Percutaneous endoscopic gastrostomy in atypical parkinsonian syndromes: survival and aspiration outcomes from a retrospective international cohort.

INTRODUCTION: Dysphagia frequently occurs in movement disorders, leading to malnutrition and aspiration. Percutaneous endoscopic gastrostomy (PEG) provides nutrition directly into the stomach, bypassing the dysfunctional swallow. However, PEG insertion is a complex decision, both clinically and ethically. Although PEG outcomes are reported in other neurological disorders, there is limited research in atypical parkinsonian syndromes such as multiple system atrophy (MSA), progressive supranuclear palsy (PSP) and corticobasal degeneration (CBD). Insertion rates remain variable, reflecting a pauci

Anti-TLR2 immunotherapy modulates neuron-to-oligodendrocyte propagation of α-synuclein in mouse and human models.

Intracellular accumulation of α-synuclein (αSyn) aggregates is a hallmark of synucleinopathies, such as Parkinson's disease (PD) and multiple system atrophy (MSA). In MSA, αSyn aggregates form glial cytoplasmic inclusions (GCIs) in oligodendrocytes, despite their low expression of αSyn. Here, we demonstrate that neuron-to-oligodendrocyte propagation of αSyn, via Toll-like receptor 2 (TLR2) contributes to GCI formation. Male transgenic mice expressing the A53T mutant human αSyn exclusively in neurons, and the preformed fibril injection model exhibited MSA-like pathology, including GCI formation

Complications and Risk Factors of Neurogenic Bladder: A Delphi Consensus.

BACKGROUND AND OBJECTIVE: Lower urinary tract symptoms secondary to neurogenic bladder impact quality of life and symptom burden significantly. These conditions are also associated with serious complications, including febrile urinary tract infections (UTIs) and impaired renal function. This study aimed to specify and rank the neurogenic bladder risk factors, their consequences, and finally the induced complications observed in neurourology. METHODS: We conducted a Delphi consensus between October 2023 and April 2025. A steering committee performed a comprehensive literature review and drafte

Inferior olive lesions in autoimmune glial fibrillary acidic protein astrocytopathy: a report of two cases.

BACKGROUND: Autoimmune glial fibrillary acidic protein (GFAP) astrocytopathy is a newly identified inflammatory nervous system disorder characterized by Immunoglobulin G autoantibodies against GFAP. While several MRI features are considered typical for autoimmune GFAP astrocytopathy, reports of lesions affecting the inferior olivary nucleus (ION) are rare. In this context, we present two cases of autoimmune GFAP astrocytopathy with lesions in the ION. CASE PRESENTATION: Case 1 involved a 64-year-old man with meningoencephalomyelitis, peripheral neuropathy, and autonomic dysfunction. His brain

Discriminative role of inflammatory indices in clinical response to ganglion impar blockade: A retrospective cohort study.

Coccydynia is a chronic pain syndrome with a complex pathophysiology. While ganglion impar blockade (GIB) is an effective treatment, predictors of response are not well-established. This study aimed to evaluate whether hematologic inflammatory indices or coccygeal morphology could predict the clinical response to GIB in patients with chronic coccydynia. We retrospectively reviewed the records of 119 patients who underwent GIB. Demographics, pain characteristics, coccygeal morphology (Postacchini-Massobrio classification), and preprocedural indices were extracted from the medical records. The p

Meteorological elements associated with vasovagal reactions in blood donors: Influence of collection setting and donation type.

BACKGROUND AND OBJECTIVES: Vasovagal reactions (VVRs) are common adverse events associated with blood donation. While previous research has focused on intrinsic donor-related risk factors, we previously identified an association between specific meteorological elements and VVR incidence. This study aims to build on our earlier findings by investigating the potential modifying effects of collection setting and donation type. STUDY DESIGN AND METHODS: This study analysed VVR incidence data from blood donations collected in northern Nara Prefecture, Japan, over a 3-year period (April 2020-March

Chromosome X-wide association study in multiple system atrophy identifies sex-differential risk loci.

The human X chromosome accounts for ∼5% of the genome. Despite its size, the role of X-chromosomal variants in human diseases is not well understood, mainly due to its distinct inheritance pattern and the frequent omission of sex chromosomes from genome-wide association studies. This study used whole-genome sequencing data from 888 multiple system atrophy (MSA) cases and 7128 controls to perform an X-chromosome-wide common variant association study. Our analyses revealed two sex-differential risk loci: one located at Xq28, associated with increased risk for MSA in females [index variant: rs489

Heart rate variability as a predictor of future depressive symptoms: A systematic review and meta-analysis.

Depression has limited early detection strategies. Heart rate variability (HRV), an indicator of autonomic regulation, has been linked to depression cross-sectionally, but its prospective value remains uncertain. We performed a meta-analysis and systematic review examining whether HRV is prospectively associated with later depressive symptom levels. PubMed, Embase, Cochrane, and PsycINFO (inception November 3, 2024) were searched for longitudinal studies of initially non-depressed baseline HRV and at least one-month follow-up assessing depressive or internalizing symptoms. Meta-analyses used m

Sympathetic signals beneath the surface: fresh insights from skin and muscle.

The sympathetic branch of the autonomic nervous system, known for its governance of the "fight or flight" response, has attracted newfound interest due to its role in maintaining bodily homeostasis in various tissue types. Sympathetic activity in the skin is often perturbed in neurological and neurodegenerative disorders. Notably, aberrant changes in the sympathetic skin response can be detected before clinical manifestations of diabetic neuropathy. Furthermore, sympathetic signaling at neuromuscular junctions in skeletal muscle has now been demonstrated to be critical for synapse integrity an

Efficacy and Safety of Ultrasound-Guided Right-Sided Stellate Ganglion Block for Reducing the Severity and Duration of Acute Paroxysmal Sympathetic Hyperactivity in Patients with Acquired Brain Injury: A Randomized Controlled Clinical Trial.

Paroxysmal sympathetic hyperactivity (PSH) is a severe autonomic disorder following acquired brain injury for which there are no effective targeted therapies. The aim of this study was to evaluate whether ultrasound-guided right-sided stellate ganglion block (RSGB) can safely and effectively attenuate acute episodes of PSH. A prospective, single-blind randomized controlled trial was conducted. Eighty patients with PSH were randomized to receive either conventional care or RSGB combined with conventional care. Primary outcomes included PSH episode intensity and duration as assessed by the Clini
