Quantitative imaging of iron dysregulation in multiple system atrophy.

Multiple system atrophy (MSA) is a rapidly progressive neurodegenerative disorder for which sensitive and objective imaging biomarkers for early diagnosis and disease monitoring remain limited. Iron dysregulation is implicated in MSA pathophysiology and represents a promising therapeutic target. We used quantitative susceptibility mapping (QSM) MRI to characterize regional patterns of iron accumulation in early-stage MSA and to explore its potential diagnostic and longitudinal utility. 3T MRI data were acquired from 38 individuals with MSA, including 10 early-stage patients with 12-month follo

Vitamin D and cardiovascular autonomic neuropathy in type 2 diabetes mellitus according to diabetic kidney disease stage.

BACKGROUND/OBJECTIVES: Cardiovascular autonomic neuropathy (CAN) may be associated with other diabetes mellitus-related complications. In addition, lower vitamin D (VD) levels have been associated with diabetic kidney disease (DKD) and diabetic neuropathy. We evaluated the relationship between serum VD and CAN in patients with type 2 Diabetes Mellitus (T2DM) in early and advanced stages of DKD. METHODS: Seventy-six T2DM patients, 28 in early DKD stage (urine albumin to creatinine ratio (UACR)): 30 to 299 mg/g - group 1), and 48 in advanced DKD stage (UACR ≥300 mg/g - group 2), participated.

Neuromodulation for Treating Adolescent Complex Regional Pain Syndrome: A Case Series.

BACKGROUND: Complex regional pain syndrome (CRPS) in adolescents is rare. When resistant to conservative treatment, therapeutic options are limited. One option is neuromodulation, but the evidence for its effectiveness is scarce. CASE REPORTS: We report 3 adolescent girls with refractory CRPS Type I who were successfully treated with neuromodulation. Two of them underwent dorsal root ganglion stimulation; the other, spinal cord stimulation. All 3 patients achieved > 50% pain reduction, restoration of function, and improved quality of life after failing multimodal conservative and minimally in

Psychological stress and immune dysregulation: Insights from neuroimmunology and the gut-brain-immune axis.

The interplay between mental health, immune function, and gut microbiota has garnered increasing attention in recent years, with mounting evidence indicating that psychological stress and depression are increasingly associated with alterations in immune responses and systemic health. Chronic stress and depression are closely associated with immune dysregulation and have been implicated in the onset and progression of autoimmune diseases, cardiovascular disorders, cancer, and other inflammation-related conditions, although these relationships are complex and not strictly causal. Emerging researc

Heart rate variability across recording conditions: Associations with depression and burnout in inpatients with stress-related disorders.

OBJECTIVE: Heart rate variability (HRV) is a non-invasive marker of the autonomic nervous system and may be a promising mental health indicator. However, the existing literature lacks clarity regarding which HRV parameter and recording condition feasibly reflects the severity of depression and burnout. This study aimed to evaluate HRV across recording conditions in relation to depression and burnout severity in inpatients with stress-related disorders. METHODS: Inpatients (n = 153) diagnosed with burnout syndrome and either depressive episode or adjustment disorder (aged 27-64 years, 88 femal

α-synuclein PET imaging: early human evidence and the path toward clinical proof-of-concept.

INTRODUCTION: Aggregation of misfolded α-synuclein is the defining pathological hallmark of synucleinopathies, including Parkinson's disease, dementia with Lewy bodies, and multiple system atrophy (MSA). Despite major advances in the field of molecular neuroimaging, the in vivo detection of α-synuclein remains one of the most challenging goals in positron emission tomography (PET), largely due to the low abundance of aggregates, structural heterogeneity of fibrillar strains, and the need for high selectivity over other amyloid proteins. AREAS COVERED: This Special Report summarizes advances i

Chemotherapy-induced peripheral neuropathy: Mechanisms of toxicity and potential natural compounds-based therapeutics.

Platinum-based drugs, such as cisplatin, carboplatin, and oxaliplatin, are increasingly being used to treat a variety of cancers, including lung, ovarian, testicular, and bladder cancer, as well as a secondary treatment for other types of cancer. These drugs primarily act by targeting the DNA of cancer cells, causing DNA damage that inhibits cell division and slows cancer progression. However, the mechanism can also lead to specific side effects, such as neuropathy. Chemotherapy-induced peripheral neuropathy (CIPN) caused by platinum drugs is a common and severe toxicity issue that significant

Heart rate monitoring using wrist photoplethysmography in Parkinson disease: feasibility and relation with autonomic dysfunction.

BACKGROUND: Cardiac autonomic dysfunction is a prevalent yet underdiagnosed non-motor manifestation in Parkinson disease (PD), and difficult to monitor in daily life. Wrist-worn photoplethysmography (PPG) allows for continuous pulse rate monitoring, with potential to measure cardiac autonomic dysfunction in PD. However, motion artifacts pose significant challenges. We propose a two-step approach to account for PPG motion artifacts, and explore the association between pulse rate characteristics and cardiac autonomic dysfunction in PD. METHODS: In 444 people with early PD, we used continuous wr

A multimodal biomarker strategy to enhance diagnostic precision in neurodegenerative parkinsonism.

Differential diagnosis of neurodegenerative parkinsonian syndromes is complicated by overlapping clinical features and frequent co-pathology that challenges the interpretation of single-protein biomarkers. We evaluated a multimodal, minimally invasive biomarker strategy integrating dermal α-synuclein and 4-repeat tau seed amplification assays (SAAs) with serum neurofilament light chain. In a prospective cohort of 166 participants (Parkinson's disease, n = 40; multiple system atrophy, n = 29; progressive supranuclear palsy (PSP), n = 77; healthy controls, n = 20) with independent external valid

Differential Target Multiplexed Spinal Cord Stimulation in Persistent Spinal Pain Syndrome Type II: A Multicenter Prospective Study.

OBJECTIVES: Differential target multiplexed spinal cord stimulation (DTM-SCS) is a novel neuromodulation paradigm designed to modulate neuroglial interactions involved in chronic pain. This study tested the hypothesis that in a real-world clinical setting, DTM-SCS is associated with sustained improvements in pain intensity and related clinical outcomes in patients with persistent spinal pain syndrome type II (PSPS-II) and that early inflammatory biomarker modulation may reflect interindividual heterogeneity in treatment response. MATERIALS AND METHODS: This was a prospective, multicenter, obs

Dynamic Modulation of the Gut Microbiome in Neurodegenerative Diseases: Mechanisms, Therapeutic Potential, and Clinical Perspective.

The gut and brain communicate bidirectionally through the gut microbiota, forming a complex network often referred to as the "microbiota-gut-brain axis." The gastrointestinal microbiome produces various metabolites, including short-chain fatty acids (SCFAs), tryptophan-derived compounds, and secondary bile acids. Research indicates that disruptions in the intestinal microbiota (dysbiosis) and impaired gut-brain axis are associated with various neurological conditions. The central nervous system (CNS) influences digestive processes via the hypothalamic-pituitary-adrenal (HPA) axis and autonomic

Blood oxygen saturation and its associations with autonomic and peripheral neuropathy in diabetes.

AIM: Individuals with diabetes have lower blood oxygen saturation (SpO2), and hypoxia is suggested to play a role in the pathophysiology of diabetic complications, including diabetic polyneuropathy (DPN). This study investigated whether lower SpO2 was associated with DPN. METHODS: The study involves participants with type 1 and type 2 diabetes from a cross-sectional study. SpO2 was measured in the supine position with fingertip pulse oximetry. To assess multiple domains of peripheral and autonomic nerve function, several different standardized tests were used, including autonomic tests, senso

TPPP/p25 amyloid seeding activity as a specific biomarker for multiple system atrophy.

Detection of α-synuclein (α-syn) amyloid seeds in human biofluids has attracted great interest for clinical diagnosis of synucleinopathies. However, as a common biomarker, α-syn lacks specificity in reliably differentiating distinct disorders. Here, we report tubulin polymerization promoting protein (TPPP/p25) as a cerebrospinal fluid (CSF) biomarker for the specific diagnosis of multiple system atrophy (MSA). We demonstrate that native TPPP/p25 is self-protected against amyloid aggregation, while disease-related mutation disrupts this protection, triggering TPPP/p25 aggregation. Cryo-electron

Circulating Neopterin Is Independently Associated with Autonomic Neuropathy in Type 2 Diabetes, but Not with Other Microvascular Complications.

Background: Chronic low-grade inflammation plays a central role in the pathogenesis of type 2 diabetes (T2DM) and its complications. Neopterin, a marker of macrophage activation and Th1-mediated immune response, has been associated with cardiovascular disease and metabolic disorders. However, its relationship with diabetic autonomic neuropathy remains insufficiently investigated. Methods: We conducted a cross-sectional study including 129 participants (93 with T2DM and 36 with obesity without carbohydrate disturbances). Clinical, anthropometric, and biochemical assessments were performed. Card

Cerebral metabolic trajectories alterations in multiple system atrophy: A longitudinal FDG-PET study.

Multiple system atrophy (MSA) is a rapidly progressive neurodegenerative disorder with different metabolic patterns in parkinsonian (MSA-P) and cerebellar (MSA-C) subtypes, but their longitudinal changes are not well understood. To characterize longitudinal changes of glucose metabolism and network connectivity in MSA using 18F-FDG-PET for early diagnosis and disease progression tracking, we retrospectively analyzed 29 MSA patients (17 MSA-P, 12 MSA-C) and 28 healthy controls. Regional glucose uptake was assessed as standardized uptake value ratios (SUVR). We examined inter-regional connectivi
