Transcranial direct current stimulation (tDCS) in psychiatric disorders in early childhood (aged under 10 years): a systematic review.

Transcranial direct current stimulation (tDCS) remains experimental for many psychiatric disorders in adults. Particularly in childhood, there is limited research on the evidence for the efficacy and mechanisms of action of tDCS on the developing brain. The objective of this review is to identify published experimental studies to examine the efficacy and mechanisms of tDCS in children with psychiatric or developmental disorders in early (prepubertal) childhood (aged under 10 years). Included Studies should meet the following criteria: (1) experimental studies (no reviews, no case reports), (2)

Overcontrol in anorexia nervosa: Elevated prefrontal activity and amygdala connectivity in a working memory task with food distractors.

Individuals with anorexia nervosa (AN) are thought to engage in excessive amounts of self-control, which may contribute to disorder development and maintenance. This "overcontrol" may explain previous findings of increased activity and connectivity in frontal brain regions involved in top-down control functions in response to diverse stimuli including emotionally salient visual food stimuli. However, these observations were made largely in tasks demanding explicit stimulus processing. Given the omnipresence of food cues and their particular relevance for AN, it deems important to test if these

Provider Perspectives of Barriers to Delivering Care for Pediatric Functional Neurological Disorder: A Thematic Analysis.

BACKGROUND: Functional neurological disorder (FND) is a multinetwork brain disorder existing at the intersection of neurology and psychiatry. FND often takes significant time to receive diagnosis and treatment. Given these delays, the purpose of the present study was to identify barriers to FND care from the provider perspective. METHODS: The Functional Neurological Disorder Society Pediatric Special Interest Group asked pediatric FND providers to specify barriers to FND treatment in their center in the United States. Two authors conducted thematic analyses to extract themes between responden

Current interpretations of the I-PACE model of behavioral addictions.

BACKGROUND AND AIMS: The Interaction of Person-Affect-Cognition-Execution (I-PACE) model of behavioral addictions is used relatively often as a scientific framework to specify research hypotheses and to interpret empirical findings in behavioral addiction research. There are, however, controversial interpretations in the literature regarding some specific elements of the model, which may require a more precise definition of specific constructs and processes that are central to the I-PACE model. METHODS: This is neither a comprehensive literature review nor a proposal for a new version of the

An overview of the challenges with the differential diagnosis of schizotypal personality disorder.

INTRODUCTION: Schizotypal personality disorder (SPD) has a long history, and there is still considerable ongoing research. Although there are overlapping features of SPD and other personality disorders, the full constellation of schizotypal features is broader. The longitudinal course of SPD is variable, with differences in trajectory manifesting lifelong clinical significance. Particularly important is the relationship between SPD and prodromal states that may result in an eventual diagnosis of psychosis. AREAS COVERED: This review covers the history and differential diagnosis of SPD, includ

Targeting of PP2 A/GSK3β/PTEN Axis in Alzheimer Disease: The Mooting Evidence, Divine, and Devil.

Alzheimer disease (AD) is a progressive neurodegenerative disease of the brain due to extracellular accumulation of Aβ. In addition, intracellular accumulation of hyperphosphorlyated tau protein which form neurofibrillary tangle (NFT) is associated with progressive neuronal injury and the development of AD. Aβ and NFTs interact together to induce inflammation and oxidative stress which further induce neurodegeneration in AD. The exact relationship between Aβ and tau, the two proteins that accumulate within these lesions, has proven elusive. A growing body of work supports the notion that Aβ ma

The Importance of Coating Surface and Composition for Attachment and Survival of Neuronal Cells Under Mechanical Stimulation.

Cell culture of neuronal cells places high demands on the surface for these cells to adhere to and grow on. Native extracellular matrix (ECM) proteins are often applied to the cell culture surface. The substrate is even more important when mechanical strain is applied to the cells in culture. These cells will easily detach and die, precluding the study of how mechanical factors affect these cells. Mechanical factors are, for example, important in the eye disorder glaucoma, which is characterized by the loss of the retinal ganglion cells (RGCs), the retinal neurons that transfer the visual info

Effective Protection Against Status Epilepticus Caused by Lithium-Pilocarpine: Combination of Midazolam and Lacosamide.

AIM: Status epilepticus causes the most severe condition related to epilepsy in terms of high mortality rate. Although status epilepticus treatment guidelines specify a treatment process based on three-stage monotherapy, effective control cannot yet be achieved in all cases. In the presented study, with electrophysiological and behavioral tests, it was aimed to investigate the effectiveness of the combination of midazolam (MDZ), one of the most commonly used benzodiazepines in the first-line treatment of status epilepticus, with the second-line antiepileptics levetiracetam (LEV), lacosamide (L

Improving the diagnosis of OSFED across clinical and research settings: the development of a diagnostic flowchart.

Other Specified Feeding and Eating Disorder (OSFED) has the highest prevalence of any eating disorder (ED), yet limited resources are available to help clinicians and researchers reliably differentiate between the different OSFED diagnoses. A tool capable of clarifying differences between OSFED subtypes and other EDs is needed to ensure that individuals receive accurate and specific diagnoses across clinical and research settings. To this end, the present report describes the development of a diagnostic flowchart developed for use in a research laboratory to allow research assistants without s

Clinical trials for neuropsychiatric syndromes in major and mild neurocognitive disorders: A CONSORT-based approach.

Neuropsychiatric syndromes (NPS) such as agitation, psychosis, apathy, and irritability are among the most disabling features of major and mild neurocognitive disorders including Alzheimer's disease, other neurodegenerative disorders (NDD), and vascular cognitive impairment. Clinical trial methodologies for the treatment of these syndromes are evolving and the first agents to reduce NPS severity has been approved. Biomarkers are rapidly becoming available to guide clinical trial decision-making. Biomarkers can confirm the diagnosis of Alzheimer's disease and are playing a larger role in non-Al

[Update on obsessive-compulsive and related disorders].

Obsessive-compulsive disorders (OCD) are common and have high treatment resistance rates. The rationale of this CME article is to provide an update on OCD in adulthood. First, recent study results on the etiopathogenesis of OCD and its correlates are summarized and the clinical manifestations and current classification criteria are presented. Insight specifiers were added in the ICD-11 criteria and the classification into subtypes and the time criterion were removed; instead, the criteria now require the symptoms to be time-consuming (e.g., lasting > 1 h/day). In addition, a separate category

Prevalence estimation of essential tremor in Hungary between 2010 and 2020 based on the National Health Insurance Fund Database.

The published prevalence of essential tremor is variable worldwide and lacking in Central Europe. We aimed to estimate its prevalence in Hungary and to explore the locally applied therapeutic approaches. We collected data from the National Health Insurance Fund database and pharmacy database registered between 2010 and 2020. Matching the specified codes of the International Classification of Diseases and the individually tailored combination of interventions, we attempted to exclude Parkinson's disease and other tremor evoking conditions. We estimated the period prevalence of essential tremor

Huntington's disease LIG1 modifier variant increases ligase fidelity and suppresses somatic CAG repeat expansion.

Huntington's disease (HD) is a fatal neurodegenerative disorder caused by inheriting an expanded CAG repeat tract in the huntingtin gene (HTT) that further expands in somatic cells over an individual's lifetime. Genome-wide association studies have provided critical insight into factors that modify the course of disease. These include DNA repair genes that alter the rate of somatic expansion and other genes that do not appear to directly influence this process. One modifier gene is DNA ligase 1 (LIG1), in which a variant specifying a lysine to asparagine substitution (K845N) is associated with

The Association Between Head Trauma and BPPV: A Nested Case-Control Study Using a National Health Screening Cohort.

Background/Objectives: Benign paroxysmal positional vertigo (BPPV) is one of the most common vestibular disorders and is characterized by transient but very severe vertigo, increasing fall risk, especially in older people. While many risk factors have been reported, there are still contradicting papers and evidence from large-scale studies remains limited. Methods: This nationwide, nested case-control study utilized Korean National Health Insurance Service-Health Screening Cohort data to investigate possible risk factors for BPPV. In particular, it examined the association between prior head t

Using the cognitive rigidity-flexibility dimension to deepen our understanding of the autism spectrum.

Autism Spectrum Disorder (ASD) is defined as a unidimensional condition, and autism traits are measured on a continuum where the high end of the spectrum represents individuals likely to have an ASD diagnosis. However, the large heterogeneity of ASD has thrown this unidimensional conceptualization into question. With the exact underlying cause(s) of autism yet to be identified, there is a pressing need to establish core, underlying dimensions of ASD that can capture heterogeneity within the autism spectrum, thereby better specifying both autistic traits and ASD symptoms. Here we describe one i

Cognitive stimulation in Parkinson's disease with mild cognitive impairment.

BACKGROUND: Cognitive impairment is among the most frequent and disabling non-motor symptoms of Parkinson's disease (PD), often emerging early and substantially impacting autonomy and quality of life. While cognitive stimulation (CS) is supported in other neurodegenerative conditions, its role in PD is less explored. This study aimed to compare the efficacy of a therapist-guided, home-based CS program, delivered via a virtual reality rehabilitation system, with a conventional in-person CS intervention. METHODS: Of 123 individuals screened, 45 patients with PD and mild cognitive impairment (PD
