OBJECTIVE: We aimed to streamline the Tap Test in terms of timing and content in order to reduce the burdensome assessment in tap-test-positive patients and optimize the protocol of CSF-TT in iNPH, and strive for building explicit connection between CSF-TT and outcome of surgery. METHODS: This retrospective cohort study enrolled a total of 69 inpatients with a positive Tap Test according to strict inclusion and exclusion criteria. Among them, 31 underwent the traditional Tap Test (T-TT, baseline-8–24 h-72 h) and 38 underwent the refined Tap Test (R-TT, baseline-24–48 h). All enrolled patients

BACKGROUND: Normal pressure hydrocephalus (NPH) is a neurological disorder in older adults, characterized by gait disturbance, urinary incontinence, and cognitive impairment, along with ventriculomegaly and normal intracranial pressure. The management of NPH often involves ventriculoperitoneal shunting (VPS), which can be programmable (P-VPS) or nonprogrammable (NP-VPS). While P-VPS offers the advantage of adjustable pressure settings, its impact on clinical outcomes and complications remains debated, particularly in resource-limited settings like Jordan.
METHOD: A retrospective review was co

BACKGROUND: Individuals with both Alzheimer’s Disease (AD) and cerebrospinal fluid (CSF) dynamics disorders, such as idiopathic normal-pressure hydrocephalus (iNPH), exhibit reduced CSF Aβ42 levels, complicating the interpretation of AD biomarkers. However, the influence of CSF dynamics on blood-based AD biomarkers remains unclear. This study investigated whether immunoprecipitation mass spectrometry (IP-MS)-based AD plasma biomarkers were associated with disproportionately enlarged subarachnoid space hydrocephalus (DESH) on MRI. METHOD: This retrospective, cross-sectional study included 509 p

BACKGROUND: Idiopathic normal pressure hydrocephalus (iNPH) overlaps clinically and pathologically with Alzheimer’s disease (AD), making differential diagnosis and prognostication challenging. The identification of reliable and minimally invasive plasma biomarkers would have major clinical implications for distinguishing iNPH from AD and for monitoring disease course and surgical outcomes. METHODS: We conducted three studies: (Study 1) cross‑sectional comparisons of plasma phosphorylated tau 217 (p‑Tau217) and amyloid-β oligomers (AβO) among healthy controls (HCs; n = 22), iNPH (n = 76), and A

OBJECTIVE: The traditional view that hydrocephalus due to obstruction in the subarachnoid space (SAS) is caused by malabsorption of CSF does not account for many experimental and clinical aspects of the disorder. Flow MRI reveals that nearly all CSF motion is pulsatile, and that hydrocephalus is linked to significant redistribution of pulsatility within the CSF pathways. Pulsatility in the cranium is governed by the cerebral windkessel system that buffers the energy of arterial pulsations via CSF to ensure smooth capillary blood flow. This study proposes a new model of intracranial thermodynam

BACKGROUND: Lumboperitoneal (LP) shunting is an established treatment for idiopathic normal pressure hydrocephalus (iNPH). However, lumbar catheter misplacement into the spinal subdural-extra-arachnoid space (SSES) can cause shunt malfunction. We describe a fluoroscopy-assisted LP shunt placement technique combined with limited intraoperative spinal myelography to facilitate catheter placement accuracy.
METHODS: Between January 2023 and June 2025, 12 consecutive patients underwent LP shunt placement using fluoroscopic guidance. Lumbar puncture and catheter insertion were performed under real-

Idiopathic normal-pressure hydrocephalus (iNPH) is a disease that can be detected early and treated promptly through neurological screening evaluations and brain imaging. Diagnosis is established based on characteristic clinical and imaging findings demonstrating not only ventricular enlargement but also tight high cortical subarachnoid spaces, along with enlargement of the Sylvian fissures and basal cisterns. Additionally, iNPH frequently coexists with neurodegenerative disorders such as Alzheimer's disease and dementia with Lewy bodies, suggesting that the iNPH brain environment is prone to

The diagnostic landscape for idiopathic normal-pressure hydrocephalus is intricate, and there is a pressing need for accurate and cost-effective methods. Because of the lack of accurate diagnostic and prognostic quantitative biomarkers, the frequent presence of comorbidities, and the limited understanding of the pathophysiology of the disorder, only a minority of patients receive disease-specific treatment. While traditional neuroimaging offers insights, its isolated diagnostic precision can be enhanced. Emerging quantitative methods analyzing cortical thickness based on standard T1-weighted b

OBJECTIVE: Hydrocephalus is a severe disorder characterized by pathological enlargement of the brain ventricles, leading to compression and deformation of brain tissue. The pathophysiological mechanisms underlying some subtypes of hydrocephalus remain poorly understood. Normal pressure hydrocephalus (NPH) continues to be a clinically significant and unresolved issue in elderly care. This study proposes a novel approach to investigate this pathology using mathematical modeling techniques.
METHODS: Using stationary multicomponent poroelasticity equations with physiological boundary conditions,

BACKGROUND AND OBJECTIVES: Glymphatic function affects brain health and could be part of the pathophysiology in idiopathic normal-pressure hydrocephalus. Elevated intracranial pressure pulsatility and increased resistance to cerebrospinal fluid (CSF) outflow (Rout) are commonly observed in idiopathic normal-pressure hydrocephalus. Whether such alterations indicate impaired glymphatic function or affect ventricle volumetrics in ordinary elderly is unknown. We investigated the associations between CSF dynamics and changes in cognitive performance, gait, and brain MRI parameters over a 10-year pe

BACKGROUND: Diagnosis and prognostication in idiopathic normal pressure hydrocephalus (iNPH) remain challenging. This study examined whether large language models (LLMs) can classify neuropsychological test (NPT) summaries for identifying iNPH and exploring postoperative shunt responsiveness.
METHODS: This retrospective study included 42 shunt-treated iNPH patients and 53 neurological controls. De-identified NPT summaries with a standardized structure across cognitive domains were evaluated using three LLMs (ChatGPT-5, Gemini 2.5 Flash, DeepSeek) under a standardized zero-shot prompt. BACKGROUND: Idiopathic normal pressure hydrocephalus (iNPH) is a neurological disorder affecting older adults for which symptoms may improve following shunting; however, the criteria for surgical referral remain unclear. While most studies rely on fixed cut-off scores for cognitive and gait tests, the present study examined decision-making based on clinical judgment to identify which factors influence referral. A secondary objective was to compare pre- and post-CSF tap test (CSF-TT) changes between the shunt and no shunt groups.
METHODS: This retrospective study included 175 patients assessed

BACKGROUND AND OBJECTIVES: Normal pressure hydrocephalus (NPH) is characterized by the classic triad of cognitive decline, gait instability, and urinary incontinence in the setting of ventriculomegaly with normal intracranial pressure. Cerebrospinal fluid diversion is the current standard treatment, yet it carries a risk of overdrainage, resulting in subdural hematoma or hygroma. Different valves have been developed to mitigate this risk, yet consensus remains unclear regarding optimal valve for NPH.
METHODS: We performed a retrospective cohort study on all patients with NPH who underwent cer
