Feasibility of a multidimensional interdisciplinary rehabilitation program for people with dementia and their informal primary caregivers - a randomized pilot trial with a caregiver focus.

BACKGROUND: To improve the situation of informal caregivers, both supportive efforts for caregiver and rehabilitation for the person with dementia, may be needed. This randomized controlled pilot trial evaluated, among caregivers, the feasibility of a person-centred multidimensional interdisciplinary rehabilitation program for community-dwelling people with dementia that incorporated an education and support intervention for informal caregivers. METHODS: Informal caregivers were randomized alongside the person with dementia to an intervention group (n=36) or usual care (n=31). Multiprofession

Oncolytic Virus-Engineered Extracellular Vesicles Rewire Glioblastoma Immunity.

Immunotherapy with oncolytic Herpes simplex virus type 1 (oHSV-1) shows promise for glioblastoma; however, clinical benefit remains uncommon due to tumour- and host-mediated immunosuppression, which limits immune targeting of residual glioblastoma stem-like cells (GSCs). To address this barrier, extracellular vesicles (EVs) harvested from oHSV-1-infected GSCs (EVOV) were evaluated for their immunomodulatory capacity by priming peripheral blood mononuclear cells (PBMCs) ex vivo and in vivo. In GSC-PBMC organoids analysed by single-cell RNA sequencing, IFN-response and antigen-presentation progr

Disease-specific body composition patterns in children with cerebral palsy and spinal muscular atrophy.

OBJECTIVES: Spinal muscular atrophy (SMA) and cerebral palsy (CP) are severe neurological conditions characterized by neurodegeneration and non-progressive brain injury, respectively, both profoundly affecting the nutritional status in children. This observational study aimed to comprehensively assess and compare anthropometry and body composition across groups of children with SMA, CP, and healthy controls (HCs). METHODS: We enrolled three age- and sex-matched groups of 40 children each: CP (Gross Motor Function Classification System IV-V), SMA (Types I-II), and HC. Anthropometry, skinfolds,

Design, Synthesis of 3-n-Butylphthalide-Hydrazine/Hydrazide Derivatives as Potential Anti-Ischemic Stroke Agents.

To identify novel therapeutic candidates for ischemic stroke, a series of hydrazine- and hydrazide-derived analogs based on 3-n-butylphthalide (NBP) were designed and synthesized in this study, and their anti-ischemic stroke activities were systematically assessed. Among all synthesized derivatives, the compound NBP-HY exerted potent antiplatelet aggregation and antioxidant activities in vitro. Consistent in vivo results revealed that NBP-HY significantly alleviated thrombus formation in both carrageenan-induced tail thrombosis and FeCl3-induced carotid artery thrombosis models. In a rat middl

[Protective effect of bloodletting at twelve Well-points of hand on neurological function by regulating cortical microglia/macrophage polarization through the IL-10/STAT1 pathway in mice with traumatic brain injury].

OBJECTIVES: To observe whether pricking blood therapy at the twelve Well points of the hand (Well-point bloodletting) can regulate the cortical phenotypic polarization of microglia and macrophages via the interleukin-10 (IL-10)/signal transducer and activator of transcription 1(STAT1) pathway, suppress excessive neuroinflammation and neuronal apoptosis, and thereby alleviate neurological deficits induced by traumatic brain injury (TBI) in mice. METHODS: Male C57BL/6 mice were used in the present study which is composed of 3 experiments. The TBI model was established by using an electronically

Cardiac troponin T and 28-day mortality in traumatic brain injury: a retrospective cohort study and machine learning prediction model.

BACKGROUND: Traumatic brain injury (TBI) frequently induces cardiac dysfunction via the brain-heart axis, yet the prognostic value of cardiac troponin T (cTnT) in TBI remains incompletely characterized. This study investigated thye association between cTnT levels and 28-day all-cause mortality in TBI patients and developed a machine learning-based prediction model. METHODS: This retrospective cohort study included 837 adult TBI patients from the MIMIC-III/IV databases (development cohort) and 121 patients from a single Chinese center (external validation cohort). The first cTnT measurement wi

The mediating role of out-of-hospital adherence between health literacy and quality of life in Chinese stroke survivors: a cross-sectional study.

BACKGROUND: Stroke, the second foremost cause of mortality globally, imposes a considerable disease burden in China. Limited health literacy and difficulties adhering to post-discharge care may be associated with poorer quality of life among stroke survivors. However, the extent to which out-of-hospital adherence statistically accounts for the association between health literacy and quality of life remains insufficiently characterized. OBJECTIVE: To examine associations among health literacy, out-of-hospital adherence, and quality of life, and to assess whether out-of-hospital adherence stati

Myelin oligodendrocyte glycoprotein antibody-associated isolated seizures in children: a retrospective single-center case series.

BACKGROUND AND PURPOSE: Isolated seizures represent a distinct, rare, and underrecognized phenotype of myelin oligodendrocyte glycoprotein (MOG) antibody-associated disease (MOGAD). In recent years, documentation of this phenotype has been limited to sporadic reports, with dedicated case series being extremely scarce. This study aimed to describe the clinical characteristics of isolated seizures in pediatric patients with MOGAD. METHODS: We retrospectively reviewed the medical records of pediatric patients diagnosed with MOG antibody-associated isolated seizures between January 2019 and Janua

Efficacy and safety of acute-phase immunotherapy for adult autoimmune encephalitis: a systematic review and network meta-analysis.

BACKGROUND: Autoimmune encephalitis (AE) is an immune-mediated inflammatory disorder of the central nervous system associated with substantial morbidity. Acute-phase immunotherapy is crucial for improving outcomes, yet the comparative efficacy and safety of available regimens remain unclear. We therefore performed a Bayesian network meta-analysis to compare acute-phase immunotherapeutic strategies for AE. METHODS: PubMed, Embase, Web of Science, and the Cochrane Library were searched from inception to October 2025 for studies evaluating acute-phase immunotherapy in AE. A Bayesian network meta

miR-378f targets CAB39 to regulate autophagy in chemotherapy-induced peripheral neuropathy.

Chemotherapy is the most common cancer therapy due to its ability to effectively destroy cancer cells. However, peripheral neuropathy is a notable side effect, which has no clearly established prevention or treatment methods. Although various therapeutic strategies using microRNAs (miRNAs) have been developed for a wide range of diseases, the role of miRNAs in neuroregeneration has yet to be elucidated. Our group previously identified five differentially expressed miRNAs in the plasma of patients with gastric cancer receiving XELOX (capecitabine and oxaliplatin) chemotherapy. Among these, miR-

Effectiveness of interventions for caregiver burden and quality of life of informal caregivers of stroke survivors: a systematic review and meta-analysis.

BACKGROUND: Informal caregivers, who play a pivotal role in post-stroke care, frequently report substantial caregiver burden and reduced quality of life (QoL), which may adversely affect caregiving capacity and stroke rehabilitation outcomes. However, evidence regarding effective interventions to lessen caregiver burden and improve the QoL of informal caregivers remains fragmented. AIMS: This study aimed to synthesize evidence on the effectiveness of interventions for reducing the caregiver burden and improving the QoL of informal caregivers of stroke survivors. METHODS: A systematic search

Deep brain stimulation and spinal cord stimulation for chronic disorders of consciousness: a decade of clinical evidence, mechanistic insights, and critical appraisal.

Deep brain stimulation (DBS) and spinal cord stimulation (SCS) have been investigated for over four decades as neuromodulatory treatments for chronic disorders of consciousness (DOC), including the vegetative state/unresponsive wakefulness syndrome (VS/UWS) and the minimally conscious state (MCS), for which no disease-modifying pharmacological therapy exists. This narrative review synthesizes literature published between 2016 and 2026 on the neurobiological rationale, clinical evidence, neurophysiological biomarkers, and safety of DBS and SCS for chronic DOC. The 'mesocircuit hypothesis', supp

Gut microbial histamine and serotonin promote cerebrovascular angiogenesis: Evidence from moyamoya disease.

Moyamoya disease (MMD) is a progressive cerebrovascular disorder characterized by arterial stenosis and compensatory neovascularization, but its upstream drivers remain poorly defined. Here, we identify Bifidobacterium pseudocatenulatum (B. pseudocatenulatum), a traditionally probiotic species, as a context-dependent pathobiont. Integrated metagenomic and metabolomic analyses revealed enrichment of B. pseudocatenulatum in MMD, closely associated with elevated microbial-derived histamine and serotonin. In mice, chronic oral administration of B. pseudocatenulatum disrupted intestinal barrier int

Multi-omics advances in sepsis-associated encephalopathy: from pathogenesis to clinical translation.

Sepsis-associated encephalopathy (SAE) represents the most prevalent neurological complication arising from sepsis, affecting approximately 70% of patients with sepsis and associated with mortality rates reaching up to 70%. Survivors frequently experience persistent cognitive deficits; however, the field currently lacks specific diagnostic biomarkers and targeted therapeutic interventions, with diagnosis primarily established through exclusion criteria. This review critically examines multi-omic investigations of SAE, evaluating whether convergent evidence across genomic, transcriptomic, prote

Stereotactic guidance for ventricular catheter placement in ventriculoperitoneal shunt surgery: a state-of-the-art review on technical rationale, clinical outcomes, and future directions.

Ventriculoperitoneal (VP) shunting remains the most frequently performed surgical treatment for hydrocephalus; however, shunt failure rates remain substantial despite technical refinements. Proximal catheter malfunction, often associated with malposition, ventricular wall contact, or choroid plexus obstruction, represents a common cause of shunt revision. Conventional freehand ventricular cannulation is based on surface anatomical landmarks and surgical experience, which may be unreliable in patients with small or distorted ventricles. Stereotactic guidance-including frame-based systems, frame

APOE ε4 accelerates plasma p-tau217 trajectory: a Bayesian joint longitudinal analysis of ADNI.

BACKGROUND AND OBJECTIVE: Plasma GFAP and p-tau217 are leading blood biomarkers for Alzheimer's disease (AD), but whether APOE ε4 modulates their longitudinal rates of change - directly or via amyloid burden - is unresolved. We tested APOE ε4 modulation of plasma GFAP and p-tau217 trajectories and their coupling with cognitive/structural decline in ADNI participants. METHODS: A total of 325 ADNI participants aged ≥ 65 (CN 181/MCI 106/AD 38) with ≥ 2 plasma GFAP and ≥ 2 p-tau217 (UPENN-Fujirebio) measurements over ≤ 10 years were analyzed. Linear mixed-effects models tested APOE ε4 × time inte

Metabolic Dyshomeostasis, neuroinflammation, and glymphatic clearance deficits as convergent drivers of Alzheimer's disease: an exposome-based conceptual framework for prevention.

The repeated failures of anti-amyloid therapies in Alzheimer's disease (AD) have exposed critical limitations of the linear amyloid cascade hypothesis and underscored the need for a broader etiological framework. The objective of this review is to synthesise mechanistic evidence for an exposome‑based model of sporadic AD that integrates metabolic, inflammatory, and sleep/circadian contributions to brain aging. Epidemiological evidence indicates that up to 40% of AD risk is attributable to modifiable environmental and lifestyle factors. These factors, collectively termed the aging exposome, rep
