Cerebral metabolic trajectories alterations in multiple system atrophy: A longitudinal FDG-PET study.

Multiple system atrophy (MSA) is a rapidly progressive neurodegenerative disorder with different metabolic patterns in parkinsonian (MSA-P) and cerebellar (MSA-C) subtypes, but their longitudinal changes are not well understood. To characterize longitudinal changes of glucose metabolism and network connectivity in MSA using 18F-FDG-PET for early diagnosis and disease progression tracking, we retrospectively analyzed 29 MSA patients (17 MSA-P, 12 MSA-C) and 28 healthy controls. Regional glucose uptake was assessed as standardized uptake value ratios (SUVR). We examined inter-regional connectivi

Cross-sectional and prospective analysis of the association of the pupillary light reflex with suicidal ideation and suicide attempts.

Predictive models of near-term suicidal behavior are not powerful and have low positive predictive value. A prior small pilot study showed a relationship between suicidal ideation (SI) or suicide attempts (SA) with rapid averaged maximum constriction velocities (aMCV) obtained during assays of the pupillary light reflex. The present report aims to replicate these findings in a larger cross-sectional study of patients with anxious and depressive symptoms and then examine the optimal value of MCV in prospective identification of near-term events which represent high risk for SA. The cross-sectio

Comorbidities in Ehlers-Danlos syndromes and hypermobile spectrum disorders.

INTRODUCTION: hypermobile spectrum disorders (HSD) and Ehlers-Danlos syndromes (EDS) are connective tissue disorders often associated with systemic manifestations such as mast cell activation syndrome (MCAS), postural orthostatic tachycardia syndrome (POTS), irritable bowel syndrome (IBS), and autoimmune conditions, including spondyloarthritis (SpA). The overlap of these conditions complicates diagnosis and treatment. This study investigates the prevalence of MCAS and other comorbidities, as well as patterns of medication use, in individuals with HSD/EDS. MATERIAL AND METHODS: this cross-sect

Prevalence and related factors of blood pressure regulation abnormalities in Parkinson's disease: Orthostatic hypotension and supine hypertension.

BackgroundOrthostatic hypotension (OH) and supine hypertension (SH) are common in patients with Parkinson's disease (PD), contributing to disease-related morbidity and mortality. However, initial OH, a transient blood pressure that decreases immediately after standing, is often unrecognized, and the interactions among OH, SH, and anti-hypertensive therapy remain unclear.MethodsThis single-centre retrospective observational study included 183 patients with PD who underwent an active supine-to-stand test with beat-to-beat blood pressure monitoring. Logistic regression was used to identify factor

The Gut-Brain-Immune Axis: Role of Microbiota Dysbiosis and Autonomic Nervous System in Infectious Diseases.

The human gastrointestinal tract harbors a complex microbial ecosystem that plays a fundamental role in maintaining immune balance and defending against infectious diseases. Disruption of this ecosystem, known as dysbiosis, has been increasingly associated with impaired immune responses and heightened susceptibility to infections. Emerging evidence highlights the importance of the gut-brain-immune axis, particularly the role of the autonomic nervous system (ANS), in regulating these interactions. This systematic review aimed to examine the relationship between gut microbiota dysbiosis and infe

Acetylcholine in Brain-Body Communication: Biological Mechanisms and Physiological Roles.

Acetylcholine (ACh) is an evolutionarily conserved neurotransmitter that is widely distributed in the central and peripheral nervous systems and plays essential roles in multiple physiological processes. This review summarizes the full biological cycle of ACh, including its synthesis, vesicular storage, release, degradation, and reuptake, and discusses the regulatory mechanisms underlying its functions in the nervous system and peripheral organs. Through nicotinic acetylcholine receptors (nAChRs) and muscarinic acetylcholine receptors (mAChRs), ACh is involved in central nervous system functio

α-Synuclein overexpression increases the tropism of pre-formed fibrils and MSA-patient derived seeds towards oligodendroglia.

Multiple system atrophy (MSA) is a rare, fatal neurodegenerative disease, pathologically characterized by glial cytoplasmic inclusions (GCIs) containing α-synuclein (αSyn). The causes leading to aberrant αSyn accumulation in oligodendroglia remain unknown. Recent evidence of increased αSyn mRNA in GCI-bearing oligodendrocytes suggests that oligodendroglia αSyn expression may be a key determinant of GCI formation. To ascertain this, we generated a new transgenic (TG) mouse line overexpressing human (h-) αSyn through the myelin basic protein promoter (MBP-hSNCA mice). Immunofluorescence combined

Artificial Intelligence Applied to the Brain-Gut Axis in Irritable Bowel Syndrome: Advancing Toward Clinical Translation.

Irritable bowel syndrome (IBS) is one of the most common functional gut disorders affecting the global population, characterized by chronic abdominal pain and altered bowel habits in the absence of structural disease. The brain-gut-microbiota axis, a bidirectional network integrating central nervous system processing, enteric and autonomic function, immune signaling, and gut microbial ecology, provides a mechanistic framework that helps explain the substantial symptom heterogeneity and variable treatment response observed across patients. Artificial intelligence (AI) and machine learning (ML)

Longitudinal Decline in Electrochemical Skin Conductance Reflects Disease Progression in Multiple System Atrophy.

BACKGROUND: Multiple system atrophy (MSA) is characterized by progressive autonomic/motor dysfunction, but robust biomarkers do not exist. Electrochemical skin conductance (ESC) provides a noninvasive measure of sudomotor function, but its longitudinal predictive value and prognostic relevance in MSA remain insufficiently studied. We aimed to comprehensively evaluate ESC in a large longitudinal cohort. METHODS: We analyzed 175 patients with MSA followed for a mean of 4.2 ± 2.1 years. Disease severity was assessed using UMSARS Parts 1 and 2, COMPASS31, SCOPA-AUT, and orthostatic blood pressure

Radon balneotherapy in rheumatologic diseases: potential mechanisms and implications for cardiovascular autonomic control and inflammation.

Radon balneotherapy, a form of low-dose radiation treatment, has been utilized for decades in managing rheumatologic diseases, particularly in Central Europe and parts of Asia. Despite ongoing debate about its efficacy, developing research suggests that low-dose exposure to radon may exert anti-inflammatory and analgesic effects, with potential implications for musculoskeletal and cardiovascular health. This narrative review aims to synthesize current knowledge on the biological mechanisms underlying radon therapy, evaluate clinical evidence for its efficacy in rheumatologic conditions, and ex

Differential Effects of Sleep Respiratory Event Types on Heart Rate Variability: Central Apnea as the Most Significant.

Background: Sleep-disordered breathing (SDB) is frequently accompanied by autonomic nervous system (ANS) dysfunction, which is closely associated with an increased incidence of cardiovascular diseases and elevated mortality risk. Heart rate variability (HRV) serves as a classic metric for evaluating sympathovagal balance; however, the specific impacts of four distinct types of respiratory events-obstructive apnea (OA), central apnea (CA), mixed apnea (MA), and hypopnea (HYP)-on HRV remain underinvestigated. Utilizing ultra-short-term HRV analysis, this study aimed to evaluate the immediate eff

Interference resistant contactless heart rate variability monitoring via millimeter wave radar.

Heart rate variability (HRV) plays a critical role in the prediction of diseases related to cardiovascular and autonomic nervous system. HRV monitoring using millimeter-wave radar has been exploited as a promising research direction for daily health monitoring and supervision of patients with skin diseases. A critical issue with these solutions includes that good monitoring performance could only be achieved with unmixed heartbeat signal, while random body movements (RBM) and interference from respiration harmonics severely hinder the separation of heartbeat signal. In this paper, we propose a

Autonomic Assessment and Management in Sports-Related Concussion.

Autonomic dysfunction, or dysautonomia, is increasingly being recognized as a complication of concussion. While the symptoms can overlap with other postconcussion sequelae, it is important to distinguish the patient suffering from postconcussion dysautonomia in order to provide proper symptomatic management and aid in recovery. This narrative review will summarize the available, high-quality literature regarding autonomic testing abnormalities in patients with a history of concussion, interpret these findings in the context of the sympathetic and parasympathetic divisions of the autonomic nerv

How we diagnose and treat hereditary transthyretin-mediated amyloidosis with polyneuropathy in the Balkan region: an expert opinion.

Hereditary transthyretin-mediated (ATTRV; v, variant) amyloidosis is a genetic disorder that causes abnormal accumulation of amyloid deposits in organs and tissues. The most common neurological manifestation is polyneuropathy (PN) of the autonomic nervous system, cardiac involvement and average survival of 6-12 years since onset of symptoms. Recent years have been marked by advancements in diagnosis and management of ATTRv amyloidosis with PN. In Balkan countries, endemic regions for some TTR gene mutations exist, with particularities in patient's access to care given the different health infr
