Respiratory failure as a seizure phenomenon. A case report.

Recurrent acute respiratory failure in an epileptic subject is described. The first episode of respiratory failure occurred while the patient was having frequent epileptic fits and was probably secondary to cerebral oedema with temporal herniation. The second occurred suddenly after 27 days during which the patient had been free from seizures. It is suggested that this episode of acute respiratory failure was the result of an epileptic seizure without any motor symptoms.

[Diabetic coma as a task for intensive therapy].

On the basis of 763 cases with diabetic coma from 1960 to 1973 is referred to the advantages of the foundation of a coma ward as department of intensive observation. It is reported on some successful changes of therapy in recent years. The introduction of methods of intensive therapy has led to a significant decrease of lethality. A sufficient administration of potassium (not less than 200 mval/die) and the prevention of a cerebral oedema are actual demands for the improvement of the prognosis of diabetic coma.

Causes of death in fulminant hepatic failure and relationship to quantitative histological assessment of parenchymal damage.

The clinical course and causes of death in 132 consecutive patients with fulminant hepatic failure and grade III or IV encephalopathy have been reviewed. 105 patients died and in 96 of these an autopsy examination was performed. In 36 patients there was cerebral oedema and the mean age of this group was significantly younger than the other fatal cases. In 28 patients death was attributed to major haemorrhage which originated in the gastrointestinal tract in 25. The prothrombin time ratio was not significantly greater in patients with major bleeding than in those without but they did have a sig

[Chronic secondary hypoparathyroidism and pseudo-brain tumour (author's transl)].

A 31-year-old man developed chronic secondary hypoparathyroidism after removal of goitre. Asymmetric cerebral oedema occured and a classical picture of pseudo-brain tumour with severe cerebral involvement developed which regressed merely on administration of calcium ions. The patient has remained well for over a year on dihydrotachysterol maintenance.

[Puttman-Reye syndrome and ekiri. Neuro-anatomical study and attempt at classification].

The authors report two cases of Puttman Reye syndrome: one, classical, involving a young child with a fatal encephalopathy with visceral fatty infiltration, the other, less typical, not showing any signs of visceral fatty degeneration. Both cases were subjected to complete anatomical examination of the brain, revealing marked cerebral oedema, in localised areas or in a laminar distribution, with degenerative changes in the neurones and slight demyelinisation. The authors, on the basis both on the classical appearances of the first case as well as the atypical features of the second, envisage a

Anticerebral oedema therapy in Reye's syndrome.

Sixteen patients with Reye's syndrome were seen over a 3-year period and treated with conservative management with an overall mortality of 31%. 6 of the 16 had illness limited to stages II and III. Intensive therapy of suspected cerebral oedema, with careful monitoring of serum osmolarity, was associated with survival in 5 of 10 patients with stage IV disease. The risks of lumbar puncture in the presence of cerebral oedema (stages III-IV) are emphasized. A change in clinical status (usually a reversal to a less severe stage and an improved electroencephalogram) was seen within hours of startin

Hyperventilation in neurosurgery.

An outline of the nature and varieties of hyperventilation is presented together with a discussion on the role of artificial hyperventilation in the management of neurosurgical patients. Attention is called to the value of gasometric investigations in the ventricular cerebrospinal fluid for evaluation of disturbances in pH of the intracranial environment and possible effectiveness of hyperventilation. The results of our measurements of cerebrospinal fluid pressure are presented in 21 cases of supratentorial cerebral tumours in which controlled ventilation with hyperventilation was conducted. I

[Potassium canreonate in neurosurgical brain edema].

The effect on the hydroelectrolytic metabolism and on the evolution of cerebral oedema of a new injectable aldosterone-antagonist, canreonate-potassium (Soldactone) was studied in two groups of subjects. The first group consisted of 14 patients who had undergone neurosurgical operations for expansive endocranial processes. The second group consisted of 18 non-operated cases of post-traumatic coma. The water-salt balance and the neurologic picture were carefully controlled during anf after 6-8 days of treatment with 600 mg/day of canrenoate-potassium by intravenous route. Comparison was also ma

Controlled trial of glycerol versus dexamethasone in the treatment of cerebral oedema in acute cerebral infarction.

10 percent glycerol was given for 6 days to 30 patients who had had acute ischaemic cerebral infarction, and the results were compared with those obtained after treating 31 similar patients with dexamethasone (16 mg. per 24 hours for 6 days). 1 patient treated with glycerol died of haemoglobinuria and acute renal failure. 6 patients treated with dexamethasone died--3 from cerebral oedema and 3 from non-neurological complications (pulmonary embolism, myocardial infarction, and aspiration pneumonia). Improvement was significantly greater in the glycerol group after 8 and 15 days. No improvement

[Case of postpregnancy diencephalosis with manifestations of psudotumor cerebri].

The authors report a female patient with postpregnancy diencephalosis in whom in close correlation with premenstrual tension syndrome acute cerebral oedema developed repeatedly with signs of tentorial herniation. Owing to treatment with substitutive cyclopeptide hormones stimulating secondarily the failing hypophysis complete disappearance of these signs was obtained.

Sudden death in rheumatoid arthritis with atlanto-axial dislocation.

A post mortem material of 11 consecutive cases of severe atlanto-axial dislocation (a.a.d.) with cord compression is reported. The total number of deaths from rheumatoid arthritis (RA) during the period of 5 years was 104, and all were autopsied. Neurological symptoms correlated poorly to fatal a.a.d. Hemiplegia was found in three cases, one of which, however, was caused by thrombosis cerebri. Spastic signs were transiently recorded in two patients and dysphagia in a further two. Five patients had a history of recent vomiting. A.a.d. was the sole or main cause of death in 8 cases and contribut

[Morphological aspects of organic brain syndromes (author's transl)].

Organic psychoses are caused by a variety of disorders. In general, they are due to diffuse dysfunction of the brain without any specific anatomical basis, but they may be modified by disorders of some distinct neuronal systems as a result of local accentuation of a dissuse morbid process or localized brain damage. Acute organic psychoses are usually caused by disorders of the blood-brain barrier (cerebral oedema), acute neuronal dysfunction of disorders of synaptic transmission. Acute lesions may be reversible or terminate in stationary defective states or progressive neuronal degeneration. I

Cerebral Malaria.

Cerebral malaria is an acute diffuse encephalopathy associated only with Plasmodium falciparum. It is probably a consequence of the rapid proliferation of the parasites in the body of man in relation to red cell invasion, and results in stagnation of blood flow in cerebralcapillaries with thromobotic occlusion of large numbers of cerebral capillaries. The subsequent cerebral pathology is cerebral infarction with haemorrhage and cerebral oedema. The wide prevalence of P. falciparum in highly endemic areas results in daily challenges to patients from several infected mosquitoes. It is thus impor

Cerebral form of high-altitude illness.

Twelve cases of severe altitude illness are reported in which the neurological signs and symptoms dominated the clinical picture. Pulmonary oedema, retinal haemorrhage, thrombophlebitis and pulmonary embolism, bronchopneumonia, and coronary-artery disease were also present in several of the patients but the primary problem seems to have been cerebral oedema. Other published cases support this impression. Patients who were returned to low altitude early in the disease fared well; two patients died, and in both cases evacuation had been delayed. The most effective prevention lies in slow ascent,

Predicting the outcome of stroke: acute stage after cerebral infarction.

On admission to hospital during the acute phase of a stroke presumed due to ischaemic infarction in one cerebral hemisphere 93 patients were examined to determine the factors associated with a poor prognosis for immediate survival. The patients particularly at risk were those who were overtly unconscious and those with any combination of impaired consciousness, dense hemiplegia, and failure of conjugate ocular gaze towards the side of the limb weakness. Necropsy evidence suggested that these signs usually indicate infarction of the whole of one middle cerebral artery territory which is often s
