Change in Optic Nerve Sheath Diameter and Cerebral Ventricular Shunt Failure in Children.

IMPORTANCE: Ocular point-of-care ultrasonography (POCUS) may be a clinically useful method to evaluate shunt dysfunction for children with hydrocephalus presenting to the emergency department (ED). OBJECTIVE: To evaluate whether a change in the optic nerve sheath diameter (ONSD) from prior asymptomatic baseline was associated with shunt failure. DESIGN, SETTING, AND PARTICIPANTS: Participants in this prospective single-center observational cohort study at a tertiary care children's hospital were enrolled from January 5, 2018, to March 2, 2022. Children 18 years or younger with cerebrospinal

eShunt system: a technical video.

The eShunt System (CereVasc, Charlestown, Massachusetts, USA) is a novel endovascularly implantable cerebrospinal fluid shunt that has recently emerged as an alternative strategy for the treatment of communicating hydrocephalus.1 It consists of the eShunt implant and its associated delivery components.2 The shunt is designed to be implanted via a transvenous approach into the cerebellopontine angle cistern, mimicking the natural physiology of arachnoid granulations.3 4 In this technical video 1, we will review the key anatomic considerations and device components that allow for the safe and ef

Recurrent paradoxical reactions as non-communicating hydrocephalus and basal meningitis in a non-HIV patient with tuberculous meningitis and tuberculoma.

BACKGROUND: Tuberculosis meningitisis the most devastating form of Tuberculosis, causing high mortality or disability. Paradoxical neuro-inflammatory reactions, which occur despite appropriate anti-TB chemotherapy, may reflect the host response to dead and dying bacterial. CASE REPORT: We report a case involving paradoxical reaction, effectively managed with emergent external ventricular drainage (EVD). The patient, a healthy 30-year-old Burmese woman, was diagnosed with disseminated tuberculosis involving pulmonary disease, vertebral osteomyelitis, and meningitis. Treatment included isoniazi

Deformation of brain in normal pressure hydrocephalus is more readily associated with slow vasomotion rather than heartbeat related pulsations of intracranial pressure.

OBJECTIVE: Enlarged brain ventricles, compressed parasagittal cerebrospinal fluid spaces, steep callosal angle, dilated sylvian fissures and focal cortical sulcal dilatation are typical imaging features of idiopathic normal pressure hydrocephalus (iNPH). The pathophysiological mechanisms behind these morphological changes are poorly understood, but the hydrodynamic concepts of communicating hydrocephalus suggest that increased heartbeat related intracranial pulsations are involved in ventricular enlargement. In this cross-sectional study we analysed the association between the radiological fin

Postoperative transient sympathetic storm after endoscopic third ventriculostomy with choroid plexus cauterization in pediatric hydrocephalus.

BACKGROUND: In recent years, the use of endoscopic third ventriculostomy combined with choroid plexus cauterization (ETV + CPC) for the treatment of pediatric hydrocephalus has gained increasing attention, particularly in North America and Africa. It has shown potential to enhance the efficacy of ETV alone and reduce the need for ventriculoperitoneal shunt placement. However, research on the potential side effects of CPC, particularly concerning postoperative sympathetic nervous system responses, remains limited. In our clinical practice, we observed that ETV + CPC may induce transient sympath

The venous angle as an anatomical landmark to evaluate external ventricular catheter placement.

BackgroundExternal ventricular drain (EVD) placement is a lifesaving procedure that accesses the ventricular system. It is typically performed freehand using external craniometric landmarks. Herein, we study the utility of digital subtraction angiograms (DSA) in determining the EVD location by comparing it to the deep venous system.MethodsA retrospective review was conducted at a single center from 2021 to 2024 to identify patients with EVD placement for communicating hydrocephalus secondary to a subarachnoid hemorrhage. Two independent reviewers assessed placement using Fargen's grade and a n

A giant monocular intra-abdominal CSF pseudocyst mimicking mesenteric cyst in a shunted child with spina bifida and hydrocephalus: A case report of diagnostic challenge and surgical management.

BACKGROUND: Cerebrospinal fluid (CSF) pseudocysts are even extremely rare but potentially serious complications of ventriculoperitoneal (VP) shunts, with an incidence reported between 1 % and 4.5 % in pediatric populations. When pseudocysts mimic mesenteric cysts or present atypically, diagnostic confusion and delayed treatment may occur, especially in patients with comorbid conditions like spina bifida and hydrocephalus. CASE PRESENTATION: We report a case of a 10-year-old male born with spina bifida who underwent early back closure and later developed communicating hydrocephalus requiring V

Reversible dementia associated with communicating hydrocephalus secondary to spinal hypertrophic pachymeningitis in ANCA-associated vasculitis.

Microscopic polyangiitis (MPA) is a subtype of antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV), a small-vessel vasculitis that can cause organ-threatening complications. Hypertrophic pachymeningitis is a rare, central nervous system manifestation of AAV rarely involving the spine. We herein report a patient with myeloperoxidase-ANCA-positive MPA presenting with progressive cognitive decline and gait disturbance associated with communicating hydrocephalus secondary to spinal hypertrophic pachymeningitis. The patient responded well to high-dose prednisolone and rituximab a

Basilar invagination in osteogenesis imperfecta-Case report.

Osteogenesis imperfecta (OI) is a genetic disorder characterized by defective type I collagen synthesis, leading to fragile bones and skeletal deformities. One of the serious complications of OI involves abnormalities of the craniovertebral junction, including basilar invagination, basilar impression, and platybasia, which can cause skull base deformities and brainstem compression. We report a case of a 19-year-old patient with OI presenting with headaches, neurological deficits, and visual impairment, whose contrast-enhanced head CT scan revealed basilar invagination, fusion of midbrain colli

Current challenges in intrauterine management of obstructive fetal hydrocephalus and future directions.

Aqueductal stenosis is a specific type of non- communicating hydrocephalus, which is characterized by narrowing or constriction of the cerebral aqueduct, also known as the aqueduct of Sylvius. Due to the blockage, the flow of cerebrospinal fluid (CSF) through the aqueduct becomes restricted or obstructed, leading to an abnormal accumulation of CSF within the ventricles and increased intracranial pressure. Progressive expansion of the ventricles leads to increasing cerebral mantle compression during fetal brain development, which can lead to irreversible damage with deleterious consequences. Ea

Communicating Hydrocephalus After Radiotherapy for a Vestibular Schwannoma.

Hydrocephalus is a known tumour complication associated with vestibular schwannomas, which can be either the obstructive or communicating (non-obstructive) type. The latter represents a rare and controversial complication following radiotherapy, typically resulting from tumour necrosis. Management generally involves cerebrospinal fluid diversion. Here, we present a case of normal-pressure and communicating hydrocephalus that developed after radiotherapy for a vestibular schwannoma. The patient was a 72-year-old woman who initially presented with imbalance and right sensorineural hearing loss.

Subacute Progression of Gait Disturbance and Consciousness Impairment Due to Communicating Hydrocephalus Associated With Vestibular Schwannoma.

Patients with vestibular schwannomas (VSs) present with vestibulocochlear nerve dysfunction such as vertigo and tinnitus. VSs occasionally develop communicating hydrocephalus as a complication, which is typically characterized by an insidious progression of symptoms. We report a case of an 84-year-old female patient with a VS who developed gait disturbance and consciousness impairment over a three-week period, ultimately resulting in an inability to walk and communicate. A thorough evaluation ruled out encephalitis and other differential diagnoses. Imaging studies demonstrated findings consist

Pediatric primary intracranial Ewing's sarcoma involving cavernous sinus: an untamed evil.

Pediatric cavernous sinus Ewing's sarcoma (CS-ES) is an exceptionally rare and aggressive form of primary intracranial Ewing's sarcoma. Primary intracranial ES itself accounts for less than 1% of all childhood ES, with ES representing less than 1% of all childhood malignancies. CS-ES poses significant diagnostic and therapeutic challenges, with only seven cases reported in the literature and a mere three in pediatric patients. We present a case of a 2-year-old boy with CS-ES, whose clinical course was marked by right orbital apex syndrome, severe headaches, and rapid neurological deterioration

Relationships Between Lateral Ventricle Size, Cerebrospinal Fluid Dynamics, and Aqueductal Resistance in Young Healthy Adults.

BACKGROUND: Ventricular enlargement and abnormal cerebrospinal fluid (CSF) circulation are closely associated in communicating hydrocephalus (NPH), yet their causal relationship remains unclear. Studying healthy populations may help clarify these mechanisms. Existing metrics for CSF dynamics and ventricular morphology are limited by physiological variability such as heart rate and brain size, and aqueductal resistance has been little studied in healthy cohorts. PURPOSE: To quantify aqueductal resistance and two ratio-based indices-the CSF stroke volume ratio of the aqueduct to the cervical re

Non-invasive intracranial pressure monitoring in management of slit ventricle syndrome-case report.

Slit ventricle syndrome (SVS) is a condition of severe headaches, usually intermittent, suffered by patients after ventricular shunting for hydrocephalus with normal or smaller ventricle size; although in some cases, the correct diagnosis and management can be a significant challenge. This case report aims to highlight the application of non-invasive ICP monitoring (nICP) technology in the diagnosis and management of SVS, providing insights into its benefits and limitations. A 6-year-old boy with a history of ventriculoperitoneal (VP) shunt placement at 11 months of age for communicating hydro

Hydrocephalus in snake bite: a rare manifestation with review of literature.

BACKGROUND: Snake bites are pretty common in the rural farming population of India. Approximately 300 species of snakes are considered medically significant by the WHO. Snake bites can be venomous or non-venomous. The toxins present in the saliva of the snake are called venom, which can cause detrimental effects on the human body, the reason being multiple enzymes and molecules that act on various tissues of the human body. The toxin can be neurotoxins or hematotoxins, or it can also have a mixed effect as well. Intracranial manifestations due to snake bites are not quite common. They may have

First Clinical Evaluation of a Modular Lumboperitoneal Shunt System With a Gravitational Unit for Communicating Hydrocephalus: A Retrospective Case Series.

BACKGROUND AND OBJECTIVES: In Europe and North America, communicating hydrocephalus (HC) is commonly treated with ventriculoperitoneal shunts, whereas lumboperitoneal shunts (LPS) are preferred in Japan and other Asian countries. However, no LPS valve with an integrated gravitational unit had been available until recently, leaving overdrainage as a persistent complication. The objective of this study was to report initial clinical experiences with a novel modular LPS valve system that incorporates a gravitational unit and to assess its safety, feasibility, and early clinical outcomes across va

Risk Factors for Ventriculoperitoneal Shunt Infection: A Systematic Review and Meta-Analysis.

Background: Ventriculoperitoneal shunting (VPS) is the mainstay of treatment for most forms of hydrocephalus; VPS infection (VPSI) is a leading cause of shunt-related morbidity and mortality. A meta-analysis of the existing literature on risk factors for VPSI is currently lacking. Herein, the authors performed a systematic review and meta-analysis to evaluate the role of different clinical factors in the development of VPSI. Methods: A systematic search in the PubMed, Scopus, and Cochrane databases was performed to identify studies comparing patients developing VPSI to controls. The following
