A granuloma Inguinale in an infant: a case report.

Granuloma inguinale (donovanosis) is a chronic granulomatous infection caused by Klebsiella granulomatis, predominantly affecting sexually active adults in endemic regions. Pediatric cases are exceedingly rare and pose diagnostic challenges, particularly in non-endemic settings. We report an unusual case of granuloma inguinale in an 8-month-old male infant from Syria who presented with multiple painless ulcerative lesions involving the genital and diaper areas. Diagnosis was established through identification of Donovan bodies on Giemsa stain and confirmed by histopathological examination. The

Granuloma inguinale (donovanosis): A historical journey through British India and beyond.

Granuloma inguinale, now commonly referred to as "Donovanosis," is a chroniculcerative bacterial disease whose historical evolution reflects important developmentsin dermatology, venereology, and microbiology. Although the condition is often associated with Col. Charles Donovan and the characteristic Donovan bodies, the initial descriptions extend further back to British India and the early observations of Surgeon Major Kenneth McLeod at Calcutta Medical College. The manuscript highlights lesser-recognized historical contributions and describes the chronology of major milestones that shaped th

Genital Herpes in an HIV-Positive Patient Mimicking Donovanosis: A Case Report.

Genital herpes is one of the most common sexually transmitted infections worldwide, primarily driven by herpes simplex virus type 2 (HSV-2). In immunocompetent individuals, the disease typically manifests as self-limiting vesicular lesions that progress to painful ulcerations. However, in patients living with human immunodeficiency virus (HIV), the clinical presentation can be remarkably atypical, chronic, and severe due to impaired cell-mediated immunity. In this specific population, hypertrophic or extensive vegetative ulcerations frequently mimic other infectious diseases and malignancy, cr

[Genital ulcers. Review article].

Genital ulcers are a frequent reason for consultation in Primary Care, primarily due to new in sexual practices in recent years. They are caused by different reasons; its origin is usually infectious and they are transmitted through sexual contact. Its etiology can be non-infectious too. This review, focused on Primary Care, aims to provide an updated overview of the clinical characteristics, diagnosis, and management of genital ulcers, both of infectious origin (genital herpes, syphilis, lymphogranuloma venereum, chancroid, and donovanosis) and non-infectious origin (acute Lipschütz vulvar).

Two for the price of one: should donovanosis and chancroid be targeted for eradication?

Half a century after smallpox eradication, this feat has not been repeated, although there is hope for the Guinea worm. This article examines whether two uncommon genital ulcerative diseases, donovanosis and chancroid, should be targeted for eradication. Many think that their decline has been such that they might be heading towards spontaneous eradication, following more effective treatments, integration of sexually transmitted infection management in primary healthcare, enhanced condom use by sex workers and promotion of male circumcision. However, examples of syphilis and mpox suggest sponta

Whether reactivation of granuloma inguinale can be a possibility in a person living with HIV?

Granuloma inguinale is one of the ulcerative sexually transmitted infections (STIs), and the incidence is very low almost worldwide. When it is associated with HIV infection, it poses a real challenge to the physician as the recommended guidelines often won't work well. Granuloma inguinale is comparatively less contagious than other STIs as the conjugal infection and vertical transmission are rare. In the present case, the patient denied any extramarital contact after marriage (for 32 years). His wife was not having any genital lesion, and their contacts were protective one. It raises the susp

Donovanosis.

Donovanosis usually causes genital ulcers with a distinct clinical appearance. The condition has been a problem in many developing countries, but recent years have seen a significant global decline in prevalence. It is well on the way to being eradicated. Donovanosis has been known under many different terminologies, and there is still debate about how the causative organism should be classified. Recent news agency and social media reports of an increase in the "flesh-eating infection donovanosis" have been false and unhelpful, only leading to hyperbole and increased stigma among those infecte

Annular Lesions Mimicking Mpox in a Patient With HIV.

Syphilis, a chronic bacterial infection caused by Treponema pallidum , is now reemerging as a global public health problem, particularly among men who have homosexual relationships. Because syphilis could mimic various diseases and could coexist with other sexually transmitted diseases, accurate recognition of syphilis, with its versatile presentations, can challenge even the most experienced clinician. Here, we presented a case of syphilis mimicking mpox in a homosexual patient with HIV infection and summarized the key points in differentiating syphilis from mpox.

Multiple sexually transmitted infections as a presenting feature of human immunodeficiency virus/AIDS.

A 45-year-old married male presented with nonhealing, painless ulcers with purulent discharge over genitals for 3 months. He had molluscum contagiosum over genitalia and forehead. A tissue smear suggested a diagnosis of donovanosis. Biopsy suggested diagnosis of molluscum contagiosum and serology was positive for human immunodeficiency virus 1 and herpes simplex 2. The patient was started on acyclovir and doxycycline. Antiretroviral therapy was initiated. The patient responded slowly over a period of 8 weeks. Immunocompromised patients having nonhealing genital ulcers must be subjected to tiss

Relapsing donovanosis rhyming with unexplained decline in CD4 count in a person living with HIV/AIDS: Long-term follow up and therapeutic outcome.

Donovanosis is a chronic genital ulcerative disease caused by the intracellular Gram-negative bacterium Klebsiella granulomatis, reported more frequently in people living with HIV/AIDS (PLHA). Here we present a case of relapsing donovanosis in a PLHA on second line antiretroviral therapy who had episodes of transient unexplained decrease in CD4 counts associated with rapid growth of the lesion and non-responsiveness to treatment followed by clinical resolution coinciding with recovery of the CD4 count.

An Ulcer by Any Other Name: Non-herpes and Non-syphilis Ulcerative Sexually Transmitted Infections.

The myriad presentations of ulcerative sexually transmitted infections, other than genital herpes and syphilis, challenge even the most astute clinician given the considerable overlap in clinical presentation and lack of widely available diagnostic resources, such as nucleic acid testing, to confirm the diagnosis. Even so, case prevalence is relatively low, and incidence of chancroid and granuloma inguinale are declining. These diseases still cause substantial morbidity and increased chance for HIV acquisition, and with the recent advent of mpox as a cause, it remains imperative to identify an

[Tropical sexually transmitted infections Summary of the SFMTSI Scientific Day of November 9, 2023].

Sexually transmitted infections (STIs) deserve more attention today than ever in the tropics. Indeed, the emergence of monkeypox in 2022 in Western countries reminds the risk of exporting STIs from the tropics as it was already known for decades with the worldwide spread of HIV/AIDS infection from tropical Africa. Some hazards are already well identified. According to WHO 2023 report STIs are increasing in the world. Antibiotic resistance is increasing for Neisseria gonorrhoeae, and already well established for Mycoplasma genitalium, whereas Treponema pallidum has become resistant to macrolide

Granuloma inguinale diagnosed on the bedside.

Donovanosis is a chronic bacterial infection caused by gram-negative bacterium Klebsiella granulomatosis and is characterized by painless beefy red granulomatous ulcer which bleeds on touch and is mostly confined to the endemic regions of the world. It is rarest among the sexually transmitted genital ulcer diseases. We hereby report a case of donovanosis in a non-endemic area who initially presented with a solitary genital ulcer. Initial tests did not reveal any aetiology. However after four days of admission, since no aetiology was coming forth, a repeat tissue smear from the lesion revealed

Genital ulcers caused by sexually transmitted agents.

Genital ulcers (GUs) represent a diagnostic challenge and can be secondary to neoplastic and inflammatory processes of different causes. Among those of infectious etiology, there are sexually transmitted infections (STIs), a very frequent reason for seeking the health service. The most common agents are herpes simplex virus and Treponema pallidum and, more rarely, Haemophilus ducreyi, Klebsiella granulomatis and Chlamydia trachomatis. A careful dermatological examination offers important diagnostic elements; however, atypical manifestations are very common. Distinctive characteristics of ulcer

The changing epidemiology of genital ulcer disease in South Africa: has donovanosis been eliminated?

OBJECTIVES: We used an in-house molecular assay for the detection of Klebsiella granulomatis in ulcer specimens collected over a 12-year surveillance period in order to determine whether a diagnosis of donovanosis could be ascribed to genital ulcer disease (GUD) of unknown aetiology in our setting. METHODS: Between 2007 and 2018, a total of 974 genital ulcer specimens with no previously identified sexually transmitted (STI) pathogens were selected from STI aetiological surveys conducted in all nine provinces of South Africa. Giemsa-stained ulcer smears from the same participants had previousl
