Focus on the excitatory and inhibitory neurotransmission imbalance in amyotrophic lateral sclerosis: a harmful disease player or a potential therapeutic opportunity?

Amyotrophic lateral sclerosis (ALS) is a progressive and fatal neurodegenerative disease affecting both upper and lower motor neurons. Evidence indicates that ALS is a "multifactorial" and "multicellular" disease; however, the causes of ALS remain elusive, as the mechanisms underlying the disease have not yet been completely clarified. One major proposed mechanism, first described in 1990, is the glutamate excitotoxicity theory. This theory suggests that excessive glutamatergic neurotransmission, combined with impaired glutamate clearance, significantly contributes to motor neuron degeneration

Immunobiology and immunotherapies in stiff-person syndrome and glutamic acid decarboxylase antibody-spectrum disorders: Present and future therapeutic prospects.

Stiff-person syndrome (SPS) is increasingly recognized as the prototypic and most common autoimmune neuronal hyperexcitability disorder that presents with stiffness in the limbs and axial muscles, stiff gait with uncontrolled falls, and episodic painful muscle spasms triggered by anxiety, task-specific phobias and startle responses, collectively leading to disability. Recent publicity has increased SPS awareness among patients and physicians, generating concerns about diagnosis, misdiagnoses, pathomechanism, and the most successful or promptly applied therapies. The paper addresses the evolvin

Dual-engineered Treg-Exosome-IKVAV nanovesicles spatiotemporally sequentially regulate neuro-immune microenvironment to promote spinal cord injury repair.

Spinal cord injury (SCI) repair remains a significant clinical challenge due to the imbalance of inflammatory microenvironment and insufficient neural regenerative ability. Current therapeutic approaches, such as pharmaceuticals, stem cell transplantation, and inorganic biomaterials, are limited by insufficient supply, poor bioactivity, and immunogenicity, severely limiting their clinical translation. To address these issues, we developed a biomimetic dual-engineered nanovesicles (Treg-Exo-IKVAV) by conjugating a neuroprotective IKVAV peptide motif with Treg-derived exosomes (Treg-Exo) using c

Adaptation of motor unit synergies in the synergetic ankle plantarflexors in ambulatory persons with incomplete spinal cord injury.

BACKGROUND: Spinal cord injury (SCI) often results in impaired motor control and coordination. Previous studies have highlighted the role of muscle synergies in coordinating motor tasks and their alterations following SCI. However, the adaptation in muscle synergy patterns at the motor unit (MU) level after SCI remains unexplored. This study aimed to investigate MU synergies and clustering in the synergetic soleus and gastrocnemius medialis (GM) muscles and to explore how these patterns are altered in persons with SCI. METHODS: High-density electromyography (HD-EMG) was used to record MU acti

Immuno-Neuroprotectants Facilitate Neurological Recovery via Sequestration of Neutrophil Extracellular Traps (NETs) and Promotion of Neuroregeneration.

Maintaining nerve integrity and rescuing/regenerating injured neurons are pivotal for spinal cord injury (SCI) repair. Herein, an immuno-neuroprotectant (INPT) is developed to mitigate secondary SCI and promote neuroregeneration via sequestration of neutrophil extracellular traps (NETs) and targeted delivery of brain-derived neurotrophic factor (BDNF). To construct the INPT, positively charged BDNF is engineered into negatively charged A-BDNF nanoparticles (A-BDNF NPs) via reversible modification with adenosine triphosphate, and A-BDNF NPs are further coated with polySia-overexpressing microgl

The ALS-associated E425K mutation uncouples DNAJC7 from the Hsp70 chaperone cycle.

DNAJC7, a member of the J-domain protein (JDP/Hsp40) family, plays a key role in protein homeostasis by regulating Hsp70 activity and preventing protein aggregation. Mutations in DNAJC7 have been linked to amyotrophic lateral sclerosis (ALS); yet, the molecular mechanisms by which these variants impair chaperone function remain poorly understood. DNAJC7 is a conserved chaperone featuring both a canonical J-domain, essential for Hsp70 activation, and three TPR domains, which serve as protein-protein binding interfaces. Here, we investigate the structural and functional consequences of the ALS-a

The utility of Gold Coast criteria for amyotrophic lateral sclerosis.

INTRODUCTION: Amyotrophic lateral sclerosis (ALS) is a rapidly progressive neurodegenerative disease. Current diagnostic criteria, including the revised El Escorial (rEE) and Awaji (AW) criteria, have limitations in sensitivity. The Gold Coast (GC) criteria were proposed to simplify diagnosis and improve early detection, but their real-world performance remains unclear. METHODS: We retrospectively analyzed 260 patients suspected of ALS who were admitted to our department between 2013 and 2022. The GC, AW, and rEE criteria were applied to data from initial hospitalization. Final diagnoses were

Afferent-driven modulation of spinal interneuron circuits across disease stages in amyotrophic lateral sclerosis.

Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disease that progressively disrupts voluntary motor command through combined cortical and spinal motor neuron degeneration. However, how spinal circuits reorganize during the disease remains poorly understood, particularly in humans. This study examined the function of excitatory and inhibitory spinal interneuron circuits that control upper and lower limb movements, using non-invasive electrophysiological techniques targeting specific afferent-motoneuron pathways at cervical and lumbar levels. These segments are clinically relevant, as

Accelerated brain aging in amyotrophic lateral sclerosis and its prognostic associations: a cohort study.

BACKGROUND: Amyotrophic lateral sclerosis (ALS) is a fatal neurodegenerative disease, appearing to be associated with accelerated brain aging. Although increased brain age has been associated with mortality risk in older adults, it remains unclear how the brain aging process is accelerated in ALS and how this acceleration relates to patient prognosis. METHODS: One hundred seventy sporadic ALS patients and 84 age- and sex-matched HCs were recruited and underwent neuropsychological tests and T1-weighted magnetic resonance imaging (MRI) scans. A brain-age prediction model was developed using a 3

Prognostic Value of Neurofilament Light Chain and Glial Fibrillary Acidic Protein in ALD-Related Myelopathy.

BACKGROUND: X-linked adrenoleukodystrophy (X-ALD) is a neurometabolic disorder caused by pathogenic variants in ABCD1, leading to slowly progressive spinal cord disease in nearly all affected men. Sensitive biomarkers to quantify disease severity and predict progression are needed for clinical care and trial design. Plasma neurofilament light chain (NfL) and glial fibrillary acidic protein (GFAP) are promising biomarkers reflecting axonal and astroglial injury. This study evaluated their prognostic value for spinal cord disease progression in X-ALD. METHODS: In a prospective, seven-year longi

Clinical and MRI morphometric characterization of symptomatic cerebellar tonsillar ectopia.

BACKGROUND: Cerebellar tonsils normally extend up to 5 mm below the foramen magnum (FM). Some individuals with tonsillar ectopia (TE) measuring under 5 mm may present with symptoms of Chiari type 1 malformation (CM1). We analyzed MRI data of the posterior cranial fossa (PCF) in affected patients to assess TE's impact on clinical manifestations. METHODS: The retrospective study examined 83 adults with 2-4 mm symptomatic cerebellar TE without syringomyelia (SM), 16 with syndromic CM1 without SM, and 19 controls. Participants with TE were divided into three groups: syndromic (TEs, n = 15), incom

Injury and therapy in a human spinal cord organoid.

Damage to the spinal cord can lead to irreversible paralysis and loss of sensory function, but translation of preclinical therapies remains elusive. We recently showed that bioactive supramolecular assemblies of peptide amphiphiles can reverse paralysis in an acute mouse model following severe spinal cord injury (SCI). Here we report the development of two human spinal cord organoid injury models to simulate SCI in vitro, a laceration of the organoid with a scalpel and a compressive contusion commonly used in preclinical models, both resulting in immediate neuronal death and the formation of g

NAD and NADPH in Neuronal Injury and Degeneration.

The review focuses on novel targets recently identified in the mechanisms of neuronal death, and specifically in axonal degeneration. Depletion of NAD triggers various death pathways depending on the nature of the NAD degradation trigger. Wallerian degeneration (WD) of axons in the peripheral nervous system follows the specific mechanism activated by NMNAT2/SARM1 - "sarmoptosis", which is also observed in other tissues, and contributes to the chemotherapy-induced neurological effects. An allosteric inhibitor of the NADase activity of SARM1, SIR2501, has been recently approved by FDA for preven

A critical role of the histone lysine demethylase JmjC1 in regulating severe malaria-associated virulence genes in Plasmodium falciparum.

Cerebral malaria involves the expression of a particular var subgroup, termed upsA, in Plasmodium falciparum, which encodes a subset of PfEMP1 variants that avidly bind to human receptors on endothelial cells, resulting in obstruction of microvasculature in the brain and severe pathogenesis. Despite the clinical importance of the upsA var genes, the mechanism of their low-frequency activation in asymptomatic or uncomplicated malaria cases remains poorly understood. Here, we identify a JmjC-domain-containing histone demethylase PfJmjC1 as a vital transcriptional silencer of upsA var genes, whic

'Got to be the right fit, the right connection': a scoping review of the peer support experiences and perspectives of adults with acquired neurological disability.

BACKGROUND: Peer support for people with acquired neurological disability is gaining worldwide popularity, yet it is conceptually unclear what constitutes valuable peer interactions. To build a greater understanding from the lived experience perspective, this scoping review synthesised literature on how adults with acquired neurological disability experience and perceive peer support. METHODS: Peer-reviewed literature published between 2013 and 2024 was identified through systematic searches of five databases. Thirty-two studies were included, covering a range of disabilities (e.g. acquired b

Poly(N-acryloyl L-tryptophan) nanoparticles: anti-inflammatory and neuroprotective effects.

Neuroinflammation is an immune response, which refers to the inflammation inside the nervous system, which involves complex processes and mechanisms that affect the brain and spinal cord. Viruses, bacteria, injuries, or autoimmune processes cause neuroinflammation, which can lead to Alzheimer's and Parkinson's diseases. Although there are reports on the development of neuroprotective and anti-inflammatory drugs or medicines that may cross the blood-brain barrier, the results are not satisfactory and demand a suitable solution. In this line, the present work is focused on developing poly(N-acry
