Comparison of Shoulder Field Anesthesia (SFA) with Interscalene Block (ISB) for Analgesia After Arthroscopic Shoulder Surgery: A Randomized Noninferiority Trial.

BACKGROUND: An interscalene brachial plexus block (ISB) is commonly used for analgesia after shoulder surgery but is associated with neurologic and respiratory complications, including hemidiaphragmatic paralysis. Surgeon-performed shoulder field anesthesia (SFA) is a landmark-guided, phrenic nerve-sparing technique that may provide effective analgesia with fewer complications. The purpose of this study was to compare SFA with ISB for pain control after arthroscopic shoulder surgery. We investigated whether SFA would provide noninferior analgesia as measured by cumulative 48-hour postoperative

Robotic nerve-sparing enucleation of thoracic sympathetic trunk schwannoma with narrow-band imaging: technical considerations.

Schwannomas arising from the thoracic sympathetic trunk can cause Horner's syndrome, and surgical resection carries a risk of further nerve injury. We describe a technique for robotic nerve-sparing enucleation of a thoracic sympathetic trunk schwannoma presenting with Horner's syndrome. After exposure of the epineurium and perineurium, careful dissection of these neural sheath structures revealed a smooth tumor surface, along which enucleation was performed. Narrow-band imaging enhanced microvascular structures within the epineurium and perineurium and improved visual contrast with the relativ

Robotic selective thoracic sympathectomy for hyperhidrosis: refined technique and anatomical variation.

Primary hyperhidrosis substantially impairs quality of life. Conventional sympathectomy risks compensatory hyperhidrosis through trunk disruption, whilst selective sympathectomy preserves the trunk while dividing preganglionic and postganglionic fibres. We describe a technique refinement and a previously undescribed anatomical variant. A 23-year-old man had severe refractory palmar, axillary, plantar and gustatory hyperhidrosis (SSDS 4) profoundly affecting his confidence, social functioning and mental wellbeing, despite conservative management. He underwent staged bilateral robotic selective

Hemi-see-saw nystagmus with superimposed pendular oculopalatal tremor: case report and systematic review.

BACKGROUND: Hemi-see-saw nystagmus (HSSN) is an uncommon subtype, characterized by elevation and intorsion of one eye with contralateral depression and extorsion, typically in a jerk pattern. It is associated with unilateral lesions of the interstitial nucleus of Cajal and the medial longitudinal fasciculus, both of which are involved in the ocular tilt reaction (OTR). Although its association with oculopalatal tremor (OPT) has been described, it is rare. METHODS: We performed a systematic review of seven databases on HSSN, extracting data on demographics, etiology, lesion location, oculomoto

Low-Volume Superior Trunk and Superficial Cervical Plexus Block With Sedation as the Sole Regional Anesthetic for Proximal Humerus Fracture Fixation in a Patient With Alzheimer Disease.

BACKGROUND Proximal humerus open reduction and internal fixation (ORIF) is usually performed under interscalene brachial plexus block (ISB) or general anesthesia (GA). ISB causes ipsilateral phrenic nerve paresis in nearly all patients, and GA may raise the risk of postoperative neurocognitive disorders in elderly patients with cognitive impairment. We report the feasibility of a low-volume regional technique, used as the sole regional anesthetic with sedation, in a patient in whom GA avoidance was desirable. CASE REPORT A 68-year-old woman (American Society of Anesthesiologists physical statu

CARE STUDY: audit of complications of regional anesthesia. A prospective multicenter cohort study.

BACKGROUND: Regional techniques are not risk-free. The aim of the study was to determine the complications associated with peripheral regional anesthesia and to identify factors that could increase the risk, to try to correct or prevent them. METHODS: This prospective observational study formed part of the CARE audit. Thirty-one hospitals across Spain participated in the study, 1528 patients scheduled for surgical procedures involving nerve and/or fascial plane blocks were included in the analysis. Data on demographics, block type and location, safety and the surgery was collected. The form a

Post-styloid parapharyngeal neurogenic tumors: imaging-based prediction of nerve origin and clinical outcomes - a single-center retrospective case series.

INTRODUCTION: Post-styloid parapharyngeal space (PPS) tumors are rare and predominantly neurogenic. Because tumors arising from the vagus nerve or the cervical sympathetic chain are associated with distinct postoperative morbidities, accurate preoperative identification of the nerve of origin is clinically important for surgical planning and risk counseling. The primary endpoint was the accuracy of imaging-based nerve-of-origin prediction using vessel displacement patterns, compared with intraoperative confirmation. METHODS: We retrospectively reviewed adult patients with post-styloid PPS tum

Iatrogenic Horner syndrome following internal jugular vein catheterization in an infant: a case report and literature review.

INTRODUCTION: Horner syndrome (HS) is a rare but recognized complication of internal jugular vein (IJV) catheterization in children, resulting from injury to the cervical sympathetic pathway. Neonates and infants are particularly vulnerable due to the compact anatomy of the neck. PRESENTATION OF THE CASE: We report a 38-day-old female infant who developed ipsilateral HS following open surgical placement of a Broviac catheter via the right IJV. The catheter was inserted under local anesthesia with sedation for prolonged intravenous therapy. Within 24 hours postoperatively, the patient presente

Transient Horner Syndrome Following Interscalene Nerve Block for Rotator Cuff Repair: A Case Report.

Horner syndrome is a well-recognized constellation of symptoms resulting from the disruption of the sympathetic pathway. It may occur in a variety of clinical contexts, including trauma, tumors, or iatrogenic causes. We report a rare case of transient Horner syndrome following rotator cuff repair with interscalene nerve block. Imaging ruled out structural or vascular causes. The syndrome resolved spontaneously by postoperative day 3. This case highlights the importance of clinician awareness regarding this benign but alarming postoperative complication. This case was presented as a meeting abs

Extended Foville Syndrome Due to a Pontine Tegmental Hemorrhage: A Clinicoradiologic Case Report.

Foville syndrome is a rare pontine tegmental syndrome classically characterized by ipsilateral abducens and peripheral facial nerve palsies with contralateral hemiparesis. Extended variants involving additional ocular motor, sensory, and autonomic pathways are uncommon, particularly when caused by hemorrhagic stroke. We report a 59-year-old Filipino male with uncontrolled hypertension who presented within hours of symptom onset with left lateral rectus paresis, left peripheral facial palsy, moderate dysarthria, right hemiparesis, and mild right hemisensory loss, with a National Institutes of H

Contrast-enhanced ultrasound combined with diagnostic nerve block for differentiating cervical sympathetic ganglion from metastatic lymph node: a case report.

Differentiating cervical sympathetic ganglion (CSG) from metastatic lymph nodes in the central neck compartment remains a diagnostic challenge, particularly in patients with thyroid carcinoma. Conventional imaging modalities, including contrast-enhanced computed tomography (CT) and ultrasound, may yield indeterminate findings due to overlapping morphological characteristics. We present a case with papillary thyroid microcarcinoma that was referred for thermal ablation. Preoperative CT suggested a suspicious level VI lymph node. Contrast-enhanced ultrasound (CEUS) combined with diagnostic ultra

Incidence of hemidiaphragmatic paralysis in superior trunk versus interscalene block upper limb surgeries: a systematic review, meta-analysis, and trial sequential analysis.

BACKGROUND: Interscalene block (ISB) is widely used for upper limb surgery but frequently causes hemidiaphragmatic paralysis (HDP). Superior trunk block (STB) has emerged as a promising alternative that preserves phrenic nerve. This study aimed to evaluate whether STB is a safer option than ISB. METHODS: This systematic review and meta-analysis followed PRISMA guidelines and was registered in PROSPERO (CRD420250654685). Randomized controlled trials (RCTs) comparing STB and ISB in adults undergoing upper limb surgery and reporting HDP as the primary outcome were included. Secondary outcomes we

Ancient Schwannoma of the Cervical Sympathetic Chain: A Rare Masquerader of Necrotic Cervical Lymphadenopathy.

Ancient schwannoma is a rare, slow-growing, benign peripheral nerve sheath tumor characterized by long-standing degenerative changes that may clinically, radiologically, and histopathologically mimic malignancy. These tumors commonly present as asymptomatic, gradually enlarging masses in the head and neck region, posing significant diagnostic challenges. We report a rare case of cervical ancient schwannoma in a 53-year-old male who presented with a painless swelling below the left ear of six-month duration. Radiological imaging suggested a necrotic cervical lymph node, and fine-needle aspirati

Case Report: Therapy of an Enormous Extracranial Carotid Artery Aneurysm With a Covered Coronary Stent in an Infant and Review of Literature.

Infectious pseudoaneurysms, though rare, pose a serious complication with mortality rates up to 77%. To date, and due to the rare incidence, no evidence-based treatment guidelines for pediatric patients are published. Different approaches have been outlined in the literature including conservative, surgical, or endovascular treatment. We report a case of a 2-year-old boy with chronic cervical lymphadenopathy for about 2 months. In an elective lymphadenectomy, a large aneurysmal bulge was noticed. MR and CT angiography revealed a particularly pronounced pseudoaneurysm of the left internal carot

Anatomical Mechanisms Underlying Clinically Reported Complications of the Infraclavicular Brachial Plexus Block: A Narrative Review.

Background: The infraclavicular brachial plexus block is a widely used regional anesthesia technique for surgery of the distal upper limb. Although generally considered safe-particularly with ultrasound guidance-a range of vascular, neurological, respiratory, and anesthetic-related complications continues to be reported. Understanding how anatomic factors can influence the occurrence of these events is essential for improving procedural safety. Objective: This narrative review aims to correlate clinically reported complications of the infraclavicular block with underlying anatomical mechanisms

Left Stellate Ganglion Pulsed Radiofrequency Suppresses Ischemic Ventricular Arrhythmias: A Translational Study.

BACKGROUND: Cardiac sympathetic hyperactivity originating from the left stellate ganglion (LSG) drives ischemic ventricular arrhythmias (VAs). Stellate ganglion block provides only transient relief, whereas surgical denervation is invasive and irreversible. A minimally invasive approach with potentially sustained effects is needed. OBJECTIVES: This study sought to investigate whether pulsed radiofrequency (PRF) of the LSG-a nonthermal neuromodulatory technique-safely suppresses ischemic VAs, and to characterize its mechanism using integrated neural and cardiac transcriptomics. METHODS: Safet
