Management of a pregnant paraplegic patient in a rehabilitation center.

As the population of paraplegic women of child-bearing age increases, the psychiatrist may be more frequently called on to care for pregnant women. Pregnancy in a paraplegic patient can be accompanied by many serious medical problems including autonomic dysreflexia, anemia, decubitus ulcers, urinary tract infections, and premature labor. This is a case report of a successful pregnancy in a spinal cord injured woman who was managed at a rehabilitation center throughout her entire pregnancy, despite developing the multiple complications described above.

Autonomic dysreflexia in the spinal cord-injured. What the physician should know about this medical emergency.

Autonomic dysreflexia is a potentially fatal condition occurring in quadriplegics and paraplegics whose spinal cord lesion lies above the greater splanchnic outflow. Prompt recognition of common and less common clinical findings and a search for the causative stimulus are essential. Bladder distention and fecal impaction are the two most likely causes. Should it persist after removal of the stimulus, hypertension requires aggressive drug therapy to control massive sympathetic outflow.

Autonomic dysreflexia in a paraplegic man with catecholamine-secreting neuroblastoma.

Autonomic dysreflexia and catecholamine secreting tumor, each of which causes paroxysmal hypertension, coexisted in a young man. Two years after neuroblastoma was diagnosed, he developed T4 incomplete paraplegia due to metastases to the spine at T5 and L3 levels. Shortly after the onset of paraplegia, paroxysmal hypertension developed. The hypertension was controlled adequately by good bowel and bladder management and oral clonidine. The paroxysmal hypertension is believed to have resulted from the synergistic effect of the high levels of circulating catecholamines from the tumor and the disru

Seizures secondary to autonomic dysreflexia.

Seizures are a little-recognized component of the syndrome of autonomic dysreflexia. Three patients who exhibited seizures during episodes of dysreflexia independent of intracerebral pathology are described. The presence of seizures in patients with traumatic myelopathy T-6 should alert the physician to a possible dysreflexic episode, and measures should be taken to avert such an episode.

5-microtransducer catheter in evaluation of neurogenic bladder function.

Recent use of the multiple microtransducer catheter in the evaluation of neurogenic bladder due to spinal-cord injuries leads us to believe that the use of the inferior edge of the symphysis pubis as the zero point for resting bladder pressure is more accurate than its superior edge, changes in resting bladder pressure at various volumes are influenced more by body position than by intravesical position of the sensor, back-to-back microtransducers indicate significant pressure difference at the external sphincter zone, and detrusor bladder neck dyssynergia during autonomic dysreflexia in patie

The effect of modified transurethral sphincterotomy on autonomic dysreflexia.

Autonomic dysreflexia continues to be a major problem in spinal cord injury patients. In our report 16 patients with spinal cord lesions above the T6 level and a history of autonomic dysreflexia also had detrusor-sphincter dyssynergia. Of these patients 12 (75 per cent) had evidence of dysreflexia during cystometry. In addition, the dysreflexic response in blood pressure was shown to correlate with the high intravesical and urethral pressures measured during cystometry. Modified transurethral sphincterotomies subsequently were performed on all patients as recommended therapy for detrusor-sphin

Catheter-induced hyperreflexia in spinal cord injury patients: diagnosis by sonographic voiding cystourethrography.

In part 1 of this study, 77 consecutive patients with spinal cord injuries and reflex bladders were examined by combined urodynamic studies and sonographic voiding cystourethrography. Of the 15 (19%) who had hyperreflexic bladders (reflex bladders that contracted when containing 125 ml or less), eight (53%) had catheter-induced hyperreflexia (proved by sonographic voiding cystourethrography without catheterization). The significant overdiagnosis influenced patient care because catheter-induced hyperreflexia did not require treatment, whereas primary hyperreflexia caused by lesions above T-5 al

A comparison of the efficacy of an alpha-I-adrenergic blocker in the slow calcium channel blocker in the control of autonomic dysreflexia.

Cystometry, using a portable CO2 cystometer, is a convenient method for detecting autonomic dysreflexia (A.D.) in response to bladder distention. Serial tracings on successive days were found to be consistent. This method was used to compare the effect of the antihypertensive drugs, phenoxybenzamine and nifedipine, in modifying the blood pressure responses of 12 tetraplegic patients. Given as regular medication twice daily, neither drug was effective in preventing A.D. responses to bladder filling, and a significant number of patients developed troublesome hypotension. Nifedipine by mouth was

Autonomic dysreflexia due to medication: misadventure in the use of an isometheptene combination to treat migraine.

Autonomic dysreflexia, a syndrome sometimes occurring in spinal cord injured (SCI) individuals, may be life-threatening. It involves, in varying degrees, hypertension, diaphoresis, headache, bradycardia, anxiety, and flushing and is believed due to unrestricted sympathetic activity below the level of the lesion in individuals with injuries above T4-6. The most frequent causes of the syndrome are urinary infections, rectal impaction, bladder distention, and decubitus ulcers. To our knowledge, medication has seldom been described as causal agent. We report here on an autonomic dysreflexic syndro

Autonomic hyperreflexia: a mortal danger for spinal cord-damaged women in labor.

Reproductive care of women with spinal cord damage demands knowledge of such women's reproductive potential and the specific complications to which these women are prone during pregnancy and childbirth, especially autonomic hyperreflexia. Fertility in cord-damaged women of reproductive age is generally undiminished as are libido, ability to have intercourse, and ability to bear children. Frequent complications of cord-damaged pregnant women include urinary tract infection, anemia, pressure sores, sepsis, unattended birth, and autonomic hyperreflexia. Autonomic hyperreflexia or autonomic dysref

Medical complications of spinal cord injury.

Recent medical advances have greatly improved the prognosis for people who sustain spinal cord injury (about 10 000 people initially survive spinal cord injury each year in the US). Physicians may become involved with the acute care of spinal cord injured persons or care for complications which develop later. Certain conditions such as heterotopic bone formation and autonomic dysreflexia are seen almost exclusively in this group of patients; other complications such as urinary tract and soft tissue infections occur quite often. Common medical complications of spinal cord injured persons are re

Autonomic dysreflexia (hyperreflexia).

This author has presented his personal experience and observations concerning acute and chronic dysreflexia. The various causes of dysreflexia are discussed. The importance of bladder balance is stressed as the number one cause with the anorectal site being second. The methods of treatment that the author uses for both acute and chronic dysreflexia are described.

Urodynamic findings and clinical status following vesical denervation procedures for control of incontinence.

We compared the results of intrathecal chemical rhizotomy performed in 3 patients, complete sacral rhizotomy in 3 and dorsal root ganglionectomy in 3. Dorsal root ganglionectomy (S2 to S4) obviated reflex bladder contractility and autonomic dysreflexia, and did not induce detrusor hypertonia. Chemical rhizotomy failed to abolish reflex activity or autonomic dysreflexia and was unsatisfactory. Complete intradural sacral rhizotomy obviated reflex activity and autonomic dysreflexia but induced detrusor hypertonicity and incontinence.

Treatment of acute autonomic dysreflexia.

Chlorpromazine has proved useful in the control of sustained dysreflexia in patients with spinal cord injury. Sustained autonomic dysreflexia can be life threatening and may occur after dysfunctional urethrovesical activity or after urological operations, such as external sphincterotomy. Given by injection chlorpromazine reduced blood pressure safely and effectively.

Spinal cord injury in the female.

A survey of 200 women, admitted with spinal cord injuries over a 20-year-period, revealed that such injuries were much less common in females than males, with a ratio of 1 female to 4.4 males. Women had a higher ratio of cervical injuries, mainly due to elderly ladies falling downstairs at home. Most women (60 per cent) achieved full independence, pressure sores were seldom a problem, and some women were able to return to work in offices and teaching. Women were very rarely at risk of renal failure. Most complete tetraplegics required permanent catheter drainage, with a 30 per cent incidence o
