Disorders of cerebrospinal fluid (CSF) dynamics, including idiopathic normal pressure hydrocephalus (iNPH), idiopathic intracranial hypertension (IIH), and spontaneous intracranial hypotension (SIH) frequently present clinical and imaging phenotypes that overlap with other disease processes. Although conventional MRI remains central for identifying characteristic morphologic features, many imaging findings have variable sensitivity, particularly across disease stages. As a result, current diagnostic pathways and treatment selection is still often reliant on invasive, snapshot-based physiologic

OBJECTIVE: To test the hypothesis that idiopathic normal-pressure hydrocephalus (iNPH) is also associated with reproducible whole-brain structural connectivity reductions detectable by diffusion MRI connectomics, beyond the established clinical triad of gait disturbance, cognitive impairment, and urinary incontinence.
MATERIALS AND METHODS: Twenty patients with iNPH (71-84 years; Evans index 32.1-43.5) diagnosed according to the Japanese guidelines and 20 age-matched healthy controls (70-88 years) underwent 3.0-T MRI. Distortion-corrected diffusion-weighted imaging (b = 2000s/mm2, 32 directio

BACKGROUND: Aquaporin-4 (AQP4) is crucial for brain fluid regulation and glymphatic system function. Idiopathic normal pressure hydrocephalus (INPH) is characterized by impaired CSF flow and is treated with shunt surgery. This study investigated AQP4 levels in INPH patients to explore its role in pathophysiology and as a potential biomarker for shunt response.
METHODS: CSF samples from 233 INPH patients and 29 controls were analysed. AQP4 levels were compared between preoperative patients and controls, before and after shunt surgery (110 patients), and between shunt responders and non-respond

BACKGROUND: Gait change after cerebrospinal fluid drainage (CSFD) in idiopathic normal pressure hydrocephalus (iNPH) is usually assessed during single-task walking. Dual-task gait assessment may provide additional diagnostic value, as both gait and cognition are commonly impaired in iNPH.
METHODS: We analyzed GAITRite gait data from 122 patients with suspected iNPH before and after CSFD and from 146 cognitively healthy controls assessed once. Normal walking, working memory dual-task walking, semantic memory dual-task walking, and fast walking were examined. Eight parameters were analyzed acro

Ventriculomegaly is a key neuroimaging feature of normal pressure hydrocephalus (NPH) and other disorders of cerebrospinal fluid (CSF) dynamics. The Evans Index (EI), defined as the ratio of maximal frontal horn width to maximal inner skull diameter, is a widely used marker of ventricular enlargement. However, manual EI measurement is susceptible to inter-observer variability and depends on accurate alignment to the anterior commissure-posterior commissure (AC-PC) plane, limiting reproducibility in large and multi-center studies. We developed a fully automated deep learning-based pipeline for

Background: Lumboperitoneal (LP) shunt surgery is an established treatment for idiopathic normal pressure hydrocephalus (iNPH). In Japan, patients undergoing LP shunt surgery are often hospitalized for several days to more than one week after surgery, even in uncomplicated cases, reflecting concerns regarding early complications, cerebrospinal fluid overdrainage, and discharge readiness in older adults. This study evaluated the feasibility and short-term safety of a perioperative optimization pathway for planned short-stay hospitalization after LP shunt surgery. Methods: This single-center ret

According to glymphatic system theory, cerebrospinal fluid (CSF) perfuses the brain's interstitial space to support waste clearance, but the magnitude of this flow and the outflow pathway of interstitial fluid (ISF) in humans remain uncertain. To achieve flow quantification, we applied a compartment-model approach applied in conjunction with serial quantitative MRI data acquired after intrathecal gadolinium administration. Using the method, we estimated CSF-to-ISF inflow to 45 ± 20 mL/h, in patients with suspected idiopathic normal pressure hydrocephalus. Tissue-specific contributions were 34

Compartmental compliance is classically defined as the ratio between volume change and resulting pressure change. Nevertheless, the concept of craniospinal compliance is inherently complex, as it involves different physiological mechanisms that vary according to temporal and volumetric scales. During lumbar infusion tests, the craniospinal system is stressed artificially, whereas during a single cardiac cycle, it operates under dynamic physiological conditions. In this study, we propose a novel approach for estimating arterial-related craniospinal compliance under physiological conditions by c

BACKGROUND: The perioperative management of antithrombotic (AT) therapy during lumboperitoneal shunting (LPS) for idiopathic normal pressure hydrocephalus (iNPH) remains controversial, and evidence to guide decisions regarding continuation of AT therapy is limited.
METHODS: We retrospectively analyzed patients with iNPH who underwent LPS at our institution. Patients were classified according to AT agent use on the day of surgery into a Continued AT therapy group and a Discontinued/No AT therapy group. Symptomatic hemorrhagic complications, chronic subdural hematoma (CSDH), and thromboembolic

BACKGROUND: Intrathecal contrast-enhanced magnetic resonance imaging (MRI), utilizing the contrast agent gadobutrol as cerebrospinal fluid (CSF) tracer is emerging as a useful method to study glymphatic function in the human brain. A consistent finding with this technique is large inter-individual variability regarding tracer propagation. In this study, we outline an approach which predicts the distribution of tracer in the parenchyma based only on geometric information from brain tissue as captured by MRI, addressing one possible explanation for this variability. METHODS: Registrations are co

We report a 78-year-old asymptomatic woman with no cardiac history and unremarkable cardiac evaluation, referred for a pre-surgical ECG showing diffuse T-wave inversion. Following ventriculoperitoneal shunting for chronic normal-pressure hydrocephalus (NPH), her ECG gradually normalized, suggesting a reversible neurocardiac effect. This case highlights a not well documented association between chronic NPH and T-wave inversions, possibly mediated by autonomic dysregulation, and underscores the importance of considering extracardiac neurologic disorders in the ECG differential diagnosis.

The glymphatic system facilitates cerebrospinal fluid (CSF)-interstitial fluid exchange and plays a key role in solute clearance and neurophysiological homeostasis. While dysfunction of this system has been shown in traumatic brain injury, stroke, meningitis, idiopathic normal pressure hydrocephalus and neurodegenerative diseases, direct measurement of glymphatic transport in humans remains elusive. We propose microGLYMPH as a translational, hypothesis-generating framework that combines established clinical cerebral microdialysis with controlled CSF tracer administration via existing clinical

Hydrocephalus is a complex neurological disorder traditionally thought to result from excessive cerebrospinal fluid (CSF) production, obstruction of CSF outflow, or impaired absorption. However, these classical theories have limitations in explaining key phenomena observed in certain clinical subtypes, such as normal pressure hydrocephalus. Recent advances in neuro-fluid dynamics have introduced the concept of the glymphatic system, a functional network in the brain that facilitates the exchange of CSF and interstitial fluid (ISF) along perivascular pathways and mediates the clearance of metab

INTRODUCTION: Idiopathic normal pressure hydrocephalus (iNPH) is frequently underdiagnosed due to non-specific symptoms and the risks of invasive testing. While disproportionately enlarged subarachnoid space hydrocephalus (DESH) is a hallmark imaging feature, manual assessment is subjective and qualitative. We developed the Disproportionately Elevated Sulcal Index (DESI), a fully automated deep learning-based volumetric biomarker, to objectively quantify these morphological changes.
METHODS: We trained a U-Net model with an EfficientNet-B0 encoder on T1-weighted MRI scans from the Baltimore L

Spinal canal stenosis is an age-related degenerative condition observed mainly in the cervical and lumbar regions. Recent research has suggested that such stenoses influence cerebrospinal fluid (CSF) dynamics in idiopathic normal pressure hydrocephalus (iNPH) patients by increasing spinal canal flow resistance thereby directly impacting craniospinal dynamic compliance and intracranial pressure (ICP). This study experimentally investigates the effects of cervical and lumbar spinal stenoses on CSF dynamics using a validated in vitro model. To simulate varying degrees of stenosis, controlled redu

BACKGROUND: Magnetic Resonance Elastography (MRE) of the brain provides quantitative biomarkers of mechanical properties of brain tissue. Prior studies have shown that an advanced 3D MRE can differentiate idiopathic normal pressure hydrocephalus (iNPH), from other neurodegenerative disorders. In this pilot study, we assessed whether a simplified 2D MRE protocol, that is more widely available, could detect stiffness differences between iNPH and non-iNPH cognitively impaired patients, thus providing preliminary diagnostic discrimination, while using a previously developed 3D MRE classifier as a

INTRODUCTION: Core cognitive deficits in iNPH include slowed information processing, psychomotor slowing and executive dysfunction. However, cognitive outcomes following shunt surgery are not well understood. This review synthesised evidence on cognitive assessment methods and outcomes following shunt surgery in iNPH.
METHODS: PubMed, Scopus, PsycINFO and Web of Science were searched for peer-reviewed studies including adults with iNPH who underwent shunt surgery and had within-subject cognitive evaluations pre- and post-operatively. Key data were extracted and study quality was assessed. Ran
