Viability of Chlamydia trachomatis in Different Anatomical Sites-a Systematic Review and Meta-analysis.

BACKGROUND: Modern assays for the detection of Chlamydia trachomatis (CT) rely on nucleic acid amplification testing (NAAT) of DNA or ribosomal RNA. However, it is also known that both viable ("living") and non-viable ("dead") CT can be detected by NAAT. Multiple laboratory techniques to measure CT viability have emerged. METHODS: We searched PubMed, EMBASE, Scopus, and Dimensions as well as conference abstracts for entries between January 2000 and May 2023. We included any studies that measured CT viability among NAAT-positive samples. Viability assays include enhanced cell culture, direct f

Prevalence of Chlamydia trachomatis and Neisseria gonorrhoeae infections and associated risk factors among pregnant women and key populations in Kenya: A multi-centre cross-sectional study.

Sexually-transmitted pathogens Chlamydia trachomatis (CT) and Neisseria gonorrhoeae (NG) cause curable but often asymptomatic bacterial infections. Missed diagnoses and treatment, leading to chronic infections, can cause clinical complications and increase transmission. Accurate prevalence estimates are essential for the effective public health control of these sexually transmitted infections, especially in Africa, where data are scarce. This cross-sectional study was undertaken across five sites in Nairobi, Mombasa, and Homabay in Kenya. Between February and July 2022, vaginal, urethral, or r

Trends in Triple-Site Testing for Chlamydia and Gonorrhea in New York City: A Cross-Sectional Study Using Health Information Exchange Data.

BACKGROUND: Sexually transmitted infections pose a threat to public health and continue to rise in New York City despite reductions in other parts of the United States. Chlamydia and gonorrhea can infect multiple anatomic sites, including the urogenital tract, pharynx, and rectum. Guidelines recommend multisite testing for select populations, yet multisite testing is not routinely performed, limiting our understanding of trends in positivity and missed infections. METHODS: We conducted a retrospective cross-sectional study of patients who underwent triple-site testing for chlamydia and gonorr

The Natural History of Mycoplasma genitalium in the Pharynx and Rectum in a Cohort of Men Who Have Sex With Men: Prevalence, Incidence, Duration, and Symptoms.

BACKGROUND: Little is known about the natural history of Mycoplasma genitalium (MG) in the throat and the rectum. METHODS: Men who have sex with men were enrolled in a longitudinal cohort study on the natural history of extragenital gonorrhea and chlamydia. Men self-collected pharyngeal and rectal specimens weekly and completed electronic symptom and sexual behavior diaries for 48 weeks. Prevalent infections were detected on the first week of testing; incident infections were defined as ≥2 consecutive weeks of positive specimens after a negative test result. We calculated duration using Kapla

Prevalence of chlamydial, gonococcal and syphilis positivity by anatomical site and sex via self-collected biospecimens using at-home testing kits among a large and diverse cohort of men and women enrolled in the MACS/WIHS Combined Cohort Study (MWCCS) in the USA.

INTRODUCTION: Cisgender men who have sex with men (MSM), transgender women, cisgender women and cisgender men who have sex with women only (MSW) have differential risk of acquiring Chlamydia trachomatis (CT) and Neisseria gonorrhoeae (GC), the two most commonly reported bacterial sexually transmitted infections (STIs) in the USA. Similarly, MSM and transgender women have a higher number of syphilis infections in the USA than other groups. The presence of any of these three STIs is a significant risk factor for HIV acquisition and transmission, unless taking antiretroviral medication for HIV pr

The Relationship Between HIV Preexposure Prophylaxis Use and Laboratory-Confirmed Bacterial Sexually Transmitted Infection Positivity Among a Diverse, Multi-City Cohort of Sexually Active Sexual and Gender Minorities in the United States.

The presence of chlamydia, gonorrhea, or syphilis infection is a significant risk factor for HIV acquisition and transmission and disproportionately impacts men who have sex with men (MSM) and transgender women. While HIV preexposure prophylaxis (PrEP) reduces HIV risk, its use may influence sexual behaviors, potentially increasing sexually transmitted infection (STI) exposure. Conversely, PrEP users are often more engaged in care, regularly screened and treated for STIs, and may access other prevention tools such as doxycycline postexposure prophylaxis. Studies on the relationship between PrE

Diagnosis and treatment of sexually transmitted infections among emergency department patients.

INTRODUCTION: Sexually transmitted infections (STIs) remain a major public health concern, with emergency departments (EDs) serving as key sites for evaluation and treatment. Despite their importance, contemporary national-level data describing ED testing and treatment practices for patients presenting with symptoms suggestive of STI are limited. METHODS: We performed a retrospective cohort study of ED encounters from 2016 to 2024 using Cosmos, a large geographically diverse electronic health record database. Patients aged ≥15 years with symptoms suggestive of an STI were included. We examine

Cervicovaginal cytokine profiles and their associations with colposcopic findings in adolescent girls and adult women in South Africa: an exploratory cross-sectional study.

BACKGROUND: Colposcopy is used to evaluate cervical inflammation and detect precancerous and other cervical lesions; however, access remains limited in many low-resource settings. Identification of cervicovaginal immune biomarkers associated with specific colposcopic findings may provide insight into mucosal immune alterations linked to increased susceptibility to sexually transmitted infections (STIs) and human immunodeficiency virus (HIV), although these associations do not imply diagnostic or predictive utility in a cross-sectional context. METHODS: In this exploratory cross-sectional stud

Implementing STI point-of-care testing using a whole-system programme model: the ProSPiRO implementation research protocol.

OBJECTIVES: Sexually transmitted infections (STIs) continue to cause substantial global morbidity and mortality, compounded by reliance on syndromic management and limited access to laboratory diagnostics. Although point-of-care tests (POCTs) can enable timely diagnosis and treatment, their adoption within health systems remains inconsistent. Here we present an implementation research protocol that aims to generate evidence on the acceptability, feasibility and suitability of a collaborative whole-system programme model to support implementation of STI POCTs and improve access to STI testing a

Genome-wide association studies of C. trachomatis identify novel tissue tropism-predicting loci and sequences that discriminate among major genome sequence clades.

Chlamydia trachomatis infections cause chronic and debilitating diseases of the eye and genital tract. Recent data suggest that infections are common at other tissue sites, including the lower intestinal tract of both men and women. Our previous work demonstrated tropism-predicting sequences in male rectal isolates, with modification in the PmpE sequence and predicted structure being highly correlated with male rectal infection. An expanded collection of strains from five different tissue sites was used to identify novel tropism-predicting markers for the different tissues. These data demonstr

High Burden of Sexually Transmitted Infections Among Pre-exposure Prophylaxis (PrEP) Users: Testing and Prevention Needs.

Pre-exposure prophylaxis (PrEP) effectively prevents HIV acquisition but does not protect against other sexually transmitted infections (STIs). Rising STI rates among PrEP users have raised concerns about risk compensation and highlight the need for integrated sexual health strategies. We assessed the prevalence, anatomical distribution, and behavioural determinants of STIs among PrEP users in Spain. This retrospective observational study included 724 individuals receiving daily tenofovir disoproxil fumarate/emtricitabine (TDF/FTC) at a tertiary hospital in Madrid between March and October 202

An interactive digital behaviour change intervention to decrease incidence of sexually transmitted infections among users of STI self-sampling websites: a feasibility RCT.

BACKGROUND: Sexually transmitted infections such as chlamydia are common among young people. If left untreated, they can have serious health consequences. Condoms are recommended for prevention, but young people report often not using them for penetrative sex. Use of web-based sexually transmitted infection testing is increasing rapidly, but these services provide little support or advice on how to prevent future infections. 'Wrapped' is a web-based intervention that aims to support young users of web-based sexually transmitted infection testing to use condoms correctly every time they have pe

HIV Screening Practices Among Youth Tested for Other Sexually Transmitted Infections in Pediatric Primary Care.

INTRODUCTION: HIV remains a significant public health concern. In Wisconsin, new cases increased by 36% during 2020 through 2022, and 22% were 13 to 24 years old. Despite recommendations for routine HIV screening, youth testing remains inadequate. This study aimed to understand HIV screening practices among youth receiving care in pediatric primary care clinics in southeastern Wisconsin. METHODS: Clinic HIV testing rates were measured in patients aged 12 to 26 undergoing gonorrhea and/or chlamydia testing at pediatric primary care clinics affiliated with a not-for-profit children's hospital.

Acceptability of self-collected oropharyngeal swabs for gonorrhoea and chlamydia screening among men who have sex with men in a sexual health clinic setting.

BACKGROUND: Self-collected oropharyngeal swabs for gonorrhoea and chlamydia screening were widely adopted during the COVID-19 pandemic to reduce transmission risks. Although self-collection is established for other anatomical sites, limited data exist on the acceptability of oropharyngeal self-collection among gay, bisexual and other men who have sex with men, particularly across levels of experience. METHODS: We conducted a cross-sectional online survey among gay, bisexual and other men who have sex with men attending Melbourne Sexual Health Centre between June 2020 and October 2022. Partici
