Potentially Hazardous Drugs in the Paediatric ICU: A Narrative Review on the Exemplary Cases of Propofol, Chloramphenicol, and Acetylsalicylic Acid.

Background: In the paediatric intensive care unit (PICU), certain drugs should be avoided or administered with strict precautions and close monitoring. This is due to their potential for toxicity or adverse effects or a lack of safety data, especially for critically ill children with organ failure. Additionally, practitioners must assess the unique pharmacokinetic and pharmacodynamic properties of drugs when treating critically ill children. In this narrative review, we highlight the risks, advantages, and disadvantages of three exemplary cases of drugs for paediatric patients hospitalised in

Understanding the microRNA signature associated with Reye's syndrome.

Reye's syndrome, an uncommon and potentially deadly childhood disease, is characterized by sudden noninflammatory brain dysfunction combined with hepatic dysfunction caused by the accumulation of fat. It was initially identified by Australian pathologist R.D.K. Reye in 1963. National monitoring of Reye's syndrome commenced in the United States in the early 1970s, resulting in strict cautions about the use of aspirin in children. MicroRNA (miRNA), small (~ 22 nt) non-coding RNA that regulate gene expression by reducing mRNA levels, plays a critical role in the onset and advancement of Reye's sy

Potential alternatives in treatment of kawasaki disease to reduce the extremely rare risk of reye syndrome.

Background/objective: Kawasaki disease is an acute, systemic vasculitis most commonly observed in young Asian children. While the exact pathogenesis remains unknown, it is thought to be triggered by a pathogenic exposure that elicits an inflammatory response several days to 2 weeks before the onset of symptoms. Symptoms include edematous hands and feet, conjunctivitis, mucositis, a diffuse erythematous rash, lymphadenopathy, and a fever lasting for at least 5 days, which is required for diagnosis. In addition, coronary vessels may exhibit aneurysms, thrombosis, and stenosis. Without treatment,

Adult-Onset Reye's Syndrome and the Risks of Low-Dose Aspirin Rechallenge: A Review of Current Literature and Future Directions.

Reye's syndrome is a rare encephalopathy with liver involvement correlated with salicylate drug use in the setting of many viral infections, especially influenza B and varicella, which are disproportionately prevalent in pediatric patients. Reye's syndrome has declined markedly since the 1960s due to public health campaigns and the adoption of alternative antipyretics. The risks associated with salicylate use in adult patients who survived childhood Reye's syndrome are unknown. We review the current literature regarding the prevalence of adult-onset Reye's syndrome, cases of recurrence, and lo

Inhibition Conversion of Aspirin into Salicylic Acid in Presence of Glycine.

Aspirin (AS) is a common drug having anti-pyretic and anti-inflammatory properties which is widely used in diverse medical conditions. The intake of AS may cause adverse effects such as gastrointestinal ulcer, tinnitus and Reye's syndrome. The adverse effects of AS arise due to conversion of AS into salicylic acid (SAL). Glycine (Gly) is a simplest non essential amino acid having anti-oxidative and anti-inflammatory effects. It also reduces the risk of obesity, hypertension, and diabetes mellitus. AS with Gly is well accepted form of the drug for the treatment of rheumatic conditions in compar

Unveiling the Influence of Glutathione in Suppressing the Conversion of Aspirin to Salicylic Acid: A Fluorescence and DFT Study.

Aspirin is a commonly used nonsteroidal anti-inflammatory drug, associated with many adverse effects. The adverse effects of aspirin such as tinnitus, Reye's syndrome and gastrointestinal bleeding are caused due to conversion of aspirin into its active metabolite salicylic acid after oral intake. Glutathione is a naturally occurring antioxidant produced by the liver and nerve cells in the central nervous system. It helps to metabolize toxins, break down free radicles, and support immune function. This study aims to investigate and explore the possibility of inhibiting aspirin to salicylic acid

Pharmacokinetics of aspirin: evaluating shortcomings in the literature.

INTRODUCTION: Aspirin is known for its therapeutic benefits in preventing strokes and relieving pain. However, it is toxic to some individuals, and the biological mechanisms causing toxicity are unknown. Limited literature is available on the role of glycine conjugation as the principal pathway in aspirin detoxification. Previous studies have quantified this two-step enzyme reaction as a singular enzymatic process. Consequently, the individual contributions of these enzymes to the kinetics remain unclear. AREAS COVERED: This review summarized the available information on the pharmacokinetics

Why Pharmacovigilance of Non-steroidal Anti-inflammatory Drugs is Important in India?

BACKGROUND: Non-steroidal Anti-Inflammatory Drugs (NSAIDs) are among the drugs that are most regularly administered to manage inflammation and pain. Over-the-Counter (OTC) NSAIDs are widely accessible, particularly in developing countries like India. This casual approach to using NSAIDs may operate as a magnet for NSAID-related adverse drug reactions (ADRs) among patients. OBJECTIVES: As patients in India are less informed about the appropriate use of NSAIDs and consumption patttern, adverse drug reactions, and the importance of reporting ADRs, the current study's objective is to promote pati

Pharmacogenetic Analysis of an 8-Year Old Girl with Reye Syndrome Associated with Use of Naproxen.

BACKGROUND Reye syndrome is a rare, yet potentially life-threatening disease characterized by acute encephalopathy and hepatic failure. This report presents the case of an 8-year-old girl with Reye syndrome and seizures after the use of naproxen. CASE REPORT An 8-year-old girl experienced a 3-day episode of fever and abdominal pain. After receiving naproxen (375 mg twice daily) starting from day -3, she exhibited hypotension, tonic seizure, and loss of consciousness (day 1). Physical examination and laboratory test results revealed acute kidney injury, metabolic acidosis, and elevated levels o

Hypothesis: 2 Major Environmental and Pharmaceutical Factors-Acetaminophen Exposure and Gastrointestinal Overgrowth of Clostridia Bacteria Induced By Ingestion of Glyphosate-Contaminated Foods-Dysregulate the Developmental Protein Sonic Hedgehog and Are Major Causes of Autism.

Epidemiological studies have found 2 significant factors associated with the increased incidence of autism spectrum disorder (ASD): the increased use of acetaminophen in the 1970s when this drug largely replaced the use of aspirin for many patients because of a fear of Reye syndrome, and the agricultural use in the 1990s of the herbicide glyphosate on crops that were genetically modified (GM) to tolerate glyphosate. The incidence of autism in the United States, where acetaminophen is widely available, is more than 1000 times greater than in Cuba, where acetaminophen is available only by prescr

The contribution of epidemiology to the understanding of neurodevelopmental disabilities.

Epidemiological approaches have played an important role in creating better understanding of developmental disabilities by delineating their frequency in populations and changes in their frequency over time, by identifying etiological factors, and by documenting pathways to prevention. Both cerebral palsy (CP) and mild intellectual disability are declining in frequency in high-income countries. The diagnosis of autism spectrum disorder has increased in recent decades, but much of this increase is a result of changing approaches to ascertainment and recording. Epidemiological studies have found

Maximum Matchings in Geometric Intersection Graphs.

The use of NSAIDs has become routine for adults and children in the management of pain. NSAIDs (other than aspirin [acetylsalicylic acid]) are also enjoying greater popularity as antipyretics since the recognition of Reye's syndrome's putative association with aspirin. In neonates, NSAIDs have been used for many years in an attempt to pharmacologically close the ductus arteriosus. This review examines the various NSAIDs and their potential and real applications in the neonatal population. For completeness, acetaminophen (paracetamol), which has weak NSAID activity and is a widely used analgesi

What are the clues for an inherited metabolic disorder in Reye syndrome? A single Centre study of 58 children.

OBJECTIVES: Reye Syndrome is an acute encephalopathy with increased liver enzymes and blood ammonia, without jaundice. The prevalence of an underlying inherited metabolic disorder (IMD) is unclear, nor the clinical or biological factors directing toward this diagnosis. Our aims were to define these clues in a large series of patients. PATIENTS AND METHODS: We retrospectively studied all patients with Reye admitted in our institution from 1995. We defined 3 groups: Group 1 with a confirmed IMD, Group 2 considered as free of IMD, Group 3 unclassified. Statistical analysis compared patients in G

Regulation of the CoA Biosynthetic Complex Assembly in Mammalian Cells.

Coenzyme A (CoA) is an essential cofactor present in all living cells. Under physiological conditions, CoA mainly functions to generate metabolically active CoA thioesters, which are indispensable for cellular metabolism, the regulation of gene expression, and the biosynthesis of neurotransmitters. When cells are exposed to oxidative or metabolic stress, CoA acts as an important cellular antioxidant that protects protein thiols from overoxidation, and this function is mediated by protein CoAlation. CoA and its derivatives are strictly maintained at levels controlled by nutrients, hormones, met

Kawasaki disease and influenza-new lessons from old associations.

Kawasaki disease (KD), an enigmatic medium vessel vasculitis, presents as an acute febrile illness predominantly affecting young children. KD appears to be a hyper-inflammatory response elicited by environmental or infectious agents (including respiratory viruses) in genetically predisposed individuals. Numerous reports from the current era of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic have described the occurrence of KD/KD-like illness in close temporal proximity to SARS-CoV-2 infection or exposure. Notably, KD has been reported in association with H1N1-pdm09 virus
