Focal ischemic stroke impairs REM sleep and inhibits c-Fos expression of SST neuron in EP of male rats.

PURPOSE: The basal ganglia contribute to sleep-wake regulation and are vulnerable to ischemic stroke. Specifically, the striatum, as an upstream input region, appears critically involved, since rapid eye movement (REM) sleep disruption occurs in both extensive (cortex and striatum) and focal striatal infarcts. Nonetheless, the post-stroke dynamics of REM sleep disturbances and their neural targets remain unclear, largely due to the technical limitations in animal models and complexity of stroke outcomes. METHODS: We modified the transient middle cerebral artery occlusion model in rats to indu

Distinct brain metabolic patterns reflect biological heterogeneity and relate to clinical outcome in chronic migraine with medication overuse.

AimChronic migraine with medication overuse (CMwMO) is a disabling phenotype of migraine with heterogenous responses to medication withdrawal and preventive treatments. We investigated whether baseline brain metabolic patterns reflect biological heterogeneity associated with 1-year clinical outcome in CMwMO.MethodsWe prospectively enrolled patients with CMwMO and healthy controls and obtained baseline headache profiles, psychological and dependence-related assessment as well as FDG-PET/MRI data. Patients received preventive treatments along with structured medication-withdrawal education. Poor

A Cross-Sectional Survey of Knowledge, Confidence, and Decision-Making in Acute Stroke in the Era of Anti-Amyloid Therapies.

BACKGROUND: Anti-amyloid therapies (AATs) for Alzheimer's disease complicate emergency stroke care due to the risk of amyloid-related imaging abnormalities (ARIA), which can mimic acute ischemic stroke (AIS) and increase hemorrhagic risk. We evaluated emergency medicine (EM) providers' (physicians and advanced practice providers) and pharmacists' knowledge, confidence, and clinical decision-making related to AAT-associated ARIA. METHODS: A cross-sectional, web-based survey of EM providers and pharmacists practicing in United States hospital-based emergency care settings between February and A

Where the Wildfires Are: Correlates of Disaster Preparedness Among Oregon Assisted Living Communities.

Assisted living communities (ALs) serve a growing population of older adults with long-term care needs, especially those with Alzheimer's and other dementias (ADRD), yet their capacity to prepare for hyper-complex climate-related disasters remains underexamined. Using geographic information systems (GIS), we mapped all licensed ALs with the Oregon Department of Forestry's wildfire hazard scores, and then assessed organizational correlates of preparedness with a representative sample of Oregon ALs (n = 321). ALs located in higher wildfire risk areas reported greater confidence in wildfire prepa

Experimental techniques to evaluate IA-MSCs-mediated alleviation of vasogenic edema following ischemic stroke.

Ischemic stroke induced cerebral edema is a devastating complication which leads to excessive swelling, increased intracranial pressure (ICP) and rapid neurological deterioration. The mortality rate of post-stroke cerebral edema is approximately 80 %. However, therapeutic options for post-stroke cerebral edema are sparce, emphasizing an urgent need to explore new therapeutic strategies for post-stroke edema. In past few decades stem cell therapy has been a promising therapeutic strategy in decreasing infarct volume and promoting functional recovery by various mechanisms following stroke. Previ

Human Stem Cell Transplantation, Immunosuppression, and Noninvasive In vivo Cell Tracking in the Mouse Brain.

Stem cell therapies have emerged as potential therapeutics of interest for many central nervous system diseases, including stroke, epilepsy, neurodegenerative diseases, and traumatic brain injury. Preclinical models are required to evaluate the migration of transplanted stem cells to various areas of the brain, monitor graft survival to improve immunosuppression regimens, and assess stem cell safety and their efficacy on disease outcomes. Understanding all these aspects of stem cells as therapeutics can help optimize parameters for translation to humans. To facilitate this overarching goal, we

Association between corpus callosum microstructure and upper limb motor outcome in moderate-to-severe ischemic stroke patients at 3-6 months post-stroke.

BACKGROUND: Corticospinal tract (CST) integrity is a well-established imaging correlate of post-stroke motor outcome but does not fully explain interindividual variability, particularly in moderate-to-severe impairment. The corpus callosum (CC) may provide complementary structural information. This study examined whether early subacute callosal microstructure is associated with upper limb motor outcome and recovery. METHODS: We retrospectively analyzed 38 patients with moderate-to-severe middle cerebral artery stroke who underwent diffusion tensor imaging during the early subacute phase. Frac

Patient-Reported Visual Function in Ataxias and Association with Clinical Oculomotor Findings.

Oculomotor dysfunction is common in ataxias, but its impact on patient-reported visual function remains insufficiently understood. To characterize patient-reported visual function across ataxias and assess the functional significance of specific oculomotor abnormalities in spinocerebellar ataxias. Participants were recruited at Massachusetts General Hospital (n = 117): 56 with genetically-defined ataxias (SCA2, SCA3, SCA6, SCA27B, CANVAS), and 61 controls. Patient-reported visual function was assessed using a 13-item subset of the Visual Activities Questionnaire targeting depth perception, vis

Differential regulation of CYP46A1 in ischemic core and peri-infarct regions of male mouse brain after permanent middle cerebral artery occlusion.

Cholesterol 24-hydroxylase (CYP46A1) regulates brain cholesterol homeostasis and synaptic plasticity, playing a crucial role in ischemic stroke. Although previous studies have reported post-ischemic CYP46A1 upregulation, its spatiotemporal dynamics remain poorly defined. To elucidate these dynamics, we investigated the expression of CYP46A1 and other essential cholesterol homeostasis-related genes from 6 h to 3 days after permanent middle cerebral artery occlusion (pMCAO) in CB-17 mice. We utilized single-cell and single-nucleus transcriptomics, regional quantitative PCR, and high-resolution i

Determinants of procedural sedation requirements during endovascular treatment for acute anterior circulation ischaemic stroke: a post-hoc analysis of the INDIVIDUATE trial.

INTRODUCTION: Procedural sedation during EVT for acute ischaemic stroke is widely used, but factors associated with analgosedative drug requirements remain poorly characterised. We aimed to identify patient and procedural factors associated with analgosedative drug requirements during EVT and explore their association with clinical outcomes. PATIENTS AND METHODS: We performed a post hoc analysis of the randomised INDIVIDUATE trial, including patients with anterior circulation ischaemic stroke undergoing EVT under procedural sedation. Sedation exposure was quantified using mean propofol, remif

Amyloid-Related Imaging Abnormalities Summit Special Proceedings: Cardiovascular and Stroke Considerations for β-Amyloid Immunotherapy in Alzheimer Disease.

BACKGROUND: New antiamyloid therapies (AAT) for treating patients with early Alzheimer disease (AD) have become available in the past 3 years. Although AAT can be beneficial in slowing progression of AD, they also come with potentially serious side effects, particularly for patients requiring anticoagulation or thrombolysis for cardiovascular conditions. Given that most patients with AD are older and likely to have cardiovascular risk factors, there is urgent need for additional guidance for clinicians about potential risks and benefits associated with AAT for patients with cardiovascular como

Parental longevity and polygenic longevity scores in relation to ageing-related factors in a population of 70-year-olds followed over six years: The Gothenburg H70 Birth Cohort Study.

As societies age, a deeper understanding of ageing-related factors that contribute to longevity is needed. We therefore investigated possible longevity factors (social, medical, and biological) in relation to parental longevity (PL) and polygenic longevity scores (PGLSs). We examined 1126 70-year-olds from the Swedish population-based Gothenburg H70 Birth Cohort study in 2014-2016 (response rate 72%), with follow-up in 2019-2022 (response rate 77.6%). Comprehensive examinations included self-reported information on parents' ages, socioeconomic factors, mental, cardiovascular, and neurological

[Gq-GPCR upregulation in reactive astrocytes: implication for neurological disorders].

Astrocytes are essential glial cells that maintain brain homeostasis. Upon brain injury, they become reactive astrocytes with altered morphology, gene expression, and function, including dysregulated Ca2+ signaling implicated in disease development and progression. P2Y1 receptor (P2Y1R), one of Gq-GPCRs, is upregulated in reactive astrocytes across multiple brain disorders, including Alzheimer's disease, epilepsy, and stroke; however, how this upregulation contributes to pathology has remained unclear. To address this question, we generated astrocyte-specific P2Y1R-overexpressing transgenic mi

Frameless stereotactic radiosurgery in the management of brain metastases: long-term oncologic and toxicity outcomes.

OBJECTIVE: Gamma Knife radiosurgery (GKRS) is a widely accepted treatment modality for brain metastases and has historically relied on pin-based frame fixation, with a frameless option introduced in recent years. Previous studies suggest similar local control and overall survival between frameless and frame-based GKRS, but data on long-term outcomes remain limited. In this study, we sought to evaluate outcomes of single-fraction frameless GKRS for patients treated at our institution between 2018 and 2024. METHODS: We retrospectively reviewed the medical records of patients treated with single

Design, synthesis and multitarget evaluation of cyclopropyl-appended thiohydantoin derivatives as dual cholinesterase and COX inhibitors with antioxidant activity: a combined experimental and computational study.

The complex pathology of Alzheimer's disease (AD), together with the limited effectiveness of current treatments, has sparked the search for multifunctional agents capable of targeting key pathological mechanisms simultaneously. In this study, a novel series of cyclopropyl-appended thiohydantoin derivatives 4a-d and their corresponding S-substituted analogues 6a-d were rationally designed and synthesized as multi-target agents. Following complete spectroscopic characterization (1H NMR/13C NMR and HRMS), the entire library of synthesized compounds was evaluated for multi-target potential, inclu

Adaptation and feasibility of the International Registry for Alzheimer's Disease and Other Dementias real-world datasets in old-age mental healthcare practice in China.

BackgroundAnti-amyloid disease-modifying therapies (DMTs) for early Alzheimer's disease (AD) are entering routine care, increasing the need for harmonized, registry-ready real-world data. The International Registry for Alzheimer's Disease and Other Dementias (InRAD) proposed a minimum dataset (MDS) and extended dataset (EDS), but their applicability to psychiatry-led old age mental healthcare practices in China is uncertain.ObjectiveTo adapt the InRAD dataset for real-world AD DMT practice across multiple psychiatry institutions in China and assess the feasibility of routine data capture for t

Immune Checkpoint B7-H3 Inhibition Reprograms Acute Neuroinflammation and Protects the Brain After Acute Ischemic Stroke.

BACKGROUND: The role of immune checkpoint B7-H3 in acute ischemic stroke prognosis and poststroke immunosuppression remains uninvestigated, despite the clinical significance of immune checkpoints with inflammaging and poststroke infections. In this study, we investigated the effect of regulating cerebral induction of B7-H3 after acute ischemic stroke and evaluated its longitudinal impact on brain damage, neuroinflammation, vascular integrity, host defense gene regulation, and functional outcomes. METHODS: C57BL/6 mice were subjected to transient middle cerebral artery occlusion and injected (

Multimodel Diffusion MRI Signatures in Atypical Parkinsonian Disorders.

Distinguishing atypical parkinsonian disorders (APS) from Parkinson's disease (PD) remains challenging due to overlapping clinical features, yet accurate differentiation is critical for prognosis and treatment. Here, we employed multimodel diffusion MRI (dMRI) analysis to characterize microstructural alterations across corticobasal syndrome (CBS), progressive supranuclear palsy-Richardson syndrome (PSP-RS), and PD, with the aim of identifying which dMRI model and regional metrics show the strongest group-level separation. We analyzed 25 CBS, 42 PSP-RS, and 21 PD participants compared to 35 age

[When Philosophy Sheds Light on Alzheimer's Disease].

This article examines our relationship with the patient through three classic problems of philosophy: the relationship between mind and body, the voluntary or involuntary nature of our actions, and finally what constitutes our identity. All three determine at once who we are, our responsibility and our relationship with others. Yet these problems, often addressed in school essays, are not confined to an academic setting but also offer a way to enrich our relationships with others.

Stress, path integration, and the entorhinal cortex.

The stress induced release of glucocorticoids influences neural excitability and plasticity in the hippocampus. Surprisingly the entorhinal cortex (EC), an adjacent region, has received little attention in stress research, even though it is also rich in glucocorticoid receptors. Grid cells in the medial EC play a decisive role in path integration (PI). Interestingly, the EC is a brain region damaged very early in Alzheimer's disease (AD). Three recent studies investigated the associations between stress, cortisol and PI. In the first study chronic stress was associated with poorer PI. Acute st
