Evaluation on the efficacy and safety of CyberKnife stereotactic radiotherapy for brainstem gliomas.

BACKGROUND: Brainstem glioma (BSG) is a rare neoplasm characterized by short survival and high mortality. Thus far, treatment efficacy for this tumor has remained limited, highlighting the critical demand for novel and effective therapeutic strategies. The utilization of CyberKnife Stereotactic Radiation Therapy(CyberKnife-SRT) for BSG may render favorable outcomes. Nevertheless, robust clinical data remain limited at present. Therefore, this study was designed to assess the efficacy and safety of CyberKnife-SRT in the treatment of BSG. METHODS: A retrospective analysis was conducted using cl

Metastatic rosette-forming glioneuronal tumour presenting as isolated primary amenorrhoea.

Primary amenorrhoea is defined as the absence of menarche by 15 years of age in individuals with normal secondary sexual characteristics and is most commonly due to constitutional delay, gonadal dysgenesis or outflow tract abnormalities. We describe an adolescent female who presented with isolated primary amenorrhoea despite normal pubertal onset and progression and no neurological symptoms. Biochemical evaluation demonstrated a central hypothalamic-pituitary pattern of hypogonadism, prompting neuroimaging. MRI revealed a large intraventricular mass causing obstructive hydrocephalus with compr

Endoscopic Third Ventriculostomy for Hydrocephalus with Interhypothalamic Adhesion: A Case Report and Literature Review.

Endoscopic third ventriculostomy (ETV) is a well-established treatment for obstructive hydrocephalus; however, anatomical variations, such as the interhypothalamic adhesion (IHA), may obscure critical third ventricular landmarks and complicate intraoperative orientation. IHA is a rare congenital band of neural tissue connecting the medial hypothalamic walls and may pose a technical challenge during ventricular endoscopy. A case of obstructive hydrocephalus in which IHA was encountered during ETV is presented, and a structured surgical approach to safely perform ventriculostomy without disrupti

Prolonged survival in pediatric diffuse intrinsic pontine glioma following intensity-modulated radiation therapy: a case report.

BACKGROUND: The aggressive WHO grade IV brainstem tumors known as diffuse intrinsic pontine gliomas (DIPGs) primarily strike children between the ages of 5 and 10. Because they are incurable, their median survival is less than a year. While traditional radiation provides short-term discomfort alleviation, more sophisticated methods like as intensity-modulated radiation therapy (IMRT) have demonstrated potential for enhancing tumor targeting and lowering treatment-related side effects. CASE PRESENTATION: A 9-year-old Indian male with persistent headaches and vomiting was diagnosed with diffuse

Rates of Comorbid Psychiatric Conditions Among Adults With Hydrocephalus at a Hydrocephalus Specialty Center.

OBJECTIVE: The relationship between hydrocephalus and comorbid psychiatric conditions among adults remains unclear. The authors characterized the prevalence of psychiatric conditions among adult patients with hydrocephalus and compared the rates of these conditions among hydrocephalus subtypes. METHODS: A retrospective study of patients ages ≥18 years from a tertiary hydrocephalus specialty center was conducted by reviewing electronic health record data. Participants (N=2,668) were required to have an ICD-10 diagnosis of hydrocephalus (G91) or congenital hydrocephalus (Q03) in their active pr

Case Report: Unclassifiable cerebellar high-grade neuroepithelial tumor with a CCDC6::RET fusion manifesting explosive recurrence.

A 20-year-old woman presented with a 1-month history of positional vertigo, occipital headaches, and progressive gait ataxia. Neuroimaging demonstrated a 3.7 × 4.2 × 3.3 cm heterogeneously enhancing mass in the left cerebellar hemisphere with fourth-ventricle compression, approximately 8 mm tonsillar herniation, and obstructive hydrocephalus. Urgent resection was performed through a midline suboccipital approach with neuronavigation and fluorescein guidance, achieving gross total removal. Histopathology showed an undifferentiated high-grade neuroepithelial malignancy with brisk mitotic activit

Hyponatremia Following Endoscopic Third Ventriculostomy in an Adolescent with an Aqueductal Web: A Case Report.

Background and Clinical Significance: Endoscopic third ventriculostomy (ETV) is a well-established cerebrospinal fluid (CSF) diversion technique for treating obstructive hydrocephalus. Here, the complication of post-ETV hyponatremia is rare. Separately, aqueductal web as a cause of obstructive hydrocephalus is also an uncommon occurrence. We present an unusual case of an adolescent who presented with late symptoms of obstructive hydrocephalus secondary to an aqueductal web and developed a delayed onset of post-operative hyponatremia after a successful ETV procedure. Pertinent aspects of the ca

An Ultra-late Recurrence with Adenoid Cystic Carcinoma-like Malignant Transformation of a Pineal Immature Teratoma after 35 Years: A Case Report.

Immature teratomas of the pineal region are a subtype of non-germinomatous germ cell tumors typically associated with early recurrence. Ultra-late recurrence decades after initial treatment is exceedingly uncommon. We report an immature teratoma case in the pineal region that recurred 35 years after subtotal resection and chemoradiotherapy, showing somatic-type malignant transformation into adenoid cystic carcinoma-like adenocarcinoma. A 16-year-old boy initially underwent ventriculoperitoneal shunting followed by tumor resection, after the pathological confirmation of an immature teratoma (gr

Cognitive decline and dementia outcomes among patients with obstructive hydrocephalus undergoing endoscopic third ventriculostomy versus shunting: A national retrospective analysis.

INTRODUCTION: Hydrocephalus is associated with cognitive morbidity that can improve after cerebrospinal fluid (CSF)-diversion however, there remains a paucity of data comparing these outcomes following endoscopic third ventriculostomy (ETV) versus CSF-diverting shunt. METHODS: This is a retrospective analysis of the 2016-2025 TriNetX database. Patients > 12 years old with newly diagnosed obstructive hydrocephalus undergoing primary ETV or shunting were identified. The primary outcome was cognitive decline or dementia at 5 years post-procedure and evaluated using 1:1 propensity score matching

PITCH (pediatric infratentorial tumors - hydrocephalus-related complications) registry study design: observational, prospective, multicenter study evaluating the number of surgeries associated with the treatment of hydrocephalus secondary to infratentorial tumors in childhood and adolescence.

INTRODUCTION: Posterior fossa tumors are the most common solid pediatric neoplasms, and more than 60% of these tumors are associated with hydrocephalus, which can be managed using different strategies, including endoscopic third ventriculostomy (ETV), ventriculoperitoneal shunt (VPS), external ventricular drainage (EVD), or direct tumor resection without CSF diversion. The safest and most effective drainage method remains controversial, and most available studies are limited to single-center retrospective analyses, often constrained by small sample sizes. Therefore, multicenter prospective stu

Central nervous system Burkitt lymphoma: fornix-to-ventricle dissemination.

Central nervous system Burkitt lymphoma (BL) is exceptionally rare in adolescents without systemic disease. We report a 16-year-old female presenting with progressive headaches and isolated fornix lesions on MRI. Following an initial presumptive diagnosis of viral encephalitis, empiric administration of low-dose dexamethasone and antivirals induced transient symptomatic improvement. A follow-up MRI 50 days later revealed a fulminant "spark-to-wildfire" imaging progression, characterized by multifocal ventricular dissemination and obstructive hydrocephalus. Obstructive hydrocephalus masquerading as impending eclampsia in a 37-week pregnant woman: a case report.

BACKGROUND: In preeclampsia, the neurologic symptoms, especially a lingering headache, can sometimes mimic more severe intracranial conditions, pushing doctors to quickly consider early delivery. Obstructive hydrocephalus during pregnancy is an uncommon but tricky diagnosis, one that could easily be missed when hypertension and proteinuria are already present, potentially delaying the identification of a neurosurgical emergency. CASE PRESENTATION: A 25-year-old first-time mother, 37 weeks and 1 day pregnant, presented with a month-long history of progressively worsening bilateral lower extrem

Extensive Pediatric Rathke's Cleft Cyst with Suprasellar, Intraventricular, and Cerebellopontine Angle Invasion: A Case Report with Long-Term Outcome.

Rathke's cleft cysts (RCCs) are benign, sellar, or suprasellar lesions that originate from remnants of Rathke's pouch. While often asymptomatic and detected incidentally, symptomatic RCCs may cause headaches, visual disturbances, and endocrine dysfunction due to mass effect. Pediatric RCCs are rare and often present distinct challenges. A 12-year-old male presented with a 4-month history of imbalance and morning headaches. Imaging revealed a large cystic lesion with suprasellar, intraventricular, and cerebellopontine angle extension, along with obstructive hydrocephalus. The patient underwent

Imaging of Pineal Region Masses.

This chapter provides a literature review of the role of imaging in the diagnosis and management of pineal tumors and cysts and other masses of the pineal region. Several types of lesions are illustrated. Imaging plays a pivotal role in the diagnostic workup, surgical planning, and follow-up. Computerized tomography plays a role in the emergency setting, owing to its wide availability. It is able to detect obstructive hydrocephalus, mass calcification, and hemorrhage. Magnetic resonance imaging is the modality of choice for tumor characterization and pretherapeutic planning. Pineal cysts are c

Case report of a rare TCF3::BEND2-fused primary intracranial neuroepithelial neoplasm in a female child.

Astroblastoma, MN1-altered, is a rare circumscribed glial neoplasm defined by alterations in the MN1 gene and often the MN1::BEND2 fusion. However, it is noted that a subset of these tumors lacks the classic astroblastic perivascular pseudorosettes, which can complicate diagnosis. We report a diagnostically challenging case in a 6-year-old female presenting with gait disturbance and limb weakness. MRI identified a well-defined fourth ventricular roof mass causing obstructive hydrocephalus. Histology post-resection revealed a solid tumor composed of epithelioid cells arranged in sheets and nest

Tectal glioma with pilocytic astrocytoma morphology carrying a histone H3 K27M mutation and malignant transformation: a literature review.

We report a case with pilocytic astrocytoma (PA) morphology carrying a histone H3 K27M mutation (H3-K27M) that progressed to PA with anaplasia. A 34-year-old woman presented with headache due to obstructive hydrocephalus from a tectal lesion. She underwent subtotal tumor resection. Histology showed a biphasic architecture with piloid cells, consistent with PA. Five months later, she developed recurrence. A second, partial resection was performed, with pathology again indicating PA. Despite subsequent radiation and chemotherapy, the tumor progressed. A third resection was done 15 months after t

Chordoid glioma of the third ventricle: case report of the first successful surgical treatment in Vietnam.

INTRODUCTION AND IMPORTANCE: Chordoid glioma is a rare, slow-growing glial tumor that most commonly arises from the anterior third ventricle or suprasellar region. Due to its rarity, deep location, and non-specific clinical and radiological features, preoperative diagnosis and surgical resection remain challenging. CASE PRESENTATION: We report the case of a 57-year-old woman who presented with a 1-month history of progressive headache and blurred vision. Magnetic resonance imaging demonstrated a well-circumscribed, homogeneously enhancing mass in the suprasellar region with upward extension t

Primary oculomotor nerve malignant peripheral nerve sheath tumor in an infant: reflex-mediated biopsy failure and tubular port-assisted diagnostic escalation.

BACKGROUND: Intracranial malignant peripheral nerve sheath tumors (MPNSTs) are exceptionally rare, and primary origin from cranial nerves is particularly uncommon. Involvement of the oculomotor nerve represents an extraordinarily rare entity. Their deep mesencephalic location, nonspecific imaging characteristics, and proximity to critical neurovascular structures pose major diagnostic challenges. In addition, surgical manipulation near the third cranial nerve may trigger trigeminocardiac or oculocardiac reflex (TCR/OCR), resulting in severe hemodynamic instability and potentially limiting mini
