Cell Therapy as Metabolic Rescue after Ischemic Stroke: Rewiring Bioenergetics, Redox Homeostasis, and Neurovascular Repair.

Ischemic stroke represents a dynamic metabolic disorder of the neurovascular unit (NVU) rather than a static vascular occlusion followed by neuronal demise. Immediate oxygen and glucose deprivation rapidly deplete ATP, disrupt the transmembrane ionic gradients, increase glutamate excitotoxicity, and overload mitochondrial with calcium. These events alter glycolytic, lipid, amino acid, and redox pathways. During the subacute and chronic phases, astrocytes, microglia, macrophages, endothelial cells, pericytes, oligodendrocytes, and surviving neurons continue to remodel substrate utilization. The

Gut microbiota-derived trimethylamine N-oxide (TMAO) and its association with neuroinflammation in migraine.

Background and objectives Migraine is a common primary headache disorder associated with neuroinflammation, with evidence implicating gut microbiota and metabolites in neurological mechanisms pathways. Since trimethylamine-N-oxide (TMAO), derived from dietary precursors by gut microbial metabolism, is associated with cardiovascular and neurodegenerative conditions, and its role in migraines is unclear, we aimed to evaluate whether serum TMAO level and related metabolites in migraine patients can be biomarkers. Methods A total of 106 participants were enrolled, consisting of healthy controls (n

Admission-Parameter Nomogram for Early Neurological Deterioration after Minor Small Subcortical Infarction: A Retrospective Cohort Study.

Early neurological deterioration (END) was common after minor subcortical infarction, but prediction was complicated by heterogeneous mechanisms, including intrinsic small-vessel disease and branch atheromatous disease (BAD). A retrospective analysis included 240 patients admitted within 24 h of symptom onset with a National Institutes of Health Stroke Scale (NIHSS) score of 5 or less. END was defined as an increase of at least 2 NIHSS points within 72 h after admission. The primary logistic model retained admission systolic blood pressure (SBP) as a continuous variable and included baseline N

Association Between Treatment Sequencing and Overall Survival in Stage IV NSCLC With Brain Metastases: A National Cancer Database Study.

BACKGROUND: The optimal sequencing of brain-directed radiation and systemic therapy in stage IV non-small cell lung cancer (NSCLC) with brain metastases remains uncertain in the era of CNS-active systemic agents. We evaluated survival outcomes using national real-world data. METHODS: We conducted a retrospective cohort study of adults diagnosed between 2010 and 2022 with stage IV NSCLC and brain metastases in the National Cancer Database who received both brain-directed radiation and systemic therapy. Treatment sequence was classified as radiation-first or systemic-first based on initiation d

Impact of semaglutide introduction on the use of triptans: an interrupted-time series.

BACKGROUND: Emerging evidence suggests that glucagon-like peptide-1 receptor agonist (GLP-1RA) may be associated with reduced migraine burden. We aimed to evaluate whether semaglutide for weight management initiation is associated with changes in triptan consumption. METHODS: We conducted a nationwide interrupted time series analysis using Danish health registers. All adults initiating semaglutide between December 2022 and December 2024 were included. The first dispensing date served as the index date, establishing a 24-month baseline and a 12-month follow-up period. The primary outcome was m

Gender-Specific Lipid Parameters Predict Excessive Daytime Sleepiness in Parkinson's Disease.

BACKGROUND: Lipid metabolism plays an important role in neurodegenerative diseases, including Parkinson's disease (PD). However, the relationship between lipid profiles and excessive daytime sleepiness (EDS) remains unclear. This study aimed to explore the relationship between lipid parameters and EDS in PD patients, especially focusing on gender differences. METHODS: A total of 176 PD patients were included in this study, and clinical data was collected. The Epworth Sleepiness Scale (ESS) was used to classify patients into PD EDS (ESS ≥ 8) and PD non-EDS (ESS < 8) groups. Univariate and mult

Migraine in Young Stroke Patients-Better Outcomes Despite Distinct Risk Profiles: Insights From the STROKE-CARD-Study.

INTRODUCTION: Migraine is a common neurological disorder and a non-traditional risk factor for ischaemic stroke. Its relationship with specific stroke subtypes and post-stroke outcomes remains unclear. We evaluated the association between migraine (with and without aura), stroke aetiology, functional outcomes, and cardiovascular risk profiles in young patients with ischaemic stroke or TIA. METHODS: Patients aged ≤ 55 years enrolled in the STROKE-CARD long-term follow-up study underwent structured face-to-face headache interviews by headache specialists using ICHD-3 criteria. Association Between CYP2C19 Genotype and P2Y12 Reaction Units in Patients Receiving Clopidogrel After Acute Ischemic Stroke: A Prospective, Observational Study.

BACKGROUND: Recurrent stroke accounts for substantial morbidity and mortality worldwide, with up to 25% of ischemic strokes occurring in patients with prior strokes. Dual antiplatelet therapy (DAPT) with aspirin and clopidogrel is standard for secondary prevention after minor acute ischemic stroke (AIS) or high-risk transient ischemic attack (TIA). However, clopidogrel efficacy is reduced in carriers of cytochrome P450 2C19 (CYP2C19) loss-of-function (LOF) alleles. Although genotype-guided DAPT has been shown to improve outcomes, the limited availability, high cost, and turnaround time of CYP2

Loss of laminin-γ1 in pericytes impairs microvascular function and cognition.

Blood-brain barrier (BBB) disruption correlates with lesion burden and cognitive dysfunction in cerebral small vessel disease (CSVD). Laminin, highly produced by pericytes in the brain, maintains BBB integrity. However, it remains unclear if laminin dysregulation contributes to the pathogenesis of CSVD. Here, we show that mice lacking laminin-γ1 in pericytes, smooth muscle cells, and fibroblasts (PKO) develop age-dependent BBB disruption via both paracellular and transcellular mechanisms. In contrast, loss of laminin-γ1 in smooth muscle cells or fibroblasts fails to affect BBB integrity. In ad

From photographer to patient in the blink of an eye: incidental optical coherence tomography detection of papilloedema and idiopathic intracranial hypertension during pregnancy.

Incidental findings during ophthalmic imaging may reveal clinically significant disease in asymptomatic individuals. Papilloedema, characterised by optic disc swelling secondary to raised intracranial pressure, is a neuro-ophthalmic emergency that can result in permanent visual loss if untreated, making early detection essential. This case report describes the unexpected identification of bilateral optic disc swelling in a clinical/ophthalmic photographer who volunteered as a model patient during optical coherence tomography (OCT) training. Despite being asymptomatic, imaging raised suspicion

Implantable Microsphere-Mediated Targeted Delivery of Mesenchymal Stem Cell-Derived Extracellular Vesicles Attenuates Neuroinflammation and Promotes Recovery After Cerebral Ischemia.

BACKGROUND: Ischemic stroke is a major cause of death and disability, in which neuroinflammation exacerbates injury. Mesenchymal stem cell (MSC)-derived extracellular vesicles (EVs) offer therapeutic potential but face translational hurdles in scalable production, rapid systemic clearance, and inefficient targeted delivery. METHODS: We engineered an implantable alginate-microsphere system encapsulating EV-secreting MSCs and displaying the RGD-4C peptide (ACDCRGDCFC) on its surface. This platform functions as a bioreactor that sustains the release of functionalized EVs with enhanced targeting

PSP-specific atrophy targets regions connecting the salience network and subcortical circuits.

Progressive supranuclear palsy (PSP) shows a characteristic but incompletely defined pattern of neurodegeneration, in part because prior imaging studies have been limited by small and heterogeneous cohorts. Here, we consolidated evidence for PSP-related gray matter (GM) loss using a coordinate-based meta-analysis and interpreted the resulting atrophy pattern in a network and molecular framework to infer disease-relevant mechanisms. We conducted an Anatomical Likelihood Estimation (ALE) meta-analysis of whole-brain morphometry studies investigating atrophy in PSP, followed by functional decodin

Serotonin depletion amplifies migraine susceptibility through trigeminal ganglion neurons sensitization: Insights from patch clamp recordings in rat model.

Serotonin (5-HT) depletion is a critical driver of migraine pathophysiology, yet its precise impact on trigeminal ganglion (TG) neurons remains unclear. This study investigates how 5-HT depletion shapes the "intrinsic neuronal susceptibility" of the first-order TG neurons by analyzing the electrophysiological properties of small-to-medium (SM) and large (L)-sized TG neurons in a rat model using patch-clamp recordings. Our findings reveal that 5-HT depletion significantly heightens the intrinsic susceptibility of TG neurons, with the electrophysiological disparities between SM and L-sized neuro

Calmodulin controls spatial and temporal specificity of calcium-induced calcium release.

Calcium dynamics controls learning and memory, and abnormal calcium dynamics have been implicated in neurodegenerative disorders, such as Alzheimer's disease (AD). Calcium dynamics are influenced by calcium-induced calcium release (CICR), which is mediated by ryanodine receptors (RyR) located on endoplasmic reticulum (ER) membrane. Calmodulin, one of the most abundant proteins in the brain, inhibits RyR2, expressed in the dendrites of hippocampal CA1 neurons, with several reported consequences: relief of this inhibition is responsible for heart failure, and enhancing calmodulin to RyR binding

N-stearoyltyrosine attenuates ischemia-reperfusion injury by inhibiting caspase-dependent apoptosis and PARP-1/AIF-mediated parthanatos.

Cerebral ischemia‑reperfusion (I/R) injury induces neuronal death via caspase-dependent apoptosis and parthanatos, a poly (ADP-ribose) polymerase 1 (PARP-1)/apoptosis-inducing factor (AIF)-mediated caspase-independent pathway. Simultaneous inhibition of these pathways may provide enhanced neuroprotection. N-Stearoyltyrosine (NsTyr), a synthetic analogue of the endocannabinoid anandamide (AEA), exhibits neuroprotective effects in various models, however, its efficacy and mechanism in middle cerebral artery occlusion (MCAO) remain unclear. Neuronal oxygen-glucose deprivation/ reoxygenation (OGD/

Factor XIa Inhibitors for Secondary Prevention After Noncardioembolic Stroke: A Systematic Review and Meta-Analysis.

OBJECTIVES: Patients with noncardioembolic ischemic stroke or transient ischemic attack (TIA) remain at substantial risk of recurrent ischemic events, despite guideline-recommended antithrombotic therapy. Factor XIa (FXIa) inhibitors may reduce thrombotic risk while preserving hemostasis. METHODS: We systematically searched MEDLINE and Scopus for randomized-controlled clinical trials (RCTs) comparing FXIa inhibitors with placebo in patients with acute noncardioembolic ischemic stroke or TIA. Primary efficacy outcome was first stroke (ischemic, hemorrhagic, or undefined), and primary safety ou

UPS-mediated P-glycoprotein regulation in Alzheimer's disease: pharmacological implications for the amyloid pathology.

Alzheimer's disease (AD) is a progressive neurodegenerative disorder marked by the accumulation of amyloid β (Aβ), tau pathology, neuroinflammation, oxidative stress, and neurovascular dysfunction. Growing evidence suggests that impaired clearance of Aβ across the blood-brain barrier (BBB), rather than overproduction alone, is a key factor driving progression of AD. P-glycoprotein (P-gp) plays a crucial role in Aβ removal and is consistently reduced in AD. This review focuses on the mechanisms underlying P-gp dysfunction, particularly its regulation by the ubiquitin-proteasome system. Herein,
