Distinctive hydrocephalus-like phenotype in NOTCH2NLC-related neuronal intranuclear inclusion disease: clinicopathological features and therapeutic implications.

NOTCH2NLC-related neuronal intranuclear inclusion disease (NIID) is a progressive neurodegenerative disorder with marked clinical heterogeneity and an increasingly recognized phenotypic spectrum. Although hydrocephalus-like presentations, i.e. ventriculomegaly with concomitant with cognition, motor, and/or bladder dysfunctions, have been observed in NIID sporadically, their prevalence, clinicopathological characteristics, and potential clinical implications remain poorly understood. This retrospective cohort study systematically characterized this newly recognized hydrocephalus-like phenotype.

Cerebrospinal fluid tap test in normal pressure hydrocephalus.

Normal pressure hydrocephalus (NPH) is a common cause of gait apraxia, cognitive impairment and urinary incontinence in older adults and can respond to cerebrospinal fluid (CSF) shunting. It can be challenging to select optimal candidates for CSF shunting given that communicating hydrocephalus is common in the normal ageing population as are the clinical symptoms of NPH. Large-volume lumbar puncture or CSF tap test can identify good candidates for CSF shunting. While neurologists are familiar with and have access to routine lumbar punctures, there are nuances about the CSF tap test for NPH to

The Lumboperitoneal Shunt Study: A Systematic Review and Single-Arm Meta-Analysis of 2696 Patients.

BACKGROUND AND OBJECTIVES: Lumboperitoneal shunt (LPS) is often regarded as a secondary treatment option for various forms of hydrocephalus. While some studies have compared its results to conventional treatments, there needs to be more comprehensive evidence systematically evaluating its isolated outcomes. METHODS: The authors systematically searched the Embase, PubMed, and Web of Science databases to identify articles of patients submitted to LPS. Random effects with single-proportion statistics were used to pool the outcomes. Subanalyses for normal pressure hydrocephalus, communicating hyd

Report of a Homozygous Nonsense Variant in the WDR91 Gene Associated With Severe Communicating Hydrocephalus, Dandy Walker Malformation, and Cerebellar Hypoplasia.

The WDR81 and WDR91 genes are important for endosome conversion, fusion, recycling, and transport. Variants in the WDR81 gene have been reported in human patients with cerebellar ataxia, impaired intellectual development, and disequilibrium syndrome type 2 (CAMRQ2, MIM #610185) and in patients with congenital hydrocephalus with brain anomalies type 3 (HYC3, MIM #617967). Both phenotypes are transmitted in an autosomal recessive inheritance pattern. Loss-of-function type variants in the WDR91 gene have also been recently described in patients with brain malformations such as hydrocephalus, brai

From shunt dependency to shunt freedom: Endoscopic third ventriculostomy in ventriculoperitoneal shunt-dependent communicating tetraventricular hydrocephalus: A case report.

BACKGROUND: Hydrocephalus represents pathological dilatation of the ventricular system caused by impaired cerebrospinal fluid (CSF) circulation and absorption, resulting in increased intracranial pressure. Ventriculoperitoneal shunting (VPS) remains the standard treatment for communicating hydrocephalus; however, it is associated with significant long-term complications, including infection and mechanical failure. Endoscopic third ventriculostomy (ETV), traditionally indicated for obstructive hydrocephalus, has increasingly been explored in selected cases of complex and communicating hydroceph

Increased free interstitial fluid in the normal appearing brain in idiopathic normal pressure and post-hemorrhagic hydrocephalus despite unchanged choroid plexus size.

PURPOSE: Adult hydrocephalus entities that require shunting may share downstream tissue-level consequences through disturbed CSF dynamics. We compared choroid plexus (ChP) size and diffusion-derived free interstitial fluid fraction (V-CSF) in normal-appearing white matter (NAWM) and gray matter among patients with idiopathic normal pressure hydrocephalus (iNPH), post-subarachnoid hemorrhage (SAH) communicating hydrocephalus, and healthy controls. METHODS: We retrospectively identified patients who underwent CSF shunting and high-resolution 3D T1-weighted MRI and multishell diffusion MRI. Diff

Autologous skull graft resorption following cranioplasty: a case report and literature review.

BACKGROUND: Autologous cranioplasty remains the first-line option for skull defect repair after decompressive craniectomy, with well-documented advantages of excellent biocompatibility, no immune reaction, and favorable cosmetic outcomes. Autologous skull graft resorption (ASGR) is a common postoperative complication that may lead to recurrent defects, poor cosmesis, and even neurological compromise. Patients with prior ventriculoperitoneal (VP) shunt are at substantially higher risk, yet long-term follow-up data and optimal management strategies in this subgroup remain poorly defined. CASE D

Post-Hemorrhagic Hydrocephalus with Secondary Cerebrospinal Fluid Pathway Obstruction in an Extremely Premature Infant: A Case Report.

BACKGROUND/OBJECTIVES: Post-hemorrhagic hydrocephalus (PHH) is a major complication of extreme prematurity associated with significant neurodevelopmental morbidity. Although post-hemorrhagic ventricular dilatation (PHVD) is a recognized consequence of intraventricular hemorrhage (IVH), progression from an apparently low-grade IVH to severe shunt-dependent disease with imaging features suggestive-but not diagnostic-of a non-communicating hydrocephalus pattern is uncommon and presents important diagnostic and management challenges. We report such a case. CASE PRESENTATION: A male infant born at

The role of stereotactic radiosurgery in the management of vestibular schwannoma in octogenarians: an international multi-institutional study.

OBJECTIVE: The incidence of vestibular schwannoma (VS) diagnosis among octogenarians has increased due to longer life expectancy and greater MRI accessibility and utilization. Stereotactic radiosurgery (SRS) offers a safe management strategy for these patients. METHODS: The authors retrospectively analyzed the data of octogenarians with VS managed with SRS across 27 institutions, assessing clinical and audiological outcomes, including survival rates, tumor control responses, complications, and posttreatment functional outcomes. RESULTS: Among 309 octogenarians with VS managed with single-fra

H3K27M-Mutant Diffuse Leptomeningeal Glioneuronal Tumor Presenting as Communicating Hydrocephalus: Case Report and Literature Review.

INTRODUCTION: Diffuse leptomeningeal glioneuronal tumor (DLGNT) with H3K27M mutation is a rare and potentially fatal condition, with only a few cases reported in the literature. We present an unusual case of DLGNT presenting with communicating hydrocephalus secondary to leptomeningeal spread, initially misdiagnosed as idiopathic intracranial hypertension (IIH), with coexisting BRAF-KIAA1549 fusion and H3K27M mutation. CASE PRESENTATION: An 11-year-old boy presented with progressive, painless bilateral vision loss. Initial neuroimaging findings were misinterpreted as IIH, and a programmable ve

Stretching the Limits: A Rare Case Report of Perimesencephalic Subarachnoid Hemorrhage During Yoga.

INTRODUCTION: Nontraumatic subarachnoid hemorrhage (SAH) is a life-threatening neurological emergency, accounting for 5-10% of strokes. While most cases result from aneurysm rupture, perimesencephalic nonaneurysmal SAH is a distinct subtype characterized by blood localized to the midbrain cisterns and a venous pathophysiology. Perimesencephalic nonaneurysmal SAH is often associated with activities increasing intracranial venous pressure. Reports linking it to low-impact exercise such as yoga are rare. A hybrid omental-parietal peritoneal fibrous sheath: a case report of an unreported cause of recurrent ventriculoperitoneal shunt failure.

BACKGROUND: Hydrocephalus often requires surgical cerebrospinal fluid diversion, and ventriculoperitoneal (VP) shunting remains one of the most widely used treatments. Distal catheter obstruction is a common cause of ventriculoperitoneal (VP) shunt malfunction. Typical abdominal mechanisms include omental wrapping, adhesions, catheter migration, perforation, and cerebrospinal fluid (CSF) pseudocyst formation. Obstruction caused by a parietal peritoneum-anchored fibrous sheath has rarely been described. CASE DESCRIPTION: A 30-years-old man underwent VP shunt placement for communicating hydroce

Understanding the role of neuroendoscopy for treatment of hydrocephalus in adults with intracranial mass lesions: a case series.

OBJECTIVE: Hydrocephalus can result from mass lesions for several reasons. Due to limited numbers of adult patients with mass lesions in studies that examine outcomes of endoscopic third ventriculostomy, success in this population is unclear. Additionally, the role of other ventriculoscopic procedures in the management of associated hydrocephalus is unclear. In this retrospective case series, the authors investigated optimal management strategies for patients with mass lesions and hydrocephalus. METHODS: A list of patients who underwent ventriculoscopic procedures or shunting procedures was c

DTI-ALPS Index and White Matter Disruption Mediate the Link Between Hydrocephalus and Cognitive Dysfunction: An MRI Study.

RATIONALE AND OBJECTIVES: A better understanding of how ventricular dilatation influences cognition in hydrocephalus might facilitate early intervention and rehabilitation for cognitive decline. We aimed to investigate whether diffusion tensor imaging analysis along the PVS (DTI-ALPS) and white matter (WM) disruption sequentially mediate the association between ventricular dilatation and cognitive decline. MATERIALS AND METHODS: 102 patients with communicating hydrocephalus (median age, 59 years), 100 patients with obstructive hydrocephalus (median age, 50 years), and 79 control subjects (med

Risks associated with ventriculomegaly and symptomatic communicating hydrocephalus following stereotactic radiosurgery for vestibular schwannoma.

OBJECTIVE: Communicating hydrocephalus may occur following stereotactic radiosurgery (SRS) for vestibular schwannomas (VSs), yet identifying individual patient risk factors associated with this post-SRS complication remains a challenge. This study examined predictors of nonobstructive ventricular enlargement and symptomatic communicating hydrocephalus following primary SRS treatment for VS via a single-center institutional cohort review and meta-analysis of the literature. METHODS: A retrospective single-institution cohort study and systematic literature review and meta-analysis examining pos

Management of Aneurysmal Subarachnoid Hemorrhage During Pregnancy with a Devastating Clinical Course: A Case Report.

Background: Aneurysmal subarachnoid hemorrhage (SAH) during pregnancy is rare, occurring in approximately 0.01-0.05% of pregnancies, most commonly in the third trimester. Its management is particularly challenging, requiring careful consideration of both maternal and fetal outcomes. Methods: We report the case of a 32-year-old woman at 31 weeks of gestation who presented with severe headache and left third cranial nerve palsy. Imaging revealed diffuse SAH with significant obstructive hydrocephalus and a 5 mm left posterior communicating artery aneurysm. Following multidisciplinary discussion,

Pericardial migration of ventriculoperitoneal shunt: a case report with review of the literature.

BACKGROUND: Ventriculoperitoneal (VP) shunting is the standard surgical treatment for hydrocephalus, yet it is associated with a wide range of complications. Distal catheter migration into the pericardial cavity is exceedingly rare and poses an immediate life-threatening risk due to potential cardiac tamponade. CASE PRESENTATION: A 1-year-old male infant, who had undergone VP shunt insertion for congenital communicating hydrocephalus at 3 months of age, presented with acute respiratory distress. Neuroimaging revealed a right-sided subdural hygroma. Chest imaging demonstrated a right pleural e

GALNS gene variants and clinical severity in pediatric mucopolysaccharidosis type IV: A retrospective cohort study from Duhok, Iraq.

Mucopolysaccharidosis type IV (MPS IV; Morquio syndrome) is a rare lysosomal storage disorder caused by pathogenic variants in the GALNS gene, resulting in deficiency of N-acetylgalactosamine-6-sulfatase and progressive accumulation of glycosaminoglycans. The disease exhibits substantial clinical heterogeneity, and the relationship between specific GALNS variants and disease severity remains incompletely understood. This retrospective cross-sectional observational study evaluated the association between GALNS variants and clinical severity in 10 children with genetically confirmed MPS IV treat

Vestibular schwannoma associated normal pressure hydrocephalus: clinical features and shunt responsiveness compared with idiopathic NPH.

BACKGROUND: Vestibular schwannoma (VS) is commonly associated with obstructive hydrocephalus due to mass effect; however, a rarer communicating form resembling normal pressure hydrocephalus (NPH) has also been described, possibly related to impaired CSF absorption from elevated CSF protein. We aimed to characterize the clinical and imaging features of VS-associated NPH (VS-NPH) and compare them with those of an idiopathic NPH (iNPH) cohort. METHODS: We retrospectively analyzed 18 patients with VS-NPH identified between 2008 and 2024. For comparison, 41 iNPH patients were drawn from a prospect
