Obstructive sleep apnea severity related hippocampal subregional volume changes and the association with hypoxia.

STUDY OBJECTIVES: Obstructive sleep apnea (OSA) is associated with cognitive impairment and brain structural changes, but the specific association of disease severity with hippocampal subfields remains unclear. This cross-sectional study aimed to delineate volumetric changes across hippocampal subfields in patients stratified by OSA severity and to investigate the association between these changes and intermittent hypoxia (IH), as well as clinical symptoms. METHODS: A study of 159 participants with OSA underwent MRI and in-hospital polysomnography. Patients were categorized into mild (n = 30)

α-Adrenergic blockade impairs ductus venosus shunting during an acute period of fetal hypoxaemia without further deficits to cerebral oxygen delivery.

The fetal circulation has unique shunts, including the ductus venosus (DV), the tone of which dictates how much substrate-rich blood returning from the placenta is streamed preferentially away from the liver and instead towards the heart. Herein, we aimed to use clinically relevant MRI techniques in sheep to measure indirectly induced changes in DV tone and the consequences of this on DV shunting, expressed as a ratio of umbilical vein (UV) flow [DV:UV (as a percentage)], and its impact on oxygen delivery to the fetal brain. At 116-117 days gestational age (term = 150 days), fetal sheep (n = 1

Huangqi Baihe Granules alleviate chemotherapy-induced intestinal mucositis in Drosophila and mice.

Chemotherapy-induced intestinal mucositis (CIM) is a significant dose-limiting adverse effect of cancer treatment. Huangqi Baihe Granules (HQBHG) derived from Dunhuang’s ancient medical texts could alleviate radiation brain injury and hypobaric hypoxia-induced acute lung injury. Here, the protective effect and mechanism of HQBHG against irinotecan (CPT-11)-induced intestinal mucositis were detected by using Drosophila melanogaster and mouse models. Oral administration of HQBHG could significantly ameliorate body injury caused by CPT-11, including increased survival rate, rescued locomotion, al

Early hypoxia-induced secretome remodeling reveals adaptive mechanisms and biomarkers of blood-brain barrier dysfunction in ischemic stroke.

Reversible disruption of the blood-brain barrier (BBB) occurs within hours after the onset of ischemic stroke (IS), offering a critical window for therapeutic intervention. However, the molecular characteristics and their potential as circulating biomarkers associated with this transient phase of BBB dysfunction remain poorly defined. To elucidate these mechanisms, we employed an oxygen-glucose deprivation (OGD) model in human cerebral microvascular endothelial cells (hCMEC/D3) to simulate early ischemic stress, and systematically profiled their secreted proteome and metabolome. By comparing w

A Whole-body Cooling Strategy in Uncontrolled Donation After Circulatory Death: Insights From a Porcine Ex Vivo Lung Perfusion Model.

BACKGROUND: Prolonged warm ischemia in uncontrolled donation after circulatory death (uDCD) potentially increases the risk of graft dysfunction in lung transplantation. To address this concern, a noninvasive, whole-body cooling approach was developed using a specialized liquid refrigerant at -21 °C. The aim of this study was to investigate whether this method reduces lung temperature and improves lung function in uDCD donors. METHODS: Fifteen pigs were randomly assigned to 3 groups. In the study group (n = 5), the entire donor body was submerged in a basin containing a coolant of -21 °C, and

Emergency Medical Individual Clinician Volume and Mortality in Trauma Patients.

IMPORTANCE: The initial treatment by emergency medical services (EMS) significantly affects the outcomes for severely injured patients. Effective control of hemorrhage, proper administration of blood products, and adherence to traumatic brain injury guidelines can reduce morbidity and mortality after trauma. Additionally, the experience of prehospital clinicians in high-acuity nontrauma conditions is associated with improved outcomes. OBJECTIVE: To evaluate the association of annual trauma patient volume and outcomes at the individual EMS clinician level. DESIGN, SETTING, AND PARTICIPANTS: A

Neural responses to acute hypoxia and hyperoxia.

The brain's high metabolic demands make it highly sensitive to oxygen level changes, with both hypoxia and hyperoxia affecting cognitive and neural function. However, the underlying neural mechanisms remain unclear. This study aimed to characterise the effects of acute normobaric hypoxia and hyperoxia on neural function and perceptual thresholds using EEG, visual evoked potentials (VEPs), and critical flicker fusion (CFF) thresholds. In a single-blind, randomised, crossover design, thirty participants inhaled 10.5%, 21%, or 100% oxygen while resting-state EEG, VEPs, and CFF thresholds were rec

Ischaemia/reperfusion and permanent ischaemia differentially affect haemoglobin properties - Possible influence of oxidative stress and adaptation to acute hypoxia.

Cerebral ischaemia is an acute state characterised by a severe decrease in the supply of oxygen (O2) to the brain, resulting in the death of neurons and glial cells. Despite multiple studies investigating the processes that lead to cell damage under ischaemic stroke, there is still a lack of information about the changes in blood properties under various phases of transient ischaemia with restoration of blood flow or under prolonged ischaemia. Blood, as a source of O2, might either contribute to the brain damage owing to the development of oxidative stress, or might prevent cell death by the m

HIF-activated priming of TRAIL-induced cell death determines epigenetic vulnerability in kidney cancer.

Activation of hypoxia-inducible factors (HIFs) supports cancer cell survival, yet how HIFs govern cell death remains unclear, despite evidence that HIF-1 acts as a tumor suppressor in cell renal cell carcinoma (ccRCC). Here, we report a cell death-priming role for HIF-1/2 in ccRCC. Through cell viability screens with chemical libraries, we identify SGI1027 and its analog MS1129 as HIF-1/2-dependent cell death inducers that specifically kill VHL-deficient ccRCC cells in vitro and patient-derived xenografts in mice. Mechanistically, SGI1027 and MS1129 induce proteasomal degradation of DNMT1/DNMT

The immune awakening: Transformative strategies against brain tumors.

Glioblastoma multiforme remains the primary brain tumor with the highest mortality rate, which is highly resistant to aggressive conventional therapies, owing to its capacity to evade the central nervous system's distinctive immunological barriers. Several mechanisms confer this tumor resistance, such as down-regulation of MHC molecules, secretion of immunosuppressive cytokines, metabolic reprogramming, recruitment of Tregs and myeloid-derived suppressor cells, and heterogeneity reinforced by the blood-tumor barrier and hypoxia. The purpose of this review is to examine the changing field of ne

Borophene/PVDF Composite Film With High Piezoelectric Coefficient for Closed-Loop Monitoring and Intervention of Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA), with the most direct symptom of snoring, is a common disorder that leads to intermittent hypoxia and cardiovascular risk. Here, we present a wearable borophene/PVDF (polyvinylidene fluoride) composite film integrated into a closed-loop OSA monitoring and intervention system. The composite film exhibits a piezoelectric coefficient of 31.9 pC·N-1, achieving effective biomechanical signal transduction within ±5% performance degradation after 10 000 compression or bending cycles. When attached to the neck, the composite detects biomechanical signals with high fidelit

Novel mechanism of neuronal hypoxia response: HIF-1α/STOML2 mediated PINK1-dependent mitophagy activation against neuronal injury.

Hypoxic stress contributes to brain disorders by causing neuronal injury, making it crucial to understand neuronal hypoxic response mechanisms for disease resistance. In the early stage of stress, neurons initiate a series of compensatory pathways to resist cell damage, but the underlying mechanisms have not been fully elucidated. In this study, we found that hypoxia transiently activates PTEN-induced kinase 1 (PINK1)-dependent mitophagy in the early stage before cell damage and neurological dysfunction. When PINK1-dependent mitophagy is inhibited, neuronal injury begins to exacerbate. Under h

Decoding myeloid heterogeneity in glioblastoma: spatial insights from transcriptomics.

BACKGROUND: Glioblastoma represents the most aggressive primary brain malignancy, characterized by a profoundly immunosuppressive microenvironment that limits therapeutic efficacy and drives disease progression. Myeloid cells—comprising tumor-associated macrophages, myeloid-derived suppressor cells, and dendritic cells—serve as central regulators of this milieu. However, their profound heterogeneity in origin, function, and localization has historically complicated efforts to neutralize their protumoral effects using conventional bulk analyses. MAIN BODY: This review synthesizes emerging insig

Cutamesine attenuates hippocampal damage via sigma-1 receptor activation following oxygen-glucose deprivation in vitro.

Cerebral ischemia is a leading cause of death and disability worldwide, due to neuronal energy failure, calcium overload, ER stress and inflammation. Sigma-1 receptor activation regulates calcium signaling between ER and mitochondria and reduces the activation of ER stress and inflammation pathways. In the present study, we investigated the neuroprotective effects of the sigma-1 receptor agonist cutamesine in rat organotypic, and acute hippocampal slices exposed to oxygen-glucose deprivation (OGD), two in vitro models of global ischemia. In organotypic hippocampal slices obtained from both sex

Mild Mitochondrial Impairment Activates Overlapping Longevity Pathways Converging on the Flavin-Containing Monooxygenase FMO-2.

A mild impairment of mitochondrial function activates the hypoxia inducible factor (HIF-1)-mediated hypoxia stress response pathway leading to a HIF-1-dependent increase in lifespan. Lifespan extension resulting from HIF-1 stabilization is dependent on activation of flavin-containing monooxygenase-2 (FMO-2). In this work, we explored the role of fmo-2 in the long lifespan of genetic mitochondrial mutants in C. elegans. We found that fmo-2, but not other fmo genes, are specifically upregulated in the long-lived mitochondrial mutants clk-1, isp-1 and nuo-6. Disruption of fmo-2 through RNA interf

Diversity and function of tumor-associated macrophages in brain metastases: mechanisms and therapeutic prospects.

Brain metastasis significantly worsens prognosis in late-stage cancer., with Its treatment hindered by the blood-brain barrier (BBB) and an immunosuppressive tumor microenvironment. Within this environment, tumor-associated macrophages (TAMs) represent the predominant immune population. Through their roles in immune modulation, angiogenesis, and tumor invasion, TAMs are critical drivers of disease progression. TAMs are highly heterogeneous. While traditionally categorized into M1 (anti-tumor) or M2 (pro-tumor) phenotypes, this dichotomy is an oversimplification. Recent single-cell studies have

Stem Cell-Derived Exosomes Improve Neurological Dysfunction in a Rat Model of Moderate-to-Severe Cerebral Palsy.

Cerebral palsy (CP), the most prevalent pediatric motor disorder with significant cognitive comorbidity (> 50%), lacks therapies addressing both impairments in moderate-to-severe cases. This study demonstrates that human umbilical cord mesenchymal stem cell-derived exosomes (hUCMSC-Exos) exert profound therapeutic effects in a rat model of moderate-to-severe CP established via bilateral carotid artery occlusion with hypoxia. Intravenously administered hUCMSC-Exos displayed sustained brain retention and significantly restored motor coordination and cognitive function. The recovery was primarily

Mitochondrial reverse electron transport regulates glioma stemness through Sirt3-HIF1α-SOX2 signaling.

Glioblastoma multiforme (GBM) is an aggressive primary brain tumor characterized by high recurrence and resistance to current therapies, largely driven by glioma stem cells (GSCs). While mitochondrial dysfunction has been implicated in GBM, the specific mitochondrial mechanisms that sustain GSC maintenance remain poorly defined. Here, we identify mitochondrial reverse electron transport (RET) as a key metabolic program enriched in GSCs relative to differentiated glioma cells. Pharmacological inhibition of RET markedly suppresses GSC sphere formation by downregulating the stemness-associated tr

Effect of a Localized Oxygen-Releasing Hydrogel Sheet on Early-Stage Infarct Evolution in a Rat Photothrombotic Stroke Model: A Preliminary Study.

Ischemic stroke triggers hypoxia, inflammation, and oxidative stress. Local oxygen delivery may prevent secondary injuries. Herein, we implanted a catalase-incorporated thiolated gelatin-based oxygen-releasing hydrogel sheet in a rat model of photothrombosis to evaluate early infarct attenuation and feasibility. Male Sprague-Dawley rats were allocated to four groups (n = 6/group): control at 24 h (G1), with hydrogel sheet at 24 h (G2), control at 72 h (G3), and with hydrogel sheet at 72 h (G4). Focal ischemia was induced with Rose Bengal and targeted illumination through a 6.0-mm cranial defec

Machine Learning-Based Prognosis Prediction in Glioblastoma Multiforme Patients by Integrating Clinical Data with Multimodal Radiomics.

Objectives: Glioblastoma multiforme (GBM) is considered the most aggressive primary brain tumor, which often exhibits tumor heterogeneity. Hypoxia is a key aspect of intratumoral heterogeneity that contributes to poor prognosis in GBM. In this study, we aimed to develop machine learning (ML) models using radiomics and clinical features for the prediction of one-year survival for GBM. Methods: Data from 35 patients in the ACRIN 6684 trial, including fluoromisonidazole (FMISO)-positron emission tomography (PET), magnetic resonance (MR) (T1, T2, and fluid-attenuated inversion recovery (FLAIR)) im
