Prognostic value of oedema findings on head CT in traumatic brain injury following road traffic accidents.

INTRODUCTION: Existing CT-based prognostic models for traumatic brain injury (TBI) are not tailored to specific injury mechanisms and lack sensitivity to cerebral oedema changes. This study aimed to develop an assessment method for cerebral oedema and compare it with conventional CT scoring systems in predicting long-term outcomes in patients with TBI following road traffic accidents (RTAs). METHODS: A retrospective analysis was conducted on 227 patients with TBI following RTAs admitted to a single center (2019.1-2023.12). A clinical prognostic model (IMPACT) was defined, comprising age, moto

Prediction of acute mountain sickness occurring at 4554 m using overnight pulse oximetry from lower altitude: A pilot study.

Acute mountain sickness (AMS) affects many individuals ascending to high altitude annually, carrying potential morbidity and mortality risks (e.g., high-altitude cerebral oedema). The objective of this pilot study was to record overnight pulse oximetry during ascent to 4554 m and determine whether: (1) extracted overnight biomarkers had any relationship to AMS; and (2) AMS at 4554 m could be predicted by oximetry biomarkers using classification models trained/validated on overnight data from lower altitudes. Twenty lowlanders (five females, 36.8 ± 18.5 years old) completed a 4 day ascent to 45

Glymphatic dysfunction following traumatic brain injury: Mechanism of waste clearance and Emerging therapeutic targets.

OBJECTIVES: Traumatic brain injury (TBI) affects millions annually, contributing to mortality and neurological deficits. The glymphatic system, the brain's perivascular waste-clearance network, has been implicated in injury mechanisms following TBI. This narrative review expands current evidence on the mechanisms of glymphatic dysfunction, imaging biomarkers and therapeutic targets. METHOD: We conducted a literature search in PubMed to screen relevant studies on glymphatic dysfunction following TBI. Of 114 initially identified articles, 45 met the inclusion criteria. Following the exclusion o

Diverging trajectories of job demands, job resources, and psychological distress before long-term mental health-related sick leave among teachers.

STUDY AIMS: The prevalence of Long COVID in children and adolescents is heterogeneous, ranging from 1% to 51%, depending on the population studied. The lack of a standardised approach for establishing a Long COVID diagnosis in children and adolescents complicates the accurate assessment of prevalence, risk factors and outcomes. The present study aimed to examine the value of standardised interviews and an adjudication process to better understand self- or proxy-reported symptoms lasting longer than 12 weeks compatible with Long COVID in children and adolescents during the COVID-19 pandemic. PAH-associated health risk from ultrafine particles: an urban field study.

Reteplase (Retavase) is a plasminogen activator, mimicking endogenous tissue plasminogen activator (t-PA), a serine protease, converting plasminogen to plasmin and thereby precipitating thrombolysis. It is a third-generation recombinant form of t-PA that operates in the presence of fibrin (i.e. it is fibrin specific). Reteplase can be administered as a bolus dose (nonweight-based), rather than an infusion, which promotes rapid and safe administration. The ease of administration of this reteplase dosage regimen (two 10U bolus doses, each over 2 minutes, 30 minutes apart) is conducive to prehosp

Early blood pressure lowering and cerebral oedema in thrombolysis-treated stroke: secondary analysis of the ENCHANTED trial.

BACKGROUND AND PURPOSE: Controversy persists over the balance of benefits and harms of early intensive blood pressure (BP) lowering in thrombolysis-treated acute ischaemic stroke (AIS) patients. The BP-control arm of the Enhanced Control of Hypertension and Thrombolysis Stroke Study (ENCHANTED) (n=2196) showed that compared with guideline-recommended management (systolic BP (SBP)<180 mm Hg), intensive BP lowering did not improve functional outcome despite reducing intracranial haemorrhage. We aimed to evaluate the relationship between the BP parameters and cerebral oedema in ENCHANTED BP-contr

Prolonged coma and cerebral oedema in a patient with an ATP1A2 variant.

ATP1A2 (OMIM 182340) encodes the α2 subunit of Na+/K+-ATPase. Variation in this gene has been associated with a spectrum of clinical phenotypes, including familial hemiplegic migraine type 2 (FHM2), epilepsy and intellectual disability. A 22-year-old woman with intellectual disability, hemiplegic migraine and epilepsy presented with persistent decreased consciousness, unexplained by initial investigations. Two weeks later, repeat imaging showed new, marked cerebral oedema with no identified cause; this eventually resolved. A year later, she had a further milder episode. An epilepsy gene panel

Nrf2 overexpression reprograms neural stem cell fate: promoting neuronal differentiation and functional recovery post-ischemic stroke via suppression of the ROS/NF-κB axis.

BACKGROUND: Neural stem cells (NSCs) have been experimentally used in multiple models and patients, offering great potential for the treatment of neurological disorders such as ischaemic stroke. However, the proliferative and differentiative limitations of NSCs transplants must be overcome to further exploit the clinical potential of NSCs-based therapies. OBJECTIVE: This study aimed to elucidate the regulatory role of Nrf2 in NSCs proliferation and neuronal differentiation in vitro, to evaluate the therapeutic potential of transplanting Nrf2-overexpressed NSCs for functional recovery in the MC

The role of neuroimaging in neurotoxicity after chimeric antigen receptor T-cell therapy.

Chimeric antigen receptor T-cell (CAR-T) therapy has become an established treatment for several haematological malignancies in relapse and is being evaluated for new indications. An important clinical challenge associated with the use of CAR-T therapy, however, is the common development of neurotoxicity. Different neurotoxicity syndromes have been reported. The best-known form of CAR-T neurotoxicity is immune effector cell-associated neurotoxicity syndrome, which can be associated with various findings on magnetic resonance imaging (MRI), including cerebral oedema and leptomeningeal enhanceme

RhoA deletion in macrophages/microglia aggravates blood-brain barrier disruption after ischemic stroke reperfusion injury by promoting endothelial cell apoptosis and pyroptosis.

Disruption of the blood-brain barrier (BBB) is an important cause of secondary injury following cerebral ischemia-reperfusion (I/R). Database analyses revealed RhoA upregulation in macrophages/microglia within I/R brain tissue; however, the role of macrophage/microglial RhoA in BBB disruption and I/R injury remains poorly understood. In this study, we verified that macrophage/microglial RhoA was significantly upregulated in I/R mice. Employing conditional knockout (cKO) mice, present study demonstrated that the macrophage/microglial RhoA deficiency exacerbates I/R injury, manifesting as enlarg

Cerebral fat embolism syndrome and the use of multimodal neuromonitoring and corticosteroid treatment.

Cerebral fat embolism syndrome is a rare complication of long-bone fractures. We describe a case of severe cerebral fat embolism after orthopaedic trauma sustained in a road traffic collision. The case is notable for the severity of neurological involvement, the application of multimodal neuromonitoring to guide care and the use of methylprednisolone as part of management. The patient was admitted to the neurointensive care unit and managed with standard neuroprotective measures. Imaging on day 10 demonstrated worsening cerebral oedema with transtentorial herniation. Intracranial pressure (ICP

A Combination of VX765 and OLT1177 Reduced Brain Injury after Intracerebral Haemorrhage via Inhibiting Inflammasome Activation.

Intracerebral haemorrhage (ICH) is a severe stroke subtype with high mortality and poor functional recovery. Neuroinflammation, mediated by NLRP3 inflammasome and caspase-1 (Casp1) activation, drives secondary brain injury, including oedema and cell death after ICH. We hypothesize that combining NLRP3 inhibitor OLT1177 and Casp1 inhibitor VX765 will provide enhanced neuroprotection against brain oedema and injury in experimental ICH. ICH was induced by injecting autologous blood into the basal ganglia in mouse models. Sixty-three C57Bl/6 male mice were randomly assigned to five groups: sham, v

Association between invasive intracranial pressure monitoring and 1-year functional outcome in ICU patients with severe stroke: a pre-planned ancillary study of the SPICE prospective cohort (SPICE-ICP).

BACKGROUND: Patients with severe stroke requiring mechanical ventilation have a severe prognosis. There are differences in the management of these patients, particularly regarding intracranial pressure (ICP) monitoring. The aim of this study was to evaluate the association between ICP monitoring and one-year functional outcome of patients with severe stroke in ICU requiring mechanical ventilation. MATERIALS AND METHODS: We conducted a predefined ancillary study of the SPICE cohort (NCT03335995), comparing patients in ICU with ICP monitoring to those receiving standard monitoring following a s

Continuous renal replacement therapy in acute liver failure.

Acute liver failure (ALF) is a devastating condition that primarily affects young adults. This often-lethal condition involves a rapid loss of hepatic function, that then leads to multiple organ failure. The accumulation of numerous toxins, especially ammonia, causes cerebral oedema and intracranial hypertension. Continuous renal replacement therapy (CRRT) is increasingly recognized as having a key role in ammonia removal in ALF and current evidence suggesting that timing of initiation, dose, and duration of therapy may influence survival. In addition to this important role of toxin clearance,

Visual Loss Following Indirect Trauma: A Diagnostic Dilemma.

Monocular vision loss from indirect trauma can result in damage to the optic nerve, retina, or surrounding structures, often occurring without direct injury to the eye. In some cases, overlapping conditions can complicate the diagnosis. We present a case of monocular vision loss following indirect trauma, caused by traumatic optic neuropathy (TON) and Purtscher's retinopathy, emphasizing the diagnostic challenges encountered. A 28-year-old gentleman with no underlying medical conditions, who was involved in a motor vehicle accident and developed a sudden onset of central scotoma in his left ey

Study on the Effects of Transcutaneous Auricular Acupuncture Stimulation of the Vagus Nerve on Post-Traumatic Cerebral Oedema.

BACKGROUND: Secondary cerebral oedema following traumatic brain injury (TBI) is a major cause of poor prognosis, primarily driven by neuroinflammation. High-mobility group box 1 (HMGB1) is a key damage-associated molecular pattern that initiates a potent inflammatory cascade, yet targeted pharmacological interventions face clinical translation challenges. Non-invasive transcutaneous auricular vagus nerve stimulation (taVNS) has shown anti-inflammatory potential, but its efficacy and specific mechanisms in treating traumatic cerebral oedema remain unclear. METHODS: A controlled cortical impact

Early Versus Delayed Anticoagulation in Acute Ischemic Stroke According to Atrial Fibrillation Subtype and Time of Diagnosis: Subgroup Analysis of the OPTIMAS Randomized Controlled Trial.

BACKGROUND: After acute ischemic stroke, it is uncertain whether the time of atrial fibrillation (AF) diagnosis (before or after stroke) or AF subtype (paroxysmal or permanent) modifies the treatment effect of early versus delayed direct oral anticoagulant (DOAC) initiation. METHODS: OPTIMAS (Optimal Timing of Anticoagulation After Acute Ischemic Stroke) was a randomized, parallel-group, open-label trial with blinded outcome assessment. Participants with acute ischemic stroke and AF were randomized 1:1 to early (within 4 days) or delayed (day 7-14) DOAC initiation. The primary outcome was a c
