Imaging of Spinal Cord Trauma.

This article outlines the evolving role of MRI in traumatic spinal cord injury (SCI) across acute to chronic stages. Conventional MRI remains indispensable for diagnosis and early prognosis, while quantitative morphometrics such as tissue bridges and spinal cord cross-sectional area provide robust and clinically applicable predictors of post-SCI recovery. Advanced techniques, including diffusion, myelin-sensitive, and functional MRI, enable the detection of microstructural damage, remote neurodegeneration, and functional reorganization. Ongoing advances in scan protocol standardization, automa

Mapping high resolution, multidimensional phase diagrams of near-physiological protein condensates.

Biomolecular condensates are membraneless compartments, crucial for organising and regulating diverse cellular processes. Current approaches to study condensate biology either use simplified recombinant protein systems with limited physiological relevance, or complex live-cell models with restricted experimental control and scalability. Here, we present ExVivo PhaseScan, a droplet microfluidics platform that couples mammalian lysate-based reconstitution with scalable analysis to generate high-resolution phase diagrams of compositionally complex protein condensates. We apply this approach to st

Atypical presacral masses in Currarino syndrome: a case series.

INTRODUCTION: Currarino syndrome is a rare congenital disorder classically defined by the triad of sacral anomaly, anorectal malformation, and a presacral mass. While anterior meningoceles and teratomas are the most commonly reported lesions, an expanding body of literature suggests a broader spectrum of presacral pathology. We present three cases from our institution that expand the phenotypic range of presacral masses associated with Currarino syndrome, including lesions not previously well described in this population. CASE PRESENTATION: Three cases are presented. Final pathologic diagnose

Optimized gGBE enables robust G-to-Y conversion in mammalian cells and zebrafish embryos.

Base editing is a powerful and precise genome-editing tool capable of introducing single-base substitutions with high resolution. Depending on the base-modification domains employed, base editors can mediate both transition and transversion base editing in mammalian cells. While deaminase-derived base editors have enabled efficient transition editing in the zebrafish genome, glycosidase-derived transversion editing remains a significant challenge. In this study, guided by structural insights into the Cas9 complex, we engineered a series of glycosidase guanine base editor (gGBE) constructs by i

Vascular profiling of the spinal cord and cortex reveals region-specific endothelial dysfunction and fibroblast remodeling in human ALS.

Amyotrophic lateral sclerosis (ALS) is a fatal neurodegenerative disorder characterized by progressive degeneration of upper and lower motor neurons. Increasing evidence suggests that neurovascular dysfunction contributes to ALS pathogenesis, but a comprehensive understanding of vascular cell alterations in the human central nervous system remains lacking. Here, to systematically characterize neurovascular changes in ALS, we curated single-nucleus transcriptomic datasets from human spinal cord and cortical tissues with a specific focus on vascular cell populations. Comparative analyses reveale

[Decision-Making for Cervical Spine Lesions: An Orthopedic Surgeon's Perspective].

In patients presenting with numbness, sensory disturbances, or motor deficits in the upper extremities, it is important to differentiate between cervical spinal cord or nerve root disorders and peripheral neuropathies. This article discusses the key points for distinguishing C8 spinal cord segment and nerve root disorders from peripheral neuropathies, particularly ulnar neuropathy and posterior interosseous nerve palsy. A comprehensive evaluation of the patterns of muscle weakness, distribution of sensory disturbances, and electrophysiological findings is essential, with careful consideration

Structural MR Imaging of the Spinal Cord.

The spinal cord is a critical central nervous system structure facilitating neural transmission and reflex activity. Spinal cord disorders manifest as myelopathy with diverse etiologies, necessitating imaging for accurate diagnosis. MR imaging is the gold standard modality due to superior soft tissue contrast and multiplanar capability, enabling detailed assessment of intrinsic and extrinsic pathologies. Conventional sequences, including T1-weighted, T2-weighted, short tau inversion recovery, and postcontrast imaging, remain foundational for detecting structural abnormalities. Emerging advance

Artifacts in MR Imaging of the Spine and Spinal Cord.

This article reviews common MR imaging artifacts encountered in spine imaging and emphasizes their clinical relevance, as artifacts may degrade image quality or mimic spinal pathology, postoperative complications, or cord lesions. It outlines practical clues for distinguishing artifactual findings, including extension beyond anatomic boundaries, appearance limited to specific planes or sequences, and absence of expected associated disease. Artifacts are organized by mechanism: motion-related artifacts such as bulk motion and cerebrospinal fluid pulsation with flow-related ghosting; field inhom

Advanced Imaging of the Spinal Cord: Quantitative, Functional, and Translational Approaches.

MR imaging of the human spinal cord presents unique challenges due to its small cross-sectional dimensions, complex microstructure, surrounding cerebrospinal fluid and other tissues, and susceptibility to motion and field inhomogeneities. Advanced imaging modalities have emerged to address these limitations by providing objective biomarkers of tissue integrity, metabolic status, and functional connectivity. This article synthesizes current evidence, outlines technical considerations and clinical applications of advanced spinal cord imaging, and provides a translational roadmap for integrating

Myelographic Imaging of Spinal Cord Disorders.

Myelographic imaging evaluates the spinal canal, nerve roots, and subarachnoid space using intrathecal contrast with fluoroscopy and computed tomography (CT), or noninvasively with MR-myelography. Although MR imaging has largely replaced conventional myelography owing to its superior soft tissue contrast and noninvasive nature, CT myelography remains essential when MR imaging is contraindicated, limited by artifact, or inconclusive. It is particularly valuable for assessing nerve root compression, spinal stenosis, cerebrospinal fluid leaks, postoperative anatomy, and complex lesions. Myelograp

Spinal Cord Myelopathies: Overview, Imaging Approach, and Differential Diagnosis: Part 1.

Myelopathy refers to a pathologic process involving the spinal cord that encompasses a heterogeneous spectrum of clinical presentations with distinct etiologies, prognoses, and management strategies. As symptoms are often nonspecific and clinical localization can be challenging, imaging-particularly MR imaging-plays a pivotal role in diagnosis. A structured, pattern-based imaging approach is essential for accurate interpretation, integrating symptom acuity and physical examination findings with lesion localization, extent, signal characteristics, enhancement patterns, temporal evolution, and a

Imaging of Degenerative Cervical and Thoracic Myelopathy.

In this article, we focus on imaging of degenerative cervical myelopathy, the most common form of spinal cord dysfunction in adults arising from its progressive compression by age-related degenerative changes in the spine. Imaging plays a key role in confirming the underlying cause of symptoms and guiding neurosurgical intervention. Emerging imaging methods promise earlier diagnosis, better prognostication, and more accurate identification of candidates for surgical intervention. Differences in imaging findings for the less prevalent degenerative thoracic myelopathy are also discussed.
