Giant Posterior Fossa Tuberculoma Mimicking Atypical Teratoid/Rhabdoid Tumor in a Child: Illustrative Case.

Intracranial tuberculomas are rare manifestations of extrapulmonary tuberculosis that can closely mimic malignant posterior fossa tumors in children, particularly in tuberculosis-endemic regions. The radiological and clinical overlap with aggressive neoplasms such as atypical teratoid/rhabdoid tumor (AT/RT) poses a significant diagnostic challenge. A 5-year-old immunocompetent boy presented with progressive headache and irritability. Magnetic resonance imaging (MRI) of the brain revealed a large (45 × 38 mm) lesion in the cerebellar vermis and left hemisphere with mass effect on the fourth ven

Animate-Inanimate Object Categorization from Minimal Visual Information in the Human Brain, Human Behavior, and Deep Neural Networks.

The distinction between animate and inanimate things is a main organizing principle of information in perception and cognition. Yet, animacy, as a visual property, has so far eluded operationalization. Which visual features are necessary and sufficient to discriminate between animate and inanimate objects? At which level of the visual hierarchy does the animate-inanimate distinction emerge? Here, we show that the categorical boundary between animate and inanimate objects is preserved even among images of objects that are made unrecognizable and only retain the low- and mid-level visual feature

Transarterial embolization for infants under 3 months of age with refractory seizures due to hemimegalencephaly: complication analysis and evolution of treatment strategy.

BACKGROUND: Hemimegalencephaly (HME) is a rare, congenital brain malformation associated with drug-resistant seizures that are challenging to manage in young infants. Hemispheric surgery in infants <3 months of age carries increased risks of blood loss and complications due to the fragility of the immature brain parenchyma and cerebral vasculature. Transarterial embolization (TAE) has emerged from an adjunct to surgery to a potential alternative to hemispherectomy in a subset of HME patients. OBJECTIVE: To review the safety, efficacy, and evolution of TAE performed in infants <3 months of age

GLAPAL-H: Global, Local, and Parts Aware Learner for Hydrocephalus Infection Diagnosis in Low-Field MRI.

OBJECTIVE: The study aims to develop a method for differentiating between healthy, post-infectious hydrocephalus (PIH), and non-post-infectious hydrocephalus (NPIH) in infants using low-field MRI, which is a safer, low-cost alternative to CT scans. The study develops a custom approach that captures hydrocephalic etiology while simultaneously addressing quality issues encountered in low-field MRI. METHODS: Specifically, we propose GLAPAL-H, a Global, Local, And Parts Aware Learner, which develops a multi-task architecture with global, local, and parts segmentation branches. The architecture se

Prediction of CSF Intervention in Fetal Ventriculomegaly via Artificial Intelligence-Powered Normative Modeling.

BACKGROUND AND PURPOSE: Fetal ventriculomegaly (VM) is common and largely benign when isolated. However, it can occasionally progress to hydrocephalus, a more severe condition associated with increased mortality and neurodevelopmental delay that may require surgical postnatal intervention. Accurate differentiation between VM and hydrocephalus is essential but remains challenging, relying on subjective assessment and limited 2D measurements. Deep learning-based segmentation offers a promising solution for objective and reproducible volumetric analysis. This work presents an artificial intellige

An Emerging Pediatric CNS Tumor: Imaging Evaluation of H3K27M and BRAF V600E Comutated Midline Tumors.

BACKGROUND AND PURPOSE: Diffuse midline gliomas (DMGs) with concurrent H3K27M and BRAF V600E mutations represent a rare and poorly studied subset of pediatric CNS tumors. While traditionally associated with poor prognosis, recent evidence suggests that mitogen-activated protein kinase (MAPK) pathway alterations may confer improved outcomes in select cases. This study aims to explore the imaging characteristics of H3K27M/BRAF V600E coaltered midline gliomas. MATERIALS AND METHODS: Eight pediatric patients (6 boys, 2 girls; age range: 3-18 years, mean: 11 years) with H3K27M/BRAFV600E-mutant mid

Clinical characteristics and genetic analysis of different Dandy-Walker malformation.

BACKGROUND: Dandy-Walker malformation (DWM) is a rare congenital malformation of the central nervous system (CNS). It is characterized by variable hypoplasia of the cerebellar vermis, cystic dilatation of the fourth ventricle, with or without an enlarged cisterna magna. Given that large-sample genetic analysis of DWM has not been conducted to date, this study aims to provide more comprehensive genetic counseling for prenatal diagnosis by summarizing the clinical features and genetic findings of DWM. METHODS: Between January 2010 and May 2025, among 22,048 pregnant women, there were 487 cases

Hypercapnia enhances airway smooth muscle contractility via STIM1-dependent Ca2+ signaling.

Hypercapnia, elevated carbon dioxide (CO2), is common in advanced chronic obstructive pulmonary disease (COPD) and predicts poor clinical outcomes. Traditionally considered a consequence of disease severity, hypercapnia may drive disease progression by promoting airway dysfunction. Here, we show that hypercapnia acts as an active stressor, driving airway smooth muscle (ASM) constriction through a stromal interaction molecule 1-dependent (STIM1-dependent) pathway. Hypercapnia rapidly activates ERK, triggering sarcoplasmic reticulum calcium (Ca2+) release via phosphorylation of the inositol 1,4,

A functional role for Septin-2 in the maintenance of the axon initial segment and in human cognitive development.

The establishment of neuronal polarity, whereby somatodendritic and axonal cellular compartments are defined, is a critical determinant for the development of neuronal networks and patterning during neurogenesis. The axon initial segment (AIS), a key structure in the establishment of this polarity, is formed through interactions between the microtubule and actin cytoskeleton, Ankyrin G, TRIM46 and multiple transmembrane and perimembranous proteins. Here, we implicate a component of the septin cytoskeleton, Septin-2, in the maintenance and function of the AIS through the study of mutations foun

Screening the composition of tubulin isotypes reveals the most abundant TUBB4B for ciliary polarity in ependymal cells.

Cilia are microtubule-based organelles that protrude from the surface of various eukaryotic cell types. Microtubules are assembled by α/β-tubulin heterodimers, all of which comprise multiple isotypes encoded by distinct genes. However, the composition and function of tubulin isotypes in cilia are largely unclear. Here, we successfully labeled the endogenous α-tubulin and β-tubulin isotypes with HA or GFP tag in cultured mouse ependymal cells (EPCs) via the CRISPR/Cas9 system. TUBA1A, TUBA1B, TUBA1C, TUBB2A, TUBB2B, TUBB4B, and TUBB5 were identified to be incorporated in ependymal cilia, with T

Factors associated with complication of cranioplasty: CT-based risk assessment for early failure of autologous-bone cranioplasty.

To determine whether preoperative noncontrast CT features predict early revision after autologous bone cranioplasty and to develop a simple CT-based risk framework. We retrospectively studied adults undergoing autologous cranioplasty at a single center (2010-2020). Preoperative CT variables and clinical indicators were evaluated as candidate predictors in the regression and machine-learning models. The primary outcome was unplanned revision surgery within 30 days. Associations were tested with multivariable logistic regression. A feature-importance derived from the random forest model was used

A congenital infant-type hemispheric glioma case with EML4::ALK fusion.

Infant-type hemispheric glioma is a high-grade cellular astrocytoma that arises in the cerebral hemispheres in early childhood. It is characterized by receptor tyrosine kinase fusions, typically involving genes such as the NTRK family, ROS1, ALK, or MET. This tumor type represents a distinct diagnostic category in the fifth edition of the WHO Classification of Tumors of the Central Nervous System. Here, we present a congenital brain tumor case that developed in the left lateral ventricle with EML4::ALK fusion, and was classified as an infant-type hemispheric glioma. The patient was found to ha

Optimal timing of cranioplasty post-decompressive craniectomy in traumatic brain injury: a systematic review, meta-analysis, and overview of ongoing trials.

BACKGROUND: The optimal timing of cranioplasty (CP) following decompressive craniectomy (DC) for the management of traumatic brain injury (TBI) remains debated. Prior studies comparing early CP (EC) and late CP (LC) report conflicting outcomes, compounded by inconsistent timing thresholds and limited attention to effect modifiers such as implant material. OBJECTIVE: To perform a systematic review and meta-analysis comparing outcomes of EC (≤ 90 days) versus LC (> 90 days) after DC for TBI, with particular evaluation of ultra-EC (< 35 days) and implant material. METHODS: MEDLINE, Embase, and

AI-based assessment of brain volume decrease after treatment with stereotactic radiosurgery versus whole brain radiotherapy.

BACKGROUND: Radiotherapy is a cornerstone in the treatment of brain metastases, but its mid- and long-term impact on brain parenchyma remains poorly understood. This study aimed to assess the differential volumetric alterations in the brain following two different modalities of Radiotherapy-Stereotactic Radiosurgery (SRS) and Whole-Brain Radiotherapy (WBRT). METHODS: Patients treated with SRS were drawn from the prospective dataset of the STEREOBRAIN study (DRKS00014694) that recruited patients with 4-10 brain metastases from 2017 to 2020. Patients that had received WBRT for the treatment of
