Challenging Dogma: Videolaryngoscopy to Limit Cervical Spine Injury?

Videolaryngoscopy is recommended for tracheal intubation of patients with known or suspected cervical spine injury, and current multidisciplinary guidance grounds that recommendation in improved first-attempt success. A separate and widely repeated rationale, that videolaryngoscopy is preferable because it produces less cervical spine movement than direct laryngoscopy, persists in teaching and at the bedside. We examine the primary literature bearing on this motion-reduction rationale. Videolaryngoscopy does reduce cervical spine motion relative to direct laryngoscopy, but the reductions are s

Clinical outcomes of nonambulatory patients caused by ossification of the ligamentum flavum in the thoracic spine.

OBJECTIVE: Thoracic ossification of the ligamentum flavum (T-OLF) may lead to severe thoracic myelopathy and loss of ambulation. Limited data are available regarding surgical outcomes in patients who are nonambulatory preoperatively. This study aimed to evaluate the surgical outcomes of posterior decompression in nonambulatory patients with T-OLF and to identify factors associated with failure to regain ambulation. METHODS: This retrospective study included consecutive patients with T-OLF who were nonambulatory before surgery and underwent posterior decompression with fusion (PDF) between Jul

TNFα promotes brain-predominant EAE independent of IFNγ and IL-17A in HLA-DR3 transgenic mice.

Multiple sclerosis (MS) is thought to be mediated by human leukocyte antigen (HLA) class II-restricted autoreactive CD4 T cells producing interleukin (IL)-17A and/or interferon γ (IFNγ), but the necessity of these cytokines for disease pathogenesis is unclear. Utilizing HLA-DR3 (DRβ1*0301) transgenic mice lacking IFNγ or IL-17A in experimental autoimmune encephalomyelitis (EAE), a model of MS, we found that neither cytokine is essential for disease induction, although both have distinct roles. The absence of IFNγ caused atypical, brain-predominant EAE characterized by high levels of IL-17A, wh

Reprogramming microglia via ShK-modified extracellular vesicles promotes immunomodulation and functional recovery after spinal cord injury.

Microglia play a dual role in tissue repair and inflammatory responses following spinal cord injury (SCI), rendering the regulation of their immune activity a key therapeutic objective during disease progression. Single‑cell RNA sequencing revealed a significant upregulation of Kv1.3 expression in microglia following SCI, implicating this channel in the initiation of early neuroinflammation. To selectively target Kv1.3, engineered extracellular vesicles displaying ShK (ShK-EVs), a highly selective Kv1.3‑blocking peptide, were developed. These ShK‑EVs efficiently accumulated at the lesion site

Tamoxifen Promotes Sustained Bifidobacterium Enrichment in the Gut Microbiota of Male and Female Rats After Spinal Cord Injury.

Spinal cord injury (SCI) triggers complex secondary injury cascades that limit neurological recovery. Tamoxifen (TAM), a selective estrogen receptor modulator, exerts neuroprotective effects in experimental SCI, although its underlying mechanisms remain unknown. Given the emerging role of gut microbiota dysregulation in SCI, we investigated whether TAM-associated recovery is accompanied by alterations in gut microbial composition and predicted function. We conducted a longitudinal, sex-inclusive analysis of 61 Sprague-Dawley rats of both sexes assigned to naïve (NAÏVE), sham-placebo (SHAM PLB)

A Pseudomembrane-Inspired Janus Hydrogel Patch With Robust Wet Adhesion and Swelling-Resistant Capabilities for Accelerated Oral Ulcer Healing.

Oral ulcer healing is hindered by the slow formation, weak adhesion, and limited protection of the native pseudomembrane. Here, a light-curable, swelling-resistant Janus hydrogel patch is developed as a pseudomembrane-inspired protective barrier. The tissue-facing adhesive layer incorporates 2-phenoxyethyl acrylate (PEA), α-lipoic acid (LA), and acryloyl NHS esters, achieving a wet tissue shear adhesion strength of 142 kPa. PEA is proposed to facilitate wet tissue contact, LA participates in the polymer network, and NHS esters provide covalent anchoring. The hydrophobic network constrains wate

Prevalence of Hypovitaminosis D and Its Association with Motor Impairment in Spinal Muscular Atrophy.

Background/Objectives: In recent years, the role of vitamin D in the etiology of central and peripheral neurological diseases has been widely investigated, and evidence suggests benefits of vitamin D replacement therapy. However, few studies have evaluated the relationship between serum vitamin D levels and patients with spinal muscular atrophy (SMA). Therefore, this study aimed to analyze serum vitamin D levels in patients with SMA and their relationship with disease type, degree of motor impairment, and the presence of musculoskeletal deformities. Methods: This was a cross-sectional analytic

Motor Neuron Disease in a Dish: The Power of Human Induced Pluripotent Stem Cell-Derived Spinal Motor Neurons and Glial Cells.

Induced pluripotent stem cells (iPSCs) have revolutionized disease modelling by providing unprecedented opportunities to investigate complex human disorders. Among others, Amyotrophic Lateral Sclerosis (ALS), the most common adult-onset motor neuron disease, clearly embodies this complexity. This disorder remains particularly challenging not only because of its marked clinical and molecular heterogeneity but also as a consequence of the composite interactions between motor neurons and the neighboring non-neuronal cells, especially astrocytes, which contribute to disease progression. The abilit

An integrative transcriptomic atlas of ALS and aging skeletal muscle defines shared and distinct molecular programs of degeneration.

Skeletal muscle atrophy and weakness resulting from loss of motor neuron innervation are shared hallmarks of amyotrophic lateral sclerosis (ALS) and age-associated sarcopenia. However, muscle wasting in ALS is rapid and severe, whereas sarcopenia progresses gradually over decades, peaking at an advanced age. The molecular mechanisms underlying muscle wasting in these conditions remain incompletely defined. Here, we used the SOD1G93A ALS mouse model and wild-type mice across the lifespan to generate an integrated single-nucleus transcriptomic atlas of skeletal muscle, mapping changes associated

Edited HSPCs rescue frataxin deficiency and inflammation-driven multisystemic pathology in Friedreich's ataxia mice.

Friedreich's ataxia (FRDA) is a multisystemic neurodegenerative disorder caused by GAA repeat expansion in intron 1 of frataxin (FXN), resulting in FXN deficiency and mitochondrial dysfunction. Current therapies do not correct the underlying genetic defect. Here, we demonstrate that ex vivo CRISPR/Cas9 excision of the GAA expansion in hematopoietic stem and progenitor cells (HSPCs) provides durable, systemic therapy for FRDA. A single infusion of gene-edited Sca1+ HSPCs, mPPL-001, into myeloablative, humanized YG8JR mice resulted in long-term engraftment of donor-derived cells across hematopoi

Surgery to treat autonomic dysreflexia due to cubital tunnel syndrome in cervical spinal cord injury: a case report.

INTRODUCTION: Autonomic dysreflexia (AD) is a potentially fatal syndrome in people with spinal cord injury (SCI) marked by episodic hypertension with flushing, diaphoresis, and bradycardia. Common etiologies include bladder distension and bowel impaction. Compressive neuropathies lack attention as a potential precipitant of AD. This report describes an individual with cervical SCI and with recurrent AD treated with ulnar nerve decompression surgery. CASE PRESENTATION: A 74-year-old woman with a remote history of C6 SCI (American Spinal Injury Association Impairment Scale Grade B) presented to

Ethical Dilemmas in Predictive Testing for Monozygotic Twins With Hereditary ALS: A Clinical Case Report.

Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disorder typically presenting in midlife and leading to death within 2-5 years of onset. Pathogenic SOD1 variants, inherited in an autosomal dominant manner, account for approximately 20% of hereditary ALS cases. Predictive genetic testing in SOD1-related ALS presents unique ethical challenges, particularly in monozygotic twins who share identical genetic information but hold differing views about testing. We describe an 18-year-old monozygotic twin who requested predictive testing for the familial SOD1 pathogenic variant i

Spinal neurocysticercosis: synthesizing evidence on clinical presentations, anatomical patterns, diagnostic approaches, and therapeutic strategies.

Spinal neurocysticercosis (NCC) is an uncommon manifestation of NCC involving the spinal cord, meninges, nerve roots, and subarachnoid spaces. Although less common than intracranial disease, spinal involvement can cause substantial neurological morbidity because even small lesions within the confined spinal canal may produce progressive myelopathy. Disease may result from hematogenous dissemination or cerebrospinal fluid (CSF) spread of Taenia solium larvae and may present as intramedullary, intradural extramedullary, subarachnoid, or, rarely, extradural lesions. Clinical manifestations are di

The isolated HEPN domain of SACSIN exhibits RNA-binding activity.

Autosomal Recessive Spastic Ataxia of Charlevoix-Saguenay (ARSACS) is a neurodegenerative disorder caused by mutations in the SACS gene, though the molecular function of its protein product, SACSIN, remains elusive. Therapeutic strategies for ARSACS are limited, mostly due to the exceptionally large size of SACSIN (~ 520 kDa), which precludes conventional gene therapy and standard molecular delivery methods. Over 200 pathogenic variants, including missense, nonsense and frameshift mutations, have been identified. Regardless of variant type, patients uniformly exhibit cerebellar ataxia, spastic

Isolated Axoplasm from Squid and Pathogenic Mechanisms in Amyotrophic Lateral Sclerosis.

The squid giant axon is an experimental model that has been instrumental in providing answers to many fundamental questions in cell biology, including the discovery of the motor protein kinesin and probing molecular mechanisms of neurodegenerative diseases such as amyotrophic lateral sclerosis (ALS). The giant axon offers scale and ease of access that is not possible in mammalian models. The power of this model stems from the fact that one can introduce reagents and proteins that would not cross a plasma membrane at defined concentrations in a buffer environment that is closely modeled on norm

A Novel Case Report of Closed-Loop Spinal Cord Stimulation for Neuropathic Pain in Cervical Spinal Cord Injury.

BACKGROUND: Neuropathic pain after spinal cord injury (SCI) is common and often refractory. Closed-loop spinal cord stimulation (CL-SCS) is a novel intervention that adjusts stimulation based on evoked compound action potentials in real time, but its use for pain in complete SCI is not well described. CASE REPORT: A 45-year-old man with complete C6-C7 SCI developed severe, diffuse neuropathic pain below the injury level refractory to multiple medications, an intrathecal fentanyl pump trial, and ketamine infusions. With extensive epidural scarring, a staged buried CL-SCS trial with a combined

Reframing traumatic brain injury and spinal cord injury as chronic multisystem disorders: toward integrated, long-term care.

Traumatic brain injury (TBI) and spinal cord injury (SCI) affect more than 220 million people worldwide and are associated with cardiometabolic, endocrine, neurological, and psychiatric sequelae for years to decades after an acute injury. However, most health systems still treat neurotrauma (TBI and SCI, collectively) as a time-limited event, leaving long-term surveillance of these patients fragmented or absent, which leads to preventable mortality and avoidable readmissions. TBI and SCI in adults should be reclassified as acute-onset multisystem chronic-risk conditions. Existing cardiovascula

Dedication to a lifelong mission: Gu Fangzhou and the Chinese road to polio elimination.

This paper reviews Professor Gu Fangzhou's contributions to polio control and elimination in China. Following the 1955 polio outbreak, he advocated for the live oral polio vaccine (OPV) route tailored to national conditions. To validate vaccine safety, he initiated human trials by testing the vaccine on himself and his infant son. He led the construction of the domestic OPV production base and pioneered the sugar-coated pill formulation to overcome cold chain constraints, ultimately facilitating the nationwide elimination of indigenous wild polioviruses by 1994. Furthermore, he established Chi

Advancement in therapeutic application of quantum dots in amyotrophic lateral sclerosis: current opportunities and challenges.

Despite advancements in healthcare settings in developed countries, the early detection and higher mortality rate associated with amyotrophic lateral sclerosis (ALS), a fatal motor neuronal disorder, remain challenging. Recently, quantum dots (QDs) have emerged as a promising nanocarrier in the prognosis and treatment of ALS owing to their unique multifunctional properties. QDs, through their photoluminescence properties upon excitation, can facilitate the identification and real-time monitoring of disease biomarkers. They also act as a nanocarrier for the targeted delivery of therapeutics, av
