The Relationship Between Inflammation and the Development of Cerebral Ischaemia and Hypoxia in Traumatic Brain Injury-A Narrative Review.

Traumatic brain injuries (TBI) represent a leading cause of morbidity and mortality globally. Whilst clinical care has significantly improved in recent years, there is still significant scope to improve patient outcomes, particularly in relation to quality of life. However, there is a window of opportunity for clinical intervention, since most of the mortality and morbidity is associated with secondary injury processes that arise after the initial trauma. In the brain, as with any tissue, inflammation plays an important role in the response to injury. However, particularly with severe injuries

Association between CHD and cerebral abscess: insights from a national database.

CHD has been historically associated with the development of cerebral abscess. This retrospective cohort study examines the association of CHD with the occurrence of cerebral abscess in an inpatient paediatric population. We analysed data from the Pediatric Health Information System database, an administrative database that captures data from participating children's hospitals in the United States. We included all patients admitted to participating paediatric ICUs between 2016 and 2021 for a total of 426,029 admissions. Including all admissions, 1,387 (0.3%) patients experienced a cerebral abs

Analysis of factors influencing short-term outcomes after early interventional therapy for acute ischaemic stroke.

OBJECTIVE: This study aimed to investigate the potential factors influencing the early efficacy outcomes of patients with acute ischaemic stroke (AIS) after endovascular interventional therapy. METHODS: A retrospective analysis was conducted involving 326 patients with AIS. The early efficacy outcomes and complications among patients after emergency cerebrovascular intervention were evaluated. Univariate analysis was performed to compare the baseline data between good outcome and poor outcome groups. Binary logistic regression analysis was used to determine the independent risk factors for po

Unmasking a Cerebral Arteriovenous Malformation Presenting as a Chronic Migraine-Type Headache at a District General Hospital.

A female in her 50s was referred by her General Practitioner (GP) to our Same Day Emergency Care (SDEC) unit due to an unresponsive migraine attack. Associated features included nausea, photophobia, phonophobia, intermittent visual aura, and unintentional weight loss over the last two weeks. Arteriovenous malformations (AVMs) are rare vascular anomalies, with a prevalence of approximately 18 per 100,000 individuals. Although often asymptomatic, they may present atypically with migraine-like headaches, posing a diagnostic challenge. Her full neurological assessment, including cranial nerves, ce

Malignant MCA Stroke in a 4-Year-Old Child: Early Decompressive Surgery as a Life-Saving Intervention.

Malignant middle cerebral artery (MCA) infarction is a rare but life-threatening condition in children. We report a case of a previously healthy 4-year-old who developed malignant left MCA infarction following bronchopneumonia. One week into bronchopneumonia, the child experienced a prolonged right-sided focal motor seizure and a reduced level of consciousness (GCS 11/15). Brain CT revealed a large left MCA infarct with mild oedema. Seventy-two hours later, the child clinically deteriorated, and repeat imaging showed midline shift with signs of impending uncal herniation. Emergency decompressi

Pediatric diabetic ketoacidosis and cerebral oedema: a systematic review.

Diabetic ketoacidosis (DKA) is present in about 30% of pediatric patients diagnosed with new-onset type 1 diabetes. Among such patients, cerebral oedema is relatively uncommon (<1%), representing about 0.1%-0.3% of deaths. However, 25% of these survivors of cerebral oedema suffer permanent neurologic deficits. Clinical presentation may be nonspecific, but the laboratory parameters of hyperglycaemia and ketosis are present in almost all cases, and treatment mainstay depends on intravenous fluids and insulin. The pathophysiology of cerebral oedema and related injury to the brain cells is not ful

Severe metabolic acidosis with elevated lactate and multi-organ dysfunction secondary to ethylene glycol toxicity.

This case report describes a male in his early 40s presenting with severe ethylene glycol (EG) poisoning from antifreeze ingestion, marked by profound metabolic acidosis, hyperkalaemia and multi-organ failure. The patient was found unconscious in the garage with shallow breathing and hypothermia, progressing to critical illness despite aggressive resuscitative measures. Diagnostic evaluations revealed high anion gap metabolic acidosis, elevated lactate, hyperkalaemia and a high osmolal gap, confirmed to be due to EG toxicity. Immediate interventions included continuous renal replacement therap

A Rare Presentation of HLH as acute encephalopathy, fever and Diabetic Ketoacidosis (DKA) followed by massive pulmonary embolism: Diagnostic and Therapeutic Challenges.

BACKGROUND: Hemophagocytic lymphohistiocytosis (HLH) is a rare, life‑threatening hyperinflammatory syndrome that often presents with nonspecific features, delaying diagnosis and treatment. Although fever, cytopenias, and organomegaly are classic, unusual presentations can obscure recognition. We report an exceptional case of secondary HLH manifesting as severe diabetic ketoacidosis (DKA) with cerebral edema in a young man with type 1 diabetes, later complicated by a massive saddle pulmonary embolism. CASE PRESENTATION: A 25‑year‑old man with a history of type 1 diabetes mellitus (HbA₁c 10.4%)

The treatment of acute symptomatic hyponatraemia in the hospital setting.

Acute symptomatic hyponatraemia results in potentially fatal cerebral oedema, whereas overly rapid correction of hyponatraemia can cause osmotic demyelination syndrome (ODS) with permanent neurological damage. To balance these two risks, we recommend a limited rapid increase of serum sodium level by at least 5 mmol/l by administration of fixed bolus(es) of hypertonic saline (HTS) to reverse symptoms of cerebral oedema, while limiting total increase to 8-10 mmol/l in the first 24 h, and each subsequent 24 h period. Neurological status, urine output, and biochemistry should be carefully monitore

Utility of transcranial Doppler and reversed jugular venous saturations for neuromonitoring in children with acute liver failure.

UNLABELLED: Paediatric Acute Liver Failure (PALF) is associated with high morbidity and mortality, with cerebral oedema a key contributor to mortality. The aim of our study was to examine the association between the findings of two neuromonitoring modalities (transcranial Doppler (TCD) and reverse venous jugular oxygen saturation (SjvO2) and mortality, as well as ammonia levels, in children with acute liver failure who received neuroprotection within 7 days of paediatric intensive care unit (PICU) admission. Acute subdural haematoma in sudden infant death: diagnostic challenges in a low-resource setting; non-accidental injury, post-mortem or resuscitation artefact?

Acute subdural haemorrhage in sudden infant death (SID) presents a forensic challenge, particularly in resource-limited settings, where distinguishing true injury from resuscitation or post-mortem artefacts is difficult. We report a case of a previously healthy 2-month-old male infant found unresponsive shortly after being placed prone following breastfeeding. Despite prolonged resuscitation, the infant died. Autopsy revealed faint anterior chest contusions consistent with resuscitation and bilateral acute subdural haemorrhages without skull fractures, retinal haemorrhages or cerebral oedema.

Bilateral Cerebellopontine Angle Masses in a 72-Year-Old Woman With Prior Malignant Melanoma and a Cochlear Implant: A Diagnostic and Management Challenge.

Cerebellopontine angle masses are most frequently vestibular schwannomas, but may also represent metastatic disease, particularly in patients with a history of malignant melanoma. We describe the case of a 72-year-old woman with a prior history of melanoma and a right cochlear implant who presented with rapidly progressive bilateral hearing loss, visual impairment, imbalance, and right-sided facial weakness. An initial non-contrast CT scan was reported as normal, although a later multidisciplinary review identified bilateral cerebellopontine angle lesions. MRI, the gold standard for characteri

Brain Shift Patterns: Upward, Lateral and Downward Herniation, Its Correlation with Clinical Patterns in Acute Intracranial Pathologies and Neurosurgical Management.

Brain shift is a critical radiological indicator of intracranial mass effect, resulting from haematoma, tumour, infarction, or cerebral oedema. Depending on the direction of displacement, herniation may occur as downward transtentorial, upward (reverse transtentorial), or lateral midline shift (MLS). This study aims to classify brain shift patterns and correlate radiological findings with clinical parameters, including level of consciousness, Glasgow Coma Scale (GCS), pupillary size and reactivity, and predicted intracranial pressure (ICP). Radiological data from CT and MRI were analysed along

Association between ASPECTS region of infarction and clinical outcome in non-acute large vessel occlusion ischaemic stroke after endovascular recanalisation.

PURPOSE: This study retrospectively investigated whether infarction in specific Alberta Stroke Program Early CT Score (ASPECTS) regions is associated with clinical outcome in patients with symptomatic non-acute internal carotid or middle cerebral artery occlusion who underwent endovascular recanalisation (ER). METHODS: Preoperative ASPECTS and region of infarction were recorded before recanalisation. Clinical outcome was evaluated 90 days after the procedure using the modified Rankin Scale; a score>2 was defined as poor outcome. Secondary outcomes included postprocedural cerebral oedema, intr

Listeria monocytogenes meningitis in immunocompetent children: A retrospective single-centre study.

OBJECTIVES: Listeria monocytogenes meningitis has traditionally been associated with specific vulnerable populations such as newborns, pregnant women, immunosuppressed transplant recipients, and older adults with compromised cell-mediated immunity. Instances involving otherwise healthy children without underlying immunological disorders are infrequently reported and considered unusual. METHODS: The medical records, clinical courses, laboratory findings, medical imaging, and responses to treatment of immunocompetent children diagnosed with Listeria monocytogenes meningitis were retrospectively

Neurovascular Pathophysiology and Emerging Biomarkers in Cerebral Malaria: An Integrative Perspective.

Cerebral malaria is a life-threatening neurological complication of Plasmodium falciparum infection and a leading cause of pediatric mortality in endemic regions of sub-Saharan Africa. It is defined clinically by coma accompanied by peripheral parasitemia, without alternative causes. Pathogenetically, cytoadherence of parasitized erythrocytes in the cerebral microvasculature, together with a widespread inflammatory response and endothelial activation, causes profound microvascular injury. This injury includes disruption of the blood-brain barrier and the development of multifactorial cerebral
