Microscopic magnetic resonance elastography of traumatic brain injury model.

Traumatic brain injury (TBI) is a major cause of death and disability for which there is no cure. One of the issues inhibiting clinical trial success is the lack of targeting specific patient populations due to inconsistencies between clinical diagnostic tools and underlying pathophysiology. The development of reliable, noninvasive markers of TBI severity and injury mechanisms may better identify these populations, thereby improving clinical trial design. Magnetic resonance elastography (MRE), by assessing tissue mechanical properties, can potentially provide such marker. MRE synchronizes mech

[Application of neuroendoscope in the treatment of skull base chordoma].

OBJECTIVE: To further explore the application, approach, indication and prognosis of neuroendoscope treatment for skull base chordoma. METHODS: A total of 101 patients of skull base chordoma were admitted at our hospital from May 2000 to April 2010. There were 59 males and 42 females. Their major clinical manifestations included headache, cranial nerve damage and dyspnea. They were classified according to the patterns of tumor growth: Type I (n = 13): tumor location at a single component of skull base, i. e. clivus or sphenoid sinus with intact cranial dura; Type II (n = 56): tumor involving

Giant atypical intraventricular meningioma presenting with visual loss in a child.

INTRODUCTION: Giant atypical intraventricular meningioma is very rare in childhood. Atypical meningiomas are characterised by an increased mitotic ratio and MIB-1 expression, and demonstrate more aggressive tumour behaviour compared with typical meningiomas. METHODS: We present the case of an 11-year-old girl who presented to the eye clinic complaining of reduced vision, and was diagnosed with a giant atypical intraventricular meningioma. RESULTS: Five years after surgical resection, there has been no recurrence of the tumour. Goldmann visual fields show a left incongruous homonymous hemiano

Facts versus theories: an everlasting struggle. The Johann Jakob Wepfer Award 2010.

Rather than a complete overview of the contribution of Utrecht to stroke research, I have selected a few subjects and attempt to put these in historical context. Johann Jakob Wepfer (1620-1695) was unique in that he approached 'apoplexy' through post-mortem observations, in the tradition of Padua. However, the interpretation of his findings in haemorrhagic and especially non-haemorrhagic stroke was still heavily influenced by the authority of Galen's writings. Wepfer's category of 'serous apoplexy' assumed that extravasation of blood serum might lead to compression of brain substance and block

Unusual manifestation of multiple myeloma: focal affection of central nervous system in a patient with chronic lymphocytic leukaemia.

INTRODUCTION: Involvement of the central nervous system as the first manifestation of multiple myeloma is very rare. AIM: To present an unusual case of the primomanifestation of a multiple myeloma in the form of a focal affection of the central nervous system in a patient with chronic lymphocytic leukaemia. METHODS AND RESULTS: A female patient diagnosed with chronic B-lymphocytic leukaemia with gradually increasing right-sided cerebellar symptomatology. The CT examination revealed expansion of the cranial vault with significant compression of brain structures. The tumour was extirpated and

TN/TN glottic carcinoma: a comparison of two fractionation schedules.

Axillary lymph node (ALN) status is considered to be the single most important prognostic indicator in patients with breast cancer. It can be assessed by various radiological, pathological and surgical techniques, the most accurate being histological examination of lymph nodes after axillary lymph node dissection (ALND). This prospective study was conducted to assess the feasibility and diagnostic accuracy of preoperative ultrasound (US) and ultrasound-guided fine-needle aspiration cytology (USG-FNAC) of ALN in patients with breast cancer. Thirty patients with FNAC-proven breast cancer, planne

[Recurrent syncope and Chiari malformation].

Downward herniation of the cerebellar tonsils through the foramen magnum (Chiari I malformation) is usually revealed by head and neck pain, often associated with brain-stem or spinal cord disturbances. Syncopes are rarely reported and may be difficult to link to their cause when they occur alone. We report two cases with brief and repetitive syncopes revealing a Chiari I malformation. These manifestations may be attributed to transient compression of brain stem or vascular structures at the craniocervical junction, triggered by intracranial pressure increase, as they disappear after posterior

Diagnosis and management of increased intracranial pressure.

Increased intracranial pressure (ICP) is a pathological state common to a variety of neurological diseases, all of which are characterized by the addition of volume to the skull contents. Elevated ICP may lead to brain damage or death by two principle mechanisms: 1) global hypoxic-ischemic injury, as a consequence of reduced cerebral perfusion pressure (CPP) and cerebral blood flow; and 2) mechanical distortion and compression of brain tissue as a result of intracranial mass effect and ICP compartmentalization. All ICP therapies have as a goal, reduction of intracranial volume. In unmonitored

Neurobehavioral functioning of adults with pituitary disease.

Neurobehavioral disorders are not infrequent in adults with pituitary disease. The disorders can be due to compression of brain structures important for cognitive and emotional function, effects of hormonal imbalance on sensitive structures, post-surgical disruption of connecting pathways, adverse reactions to medical therapy, and adverse delayed effects of radiation therapy. A multidisciplinary team approach to the treatment of pituitary tumors will allow for the early diagnosis of neurobehavioral disorders and the institution of pharmacologic and behavioral interventions.

[The hemodynamic aspects in predicting the course of hemorrhagic stroke (based on ultrasonic dopplerographic data)].

A comparative analysis was performed of clinical-neurological-hemodynamic-structural changes in the acute phase of hemorrhagic insult (HI) in 56 patients who ranged from 19 to 59 years old. Intracerebral hematomas (ICH) appeared to be more commonly seen. Main emphasis was put on potentialities of Doppler Sonography (DS) in hemodynamic assessment of cerebral bloodflow condition in patients with hemorrhagic insult. Parenchymal hemorrhage was found out to play the greatest pathogenetic part in the time course of cerebral decompensation in HI, with subarachnoidal hemorrhage making a lesser contrib

Macrocephaly is not always due to hydrocephalus.

Macrocephaly is a clinical symptom with a heterogeneous cause. Besides enlarged ventricles (hydrocephalus) it can be caused by increased brain volume, expanded extracerebral spaces, intracranial cysts, and dilated vessels. Drawing on the experience of more than 8 years using transfontanellar sonography including Doppler techniques, some typical clinical and sonographic features of several forms of macrocephaly are addressed. Besides isolated familial or sporadic megalencephaly, extracerebral fluid collections are the main explanation of macrocephaly. Subdural effusions with compression of the

Changes of frequency spectrum of the CSF pulse wave caused by supratentorial epidural brain compression.

The frequency spectrum of the CSF pulse wave and amplitude transfer function from arterial to CSF pressure were estimated in the lateral ventricle and cisterna magna during the course of supratentorial epidural compression of brain in anaesthetised ventilated cats. Ventricular fluid pressure, the amplitude of spectral components of the CSF pulse and the amplitude of transfer function in the supratentorial compartment were increased in an exponential manner as epidural compression advanced. A transtentorial pressure gradient developed when epidural compression exceeded a critical level. Conduct
