Acute intracranial hypertension occurring during infusion of stable plasma protein solution.

A case of traumatic cerebral oedema is reported in which the infusion of small volumes of stable plasma protein solution (SPPS) resulted in acute and massive increases in intracranial pressure. The batch of SPPS responsible was found to have high levels of prekallikrein activator which is thought to be the cause of the systemic hypotension previously described with plasma proteins solutions. A second batch of SPPS with low levels of prekallikrein activator generated much smaller increases in intracranial pressure. The mechanism of this phenomenon is discussed and a recommendation made for the

Fulminant hepatic failure in childhood: an analysis of 31 cases.

To document the clinical features and complications of fulminant hepatic failure in childhood, 31 consecutive cases (of whom only 9 survived) were reviewed. Of 26 children with acute hepatitis (HbSAg-negative), liver function steadily deteriorated in all but 2, and encephalopathy occurred within 3 weeks of the onset of symptoms in all except 3 of them. Eight of these patients survived, as did one of 3 in which this deterioration was caused by paracetamol overdosage. Single cases due to Amanita phalloides and halothane died. Encephalopathy lasted from 2 to 16 days in the survivors, and from one

Cerebral complications in the treatment of accelerated hypertension.

The malignant phase of hypertension is invariably fatal unless treated, and rapid reduction of arterial pressure is thought to be the treatment of choice. Ten patients with accelerated hypertension are described in whom abnormal neurological signs developed following the rapid reduction of arterial pressure. Three patients died without recovering from the neurological damage. A fourth died of an unrelated cause a month later. Areas of ischaemic damage were found in the brains of three of these cases. Of the six survivors, four were left with some permanent neurological disability. It is likely

Angiographic diagnosis of cerebral oedema.

Cerebral oedema is usually associated with brain vascular lesions, such as infarction, neoplasms, angiitis, thrombophlebitis, etc. and we consider that angiographic examination is essential for the diagnosis. In this paper we analyse the angiographic signs of cerebral oedema and the differential diagnosis between oedema and any associated lesions. We also consider that it is very important to establish a diagnosis between primary oedema and hydrocephalus with a view to establishing the correct treatment in each case.

[Vasogenic cerebral oedema. Changes in membrane ATPases. Correction by a phospholipid precursor (author's transl)].

In vasogenic cerebral oedema, there is progressive quantitative and qualitative impairment of mitochondrial ATPase and of Na/K/ATPase. This impairment, which reflects the intracellular component of cerebral oedema, would appear to be related to changes in the phospholipid environment of the cell membrane enzymes. CDP choline, a metabolic phospholipid precursor, is to a certain extent capable of correcting this disturbed activity and at the same time reduce oedema.

[The importance of axial computer tomography of the neurocranium in neurotraumatology (1) (author's transl)].

Computer tomography enables exact diagnostic clarification of intracranial lesions in trauma of the neurocranium. Subdural or epidural as well as intracerebral haemorrhages can be localised, and a circumscribed contusion and cerebral oedema become tangible. The article reports on 90 patients subjected to computerized tomography following trauma of the neurocranium. In about 50% of the cases, the trauma had been caused by a traffic accident. About 27% of the accident victims were children and adolescents. In about one-half of the examined persons, subdural or epidural haemorrhages were found, w

[Noninvasive screening of cerebral death by Doppler sonography (author's transl)].

Directional Doppler sonography of the supratrochlear and extracranial cerebral arteries was performed on 16 patients without any cerebral flow, as confirmed angiographically. The findings were inconsistent for the supratrochlear artery, but the internal carotid and vertebral arteries had characteristic changes in the Doppler curve with clearly diminished systolic flow velocity and flow reversal during diastole. These results were never seen in arteriosclerotic occlusion, hypoxaemic or toxic cerebral oedema. Doppler sonography thus helps to formulate indications for early cerebral angiography.

Papilloedema and optic atrophy in chronic hypercapnia.

Two patients are described with hypercapnia of 10 and 13 years duration. Both patients had papilloedema at different stages of their illness and one patient developed optic atrophy. Whether such changes are due to local retinal vascular changes, general change in cerebral vasculature or the result of raised intracranial pressure is unknown. A combination of all seems likely. Computerized axial tomography brain scan on Case 2 was compatible with cerebral oedema and the development of optic atrophy (Case 1) suggests cerebral oedema and consequent raised intracranial pressure as the major factor

A quantitative histological study of the effects of acute triethyl lead poisoning on the adult mouse brain.

The effects of a single injection of triethyl lead chloride on the mouse brain was studied 1, 3, 5, 7 and 30 days postinjection using quantitative histological techniques. The total number of glia in the anterior commissure was significantly reduced following injection but by 30 days postinjection had returned to normal. The number of neurons and the number of glia in the indusium griseum did not change significantly. The number of mitotic cells in the subependymal layer fell slightly from 1 to 3 days postinjection then returned to normal 5 days postinjection. The number of pyknotic cells in t

Water intoxication after oxytocin-induced midtrimester abortion.

Two cases of water intoxication assoicated with midtrimester induction of abortion with oxytocin are presented. Both patients recovered from this potentially fatal condition which is associated with acute hyponatraemia. Some of the pertinent literature is reviewed and the possible mechanisms of causation and measures for prevention and treatment are discussed.

[Dexamethasone-21-phosphate in the treatment of cerebral edema. Evaluation of the effects on diuresis, natremia and natriuria].

An evaluation was made of variations in blood and urinary sodium following the administration of 24 mg/day desamethasone-21-phosphate for at least 9 days, starting from admission, as cerebral antioedemigenic therapy in 30 patients with nerve traumas. The changes observed were within normal limits. This absence of interference with the hydro-electrolyte balance is a strong point in favour of the use of the drug in the treatment of cerebral oedema. The exact mechanism of action of this type of glycocorticoid, however, is still unknown.

Reye's syndrome associated with respiratory syncytial virus infection.

An upper respiratory tract infection in a 22-month-old boy was followed by rapid loss of consciousness, hypoglycaemia, uraemia, and death. Necropsy examination showed fatty change of liver and kidneys, severe cerebral oedema, bronchiolitis, and endocardial fibroelastosis affecting the left ventricle. Immunofluorescence staining showed infection with respiratory syncytial virus (RSV). The clinical and pathological findings were those of Reye's syndrome, not previously reported accompanying RSV infection.

[Renocerebral oxalosis following glycerin infusion].

At the autopsy of a 55 years old male patient renocerebral oxalosis was found, the most frequent cause of which--the ethylenglycol poisoning could be excluded by toxicological examination of the organs. Authors suppose that intravenous administration of 500 ml of 10 per cent solution of glycerol several hours before the death for the treatment of cerebral oedema might have caused the renocerebral oxalosis. The fact that in the catabolism of glycerol, ethylenglycol and xylit causing most frequently renocerebral oxalosis glycoxylic acid, precursor of the oxalic acid in the organism is formed, se

[Justification of bitemporal decompressive trepanation with duroplasty in severe closed craniocerebral injury].

In serious craniocerebral traumata in which the cerebral oedema cannot be controlled by conservative methods, the authors again recommend the performance of a bitemporal relieving trepanation reaching far in basal direction (adults 5 cm ø, children 4 cm ø). In case of closed traumata, the operation is started on the more affected side, in case of open injuries trepanation is first carried out on the other side. Detailed indications about the accompanying oedema treatment. The wide opened dura is covered with the temporalis fascia or with a free transplant.

Effect of haemodilution on experimental cerebral oedema in cats.

The effect of haemodilution with Dextran 60 on experimental cerebral oedema produced by the modified method of Ishii, Hayner, Kelly & Evans (1959) is described. The intracranial pressure was measured from the cisterna magna or epidurally in the parieto-occipital region. In the group of cats subjected to haemodilution, with a haematocrit of 27.20 +/- 1.48%, the intracranial pressure first increased by 500% and, after 1.5 h, began to fall gradually so that at the end of the experiment it was only 160% of initial value. In the other experimental group in which haemodilution with a haematocrit of
