Safety of Electroconvulsive Therapy With Implanted Ventricular Shunts: A Systematic Review and Case Series.

OBJECTIVES: We aim to determine the safety and outcomes of electroconvulsive therapy (ECT) in patients with ventricular shunts, with a focus on shunt function after treatment. METHODS: We conducted a retrospective analysis of all patients with ventriculoperitoneal shunts who underwent ECT at the Massachusetts General Hospital between November 2021 and November 2024. We also conducted a systematic review of the literature to identify all studies published between November 1, 1964, and November 1, 2024, reporting the use of ECT in patients with implanted cerebrospinal fluid shunts. RESULTS: Ou

Association of rurality with reduced shunt survival in child and young adult hydrocephalus: a prospective nationwide cohort from the Australasian Shunt Registry.

OBJECTIVE: Patients with ventriculoperitoneal shunts have a lifelong risk of requiring revision procedures that may be time critical. Many of these patients reside distantly from neurosurgical services; however, the influence of this key sociodemographic factor on the outcomes of patients with ventriculoperitoneal shunts has not been previously studied. The Australasian Shunt Registry provides a rare opportunity to study the association of rurality with ventriculoperitoneal shunt outcomes across a large landmass with a broadly distributed population. METHODS: Data were extracted from the Aust

Enhancing reporting standards and cumulative evidence in ventriculoperitoneal shunt studies: a systematic review and reporting guideline proposal.

OBJECTIVE: Ventriculoperitoneal shunt (VPS) placement remains the primary treatment for hydrocephalus. However, the literature on this topic is heterogeneous, with studies assessing and reporting surgical and clinical outcomes in different ways, lacking standardization. The authors aimed to evaluate the quality of these studies and propose a reporting guideline focusing on essential elements to ensure reproducibility and comparability. METHODS: Following PRISMA guidelines, the authors systematically searched PubMed, Embase, Web of Science, and the Cochrane Library databases. Eligible studies

Hemispherectomy in infants: an institutional experience with 21 patients.

OBJECTIVE: Drug-resistant epilepsy (DRE) can arise early in life due to multiple structural abnormalities. In patients with the disorder, seizures can be difficult to control and can significantly impair neurodevelopment. Hemispherectomy is well described as a treatment for refractory hemispheric-onset epilepsy, but its use in infants has been limited due to concerns over surgical risk and physiological tolerance. The aim of this review was to summarize an institutional experience with hemispherectomy in infants to demonstrate the safety and efficacy of the technique. METHODS: A retrospective

Clinical and health economic impact of the BASICS trial on ventriculoperitoneal shunt surgery: UK Shunt Registry analysis.

OBJECTIVE: The aim of this study was to determine the clinical and health economic impact of the British Antibiotic and Silver Impregnated Catheters for ventriculoperitoneal Shunts (BASICS) trial on UK shunt surgery practice and shunt infection rates. METHODS: This retrospective study used UK Shunt Registry data to compare antibiotic and standard shunt use in patients undergoing the first insertion of a ventriculoperitoneal shunt during pre-BASICS (January 2004 to June 2013) and post-BASICS (January 2018 to December 2021) periods. Patients of any age with hydrocephalus who underwent a primary

Clinical presentation of hydrocephalus following pre- and postnatal myelomeningocele repair: a Hydrocephalus Clinical Research Network study.

OBJECTIVE: Despite improvements in rates of hydrocephalus with prenatal repair of myelomeningocele (MMC), a significant number of children still require CSF diversion. While previous studies have focused on uncovering predictors of future hydrocephalus, differences in timing and presentation of MMC-associated hydrocephalus following pre- and postnatal surgery are not well-characterized. This study aimed to determine how age, head size, and ventricle size differ at hydrocephalus presentation in patients treated with pre- and postnatal surgery for MMC. METHODS: The Hydrocephalus Clinical Resear

Minimally invasive abdominal catheter placement in ventriculoperitoneal shunt patients is associated with lower abdominal catheter complications: a single center experience.

PURPOSE: Ventriculoperitoneal (VP) shunt placement represents one of the modalities for hydrocephalus (HC) requiring cerebrospinal fluid (CSF) flow diversion and is considered a procedure with a high complication rate. Reducing VP-shunt procedure time and optimizing abdominal catheter placement could help surgical VP-shunt complication rates. METHODS: A 7-year, single-center retrospective analysis was performed on adult patients who underwent minimally invasive (MI) trocar-guided direct peritoneal puncture (through the outer and inner fascia) for abdominal catheter insertion during VP-shunt im

Neonatal Dandy-Walker syndrome: a case report.

Dandy-Walker syndrome (DWS) is usually identified prenatally or after hydrocephalus-related symptoms emerge. We report a term male neonate in whom classic DWS was incidentally discovered during admission for purpura and mild tachypnea. Although fetal magnetic resonance imaging (MRI) had suggested vermian agenesis, definitive postnatal brain MRI on day 6 confirmed complete vermian absence, left cerebellar hypoplasia, and a large posterior-fossa cyst communicating with the fourth ventricle. The purpura and respiratory symptoms resolved rapidly with supportive care, and the infant was discharged

Evaluation of risk factors for obstruction of the intraventricular catheter after ventriculoperitoneal shunting in dogs with congenital internal hydrocephalus.

BACKGROUND: Ventriculoperitoneal shunting (VPS) is the standard treatment for congenital internal hydrocephalus in dogs; however, obstruction of the ventricular catheter is a frequent and serious complication. In human medicine, several predictors of catheter obstruction have been identified, while corresponding risk factors in veterinary patients remain largely unknown. OBJECTIVES: This study aimed to identify potential risk factors for ventricular catheter obstruction after VPS implantation in dogs with congenital internal hydrocephalus. METHODS: A retrospective cohort study was conducted

Sequencing of Cranioplasty and Shunt Surgery After Decompressive Craniectomy: A Swedish Multicenter Study.

BACKGROUND AND OBJECTIVES: Patients with severe brain injury requiring decompressive craniectomy are at increased risk of developing chronic hydrocephalus, often necessitating both cranioplasty and shunt surgery. The optimal sequence of these procedures remains unclear, with limited and conflicting evidence on associated complication rates and outcomes. The aim of this study was to investigate clinical practices and outcomes associated with 3 procedural sequences: (1) cranioplasty before shunt, (2) simultaneous cranioplasty and shunt, and (3) shunt before cranioplasty. METHODS: In this multic

A complicated case of ventriculoperitoneal shunt migration into the right heart ventricle after malresorptive hydrocephalus: illustrative case.

BACKGROUND: Malresorptive hydrocephalus occurs when CSF is inadequately absorbed, often following surgery. Ventriculoperitoneal (VP) shunt implantation is a well-established intervention to manage hydrocephalus by diverting excess CSF to the peritoneal cavity. OBSERVATIONS: This case report describes a rare complication following VP shunt implantation in a 62-year-old male patient with malresorptive hydrocephalus after prior aneurysm clipping. Despite laparoscopic intraoperative confirmation of correct intraperitoneal VP shunt catheter placement, postoperative imaging revealed migration of th

Candida albicans Meningoencephalitis After Vestibular Schwannoma Surgery: An Autopsy-Confirmed Case Report.

Background and Clinical Significance: Cerebral candidiasis (Candida albicans meningoencephalitis) is a rare but severe central nervous system (CNS) infection, usually associated with neurosurgical procedures or indwelling devices. Diagnosis is challenging due to frequent negativity of cerebrospinal fluid (CSF) cultures, and mortality remains high despite antifungal therapy. Case Presentation: We describe a 64-year-old woman who underwent retrosigmoid resection of a left vestibular schwannoma. The early postoperative course was complicated by fever, neurological deterioration, and hydrocephalus
