Central demyelinating disorders, notably myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD), exhibit diverse clinical and radiological manifestations, thereby complicating accurate diagnosis. We present the case of a 78-year-old female patient who developed progressively worsening bilateral lower limb numbness and weakness, accompanied by fecal incontinence over a two-month period. Spinal MRI identified a longitudinally extensive lesion within the spinal cord, and serological analysis revealed MOG antibodies at a titer of 1:100. The patient experienced partial clinical imp

OBJECTIVE: Vascular injuries of the spinal cord are less common than those involving the brain; however, they can be equally devastating. This article discusses the diagnosis and management of ischemic and hemorrhagic vascular disorders of the spinal cord.
LATEST DEVELOPMENTS: Clinical suspicion remains the mainstay for recognizing vascular myelopathies, yet diagnoses are often delayed and challenging in part because of their rarity and atypical manifestations. Noninvasive imaging such as CT and MRI continues to improve in spatial resolution and diagnostic precision; however, catheter-based s

PURPOSE OF REVIEW: Neurologists should be able to identify clinical and neuroimaging features that distinguish vascular disorders from other causes of myelopathy.
RECENT FINDINGS: Although certain clinical features suggest a vascular etiology in acute and chronic myelopathy settings, accurate MRI interpretation within the clinical context is key. Recent studies have shown vascular myelopathies are frequently misdiagnosed as transverse myelitis, and recognition of this diagnostic pitfall is important. Many different vascular mechanisms can cause myelopathy; this article provides a comprehensiv

Vascular disorders of the spinal cord are uncommon yet under-recognized causes of myelopathy. Etiologies can be predominantly categorized into clinical and radiographic presentations of arterial ischemia, venous congestion/ischemia, hematomyelia, and extraparenchymal hemorrhage. While vascular myelopathies often produce significant morbidity, recent advances in the understanding and recognition of these disorders should continue to expedite diagnosis and proper management, and ideally improve patient outcomes. This article comprehensively reviews relevant spinal cord vascular anatomy, clinical

BACKGROUND: Vascular lesions of the spinal cord are rare but potentially devastating conditions whose accurate recognition critically determines the clinical outcome. Several conditions lead to myelopathy due to either arterial ischemia, venous congestion or bleeding within the cord. The clinical presentation varies, according with the different aetiology and mechanism of damage.
PURPOSE: The aim is to provide a comprehensive review on the radiological features of the most common vascular myelopathies, passing through the knowledge of the vascular spinal anatomy and the clinical aspects of th

PURPOSE OF REVIEW: Vascular myelopathies include several diagnoses that are often misdiagnosed or undertreated. Some represent neurologic emergencies, such as spinal cord infarction, and others can be disabling if they remain unrecognized, such as spinal dural arteriovenous fistulas. This review describes the clinical characteristics and current therapeutic strategies for the most common vascular myelopathies and emphasizes practical concepts for the clinician.
RECENT FINDINGS: Although none of the vascular myelopathies have been studied in large prospective studies and their treatments have

Acute nontraumatic myelopathies of childhood include inflammatory, infectious, and vascular etiologies. Inflammatory immune-mediated disorders of the spinal cord can be categorized as idiopathic isolated transverse myelitis, neuromyelitis optica, and multiple sclerosis. In recent years, human T-cell lymphotropic virus type 1, West Nile virus, enterovirus-71, and Lyme disease have been increasingly recognized as infectious etiologies of myelopathy, and poliomyelitis remains an important etiology in world regions where vaccination programs have not been universally available. Vascular etiologies

Vascular abnormalities of the spinal cord are an important cause of myelopathy. Clinicians need to be aware of these disorders as they can present with a variety of neurologic symptoms ranging from acute spinal neurologic emergencies, relapsing/remitting spells to gradually progressive dysfunction. The unique topography and vascular anatomy of the spinal cord lends to the variety of clinical presentations. Both ischemic and hemorrhagic insults can occur. Increased clinical suspicion, better detection with newer imaging modalities and early treatment can often impact outcomes. The authors revie

The clinical symptoms and MRI characteristics of transverse myelopathy (TM) due to non-compressive causes are reviewed, with special emphasis on the differential diagnosis between inflammatory demyelinating lesions, and metabolic and vascular myelopathies. Inflammatory transverse myelopathies are the commonest and most difficult ones to identify. The differentiation between clinically isolated syndromes, multiple sclerosis, neuromyelitis optica, acute disseminated encephalomyelitis and metabolic causes is based on both clinical symptoms and paraclinical signs including magnetic resonance imagi

Vascular myelopathies of the spinal cord have not been described in Suidae, and are a rare finding in companion animals. An 8.5-yr female African warthog (Phacochoerus africanus) presented with an acute onset of tetraparesis. Based on neurologic findings, a cervical spinal cord lesion between C7-T2 was suspected. Magnetic resonance imaging revealed severe intramedullary hemorrhage with suspected abnormal vessels in the spinal cord at the level of the seventh cervical vertebrae. The acute onset of clinical signs and rapid deterioration of neurological status precluded surgical managements. A va

Differential diagnosis of non-traumatic, acute transverse spinal cord syndromes should cover compressive myelopathy (mostly hematomas or tumors), inflammatory myelitis and vascular myelopathies. Since acute pathologies of the spinal cord primarily result in flaccid para- or tetraparesis accompanied by areflexia or hyporeflexia (spinal shock), acute polyradiculoneuritis and the cauda equina syndrome must also be weighed into the differential diagnosis. Paraplegia may ultimately also be of psychogenic origin. The clinical picture is characterized by the rapidity of progression, the possible invo

Spinal cord vascular malformations are often a cause of spinal myelopathy. Endovascular surgical approaches to spinal cord vascular malformations have become an important adjunct and often the primary treatment of these disorders. Spinal cord vascular malformations may be divided into several categories based on their anatomy and location. They may present with a wide range of symptoms far removed from the vascular pathology. Early recognition and treatment can effect a better outcome and often reverse presenting neurologic deficits. Emerging magnetic resonance imaging techniques allow visuali

The most common disease of the spinal cord in human immunodeficiency virus (HIV) infection is vacuolar myelopathy. Pathology studies have demonstrated that vacuolization in the thoracic spinal cord is present in more than a third of patients with AIDS. The disease, however, manifests clinically only when the vacuolization in the spinal cord has become severe, with prominent myelin loss in the lateral and posterior columns. Vacuolar myelopathy presents usually with slowly progressing spastic paraparesis, accompanied by loss of vibratory and position sense and urinary frequency and urgency. In m

Diagnosis of myelopathies of vascular origin is difficult and they are probably underdiagnosed at this time because of the lack of diagnostic tools. A recent report of a 58 year old patient who developed ASAS after an episode of cardiac arrest pointed out the importance of MRI and somatosensory evoked potentials (SEP) to support the diagnosis. MRI with T2 weighted imaging demonstrated diffuse signal abnormalities in both gray matter and surrounding white matter below T7. Furthermore, SEP latencies showed a delay between T6 and T7. Therefore, new technologies including MRI and SEP may improve t

Acute myelopathies (acute transverse myelopathies) can be divided into two major groups. Vascular myelopathies are caused by aortic disease, surgery, or intrinsic vascular disease of cord vessels. Inflammatory/demyelinative myelopathies include lupus erythematosus, various viral infections, and primary demyelinating disease. Criteria for diagnostic decisions and recent data from magnetic resonance imaging are reviewed.

Four cases of acute onset myelopathy in which clinical and instrumental findings led to the diagnosis of anterior spinal artery syndrome are reported. The physiopathology, clinical medicine and treatment of vascular myelopathies are discussed on the basis of the series presented and reported data. It is also pointed out that vascular aetiology underlies a certain number of acute myelopathies, otherwise classified indiscriminately as myelitis.
