From the past to the present: evolving theories in the pathophysiology of normal pressure hydrocephalus.

Over recent decades, various hypotheses and theoretical frameworks have been advanced to elucidate the aetiology of normal pressure hydrocephalus (NPH). This reversible neurological condition, characterised by the classical clinical triad of gait disturbance, urinary incontinence and cognitive impairment, represents a multifactorial interplay of pathophysiological processes that co-occur, rather than originating from a single, defined cause. Despite extensive research efforts, the precise aetiology and underlying pathophysiological pathways remain indeterminate. Contributory factors such as dy

Sex Disparities in Normal Pressure Hydrocephalus Diagnosis and Urinary Outcomes.

IMPORTANCE: Idiopathic normal pressure hydrocephalus (iNPH) is characterized by dementia, gait disturbance, and urinary incontinence (UI). While gait and cognitive impairments trigger a focused workup, UI is often excused by female patients and doctors as normal aging in women. OBJECTIVES: The objectives of this study were to evaluate sex differences in time from UI documentation to iNPH diagnosis and describe differences in postoperative UI outcomes following ventriculoperitoneal shunt placement. STUDY DESIGN: This was a single-center, retrospective study of patients treated for iNPH who

Drebrin is a novel biomarker of cognitive deterioration in Alzheimer's disease.

BackgroundThe cerebrospinal fluid (CSF) and plasma amyloid-β (Aβ)40/42 ratio, p-217tau and p-181tau, and neurofilament light chain are biomarkers of Aβ proteinopathy, tau proteinopathy, and neuronal injury, respectively, in Alzheimer's disease (AD). However, direct biomarkers of cognitive function have yet to be identified.ObjectiveTherefore, the present study investigated the potential of CSF and plasma levels of drebrin, a postsynaptic protein, as biomarkers of synaptic activity and cognitive function in the human brain in clinical settings.MethodsWe developed a novel ELISA to measure CSF an

Inflammatory protein profiles and shunt response in iNPH.

INTRODUCTION: Neuroinflammation in the context of idiopathic normal pressure hydrocephalus (iNPH) is poorly studied. Currently, no single objective test can reliably predict outcomes after shunt surgery. The aim was to investigate whether neuroinflammatory proteins in cerebrospinal fluid (CSF) are associated with the characteristic symptoms of iNPH and whether they can predict outcome after shunting. METHODS: Neuroinflammatory proteins were analyzed from preoperative CSF using proximity extension assay (PEA). In total, 92 proteins were analyzed from 74 patients with iNPH referred to shunt surg

Management of Low/Negative Pressure Hydrocephalus: A Systematic Review of Etiology, Pathophysiology, and Treatment.

Negative- and low-pressure hydrocephalus is an uncommon and frequently misdiagnosed condition lacking standardized diagnostic and management protocols. We conducted a systematic review of published studies from 1980 to 2024 to synthesize evidence on etiology, pathophysiology, and treatment. Thirty studies reporting 215 patients were identified, with antecedents including traumatic brain injury, aneurysmal subarachnoid hemorrhage, normal-pressure hydrocephalus, postoperative cases, and spontaneous presentations. Four interacting models were commonly described: viscoelastic brain change with red

Parkinsonism in Idiopathic Normal Pressure Hydrocephalus or Hydrocephalic Presentation in Parkinson's Disease: A Report of Three Cases.

Idiopathic normal pressure hydrocephalus (iNPH) occasionally presents with parkinsonism, in addition to its classic triad of symptoms. However, it remains unclear whether such parkinsonism reflects an inherent manifestation of iNPH or comorbid Parkinson's disease, and whether it is levodopa-responsive. We report three patients exhibiting both hydrocephalus and parkinsonism. Two improved with both levodopa and shunt surgery; one improved only with levodopa and not with spinal tap. In one dual responder, dopamine transporter uptake improved postoperatively, and motor symptoms remained stable des

Electroconvulsive Therapy Ameliorates Neuropsychiatric Symptoms in an Older Adult Patient With Nonshunted Normal Pressure Hydrocephalus.

We report an 80-year-old male with nonshunted normal pressure hydrocephalus (NPH) presenting with an episode of recurrent major depressive disorder who underwent electroconvulsive therapy (ECT) with good effect and consequent amelioration of neuropsychiatric symptoms. NPH had been diagnosed several years prior, although a shunt was not pursued. On admission, it proved initially challenging to separate a mood disorder from behavioral and psychological symptoms of dementia (BPSD), as is the case for many patients with NPH. Following the diagnosis of recurrent major depressive disorder, the patie

Assessment of patients affected by INPH after external lumbar drainage: what is the better timing?

BACKGROUND: Idiopathic normal pressure hydrocephalus (iNPH) is a potentially reversible neurological condition characterized by cerebrospinal fluid (CSF) accumulation contributing to clinical symptoms. Removal of excess CSF can lead to improvement, and among the available approaches, external lumbar drainage (ELD) offers superior diagnostic accuracy. However, the optimal timing for patient evaluation following ELD remains unclear. METHODS: We recruited 54 adult iNPH patients, assessed at baseline (T0), 48 hours (T1), and 10 days (T2) after ELD. Balance performance was measured using clinical s

Optimizing Initial Shunt Pressure in Idiopathic Normal Pressure Hydrocephalus.

BACKGROUND: Although setting a low initial opening pressure in a programmable valve with a gravitational unit seems to be the gold standard treatment for idiopathic normal pressure hydrocephalus (iNPH) patients, there are no studies in the literature describing the optimal initial low-pressure setting. Here, we sought to describe the optimal initial opening pressure setting for iNPH in terms of both efficacy and safety. MATERIALS AND METHODS: Patients with probable iNPH underwent shunt surgery with a programmable valve with a gravitational unit, using either an opening pressure of 3 or 6.5 cm

Association between cerebrospinal fluid volume and frailty in community-dwelling older adults: a cross-sectional study.

BACKGROUND: Impaired cerebrospinal fluid (CSF) dynamics may affect brain health in older adults and contribute to age-related changes in brain structure. Disproportionately enlarged subarachnoid-space hydrocephalus (DESH) is a neuroimaging finding associated with impaired CSF dynamics. However, the association between frailty-a condition characterised by increased vulnerability in late life-and DESH-related CSF space volumes remains poorly understood. Therefore, in this study, we aimed to investigate this association. METHODS: This cross-sectional study was conducted using data from 1,395 com

Excessive daytime sleepiness in patients with idiopathic normal pressure hydrocephalus: A controlled cross-sectional study.

BACKGROUND: Idiopathic normal pressure hydrocephalus (iNPH) is characterized by gait disturbance, cognitive impairment, and urinary incontinence, yet non-motor symptoms such as sleep disturbances remain underexplored. OBJECTIVE: To investigate subjective daytime sleepiness and sleep apnea in patients with iNPH compared with healthy controls, and to assess their association with disease severity. METHODS: In this cross-sectional study, 46 iNPH patients and 74 age- and sex-matched controls were evaluated with the Epworth Sleepiness Scale (ESS). Apnea was assessed using a semi-structured freque

Development and temporal validation of an odds ratio-weighted prognostic score (NPH-RKP) for idiopathic normal pressure hydrocephalus shunt surgery: a retrospective cohort study.

BACKGROUND: Idiopathic normal pressure hydrocephalus (iNPH) is a major cause of reversible gait and cognitive impairment in older adults, yet postoperative outcomes remain difficult to predict. Existing prognostic tools are limited by methodological shortcomings, including lack of temporal validation, dependence on subjective radiological markers, insufficient events-per-variable ratios, and absence of transparent graded risk stratification. To address these gaps, we developed the first temporally validated, odds ratio-weighted prognostic scoring system specifically designed for iNPH surgical

Neurophysiological insights into iNPH: ABR and CAEP responses to lumbar puncture and shunt surgery.

BACKGROUND: Idiopathic normal pressure hydrocephalus (iNPH) is a neurological syndrome resulting from increased cerebrospinal fluid volume without significant increase in intracranial pressure. The diagnosis is difficult and investigating other tools that can assist in this process is essential. Auditory Evoked Potentials (AEPs) can be used to assess the auditory pathway integrity and monitor cognitive auditory functions. The objective of this study was to investigate the applicability of the AEPs as a potential monitoring tool for detecting clinical improvements associated with cerebrospinal

Analysis of radiological markers in normal pressure hydrocephalus and their association with postoperative outcomes following shunt procedures.

Several radiological signs have been recognized as valuable adjuncts to clinical criteria in the diagnosis of normal pressure hydrocephalus (NPH). Certain magnetic resonance imaging (MRI) findings may also provide information on postoperative outcomes following ventricular shunting. However, the evidence remains inconclusive. We conducted a retrospective cohort study including patients from our institution's Clinical Care Center for NPH. Preoperative MRI findings-specifically Evan's Index (EI), callosal angle (CA), disproportionately enlarged subarachnoid space hydrocephalus (DESH), and perive
