Independent risk factors for clinically significant acute poisoning in children presenting to the emergency department: a 4-year cohort study of 2,345 cases.

BACKGROUND: Acute poisoning is a major pediatric emergency department (ED) presentation, yet evidence-based tools for early risk stratification at triage remain limited. This study aimed to characterize epidemiological trends and identify independent risk factors for clinically significant poisoning (Poisoning Severity Score [PSS] ≥ 2) in children. METHODS: We conducted a retrospective cohort study of 2,345 children (<18 years) with acute poisoning (2021-2024). Patients were stratified into mild (PSS 0-1) and clinically significant (moderate-to-severe) groups. Multiple imputation handled miss

[Severe acute poisoning with methadone and α-pyrrolidinovalerophenone complicated by circulatory arrest].

This report presents a case of acute severe poisoning with methadone and α-pyrrolidinovalerophenone in a 41-year-old male patient complicated by circulatory arrest. The poisoning occurred following the intravenous injection of 0.5 g of methadone and intranasal administration of α-pyrrolidinovalerophenone. The case manifested as a critical circulatory arrest with a significant depression of consciousness, reaching a coma II level, alongside cytotoxic edema affecting the white matter of the brain and corpus callosum. Notably, the serum S-100 protein concentration was increased to 152.1 ng/L. The

Post-marketing safety evaluation of mirogabalin using the JADER database.

BACKGROUND: Mirogabalin, a novel third-generation α2-δ calcium channel ligand, is approved for diabetic peripheral neuropathic pain (DPNP). However, its post-marketing safety profile remains insufficiently characterized, particularly regarding novel signals and comparative safety profiles with established therapies. OBJECTIVES: This study aimed to identify and characterize adverse event (AE) signals associated with mirogabalin, evaluate demographic differences, and compare signal strength with pregabalin and gabapentin. METHODS: We conducted a pharmacovigilance analysis using the Japanese Ad

Acute bilateral vision and hearing loss following high-dose selenium ingestion: a case report.

BACKGROUND: Selenium is an essential trace element widely used in health supplements. Reported cases of selenium poisoning have mainly involved hair loss, nail changes, and liver dysfunction, whereas toxic encephalopathy is uncommon. To date, no cases of selenium poisoning causing vision or hearing loss have been reported. CASE PRESENTATION: We report a 27-year-old woman employed in health supplement research and development who was occupationally exposed to multiple selenium compounds. During product testing in April 2025, she ingested selenium-containing samples. Based on the product formul

Successful treatment of acute organophosphorus pesticide poisoning complicated by intestinal necrosis with exploratory laparotomy: a case report.

OBJECTIVES: Acute organophosphorus pesticide poisoning (AOPP) constitutes a common yet life-threatening emergency, particularly prevalent in rural regions of Asia. The condition results from inhibited cholinesterase activity, which induces acetylcholine accumulation at neural synapses. Clinically, its manifestations are categorized into muscarinic, nicotinic, and central types. Among these, mortality typically arises from respiratory paralysis, while neurological sequelae are frequently encountered. Nevertheless, reports of AOPP-induced intestinal necrosis remain exceedingly uncommon both dome

Agent-Specific Clinical Profiles of Antibiotic-Associated Encephalopathy: A Dual-Database Pharmacovigilance Study Using JADER and FAERS.

Background/Objectives: Antibiotic-associated encephalopathy (AAE) is a serious adverse reaction whose presentation is thought to differ between antibiotics; whether these agent-specific profiles are consistently reflected in spontaneous reports across independent data sources is unknown. Methods: Using the Japanese Adverse Drug Event Report (JADER) database and the US FDA Adverse Event Reporting System (FAERS, 2004-2026), we studied metronidazole, cefepime, ceftriaxone, ceftazidime, and levofloxacin. Encephalopathy was defined by the MedDRA terms Encephalopathy or Toxic encephalopathy, and rep

Familial acute necrotizing encephalopathy temporally associated with SARS-CoV-2 infection: Two case reports and literature review.

RATIONALE: Acute necrotizing encephalopathy (ANE) is a rare but life-threatening parainfectious encephalopathy characterized by rapid neurological deterioration following viral infections. Although most cases occur sporadically, familial clustering is uncommon and suggests the involvement of genetic susceptibility factors. The emergence of coronavirus disease 2019 (COVID-19) has broadened the spectrum of viral triggers associated with ANE; however, familial COVID-19-associated ANE remains rarely reported. PATIENT CONCERNS: We retrospectively reviewed 2 siblings who developed ANE following lab

Methylene blue for treating toxic encephalopathy due to acute nickel poisoning: A case report.

BACKGROUND: Acute nickel poisoning is a rare clinical entity with no established management guidelines. Diagnosis is often delayed by non-specific clinical presentations that can mimic other common toxidromes, such as organophosphate poisoning. CASE SUMMARY: A female in her late twenties presented with a Glasgow Coma Scale of 3, profound hypotension, miosis, and hypersalivation following intentional ingestion of an unlabeled toilet cleaner. Initial management for suspected cholinergic toxidrome and corrosive ingestion failed to achieve clinical stability. The patient developed refractory vaso

Expression of V‑domain Ig suppressor of T cell activation in ovarian cancer and its regulation of follicular regulatory T‑cell function via the AKT/MAPK pathway.

BACKGROUND: Patients with eating disorders (ED) often suffer from compulsive exercise behavior, which is associated with lower short-term response to treatment and poorer long-term outcome. Evidence-based interventions specifically targeting compulsive exercise behavior have been scarce so far. We developed a manualized group therapeutic approach integrating cognitive-behavioral therapy, exercise therapy and exposure with response management to promote healthy exercise behavior. Our objective was to examine the feasibility and acceptance of this new approach as add-on to regular inpatient trea

Lush Ice High: A Rare Case of Nicotine E-Liquid-Induced Toxic Encephalopathy.

E-cigarette consumption has increased globally, partly due to its perceived role as a harm-reduction alternative to conventional cigarettes. It has been linked to various neurotoxic pathologies including stroke, cognitive dysfunction, and neurodevelopmental disorders. Acute e-liquid intoxication can lead to a fatal syndrome of respiratory failure, cardiovascular instability, acute encephalopathy, and gastrointestinal distress. We present a case of toxic encephalopathy due to e-liquid intoxication directly from an e-cigarette device. Our patient is a 59-year-old man who presented to the hospita

Metronidazole-induced encephalopathy and polyneuropathy.

Though effective, metronidazole poses multiple side effects, especially with prolonged therapy or high dosage. Among these are metronidazole-induced peripheral neuropathy and encephalopathy, which are severely debilitating. We describe a middle-aged man with a liver abscess who was treated with metronidazole 2.4 g/day for nearly 2 months, and who went on to develop dysarthria, ataxic gait and sensory loss, including numbness and burning paraesthesia in his limbs. Neurological assessment comprised nerve conduction studies (NCS) and magnetic resonance imaging (MRI). NCS indicated axonal neuropat
