From lactate to lactylation: novel pathological mechanisms and potential therapeutic targets for high-altitude cerebral oedema.

BACKGROUND: High altitude cerebral edema (HACE), a fatal terminal stage of acute mountain sickness (AMS), is triggered by rapid exposure to hypoxia at high altitudes. The pathophysiology of HACE is complex, involving multiple key processes including energy metabolism disorders, oxidative stress, blood-brain barrier (BBB) injury, and neuroinflammation, all of which interact to drive disease progression. Lactylation, a novel epigenetic regulatory mechanism discovered in 2019, provides a fresh perspective for HACE research. METHODS: This study integrates the latest research findings on the patho

Opioid-induced respiratory depression in cosmetic surgery: A medico-legal analysis of a fatal adverse event.

Aesthetic surgery is an elective procedure intended to enhance physical appearance under controlled and safe conditions. In the eyes of public opinion, life-threatening complications are considered unacceptable, as they violate the core expectation of safety. The authors present a case of death following a cosmetic surgery procedure and offer a literature review on fatalities associated with this type of intervention. The case is of particular interest due to the clinical and medico-legal methodological issues it raises. A middle-aged woman underwent major elective cosmetic surgery under local

Traumatic intraventricular haemorrhages: clinical indicators determining morbidity and mortality in paediatric patients.

UNLABELLED: Intracranial haemorrhages and traumatic brain injury are significant causes of morbidity and mortality in paediatric patients. Traumatic intraventricular haemorrhage (tIVH) is the least common type of intracranial haemorrhage and typically occurs in association with other intracranial injuries. This study aimed to describe the clinical characteristics of children diagnosed with tIVH, and to identify risk factors associated with poor prognosis. Paediatric patients diagnosed with tIVH in the paediatric emergency department between January 2010 and December 2024 were retrospectively r

The role of transcranial doppler in localizing stroke lesion: A valuable tool in low resource setting.

The use of transcranial Doppler (TCD) ultrasound in acute cerebrovascular events has not been widely adopted although it can help clinicians make informed decisions in the early evaluation of patients. A 74-year-old female presented to the emergency department with an acute onset of weakness of the left upper and lower limbs, along with slurring of speech, for the last 12 hours. Non-contrast CT of the head showed a hypodense area in the right frontotemporal region, suggestive of cerebral oedema. A TCD study through the transtemporal window showed increased pulsatility index (PI) of right inter

Hydration Care After Stroke: A Systematic Review of International Clinical Practice Guidelines.

Background/Objectives: Hydration status at the time of stroke has been identified as a predictor of both vital and functional prognosis. Many studies have demonstrated that dehydration is associated with poorer outcomes, yet the prevalence of dehydration in those affected by stroke remains high. In this review, we systematically identify, appraise and summarise international clinical practice recommendations regarding hydration care after stroke. Methods: International clinical practice guidelines, published since 2009, were identified through a combination of searches of four online databases

Advances in imaging studies of contusion penumbra.

Traumatic brain injury (TBI) is a serious human health hazard. The contusion penumbra (CP) is the area of injury that lies between the core of the injury and normal-looking brain tissue. CP is the main site where secondary injury occurs. Additionally, it offers a major therapeutic target for preserving neurological function post-TBI. Using different imaging modalities, specific cellular and tissue changes in CP as well as contusion core and surrounding normal brain tissue were evaluated. Even though the existing imaging techniques are widely used in clinics, these techniques often do not chara

Computed Tomographic Patterns of Traumatic Brain Injury in a Nigerian Tertiary Hospital.

BACKGROUND: Traumatic brain injury (TBI) remains a major cause of morbidity and mortality worldwide, particularly in low- and middle-income countries where road traffic accidents constitute a significant public health burden. Neuroimaging, especially computed tomography (CT), plays a pivotal role in the early diagnosis, management, and prognostication of TBI. Understanding the spectrum of neuroimaging findings in affected patients is essential for improving patient care and resource allocation, which is the aim of this study. METHODOLOGY: This retrospective descriptive study reviewed cranial

An Update on Osmotherapy in Acute Brain Injury: Current and Emerging Strategies.

Intracranial hypertension is a major modifiable pathway of secondary brain injury after acute neurological insults. Osmotherapy is an early cornerstone of tiered intracranial pressure management. Hypertonic saline and mannitol reliably reduce intracranial pressure through osmotic reduction in brain water, with additional haemodynamic and rheological effects. However, neither agent has shown consistent improvement in survival or long-term neurological recovery. Current practice is therefore guided by physiological rather than patient-centred outcomes. This review summarises the physiological ba

Predictors of seizures after endovascular aneurysm exclusion: an updated systematic review and meta-analysis.

UNLABELLED: Endovascular aneurysm exclusion has revolutionized neurovascular care, yet postoperative seizures remain a significant cause of morbidity. As endovascular technologies evolve from simple coiling to complex flow diversion, identifying factors associated with postoperative seizures is essential for improving risk stratification and neurocritical care. This systematic review and meta-analysis, registered in PROSPERO (CRD420261375370), followed PRISMA 2020 guidelines. A comprehensive search of MEDLINE, Scopus, Embase, and the Cochrane Library was conducted through April 13, 2026. Studi

ACP5-positive macrophages contribute to cerebral oedema and neuroinflammation after traumatic brain injury.

BACKGROUND: Traumatic brain injury (TBI) is a leading cause of death and long-term disability worldwide. Secondary cerebral oedema, driven by neuroinflammation and blood-brain barrier (BBB) disruption, critically determines poor outcomes, yet effective targeted therapies remain absent. This study aimed to identify immune cell subsets associated with post-TBI cerebral oedema and to explore their functional contributions in preclinical models. METHODS: We integrated single-cell transcriptomic datasets from a mouse TBI model (GSE175430) and human intracerebral haemorrhage (ICH) perihaematomal oe

Diabetic Ketoacidosis Complicated by Cerebral and Pulmonary Oedema in an Adult.

Diabetic ketoacidosis (DKA) is a life-threatening metabolic emergency. While cerebral and pulmonary oedema are recognised complications in paediatric populations, they are exceptionally rare and poorly characterised in adults. This case report describes an adult patient with severe DKA who developed both cerebral and pulmonary oedema during treatment. A previously healthy 36-year-old man presented with a two-week history of polyuria and polydipsia and progressive dyspnoea. He was alert and tachycardic and exhibited Kussmaul respirations. Initial investigations confirmed severe DKA: serum gluco

Resveratrol and Curcumin in Stroke Therapy: From Experimental Evidence to Clinical Perspectives.

Stroke remains one of the leading causes of death and long-term neurological disability worldwide, while currently available therapeutic strategies are limited by narrow treatment windows and incomplete neuroprotection. In this context, plant-derived polyphenols have attracted increasing attention as potential adjunctive agents because of their multimodal biological activity. This review focuses on resveratrol and curcumin, two of the most extensively investigated polyphenols, and evaluates their potential role in the prevention and treatment of ischaemic and haemorrhagic stroke. Evidence from

Sequential NIHSS-NLR monitoring after endovascular thrombectomy: malignant cerebral oedema and inflammatory futility in a two-phase derivation study.

INTRODUCTION: Successful reperfusion after endovascular thrombectomy (EVT) does not guarantee recovery. Malignant cerebral oedema (MCE) complicates approximately one in eight patients, and static pre-procedural models do not capture evolving post-reperfusion risk. We evaluated a two-phase sequential clinical-inflammatory monitoring framework. METHODS: We conducted a retrospective consecutive-patient cohort study at Xiangya Hospital (2021-2025) including 306 successfully reperfused EVT patients. Phase 1 combined the 24-h NIHSS trajectory with neutrophil-to-lymphocyte ratio (NLR) direction for
