Long-term follow-up of percutaneous radiofrequency sacral rhizotomy.

A series of 7 patients undergoing percutaneous radiofrequency sacral rhizotomy to change a hyperreflexic to an areflexic bladder was reported in 1978. A patient from this original series was seen for treatment nine years later with successful preservation of good vesical capacity enabling him to use Credé maneuver and intermittent catheterization to evacuate his urine. In addition, there has been no recurrence of symptoms of autonomic dysreflexia during that period.

Medical Concerns Among Wheelchair Road Racers.

In brief: Before participating in the 1987 Peachtree Road Race in Atlanta, 43 wheelchair athletes completed a questionnaire about their training methods, injuries, and medical concerns. The injuries reported most frequently were abrasions and musculoskeletal syndromes of the upper extremities. Of particular interest in the study were the types of residual neurologic deficits and autonomic dysfunction among athletes with spinal cord injury. These deficits included altered temperature regulatory mechanisms, neurogenic bladder and bowel, spasticity, and autonomic dysreflexia. Athletes also report

Ovarian cyst and autonomic dysreflexia.

Autonomic dysreflexia may present itself as a medical emergency in quadriplegics and paraplegics above the T6 level. Common causes are bladder/bowel dysfunction and pressure ulcers. A case study is presented in which an ovarian cyst was the cause of autonomic dysreflexia. Excision of the ovarian cyst stopped recurrence of paroxysmal hypertension and completely alleviated signs and symptoms of autonomic dysreflexia.

Action of clonidine on micturitional reflexes in decerebrate cats.

We explored the urodynamic action of clonidine (preferential alpha 2-agonist) and yohimbine (preferential alpha 2-blocker) in decerebrate cats. These animals retain catecholaminergic fibers from the complex of the locus coeruleus to the spinal cord and have synergic and faster micturition cycles than normal cats. Twenty-nine male cats were made decerebrate at intercollicular level under ether anesthesia. Voiding, vesical, urethral and blood pressures, and the EMG of the external sphincter-pelvic floor and leg muscles were studied. Preparations with the urethrovesical junction opened and closed

Autonomic dysreflexia: a potentially fatal complication of somatic stress in quadriplegics.

A 44-year-old patient with known, stable, long-standing quadriplegia sustained a midshaft left femur fracture in a motor vehicle accident. He presented to the emergency department (ED) two days after the accident with a chief complaint of a swollen left thigh. Management in the ED included x-ray studies and some incidental manipulation during the process of examination and splinting. While awaiting discharge, the patient, although asymptomatic, was observed to be very profusely diaphoretic above the level of his cord lesion. Blood pressure was then noted to be 230/110, far above his admission

Sympathetic activity and blood pressure increases with bladder distension in humans.

Microneurographic recordings of muscle nerve sympathetic activity, which is governed by baroreceptors and involved in blood pressure regulation, were made in the peroneal nerve in 16 healthy volunteers during physiological bladder distension. When the urge to urinate was pronounced, sympathetic outflow increased from a baseline level of 16.3 +/- 1.7 to 23.2 +/- 1.9 bursts/min (mean +/- SEM, p less than 0.01). There was a concomitant significant rise in both systolic and diastolic blood pressure, from 125 +/- 2/74 +/- 2 to 140 +/- 4/84 +/- 3 mm Hg. After micturition, sympathetic activity and bl

Clinical observations in vibratory stimulation of the penis of men with spinal cord injury.

Less than 10% of men with complete spinal cord injury experience ejaculation in intercourse. Vibratory stimulation of the penis triggers a predictable series of body reactions in most men with lesions above the thoracic 11th spinal cord level. These reactions are very similar to the physical manifestations of the sexual response in men with intact nervous systems. Between half and three quarters of men with lesions above the thoracic 11th spinal cord level ejaculate. The spermatozoa count is within the normal range, but the percentage of motile spermatozoa is low. The reactions leading to ejac

Care of the spinal cord-injured patient.

Family physicians are often unfamiliar with the care of patients with spinal cord injuries because they may have only one such patient in their practice. Urinary tract infections, constipation, and decubitus ulcers are the most common problems, and autonomic dysreflexia the most serious emergency that family physicians treat in this population. This article addresses these areas, as well as spasticity, sexuality, depression, and the acute abdomen.

[Transurethral electroresection of the external urethral sphincter in the urological management of male tetraplegics].

Transurethral electroresection of external urethral sphincter (TUR-sph) was performed in sixteen male tetraplegics to obtain good voiding efficiency and to prevent autonomic dysreflexia due to detrusor sphincter dyssynergia. Ratio of residual urine as a parameter of voiding efficiency and blood pressure and plasma noradrenaline level during voiding as parameters of the significance of autonomic dysreflexia were evaluated before and after TUR-sph, and all these parameters had significantly improved after TUR-sph. After discharge from the hospital, all patients urinated comfortably with the mane

Spinal cord lesions at different levels affect either the adrenergic or vasoactive intestinal polypeptide-immunoreactive nerves in the human urethra.

The urethras from 1 patient with cervical (C1-2) and 2 patients with thoracic (T10) spinal cord lesions were studied histochemically and immunohistochemically for adrenergic and vasoactive intestinal polypeptide-immunoreactive nerves. Dense vasoactive intestinal polypeptide-immunoreactive but not adrenergic nerves were found in the urethral smooth muscle, around the blood vessels and at the base of the mucosa in the patients with thoracic lesions. In contrast, adrenergic but not vasoactive intestinal polypeptide-immunoreactive nerves were found associated with the smooth muscle of the urethra

Extracorporeal shock wave lithotripsy in traumatic quadriplegic patients: can it be safely performed without anesthesia?

A total of 5 traumatic quadriplegic patients underwent 10 extracorporeal shock wave lithotripsy treatments with either a local (bupivacaine 0.25 per cent) field block or no anesthesia. Significant intraoperative hypertension as determined by the anesthesiologist occurred in 2 patients and responded to intravenous hydralazine. The complete clinical syndrome of autonomic dysreflexia did not occur. Three patients are free of stones and 2 have insignificant residual caliceal fragments. Extracorporeal shock wave lithotripsy may be performed safely in traumatic quadriplegic patients without the adde

Lower extremity fractures in paraplegics: pattern, treatment, and functional results.

Eighty-seven lower extremity fractures in 71 paraplegics were retrospectively evaluated to determine adequacy of treatment, functional result, and range of motion of the hip and knee. Most fractures treated closed healed without difficulty, but with a complication rate of 19%. Length of hospitalization averaged almost 3 months in the group treated closed, and residual hip and knee stiffness was the major complication. This was not a problem except in high demand patients, such as wheelchair athletes, in whom decreased range of motion was a significant limitation. Treatment with circular casts

Effects of botulinum A toxin on detrusor-sphincter dyssynergia in spinal cord injury patients.

We evaluated the ability of low doses of botulinum A toxin, an inhibitor of acetylcholine release at the neuromuscular junction, to denervate and relax the spastic rhabdosphincter in 11 men with spinal cord injury and detrusor-sphincter dyssynergia. Toxin concentration, injection volume, percutaneous versus cystoscopic injection of the sphincter and number of injections were evaluated in 3 treatment protocols. All 10 patients evaluated by electromyography after injection showed signs of sphincter denervation. Bulbosphincteric reflexes in the 10 patients evaluated after injection were more diff

A life-threatening respiratory complication of gastro-oesophageal reflux in a patient with tetraplegia.

Recurrent attacks of life-threatening dyspnoea and choking occurred in a patient with tetraplegia. Conventional investigations for gastro-oesophageal reflux were normal, but 24-hour oesophageal pH recording revealed gross reflux in association with an attack of dyspnoea. Surgical correction of the reflux abolished the attacks. The possibility of autonomic dysreflexia as the mechanism linking reflux and respiratory symptoms in this patient is discussed.

Orthostatic hypotension-induced autonomic dysreflexia.

Three patients with chronic traumatic cervical myelopathy had severe orthostatic hyperhidrosis. Orthostatic challenge revealed that hypotension preceded hyperhidrosis, hypertension, and chills, all manifestations of autonomic dysreflexia. Treatment of orthostatic hypotension with fludrocortisone acetate relieved these symptoms. Therefore, orthostatic hypotension may trigger autonomic dysreflexia and the usual way of managing such patients, propping them upright, may be counterproductive.

Rehabilitative neurosurgery.

The neurosurgical role in rehabilitation rehabilitation was studied. Over a 5-year period, 850 individuals were referred to a rehabilitation center after initial acute care. Surgery was indicated for 66 patients. There were 28 quadriplegic or paraplegic individuals with intractable spasticity. Percutaneous radiofrequency foramenal rhizotomies were found to be 98% effective in relieving posttraumatic spasticity. In 14 patients with cognitive impairment, intellectual improvement had reached a plateau level. These persons underwent computed tomography scanning and cisternography, revealing signif
