A Clearance-Repair-Activation Framework for Alzheimer's Disease: A Perspective and Validation Roadmap.

Alzheimer's disease (AD) is a heterogeneous clinicobiological syndrome involving impaired protein clearance, neuroinflammation, synaptic and cellular injury, network dysfunction, and progressive functional decline. Amyloid-directed antibodies provide an evidence-based treatment backbone for selected patients with early symptomatic AD, but adjunctive strategies may be needed to address other disease mechanisms. This Perspective proposes a clearance-repair-activation framework for hypothesis-driven research. Clearance includes established pharmacological treatment and risk management as well as

Application of IgA-Seq in schizophrenia to identify key gut bacteria that promote abnormal behavior in mice.

Schizophrenia (SCZ) has a complex etiology, and growing evidence implicates the microbiota-gut-brain axis. Because abundance-based profiling does not identify taxa closely engaged by mucosal immunity, we used immunoglobulin A (IgA)-based bacterial sorting to prioritize candidate SCZ-associated gut bacteria and evaluated cultured consortia in mouse models. Fecal samples were collected from participants with SCZ, other mental disorders, and healthy controls, which underwent 16S rRNA gene sequencing before and after magnetic separation of IgA-coated (IgA+) and non-IgA-coated (IgA-) bacteria. Taxa

Developmentally Guided Differentiation of Ventral Midbrain Dopaminergic Cells Validated in Proteopathy-Based Parkinson's Disease Models.

Parkinson's disease (PD) involves progressive degeneration of nigrostriatal ventral midbrain dopaminergic (vmDA) neurons and Lewy body proteopathy, necessitating scalable, lineage-faithful donor cells. However, variability in human pluripotent stem cell (hPSC) differentiation and incomplete ventral midbrain identity limit translational use. Here, we establish a developmentally guided differentiation protocol that reproducibly specifies EN1-positive vmDA progenitors through temporally optimized SHH, WNT/β-catenin, and FGF8 signaling. The resulting progenitors mature into vmDA neurons exhibiting

From reference epigenomes to response epigenomes: a public-health case built on the developing brain.

Reference epigenome projects, such as the NIH roadmap, ENCODE, and the international human epigenome consortium, have started mapping the baseline regulatory states of human cell types with extraordinary detail. However, these projects were not designed to address the urgent questions posed by exposome science and child-health policy: how these states change after environmental exposure, in whom, during which developmental window, and whether they can recover. I argue that the missing object is a human response epigenome: the cell-type-specific change in epigenomic state over time following a

Tocilizumab, atezolizumab, and stereotactic radiotherapy in recurrent glioblastoma: primary analysis of safety and efficacy of a phase 2 trial, NRG Oncology-BN010.

BACKGROUND: Preclinical data suggest that fractionated stereotactic radiotherapy (FSRT) may sensitize immunologically-cold tumors to immune checkpoint inhibition by stimulating neoantigen release and that inhibition of interleukin-6 (IL-6) permits immunosuppressive tumor-associated macrophages to become immunostimulatory. This study aimed to test the safety and efficacy of the combination of tocilizumab (anti-IL6R), atezolizumab [anti-programmed death-ligand 1 (PD-L1)], and FSRT in patients with recurrent glioblastoma (rGBM). METHODS: A multicenter, single-arm, phase 2 trial was conducted wit

From heteroplasmy shift to direct base correction: mitochondrial genome editing for disease modeling and therapy.

Mitochondria are essential organelles that support cellular energy metabolism and contribute broadly to tissue homeostasis, stress responses, and disease pathophysiology. As the mitochondrial genome encodes core components of oxidative phosphorylation, genetic alterations in mitochondrial DNA (mtDNA) can profoundly affect cellular and tissue function. Nevertheless, compared with the nuclear genome, mtDNA remains difficult to interrogate and manipulate because of longstanding technical barriers to mitochondrial genome engineering. Consequently, many fundamental questions surrounding mtDNA biolo

Prioritizing clinical data for psychiatric inpatient dashboards: insights from a nationwide survey of German university centers.

INTRODUCTION: As digital data collection becomes increasingly integrated into the treatment of patients with affective disorders, the use of dashboards to visualize this information for clinicians is gaining importance. However, the question of which parameters should be prioritized for display remains largely unaddressed. This study aims to identify the parameters that physicians working in psychiatric facilities consider most important for inclusion in dashboard infrastructures supporting the inpatient care of patients with affective disorders. METHODS: From July 2024 to August 2024, we con

Randomized double-blind inpatient study of a gluten-free diet for negative symptoms in people with schizophrenia.

BACKGROUND: There are no FDA-approved treatments for negative symptoms in schizophrenia and related disorders (SRD). In an SRD subgroup with systemic and central inflammation and elevated anti-gliadin immunoglobulin G antibodies (AGA IgG+), previous findings suggested that a gluten-free diet (GFD) improved negative symptoms. METHODS: We conducted a five-week double-blind randomized placebo-controlled inpatient trial comparing a GFD versus a gluten-containing diet (GCD) in people with SRD and elevated AGA IgG+ (N = 39; n = 21 GFD and n = 18 GCD). A Single-Cell and Spatial 3D Multi-omic Atlas of Developing Human Basal Ganglia and Inhibitory Neurons.

The human basal ganglia (BG), subcortical nuclei fundamental to motor regulation and cognitive modulation, is constructed from neurons produced during gestation in the adjacent ganglionic eminences (GEs). GEs are transient structures in the ventral prenatal brain that also generate GABAergic inhibitory neurons which migrate to destinations in the BG, cortex and other destinations. This study aims to elucidate the epigenomic and 3D-genomic dynamics involved in the specification and maturation of GEs and GE-derived neurons, using single-nucleus methyl-3C sequencing (snm3C-seq), highly-multiplexe

Trends in net survival for patients with brain tumors in Japan, 2000-14 (CONCORD-3).

BACKGROUND: Brain tumors are a serious health issue, and survival is an effective indicator of how the healthcare system manages them. We examined net survival trends for patients diagnosed with a primary brain tumor during 2000-2014 in Japan, using data from 16 regional population-based cancer registries participating in the CONCORD-3 study. METHODS: We included patients aged 15-99 years, with follow-up completed either 5 years after diagnosis or up to 31 December, 2014. We estimated 5-year net survival by age group and morphological subtype using the Pohar Perme estimator, and age-standardi

Locus coeruleus modulation of neurophysiological sensory selectivity differs in autism and other mental health conditions.

Sensory symptoms are common in autism and may result from differences in sensory processing. The locus coeruleus-norepinephrine (LC-NE) system modulates sensory processing by increasing selectivity to salience. To specify this mechanism, we manipulated LC-NE activity and explored sensory selectivity in autistic and non-autistic adolescents. We assessed pupillometry and electroencephalography (EEG) in 52 autistic adolescents (ASD), 55 healthy controls (CON), and 43 adolescents with other mental health conditions (MHC) during a passive auditory oddball task. A handgrip exercise manipulated LC-NE

Assessment of clinical and neuroimaging efficacy of treatment targeting tau pathology in mild cognitive impairment and mild to moderate Alzheimer's disease with hydromethylthionine mesylate using external control data.

BACKGROUND: Hydromethylthionine mesylate (HMTM) targets tau pathology and also has tau-independent symptomatic activity. A traditional randomised placebo-controlled trial (RCT) was precluded by loss of blinding due to urinary colouration and therapeutic activity at the minimum dose required to maintain blinding. OBJECTIVE: To evaluate the efficacy of HMTM in participants with mild cognitive impairment (MCI) and mild to moderate dementia due to Alzheimer's disease (AD). METHODS: Because a traditional RCT was not feasible without loss of blinding, we compared HMTM 16 mg/day in TRx-237-039 with

The effects of GLP-1 receptor agonists on Alzheimer's pathophysiology: A systematic review.

BACKGROUND: The incidence of Alzheimer's disease (AD) is increasing globally but there are limited effective therapies available. Recently, evidence has demonstrated a role of GLP-1 receptor (GLP-1R) agonists, commonly used in the treatment of type 2 diabetes, may have therapeutic potential in AD. GLP-1R agonists have exhibited their neuroprotective role by targeting tau hyperphosphorylation and the accumulation of beta-amyloid (Aβ) plaques. This systematic review aims to evaluate the effectiveness of liraglutide, semaglutide, exenatide and dulaglutide on AD pathology with a focus on the key b

rAAV9 vector biodistribution in nonhuman primate brain and spinal cord following lumbar intrathecal infusion.

INTRODUCTION: Delivery of recombinant adeno-associated virus serotype 9 (rAAV9) into cerebrospinal fluid bypasses the blood-brain barrier and can permit efficient brain tissue transduction. Whereas intracisterna magna injection (ICM) and other routes have been considered, lumbar intrathecal (IT) delivery offers a minimally invasive approach for treating genetic disorders of the central nervous system. To evaluate central nervous system biodistribution, we quantified vector genome (vg) copies of five rAAV9 vectors in cynomolgus macaques following ICM or lumbar IT delivery. METHODS: A total of

Functional neuroanatomy of musical object processing in Alzheimer's disease and frontotemporal dementia.

Music, besides its emotional and social resonance, models a complex sensory environment exemplifying auditory objects corresponding to sources (musical instruments) and information streams (melodies). These musical dimensions can be variably preserved or blighted by neurodegenerative disease. Music is, therefore, an attractive way to investigate the neural mechanisms of sensory object processing in these diseases. Here, we assessed the functional neuroanatomy underlying the perceptual, semantic, and apperceptive processing of musical objects in Alzheimer's disease and temporal variant frontote

Digital Atlases to Unlock the Potential of Brain Biorepository Tissues for Interdisciplinary Research.

Human brain tissue preserved in biorepositories is foundational for the structural, cellular, and biomolecular research necessary for a mechanistic understanding of neurological diseases. Realizing the research potential of these valuable resources requires well-characterized research-relevant tissue that can be efficiently identified by investigators and incorporated into the conceptual and computational frameworks of interdisciplinary research. Several large-scale efforts to improve research reliability and reproducibility have sought to characterize and annotate the processes by which these
