Acute disseminated encephalomyelitis and reactivation of cerebral toxoplasmosis in a child: Case report.

Toxoplasma gondii is an opportunistic parasite that infects a broad range of hosts including humans. The chronic latent phase of the disease manifests as intra-neuronal cerebral cysts tightly controlled by the host immune system. In immunocompromised patients, reactivation of cerebral toxoplasmosis can have severe neurological outcomes that may sometimes lead to death. Despite the efficient prophylactic and treatment measures taken against the rare reactivation of cerebral toxoplasmosis, many reports including several recent ones revealed the still occurrence of this spectrum of disease. Hydatidosis Following Giant Cerebral Hydatid Cyst Operation.

BACKGROUND: Hydatid cysts (HCs) are caused by parasites, the larvae of Echinococcus granulosus. They usually affect the liver and lung, but primary brain involvement is rarely seen. HCs may cause a mass effect in the brain. For this reason, surgical excision must be performed. Complications may rarely occur after surgery. Recognition of complications is of vital importance. CASE DESCRIPTION: A 45-year-old man presented with complaints of personality disorder and severe, long-standing headache. The patient had no neurologic deficit. However, there were symptoms associated with increased intrac

ORAL-FACIAL-DIGITAL SYNDROME TYPE I (CLINICAL CASE).

The oral-facial-digital syndrome belongs to a group of hereditary diseases, manifested by multiple birth defects (usually, the face and fingers). At the current stage, there are 14 genetic variations of the oral-facial-digital syndrome. The presence of various abnormalities of the oral cavity, face and fingers is common for all of them, but each syndrome has a specific phenotype or type of inheritance. The etiology of this syndrome is unknown. It is inherited in an X-linked dominant pattern. Aim of the study: to describe and analyze the clinical case of oral-facial-digital syndrome. Data of th

Teratogenic bluetongue and related orbivirus infections in pregnant ruminant livestock: timing and pathogen genetics are critical.

Congenital infections of domestic animals with viruses in several families, including Bunyaviridae, Flaviridae, Parvoviridae, and Reoviridae, are the cause of naturally occurring teratogenic central nervous system and/or musculoskeletal defects (arthrogryposis) in domestic animals. Congenital infections of ruminant livestock with bluetongue virus (BTV) and some related members of the genus Orbivirus (family Reoviridae) have clearly shown the critical role of gestational age at infection in determining outcome. Specifically, fetuses infected prior to mid-gestation that survive congenital BTV in

Cerebral and non-cerebral coenurosis: on the genotypic and phenotypic diversity of Taenia multiceps.

We characterised the causative agents of cerebral and non-cerebral coenurosis in livestock by determining the mitochondrial genotypes and morphological phenotypes of 52 Taenia multiceps isolates from a wide geographical range in Europe, Africa, and western Asia. Three studies were conducted: (1) a morphological comparison of the rostellar hooks of cerebral and non-cerebral cysts of sheep and goats, (2) a morphological comparison of adult worms experimentally produced in dogs, and (3) a molecular analysis of three partial mitochondrial genes (nad1, cox1, and 12S rRNA) of the same isolates. Prime-boost vaccination with toxoplasma lysate antigen, but not with a mixture of recombinant protein antigens, leads to reduction of brain cyst formation in BALB/c mice.

INTRODUCTION: Infection with the ubiquitous parasite Toxoplasma gondii is a threat for immunocompromised patients and pregnant women and effective immune-prophylaxis is still lacking. METHODS: Here we tested a mixture of recombinant T. gondii antigens expressed in different developmental stages, i.e., SAG1, MAG1 and GRA7 (SMG), and a lysate derived from T. gondii tachyzoites (TLA) for prophylactic vaccination against cyst formation. Both vaccine formulations were applied systemically followed by an oral TLA-booster in BALB/c mice. RESULTS: Systemic priming with SMG and oral TLA-booster did n

Experimental cerebral and non-cerebral coenurosis in goats: A comparative study on the morphological and molecular characteristics of the parasite.

This experimental study was conducted to test whether cerebral and non-cerebral forms of Coenurus cerebralis belong to one origin or they are originated from two different tape worms. In the first step of the study, two groups of dogs were orally infected with the protoscoleces of cerebral and non-cerebral cysts and four months after infection, the adult worms were collected and morphologically characterized. Then the obtained eggs from two groups of adult worms were orally inoculated to two groups of goats to trace the predilection sites and also to compare the morphological and molecular cha

Treatment of giant congenital cysts of the midline in adults: Report of two cases and review of the literature.

BACKGROUND: Giant cysts of the midline, not associated to a tumor, are exceptional finding in the brain of adults. Here we present two cases of symptomatic giant cerebral cysts of the midline occurred in an elderly and in a young adult patients both treated with mini-invasive unilateral neuroendoscopic procedure. In the recent literature (since 1999) similar cases have not been reported. Beside the clinical report, review of literature and major anatomical features of the region are described. CASE DESCRIPTION: These two adults (82 and 41 years old respectively) had a slow progressive develop

Surgical Planning for the Treatment of a Patient with Multiple, Secondary, Intracranial Echinococcal Cysts.

A 27-year-old man with a 2-year history of recurrent hospitalizations for various neurologic and cardiologic emergencies was admitted to our hospital presenting with left hemiparesis, which gradually progressed to quadriparesis, bilateral hemianopsia, intracranial hypertension syndrome, and seizures. A diagnosis of echinococcosis was made, based on the radiologic findings of multiple cerebral hydatid cysts and a sizable cyst of the heart. The hydatid cyst of the heart was treated first with a thoracotomy, and after a month he underwent three consecutive surgical operations for the removal of s

Disseminated neurocysticercosis presenting as isolated acute monocular painless vision loss.

Neurocysticercosis, the most common parasitic infection of the nervous system, is known to affect the brain, eyes, muscular tissues and subcutaneous tissues. However, it is very rare for patients with ocular cysts to have concomitant cerebral cysts. Also, the dominant clinical manifestation of patients with cerebral cysts is either seizures or headache. We report a patient who presented with acute monocular painless vision loss due to intraocular submacular cysticercosis, who on investigation had multiple cerebral parenchymal cysticercal cysts, but never had any seizures. Although such a visio

Relative etiological importance of adenoid hypertrophy versus sinusitis in children with persistent rhinorrhoea.

AIM: Aplastic anemia (AA) affects the Asian population two to three fold more than people in other regions. Besides the host genetics and socioeconomic status, several other environmental exposures have been linked with an AA etiology. We aimed to examine the association of various environmental exposures with AA occurrence among Pakistani individuals. SUBJECTS AND METHODS: A case-control study was conducted in Karachi, Pakistan, where cases (diagnosed AA patients) were selected from the National Institute of Blood Disease and Bone Marrow Transplantation (NIBD), while for each case, a single

Evidence that the Echinococcus granulosus G6 genotype has an affinity for the brain in humans.

The present study investigates the molecular characteristics of cerebral Echinococcus cysts. A total of 10 specimens of cerebral Echinococcus cysts, including six formalin-fixed paraffin blocks and four intact cerebral cysts, were used for this study. The target DNA was successfully amplified from eight samples and sequenced. BLAST analysis indicated that sequenced isolates belong to the Echinococcus granulosus (G6) genotype. All of the eight sampled brain cysts belonged to the G6 genotype, while all of the eight liver cysts belonged to G1. This is a strong indication that G6 has a higher affi

Primary multiple cerebral hydatid disease: still symptomatic despite pathologically confirmed death of the cyst.

Hydatid disease is a life-threatening parasitic infestation caused by Echinococcus granulosus. Infection with E. granulosus typically results in the formation of hydatid cysts in liver, lungs, kidney and spleen. Majority of the intracranial cysts are secondary and solitary. Multiple primary cerebral cysts are uncommon. Surgical and medical management of a 14-year-old boy with multiple primary hydatid cysts are presented. 14 cysts, which were symptomatic due to their mass effect, were surgically removed, whereas a deep-seated asymptomatic cyst was followed-up with medical treatment. Despite pro

[Brain hydatidosis: report of four cases].

In only 2% of all cases of hydatidosis, the cysts are located in the brain. We report a 12-year-old male, a 5-year-old girl and a 19-year-old female consulting for intracranial hypertension and a 13-year-old male consulting for a left hemiparesis. Imaging studies found intra-cerebral cysts with characteristics of hydatidosis in all. All lesions were completely removed surgically and the pathological study of the excised piece confirmed the diagnosis of hydatidosis.

Cerebroretinal microangiopathy with calcifications and cysts: characterization of the skeletal phenotype.

Cerebral cysts and calcifications with leukoencephalopathy and retinal vascular abnormalities are diagnostic hallmarks of cerebroretinal microangiopathy with calcifications and cysts (CRMCC). Previous studies have suggested that skeletal involvement is also common, but its characteristics remain unknown. This study aimed to assess the skeletal phenotype in CRMCC. All Finnish patients with features consistent with CRMCC and for whom radiographs were available were included. Clinical information pertinent to the skeletal phenotype was collected from hospital records, and all plain radiographs we

Choroidal fissure cerebrospinal fluid-containing cysts: case series, anatomical consideration, and review of the literature.

BACKGROUND: Cerebrospinal fluid (CSF)-containing cysts at the level of the choroidal fissure are rare embryological entities infrequently described in the literature because of their benign nature. On the occasion of a case series, we present an overview of the literature and discuss anatomical and embryological location, imaging characteristics, presenting symptoms, and treatment indication of these lesions. METHODS: We identified, in a retrospective study of a database with 81 patients harboring 86 supratentorial intracranial cysts, six patients with a CSF-containing cyst at the level of th

A case of leukoencephalopathy, cerebral calcifications and cysts.

Triad of leukoencephalopathy, cerebral calcifications and cysts (LCC) is a recently reported rare disease named 'Labrune syndrome' after the first case was reported in 1996 by Labrune et al. Herein, we report a case of a 36-year-old man with mild right-sided weakness and seizures for 5 years. CT of brain revealed extensive calcification involving bilateral basal ganglia, right thalamus and bilateral deep cerebellar nuclei. A supratentorial cystic lesion with blood fluid level was seen in left occipitotemporal region. MRI examination revealed diffuse symmetric white matter hyperintensity sugges

Use of statewide hospital discharge data to evaluate the economic burden of neurocysticercosis in Los Angeles County (1991-2008).

Statewide hospital discharge data were used to assess the economic burden of neurocysticercosis in Los Angeles County (LAC) from 1991 through 2008. A neurocysticercosis hospitalization was defined as having a discharge diagnosis of cysticercosis in addition to convulsions, seizures, hydrocephalus, cerebral edema or cerebral cysts. This study identified 3,937 neurocysticercosis hospitalizations, with the number of annual hospitalizations remaining relatively unchanged over the study period (R(2) = 0.01), averaging 219 per year (range 180-264). The total of all neurocysticercosis hospitalization

Diagnosis and treatment of patients with nevoid basal cell carcinoma syndrome [Gorlin-Goltz syndrome (GGS)].

BACKGROUND: The nevoid basal cell carcinoma syndrome (NBCC) is a rare and autosomal dominant inherited disease with well-defined characteristics, summarized by Gorlin and Goltz in 1960. In the head and neck region, cerebral calcifications, basal cell carcinoma (BCC) and multiple keratocysts of the jaws are the predominant findings. The aim of this study was to determine the diagnostic findings and the therapy for patients with NBCC. PATIENTS AND METHODS: The medical files of 17 patients with NBCC, treated in a single institution (females: 9, males: 8) were evaluated. Thirteen patients were al

Endoscopic treatment of midline cerebral cysts.

Axial cerebral cysts of various location and origin are relatively rare lesions. Endoscopic neurosurgery has made them amenable for safe intervention even in vital brain regions. Neuroendoscopic treatment was used by the author in 35 patients during the last 7 years without permanent morbidity or mortality. The aim of the surgery was to fenestrate the cyst or to perform cyst exstirpation to relieve the pressure effect on the surrounding functioning neural tissue and to reestablish the patency of cerebrospinal fluid pathways, as well as histological verification of the cyst origin.
