Relationship between intracranial pressure waveform parameters and quantitative biomarkers of Alzheimer's disease from brain tissue and cerebrospinal fluid in idiopathic normal pressure hydrocephalus.

Idiopathic normal pressure hydrocephalus (iNPH) is a complex disturbance of cerebrospinal fluid (CSF) dynamics and intracranial compliance with common overlap with Alzheimer's disease (AD). Little knowledge exists about the relationship between AD biomarker burden in brain tissue and CSF and the waveform components of intracranial pressure (ICP) homeostasis. In this study 10 patients with iNPH underwent analysis of cortical brain biopsy and CSF for amyloid-beta 40 and 42 (Aß-40, Aß-42) and phosphorylated tau (p-tau). Patients underwent prior overnight ICP monitoring with analysis of pulse pres

Safety, tolerability, and efficacy of acetazolamide in idiopathic normal pressure hydrocephalus (DRAIN) in Sweden: a randomised, double-blind, placebo-controlled, phase 2 trial.

BACKGROUND: Idiopathic normal pressure hydrocephalus is a potentially reversible cause of gait disturbance, cognitive impairment, and urinary incontinence, typically in adults aged 60 years or older. Ventriculoperitoneal shunt surgery improves symptoms but is associated with complications and reoperations, and no pharmacological treatment has been proven effective. We aimed to assess whether acetazolamide improves gait in patients with idiopathic normal pressure hydrocephalus awaiting shunt surgery. METHODS: In this investigator-initiated, randomised, double-blind, placebo-controlled, phase 2

Normal pressure hydrocephalus plus atypical presentation with symptomatic resolution following the restoration of CSF flow: illustrative case.

BACKGROUND: Normal pressure hydrocephalus (NPH) is a condition characterized by enlarged intracerebral ventricles with normal intracranial pressure. The typical clinical presentation includes cognitive decline, gait disturbances, and urinary incontinence. While the classic triad of symptoms is well documented, atypical presentations, including focal deficits and aphasia, pose significant diagnostic challenges. OBSERVATIONS: This is the case of a previously healthy 70-year-old patient presenting with rapidly progressive gait disturbances, right hemiplegia, cognitive decline, and mutism. The fo

Idiopathic normal pressure hydrocephalus: validation of the DESH score in the Capital Region of Denmark.

OBJECTIVE: Previous research has shown that the Disproportionally Enlarged Subarachnoid-space Hydrocephalus (DESH) score may be a prognostic factor for shunt surgery response for idiopathic normal pressure hydrocephalus (iNPH) patients at 1-month follow-up after shunt surgery. The objective of the current study was external validation in an independent cohort with a longer follow-up period. Secondarily, to assess whether the grading system was confounded by imaging modality. METHODS: Preoperative baseline characteristics, magnetic resonance imaging (MRI) and/or computed tomography (CT) scans

Acetazolamide in hydrocephalus management: Mechanistic insights and clinical challenges.

Hydrocephalus is a heterogeneous neurological disorder characterized by impaired cerebrospinal fluid (CSF) dynamics, resulting in ventriculomegaly, neurological dysfunction, and substantial morbidity across pediatric and adult populations. Surgical CSF diversion remains the standard treatment but risks infection, shunt malfunction, and lifelong dependency, underscoring the need for potential nonsurgical adjuncts. Acetazolamide, a carbonic anhydrase inhibitor, has been investigated as a pharmacological approach to reduce CSF production, lower intracranial pressure, and modulate cerebral blood f

Cerebrospinal fluid shunt outcomes in patients with idiopathic normal pressure hydrocephalus and an Evans index below 0.3: a prospective study.

BACKGROUND: An Evans index greater than 0.3 is a key diagnostic criterion for idiopathic normal pressure hydrocephalus (iNPH). However, some patients with iNPH symptoms and an Evans index below 0.3 achieve favorable outcomes after cerebrospinal fluid (CSF) shunt surgery. This study aimed to evaluate the clinical and radiographic characteristics and CSF shunt outcomes of these patients. METHODS: This prospective study enrolled patients presenting with iNPH symptoms and an Evans index below 0.3. Patients who showed clinical improvement after a spinal tap test underwent CSF shunt placement via v

Cytokine markers in intracerebral hemorrhage are linked to ventricular imaging and outcomes.

OBJECTIVE: Central nervous system (CNS) inflammation after intracerebral hemorrhage (ICH) is substantial, but its relationship to radiographic severity and outcome remains unclear. Cerebrospinal fluid (CSF) obtained via external ventricular drains (EVDs) offers a direct window into CNS inflammation. We hypothesized that a stronger proinflammatory CSF cytokine response would be associated with ventricular enlargement and worse functional outcome. METHODS: In this study, 56 ICH patients and 25 controls with normal pressure hydrocephalus were analyzed. CSF (days 0-16) and plasma (days 0-7) sampl

Three-dimensional kinematic gait signatures of idiopathic normal pressure hydrocephalus: a biomechanical framework toward objective diagnosis.

BACKGROUND: Idiopathic Normal Pressure Hydrocephalus (iNPH) is a leading cause of reversible gait disturbance in older adults, yet diagnosis and treatment selection remain limited by low-sensitivity clinical assessments. Conventional tests such as the 10-metre walk and Timed Up-and-Go capture overall performance but overlook the multidimensional, joint-level alterations that characterise iNPH gait. This study aimed to deliver the first comprehensive, three-dimensional (3D) biomechanical characterisation of iNPH gait to inform more objective diagnostic frameworks. METHODS: Twenty-three partici

Comparison of Albumin and Ischemia-Modified Albumin Levels in Concurrent Blood and Cerebrospinal Fluid in Patients with Spontaneous Subarachnoid Hemorrhage and Normal Pressure Hydrocephalus.

Background and Objectives: Ischemia-modified albumin (IMA) has previously been identified as a biomarker for early ischemia, rapidly formed by acidosis and free radical modification of the N-terminus of human serum albumin. This study aimed to compare albumin and IMA levels in blood and cerebrospinal fluid (CSF) from 30 patients with spontaneous subarachnoid hemorrhage (SAH) and 15 patients with normal pressure hydrocephalus (NPH) at a single center between 2021 and 2022. Materials and methods: This prospective study included 30 patients diagnosed with subarachnoid hemorrhage (SAH), confirmed

Economic analysis and healthcare implications of underdiagnosed idiopathic normal pressure hydrocephalus in Italy's aging population.

BACKGROUND: Idiopathic normal pressure hydrocephalus (iNPH) is a potentially reversible neurological condition predominantly affecting individuals over 65 years of age. Despite its treatability through cerebrospinal fluid (CSF) shunting-with clinical improvement in up to 80% of appropriately selected patients-iNPH remains substantially underdiagnosed, with only approximately 13% of affected individuals in Europe receiving appropriate care. This study evaluates the long-term epidemiological and economic burden of iNPH underdiagnosis in Italy over the decade 2024-2033, quantifying potential savi

6-minute walk test and the 3-metre backward walk test are sensitive to postoperative gait improvement in idiopathic normal pressure hydrocephalus.

BACKGROUND: Gait impairment is a core symptom in idiopathic normal pressure hydrocephalus (iNPH), but commonly used tests like the 10-metre walk test (10MWT) may underestimate postoperative improvements. We evaluated the 6-minute walk test (6MWT) and 3-metre backward walk test (3MBWT) as outcome measures compared with 10MWT and Timed Up and Go (TUG) and explored sex-related and age-related differences in shunt-operated patients with iNPH. METHODS: A retrospective study of 291 shunted patients with iNPH (62.5% male; mean age 73.3±6.2 years) was conducted. Gait function was assessed preoperativ

Gut Microbiome Differences Between Early Alzheimer Disease and Idiopathic Normal Pressure Hydrocephalus.

BACKGROUND: Alzheimer disease (AD) and idiopathic normal pressure hydrocephalus (iNPH) are neurodegenerative diseases causing memory decline. Previous studies have demonstrated an altered gut microbiome (GM) in both conditions. In this study, we compared the GM composition between the groups to find out how if the GM composition differed between the cognitively healthy individuals (CO) and AD groups, as well as between the AD and iNPH groups. METHODS: Thirty-seven CO participants, 21 mild AD patients and 10 participants with shunted iNPH gave fecal samples, which were subjected to 16S amplico

Efficacy and Safety of CSF Shunting for Idiopathic Normal Pressure Hydrocephalus: A Systematic Review and Meta-Analysis.

Idiopathic normal pressure hydrocephalus (iNPH) is a potentially reversible neurological condition characterized by gait disturbance, cognitive impairment, and urinary dysfunction. CSF shunting remains the standard treatment; however, reported efficacy and complication rates vary across studies. This systematic review and meta-analysis aimed to evaluate the effectiveness and safety of CSF shunting in patients with iNPH based exclusively on evidence from randomized controlled trials (RCTs). The review was conducted in accordance with Preferred Reporting Items for Systematic Reviews and Meta-Ana

Speed of clinical response to external lumbar drainage as a predictor of outcome of ventriculoperitoneal shunt in normal pressure hydrocephalus: A prospective cohort study.

BACKGROUND: External lumbar drainage (ELD) is definitely a known predictor of ventriculoperitoneal shunt (VPS) outcome in normal pressure hydrocephalus (NPH). However, the speed of clinical response following ELD has not been studied well enough to predict shunt outcome. This study aims to assess the predictive value of ELD response for VP shunt outcome. METHODS: A prospective observational study of 67 patients diagnosed with NPH based on clinical and radiological criteria. All patients underwent ELD, and the time to improvement in gait after ELD was calculated. Those who showed improvement w
