Ketogenic diet enhances cardiac autonomic stability in pediatric drug-resistant epilepsy: A longitudinal heart rate variability study.

OBJECTIVE: Pediatric drug-resistant epilepsy(DRE) is frequently associated with autonomic dysregulation, which contributes to increased cardiovascular morbidity. This study evaluated the impact of a ketogenic diet(KD) therapy on cardiac autonomic regulation in children with DRE using heart rate variability(HRV) metrics. METHODS: In this prospective longitudinal observational study, 13 pediatric patients with DRE were enrolled. All patients underwent 24-hour ambulatory ECG (Holter) monitoring at baseline and after six months of ketosis.Time-domain HRV parameters (SDNN,SDANN,RMSSD, pNN50) and he

Neuroimmune Dynamics in Diseases of the Arterial Wall: Mechanistic Foundations and Translational Prospects.

PURPOSE OF REVIEW: Peripheral nerves are increasingly recognized as regulators of vascular pathology, shaping arterial tone, inflammation, and structural remodeling. This review delineates how neural circuits interface with the aortic wall, considers sympathetic and sensory pathways in atherosclerosis and aneurysm pathophysiology, and discusses emerging experimental approaches and therapeutic strategies. RECENT FINDINGS: Autonomic and sensory neural circuits are increasingly recognized as modulators of arterial disease. In experimental atherosclerosis, neuroimmune crosstalk shapes vascular in

How we diagnose and treat hereditary transthyretin-mediated amyloidosis with polyneuropathy in the Balkan region: an expert opinion.

Hereditary transthyretin-mediated (ATTRV; v, variant) amyloidosis is a genetic disorder that causes abnormal accumulation of amyloid deposits in organs and tissues. The most common neurological manifestation is polyneuropathy (PN) of the autonomic nervous system, cardiac involvement and average survival of 6-12 years since onset of symptoms. Recent years have been marked by advancements in diagnosis and management of ATTRv amyloidosis with PN. In Balkan countries, endemic regions for some TTR gene mutations exist, with particularities in patient's access to care given the different health infr

Relationships between brain functional connectivity and resting cardiac autonomic profiles in functional neurological disorder: A pilot study.

BACKGROUND: Functional neurological disorder (FND) is associated with alterations in functional brain networks, yet relationships between peripheral autonomic physiology and brain architecture remain poorly characterized. This pilot study examined associations between cardiac autonomic metrics and resting-state functional connectivity (rsFC) in FND. METHODS: Twenty females with FND and 23 age-matched female psychiatric controls (PCs) completed questionnaires, 10-min resting photoplethysmography recordings, and same-day resting-state fMRI. Interbeat interval (IBI) and heart rate variability (H

Autonomic Dysfunction as a Potential Pathophysiological Link Between Chronic Low Back Pain and Coronary Artery Disease: A Systematic Review.

Chronic low back pain (CLBP) is a leading cause of disability worldwide and is increasingly recognized as a condition associated with systemic physiological alterations beyond musculoskeletal dysfunction. Emerging evidence suggests that autonomic nervous system dysregulation may be associated with impaired cardiovascular regulation and increased cardiovascular risk in individuals with CLBP. This systematic review aimed to evaluate the current evidence regarding autonomic dysfunction in CLBP and its potential association with cardiovascular disease. A comprehensive literature search was conduct

Does immediate orthostatic hypotension in adults have clinical implications for syncope and falls? An analysis of the Irish longitudinal study of ageing.

BACKGROUND AND AIMS: Orthostatic hypotension is a common cause of syncope and falls; however, it is unclear if the association differs by variant. One of the variants is immediate orthostatic hypotension (formerly initial), which may be of lesser clinical importance than other variants. The Irish Longitudinal Study of Ageing offers a unique opportunity to evaluate the association of orthostatic hypotension variants with falls and syncope events in adults 50 years and older. METHODS: Blood pressure (BP) and heart rate were measured supine and standing using phasic blood pressure techniques. In

Elevated Serum Soluble CD163 Indicates Macrophage Activation: A Potential Biomarker for Inflammation in Complex Regional Pain Syndrome?

BACKGROUND: Several studies point towards the involvement of the immune system (especially the monocyte-macrophage system) in the pathophysiology of Complex Regional Pain Syndrome (CRPS), as shown by increased levels of the cytokines tumor necrosis factor (TNF)-α and interleukin (IL)-6. These cytokines are primarily released from pro-inflammatory M1 macrophages, but are difficult to measure in a clinical setting. Investigating the activity of tissue-resident macrophages by measuring soluble CD163 (sCD163) in serum is much more clinically applicable. sCD163 could potentially be a biomarker for

Diagnostic Strategies for Neuroblastoma in Children With Horner Syndrome: A Proposed Diagnostic Algorithm.

Neuroblastoma constitutes the most frequent solid tumor in children under 2 years of age. It is most often located in the abdomen; however, it may also be found in the mediastinum or the neck, and its first and only symptom may be Horner syndrome (HS). Because various imaging and laboratory examinations may be employed in order to detect the tumor, the objective of our study was to assess their sensitivity in detecting neuroblastoma in children with HS. The study comprised data from 14 oncologic departments in Poland and involved children with a clinical diagnosis of neuroblastoma. In total, 7

Pupillary disorders.

This chapter provides a comprehensive overview of various conditions affecting the pupil, encompassing anatomic, developmental, and pathologic aspects. The relevant anatomy and embryology of the iris is described, highlighting the sphincter and dilator pupillae muscles which control pupil size. A few examples of iris pathologies (congenital and acquired) are given for illustrating ocular disorders which affect pupil size and shape. The majority of this chapter discusses the autonomic pathways, parasympathetic and sympathetic, to control pupillary movement. Included are details of the anatomic

Involuntary inspiratory sigh in multiple system atrophy: A clinical and cerebral blood flow study.

BACKGROUND: Involuntary inspiratory sigh (IIS) is a characteristic symptom of multiple system atrophy (MSA) and a supportive non-motor feature in the current diagnostic criteria. However, IIS during wakefulness has not been systematically investigated. OBJECTIVES: This study explored the clinical significance and neural correlates of IIS by comparing clinical characteristics and cerebral blood flow (CBF) single-photon emission computed tomography findings between patients with and without IIS, and evaluated a practical method for IIS assessment. METHODS: We retrospectively investigated 90 pa

Central circuits in control of energy expenditure: connecting energy and temperature homeostasis.

Energy expenditure (EE) is the dynamic physiological process that balances energy intake to maintain body weight and body temperature. Environmental changes induce EE adaptations that limit fluctuations in body weight or body temperature by balancing between body temperature defense and energy conservation. The central nervous system (CNS) orchestrates these adaptations by integrating multiple external and internal cues to coordinate thermogenesis together with other behavioral and metabolic processes. Key hypothalamic and brainstem nuclei form a distributed network that modulates autonomic ou

A giant cervical ganglioneuroma in a middle-aged man: a case report.

Cervical ganglioneuroma is a relatively rare benign neurogenic tumor for which standardized evidence-based guideline is lacking. It is more common in women, adolescents, and children and presents as an asymptomatic neck mass. Preoperative imaging and needle biopsy are insufficient to confirm the diagnosis, making postoperative histopathological examination of a specimen the gold standard. Herein, we present the case of a middle-aged man with a giant ganglioneuroma of the neck, discussing the relevant diagnostic and therapeutic characteristics. The patient was a 43-year-old man who had been dia

Plasma IL-24 is associated with time to first fall in Parkinson's disease.

Plasma inflammatory protein biomarkers associated with clinical milestone heterogeneity in Parkinson's disease (PD) remain undefined. We measured 92 inflammation-related plasma proteins in 82 PD patients using the Olink Proximity Extension Assay. No proteins differed significantly between fallers and non-fallers. However, among fallers, plasma interleukin-24 showed the strongest correlation with observed time to first fall (unadjusted r = 0.752, q = 0.071), and this signal was also observed in an exploratory covariate-adjusted model (q = 0.030). Colony-stimulating factor 1 showed a comparable

Metabolic dysfunction-associated steatotic liver disease in spinal cord injury and obesity: A narrative review of divergent mechanisms, convergent outcomes.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is most frequently attributed to obesity. However, spinal cord injury (SCI) also induces a rapidly developing MASLD phenotype in the absence of obesity. This narrative review synthesizes emerging evidence to compare neurogenic and obesogenic drivers of MASLD (neurogenic MASLD vs obesity-associated MASLD [oMASLD]) to uncover mechanistic pathways that converge on liver injury despite markedly different initiating stimuli. While neurogenic MASLD is characterized by abrupt and pronounced sympathetic nervous system dysregulation, oMAS
