ALS-linked mutant TDP-43 in oligodendrocytes induces oligodendrocyte damage and exacerbates motor dysfunction in mice.

Nuclear clearance and cytoplasmic aggregation of TAR DNA-binding protein of 43 kDa (TDP-43) are pathological hallmarks of amyotrophic lateral sclerosis (ALS) and its pathogenic mechanism is mediated by both loss-of-function and gain-of-toxicity of TDP-43. However, the role of TDP-43 gain-of-toxicity in oligodendrocytes remains unclear. To investigate the impact of excess TDP-43 on oligodendrocytes, we established transgenic mice overexpressing the ALS-linked mutant TDP-43M337V in oligodendrocytes through crossbreeding with Mbp-Cre mice. Two-step crossbreeding of floxed TDP-43M337V and Mbp-Cre

Midterm Morphological Change of Kommerell's Diverticulum after Hybrid Thoracic Endovascular Aortic Repair.

BACKGROUND: Kommerell's diverticulum (KD) is associated with a high incidence of right-sided aortic arch (RAA). Hybrid thoracic endovascular aortic repair (TEVAR) is an effective and less invasive alternative to open repair. However, the long-term results regarding KD diameter regression or symptom improvement remain inadequately described. METHODS: Nine patients underwent TEVAR for KD associated with RAA between January 2016 and September 2023 at our university hospital and affiliated institutions. A hybrid procedure was performed to exclude KD by blocking the proximal blood flow with TEVAR

Skeletal muscle hypertrophy and enhanced mitochondrial bioenergetics following electrical stimulation exercises in spinal cord injury: a randomized clinical trial.

We examined the combined effects of neuromuscular electrical stimulation-resistance training (NMES-RT) and functional electrical stimulation-lower extremity cycling (FES-LEC) compared to passive movement training (PMT) and FES-LEC on mitochondrial electron transport chain (ETC) complexes and citrate synthase (CS) in adults with SCI. Thirty-two participants with chronic SCI were randomized to 24 weeks of NMES-RT + FES [n = 16 (14 males and 2 females) with an age range of 20-54 years old] or PMT + FES [n = 16 (12 males and 4 females) with an age range of 21-61 years old]. The NMES-RT + FES group

Safety and Feasibility of Early Activity-Based Therapy Following Severe Traumatic Spinal Cord Injury: Results from a Single-Arm Pilot Trial.

Early activity-based therapy (E-ABT) has the potential to decrease complications and radically improve neurofunctional recovery following traumatic spinal cord injury (TSCI). Unfortunately, E-ABT after TSCI has never been attempted in humans due to practical obstacles and potential safety concerns. This study aims to report on the safety and feasibility outcomes of the Protocol for Rapid Onset of Mobilization in Patients with Traumatic SCI (PROMPT-SCI) trial: the first-ever trial of E-ABT in critically ill patients who suffered a severe TSCI. To do so, 45 patients with severe TSCI were recruit

Intermittent adaptation to pelvis perturbation during walking enhances retention and generalization of motor learning in people with incomplete spinal cord injury.

The purpose of this study was to determine whether the intermittent adaptation to pelvis perturbation load enhances retention of improved weight transfer and generalization of motor skills from treadmill to overground walking, compared with effects of the continuous adaptation. Fifteen individuals with incomplete SCI participated in two experimental sessions. Each session consisted of (1) perturbed treadmill walking with either intermittent (i.e., interspersed 3 intervals of no perturbation) or continuous (no interval) adaptation to novel walking patterns induced by external pelvis perturbatio

A patient with a PTPN11 gene variant complicated with Chiari I malformation and syringomyelia and a review of literatures.

BACKGROUND: According to previous literature reports, PTPN11 gene variants account for approximately 50% of Noonan syndrome (NS) cases and 85% of Leopard syndrome (LS) cases. Several patients who were diagnosed with NS or LS complicated with Chiari I malformation (CIM) and/or syringomyelia have been reported to have a PTPN11 variant. However, it is not always clear whether the association between CIM and/or syringomyelia and PTPN11 variants is real or random. We try to explain this phenomenon by reporting a clinical case and making a mini-review. METHODS: We retrospectively described a clinic

Using Artificial Intelligence to Identify Three Presenting Phenotypes of Chiari Type-1 Malformation and Syringomyelia.

BACKGROUND AND OBJECTIVES: Chiari type-1 malformation (CM1) and syringomyelia (SM) are common related pediatric neurosurgical conditions with heterogeneous clinical and radiological presentations that offer challenges related to diagnosis and management. Artificial intelligence (AI) techniques have been used in other fields of medicine to identify different phenotypic clusters that guide clinical care. In this study, we use a novel, combined data-driven and clinician input feature selection process and AI clustering to differentiate presenting phenotypes of CM1 + SM. METHODS: A total of 1340

Early feasibility of an embedded bi-directional brain-computer interface for ambulation.

Current treatments for paraplegia induced by spinal cord injury (SCI) are often limited by the severity of the injury. The accompanying loss of sensory and motor functions often results in reliance on wheelchairs, which in turn causes reduced quality of life and increased risk of co-morbidities. While brain-computer interfaces (BCIs) for ambulation have shown promise in restoring or replacing lower extremity motor functions, none so far have simultaneously implemented sensory feedback functions. Additionally, many existing BCIs for ambulation rely on bulky external hardware that make them ill-

Assisting Sit-To-Stand Through Event-Based Electrical Stimulation of Trunk Control: A Preliminary Study.

Independence afforded by functional mobility is invaluable yet inaccessible for those with spinal cord injury (SCI). Regaining mobility skills like sit-to-stand (STS) promotes independence and recovery. Previously, functional electrical stimulation (FES) of leg muscles and exoskeletons have assisted STS, but limitations exist. FES causes fatigue and spasms while exoskeletons cannot activate muscles directly nor suit those with poor trunk control. Thus, hybrid FES exoskeletons are promising but rarely target trunk muscles. This study examined FES of erector spinae for STS assistance with a wear

Fasciculation potentials are related to the prognosis of amyotrophic lateral sclerosis.

Some prognostic biomarkers of amyotrophic lateral sclerosis (ALS) have been described; however, they are inadequate for satisfactorily predicting individual patient outcomes. Fasciculation potentials (FPs) on electromyography (EMG) are useful for the early diagnosis of ALS, and complex FPs are associated with shorter survival in ALS. In this study, we investigated the relationship between the proportion of muscles with FPs, biochemical markers, and the prognosis of ALS. 89 Patients with ALS were retrospectively classified into three groups based on the interval from onset to death or tracheost

Paclitaxel alters the microvascular network in the central and peripheral nervous system of rats with chemotherapy-induced painful peripheral neuropathy.

BACKGROUND AND AIMS: Chemotherapy-induced peripheral neurotoxicity (CIPN), with paraesthesia, numbness, dysesthesia and neuropathic pain ranks among the most common dose-limiting toxicity of several widely used anticancer drugs. Recent studies revealed the microvascular angiogenesis as a new important actor, beside peripheral neurons, in the neurotoxicity and neuropathic pain development and chronicisation. The aim of this work is to elucidate the role of vascular alterations in CIPN. METHODS: We evaluated the severity of CIPN with neurophysiological, behavioural and neuropathological analysi

Semaglutide Ameliorates Diabetic Neuropathic Pain by Inhibiting Neuroinflammation in the Spinal Cord.

UNLABELLED: Glucagon-like peptide 1 (GLP-1) receptor agonists are frequently used to treat type 2 diabetes and obesity. Despite the development of several drugs for neuropathic pain management, their poor efficacy, tolerance, addiction potential, and side effects limit their usage. Teneligliptin, a DPP-4 inhibitor, has been shown to reduce spinal astrocyte activation and neuropathic pain caused by partial sciatic nerve transection. Additionally, we showed its capacity to improve the analgesic effects of morphine and reduce analgesic tolerance. Recent studies indicate that GLP-1 synthesized in

Cord compression due to atypical T-cell lymphoma from paraspinal soft tissue: Report of a case.

During lymphoma's natural history of disease, 5-10% of cases may develop Central Nervous affectation. We present the case of a 57-years-old man with less than 24 h of onset symptoms of paraparesis, lower limb hypoesthesia and sphincter dysfunction who was operated due to dorsal tumor with epidural component which caused severe cord compression. Pathological analysis concluded atypical T-cell lymphoblastic lymphoma, a rare subtype of lymphoma which accounts 1%-2% of all Non-Hodgkin Lymphomas. Our case was particularly aggressive and atypical due to its origin in paraspinal soft tissue. Despite

High-frequency spinal cord stimulation in treatment of phantom lower limb pain following spinal cord injury: A case report.

INTRODUCTION: Pain management in patients with complete spinal cord injury is complex. CASE REPORT: We report a successful case of managing neuropathic, phantom limb, and back pain below the level of spinal cord injury (T5 American Spinal Injury Association [ASIA] A) using a 10 kHz high-frequency spinal cord stimulator (SCS) over a 6-month follow-up period. CONCLUSION: The effectiveness of this approach may be attributed to its ability to modulate supraspinal pain processing, allowing for targeted relief of various pain mechanisms below the level of injury.
