Obstructive hydrocephalus secondary to brainstem sagging in recurrent cerebrospinal fluid-venous fistulas: a diagnostic and therapeutic challenge. Illustrative case.

BACKGROUND: The association of idiopathic intracranial hypertension (IIH) with spontaneous skull base CSF leaks is well known; however, its role in spinal CSF leaks is poorly established. CSF-venous fistula (CVFs) are a recognized subtype of spinal CSF leaks, but they rarely recur after successful treatment. Brainstem sagging is a known consequence of spontaneous intracranial hypotension with variable clinical sequelae. Brainstem sagging has not previously been demonstrated to cause obstructive hydrocephalus. OBSERVATIONS: The authors describe the case of a female in her early 5th decade with

Anatomical Surgical Trajectory for Endoscopic Transcortical Third Ventriculostomy: A Neuronavigation-Guided Cadaveric Study With Pediatric Clinical Correlation.

BACKGROUND AND OBJECTIVES: Endoscopic third ventriculostomy (ETV) is a widely used surgical treatment for obstructive hydrocephalus. While the anatomical landmarks of the third ventricular floor are well described, the organization of white matter (WM) tracts along the transcortical ETV corridor and their correlation with neuronavigation and endoscopy remain insufficiently clarified. This study aimed to anatomically map the transcortical ETV trajectory from cortical entry to the third ventricular floor using stepwise cadaveric dissections integrated with neuronavigation and clinical correlatio

Radiologic Characteristics and Clinical Correlates of Quadrigeminal Cistern Arachnoid Cysts: A Retrospective Study.

Introduction Quadrigeminal cistern arachnoid cysts (QACs) represent a rare subgroup of arachnoid cysts. Patients may present with headache, visual disturbances, gait imbalance, or nausea, typically secondary to obstructive hydrocephalus. Hydrocephalus is frequently assessed using the Evans Index (EI). Vertebral artery hypoplasia (VAH) is generally defined as a decrease in the diameter of the vertebral artery. It has been associated with posterior circulation ischemia, vertebrobasilar insufficiency (VBI), vertigo, and headache. However, the relationship between QAC-related mass effect and vascu

Strategy of adult thalamic glioma surgery: Thoughts and practices based on thalamic glioma classification.

Adult thalamic gliomas (ATGs) present a significant surgical challenge due to their deep location and proximity to critical brain structures. To standardize the surgical approaches, we proposed a novel anatomically-guided classification that divides thalamic gliomas into four distinct zones, each associated with specific surgical approaches. This retrospective study included 201 patients (median age 40 years) with WHO grade 2–4 thalamic gliomas treated at West China Hospital from 2012 to 2024. Utilizing two vertical lines transecting the internal capsule genu and the third ventricle midpoint,

Pediatric craniopharyngioma: when hypoglycemia reveals a brain tumor. Illustrative case.

BACKGROUND: Craniopharyngiomas (CPs) usually manifest with symptoms of intracranial hypertension secondary to obstructive hydrocephalus, visual disturbances, or growth failure. Hypoglycemia as the sentinel presentation is exceptionally rare and reflects hypothalamic-pituitary dysfunction. OBSERVATIONS: A previously healthy 4-year-old girl presented with progressive headache and vomiting for 4 days, followed by coma (Glasgow Coma Scale score 6). Emergency testing revealed profound hypoglycemia (capillary glucose 40 mg/dL [2.2 mmol/L]); consciousness returned rapidly after an intravenous dextro

Evaluation of an artificial intelligence model for the identification of obstructive hydrocephalus on computed tomography of the head.

OBJECTIVE: Obstructive hydrocephalus is a critical radiographic finding requiring emergent treatment. Its identification on head CT by an AI model could facilitate sooner life-saving interventions, although there are common co-occurring findings, including intracranial hemorrhage, that can confound this interpretation. This external validation assessed the accuracy of an AI model at identifying obstructive hydrocephalus, including in the presence or absence of other findings. MATERIALS AND METHODS: This retrospective cohort included 177 thin (≤ 1.5 mm) series and 194 thick (> 1.5 and ≤ 5 mm)

Case Report: Primary Fourth Ventricle Outlet Obstruction in Childhood - New Clinical Insights and Long-Term Outcome.

INTRODUCTION: Primary fourth ventricle outlet obstruction (PFVOO) is a rare cause of obstructive hydrocephalus in children. Diagnosis often requires high clinical suspicion and advanced magnetic resonance imaging (MRI) sequences, and the optimal management remains a matter of debate. CASE PRESENTATION: We report a 3-year-old girl with unexplained tetraventricular hydrocephalus who presented with progressive macrocephaly, intermittent headaches, and atypical stereotyped head movements. She had previously undergone ventriculoperitoneal shunting in infancy, which failed after 18 months. High-res

Ventriculo-gallbladder shunt reflux: A case study and review of the literature.

BACKGROUND: Ventriculo-gallbladder (VGB) shunt represents an uncommon yet well-documented surgical intervention for the treatment of hydrocephalus through cerebrospinal fluid (CSF) diversion into the gastrointestinal tract through the gallbladder. When other ventricular shunt types have failed or are contraindicated, VGB shunts are effective. In exceptionally rare circumstances, biliary reflux can occur and cause accumulation of bile in the ventricular system. The authors report a case of bile reflux through a ventricular gallbladder shunt with eventual patient recovery after surgical interven

Occult cerebral hemorrhage induced by high-dose oral methanol poisoning: a case report.

BACKGROUND: Acute methanol intoxication is life-threatening and typically presents with severe metabolic acidosis and central nervous system injury. Intracranial hemorrhage associated with methanol poisoning is uncommon and may be clinically occult, potentially delaying diagnosis and intervention. CASE: We report a case of high-dose oral methanol poisoning presenting with profound metabolic acidosis and coma. Emergent cranial CT revealed basal ganglia hemorrhage. The patient underwent intermittent hemodialysis and received supportive pharmacotherapy. Owing to obstructive hydrocephalus and neu

Bilateral Blindness in a Patient With Vestibular Schwannoma and Empty Sella Syndrome: A Case Report.

We report a rare clinical presentation of a T4b vestibular schwannoma causing severe compression of the brainstem and fourth ventricle, associated with obstructive hydrocephalus and empty sella syndrome, in a patient presenting with profound neurological and ophthalmological deficits, most notably complete bilateral blindness. Radical microsurgical resection was achieved via a lateral suboccipital (retromastoid) approach, resulting in gross-total tumor removal with preservation of most cranial nerves. Postoperatively, the patient developed House-Brackmann grade III facial palsy. Hydrocephalus

Isolated primary craniopharyngioma of the fourth ventricle: a rare case report and literature update.

BACKGROUND: Craniopharyngiomas usually occur in the saddle and suprasellar regions, and primary cases occurring in the fourth ventricle are extremely rare, with only eight definite cases reported in the English literature. In this article, we present the ninth case of primary craniopharyngioma of the fourth ventricle. CASE DESCRIPTION: The patient was a 51-year-old man who complained of progressive occipital headache, unsteady gait and vomiting. Imaging showed a cystic occupying lesion in the fourth ventricle with calcification of the cystic wall, which compressed the aqueduct and led to obst

From Balloon to Brain Ballooning: A Case of Obstructive Hydrocephalus in a Child.

BACKGROUND: Intraventricular simple cysts, often termed intraventricular arachnoid cysts, are rare benign intracranial lesions in children and are usually asymptomatic. When they become symptomatic, the presentation is typically related to obstructive hydrocephalus or seizures. Clear clinical triggers for abrupt decompensation are not well defined. OBSERVATIONS: A previously healthy 6.9-year-old boy presented in stupor with acute obstructive hydrocephalus. The only notable antecedent event was repetitive, forceful balloon inflation the night before, followed by early-morning headache and vomi

Obsessive-compulsive disorder onset and clinical course in the context and treatment of pineal region germinoma and obstructive hydrocephalus: a case report.

Obsessive-compulsive disorder (OCD) secondary to structural brain pathology is rarely described in pediatric populations. This single-case report describes a child with a pineal germinoma and obstructive hydrocephalus who developed prominent obsessive-compulsive symptoms, highlighting a potentially reversible neurobiological contributor to atypical pediatric OCD. The case illustrates the temporal association between symptom evolution, neurosurgical and oncologic management, and subsequent changes in obsessive-compulsive behaviors, contributing to understanding the role of cortico-striato-thala

Investigating Prognostic Factors for Primary Endoscopic Third Ventriculostomy in Adults and Elderly Patients with Noncommunicating Hydrocephalus.

BACKGROUND: Endoscopic third ventriculostomy (ETV) is a well-established treatment for obstructive hydrocephalus, offering durable cerebrospinal fluid diversion without the long-term risks of shunt dependency. While prognostic factors such as patient age, etiology, ETV Success Score (ETV-SS), and radiological signs of third ventricle bowing are validated in pediatric cohorts, their predictive value in adults and elderly patients remains unclear. METHODS: Adult patients diagnosed with obstructive hydrocephalus and consecutively treated with primary ETV were retrospectively evaluated. The prima

Dolichoectasia of Anterior and Posterior Circulation With Thrombosed Saccular Aneurysm of Vertebral Artery Presenting as Obstructive Hydrocephalus: A Case Report.

Vertebrobasilar dolichoectasia (VBD) is characterized by vascular elongation, widening, and tortuosity of cerebral vasculature. There are only a few reported cases of VBD as a cause of hydrocephalus. Here, we report a case in a 55-year-old male who presented with sudden vomiting for a day and worsening of headache for two weeks. He was diagnosed with CT imaging with angiographic findings: thrombosed saccular aneurysm of the vertebral artery along with dolichoectasia of the anterior and posterior circulation.

CT ventriculographic identification and neuroendoscopic treatment of acquired hydrocephalus due to cerebral aqueduct lesions: a two-case report.

Hydrocephalus secondary to cerebral aqueduct lesions poses significant diagnostic and therapeutic challenges, particularly when causative lesions are occult on conventional neuroimaging. We present two pediatric cases of acquired obstructive hydrocephalus, in which routine laboratory tests were unremarkable except for mildly elevated tumor markers. Both patients exhibited acute symptoms of intracranial hypertension. Magnetic resonance imaging (MRI) demonstrated supratentorial hydrocephalus with periventricular edema but failed to visualize a definitive obstructive lesion. Subsequent cerebral v

Endoscopic Biopsy of Intra- and Paraventricular Brain Lesions: Practical Advantages and Clinical Experience.

Background and Objectives: Endoscopic biopsy of brain lesions plays an important role in the management of intra- and periventricular lesions. While the diagnostic yield of this technique has been reported with varying success across studies, its outcome is likely influenced by specific technical nuances of the procedure. However, the relationship between these technical factors and diagnostic accuracy remains understudied in the current literature. We aim to describe the procedural rationale, key anatomical considerations, and technical nuances of the endoscopic biopsy of intra- and paraventr

The pattern and outcome of infants and neonates undergoing endoscopic third ventriculostomy in Tanzania: an observational cohort study.

OBJECTIVE: Hydrocephalus in infants is a substantial public health burden in East Africa. In sub-Saharan Africa, hydrocephalus is estimated to affect more than 100,000 new infants annually. Endoscopic third ventriculostomy (ETV) is considered a safe procedure and is a method of choice for the treatment of obstructive hydrocephalus. ETV outcomes in children older than 2 years are abundant; however, minimal outcomes data for infants and neonates younger than 2 months are available in the literature. The authors therefore sought to evaluate the patterns of presentation, endoscopic anatomy, and cl

Multilayer dural repair using absorbable artificial dura in the posterior fossa: A case report on the "sandwich technique".

BACKGROUND: Achieving watertight dural closure in posterior fossa surgery is often challenging due to the anatomical complexity of this region, particularly when suturing is not feasible near critical venous structures. These circumstances increase the risk of cerebrospinal fluid (CSF) leakage, a well-recognized complication. Traditional single-layer techniques may be insufficient to achieve a watertight seal. To address this limitation, we developed a novel multilayer duraplasty method, termed the "Sandwich Technique," which combines an absorbable collagen matrix with autologous fascia. METH
