MiR‑204‑5p modulates glial morphology and apomorphine‑induced rotational asymmetry without dopaminergic neuroprotection in a preclinical mouse model of Parkinson's disease.

Parkinson's disease (PD) is characterized by progressive nigrostriatal dopaminergic degeneration and a dysregulated neuroinflammatory environment. Here, we investigated whether intranigral delivery of a microRNA‑204‑5p mimic (miR‑204-5p) at the time of lesion modulates dopaminergic‑dependent rotational asymmetry, glial morphology, and striatal medium spiny neurons (MSNs) recruitment in a unilateral 6‑OHDA mouse model. Adult C57BL/6 mice received a unilateral striatal 6‑OHDA injection and simultaneous intranigral infusion of a synthetic miR‑204‑5p mimic or vehicle, and apomorphine‑induced rotat

Proteome-Wide Mendelian Randomization Identifies Therapeutic Targets for Ischemic Stroke.

BACKGROUND: Ischemic stroke carries substantial residual risk despite acute revascularization and secondary prevention, and small-vessel disease (SVD) lacks specific preventive therapy. Mendelian randomization (MR) using cis-acting protein quantitative trait loci (pQTLs) identifies circulating proteins whose lifelong modulation alters stroke risk. METHODS: We screened 589 conditionally independent cis-pQTLs from the INTERVAL study against four GIGASTROKE outcomes: any ischemic stroke (AIS), large-artery atherosclerosis (LAA), cardioembolic stroke (CES), and SVD. Wald ratio estimates were corr

Urgent versus scheduled resection in high-grade glioma: patient selection and healthcare utilization in a single-center cohort.

PURPOSE: Patients with high-grade glioma reach surgery during the initial, index hospitalization (urgent), or through scheduled/outpatient pathways. There is a paucity of literature examining what patients require expedited versus elective surgery. This single-center retrospective study describes complications, utilization, and survival by pathway. METHODS: Adults with WHO grade 3-4 gliomas undergoing primary resection (2021-2026) were classified as urgent (admitted without a planned resection date) or scheduled (discharged and readmitted for planned surgery, or managed exclusively as an outp

When to Start Oral Anticoagulant Therapy Post-Stroke: From Common to Rare Conundrums.

PURPOSE OF REVIEW: This review summarizes recent evidence on when to start oral anticoagulant therapy following stroke, including both common and rare clinical conundrums. RECENT FINDINGS: While previous recommendations based on Diener's rule suggested waiting up to 12 days based on stroke severity and neuroradiological findings, newer data support earlier initiation or resumption of anticoagulation, particularly for mild and moderate ischemic strokes, generally within 4 days. In patients with a history of intracerebral hemorrhage, anticoagulation resumption involves weighing thromboembolic r

Very Low Frequency of SCA27B Among Undiagnosed Cerebellar Ataxia Patients in Peru.

Spinocerebellar ataxia type 27B (SCA27B) is a recently described autosomal dominant ataxia caused by a pathogenic GAA repeat expansion in the FGF14 gene. The frequency and the distribution of FGF14 repeat alleles remain largely unexplored in underrepresented populations, particularly in Latin America. To investigate the frequency of SCA27B and characterize the allelic distribution of the GAA-FGF14 microsatellite locus in a Peruvian cohort of individuals with ataxia of unknown molecular etiology. We analysed a cohort of affected individuals evaluated at a referral center for neurogenetic disord

Dynamic long-term modulation of cognition, brain structure and covariance networks in mild cognitive impairment due to AD by donepezil: A 36-month longitudinal study.

BACKGROUND: Whether donepezil exerts long-term efficacy in mitigating cognitive decline, preserving brain structure, and restoring impaired structural covariance networks in patients with mild cognitive impairment due to Alzheimer's disease (AD-MCI) remains controversial. OBJECTIVE: This study aimed to characterize the longitudinal dynamic alterations in cognition, brain morphology, and structural covariance network topology in AD-MCI patients receiving continuous 36-month donepezil treatment, thereby elucidating the long-term modulatory effects of donepezil on AD progression. METHODS: Cross

Potential sphenopalatine ganglion block efficacy as an alternative to epidural bloodpatch for post-dural puncture headache: a monocentric french study.

BACKGROUND: Post-dural puncture headache (PDPH) is a common complication of lumbar puncture caused by cerebrospinal fluid leakage and intracranial hypotension. Epidural blood patch (EBP) is considered the gold standard treatment but remains invasive. Sphenopalatine ganglion block (SPGB), a non-invasive technique, has shown promising results in recent studies, but it was not evaluated as an alternative of EBP. This study aimed to evaluate the effectiveness of SPGB in PDPH by determining whether it could reduce the need for EBP in clinical practice. METHODS: We conducted a prospective monocentr

Molecular-genetic mechanisms of cerebral arteriovenous malformations: A systematic review of germline, somatic and experimental evidence.

Cerebral arteriovenous malformations (AVMs) are high-flow vascular anomalies that carry a considerable burden of haemorrhagic morbidity. Work over the past decade has shown that both inherited syndromes and apparently sporadic lesions can be traced to alterations in a handful of signalling pathways, but the relevant evidence sits awkwardly between human genetic studies, animal models, and molecular profiling experiments. To bring together the current evidence on the molecular and genetic underpinnings of cerebral AVM formation, clinical course, and candidate therapeutic targets. We searched ME

Advances in Single-Cell and Spatial Omics for Glioblastoma Precision Medicine.

PURPOSE OF REVIEW: Glioblastoma is the most aggressive and fatal primary malignant brain tumour in adults, characterised by extensive intra-tumour heterogeneity, diffuse infiltration into surrounding brain tissue. Despite advances in surgery, radiotherapy, and chemotherapy, patient prognosis remains poor, underscoring the critical need for improved biological insight to guide precision medicine potential strategies. Although traditional bulk genomic and transcriptomic analyses have made a substantial contribution to understanding glioblastoma pathophysiology, such average molecular signals acr

Design and Synthesis of Novel β-Naphthol Dithiocarbamate Hybrids: A Convergent Multifaceted Approach Towards AChE and Amyloid-β Inhibition for Alzheimer's Disease.

Alzheimer's disease (AD) is an irreversible and chronic neurodegenerative disease, posing a growing global health burden, necessitating the development of multitarget-directed ligands (MTDLs) to combat its complex, multifactorial pathology. In this research, a new series of β-naphthol-based dithiocarbamate derivatives (5a-5g) were designed, synthesized using a rational drug design approach, and their structures were elucidated using FTIR, UV, and NMR spectra analysis. Reportedly, compound 5d showed characteristic C-N, C = S, and C-S vibrations in FTIR, while distinct aromatic and piperidinyl p

Discovery of LG-NE-29: An Orally Bioavailable GSPT1 Molecular Glue Degrader Effective in Glioblastoma Xenograft Models.

The clinical management of glioblastoma (GBM) is severely hindered by rapid resistance to temozolomide (TMZ). Through viability-based phenotypic screening, we identified the translation termination factor GSPT1 (G1 to S phase transition 1) as a critical vulnerability in GBM cells. Structure-activity relationship optimization of an isoindolinone scaffold, specifically focused on improving metabolic stability, led to the discovery of LG-NE-29, a potent molecular glue degrader. This compound efficiently promotes CRBN-dependent ubiquitination and subsequent proteasomal degradation of GSPT1. LG-NE-

Discovery of a BBB-Penetrant Lanosterol Derivative as an Aβ Aggregation Inhibitor for Alzheimer's Disease.

The abnormal aggregation of amyloid-beta (Aβ) peptides is a core driver of Alzheimer's disease (AD). Developing central nervous system (CNS)-penetrant small-molecule Aβ inhibitors offers a complementary strategy to overcome the high costs and severe side effects of antibodies. Lanosterol has demonstrated excellent potential in inhibiting abnormal protein aggregation. However, poor physicochemical and pharmacokinetic profiles strictly hinder its direct therapeutic application. Herein, rational modifications on lanosterol were conducted to modulate the hydrophilic-lipophilic balance around the 3

Discovery and Optimization of a Novel Brain-Penetrant Colchicine-Binding Site Ligand for Treatment of Glioma.

The limited number of approved drugs for treatment of brain cancers is very apparent. Temozolomide and Vorasidenib continue to be the only approved brain-permeable, orally available drugs. A novel class of highly brain-penetrant tubulin-binding ligands was discovered through rigorous structure-activity-relationship (SAR) studies. The lead compounds were validated through tubulin polymerization assays and co-crystallization, demonstrating destabilization of microtubules by binding to the colchicine site of tubulin. The ligands maintain core interactions with tubulin like those of colchicine; ho

Enzyme-Targeted Therapeutics for Neurological Disorders.

Neurological disorders represent one of the leading causes of disability and mortality worldwide, and their prevalence is expected to increase substantially with population aging. Despite major advances in neuroscience, effective disease-modifying therapies remain unavailable for most chronic disorders of the central nervous system (CNS), highlighting the need for novel therapeutic strategies. Enzymes are attractive drug targets because they regulate fundamental processes including neurotransmission, signal transduction, energy metabolism, protein homeostasis, epigenetic regulation, lipid meta

Design, Synthesis and Evaluation Donepezil-Based Dual BuChE/FAAH Inhibitors for Alzheimer's Disease Therapy.

Alzheimer's disease (AD) is multifactorial, prompting multi-target therapy. A series of donepezil-based derivatives were designed, synthesized, and evaluated as dual inhibitors of human butyrylcholinesterase (hBuChE) and fatty acid amide hydrolase (hFAAH). Among them, lead G15 displayed potent and balanced inhibition with IC50 values of 2.44 μM and 0.17 μM, respectively, and favorable selectivity over carboxylesterases CES1/2 (SI 14.4 and 8.4). G15 not only acted as a time-dependent, mixed-type hFAAH inhibitor and a pseudo-irreversible hBuChE inhibitor, but also exhibited good drug-like proper

Exercise-based cardiac rehabilitation after stroke and the risk of incident dementia: an observational study from a global federated network.

INTRODUCTION: Dementia is a common and disabling complication after stroke. Exercise-based cardiac rehabilitation (EXCR) may influence vascular and inflammatory mechanisms relevant to cognitive decline. We investigated the association between EXCR initiation after stroke and incident dementia. PATIENTS AND METHODS: Using TriNetX, we identified adults with ischaemic stroke or TIA. Patients with recorded ICD-10-CM codes indicating an NIHSS score ≥ 16 were excluded from the primary analysis. Patients initiating EXCR within 6 months were compared with those without documented post-index EXCR. Pro

Longitudinal blood pressure trajectories and time to stroke among hypertensive patients: A Bayesian multivariate joint modeling study in Benishangul-Gumuz, Ethiopia.

BACKGROUND: Stroke is a major cause of morbidity and mortality worldwide, with hypertension being its most important modifiable risk factor. Although the association between elevated blood pressure and stroke is well established, the extent to which longitudinal blood pressure patterns influence the timing of stroke remains poorly understood, particularly in low-resource settings. We investigated the association between longitudinal systolic and diastolic blood pressure trajectories and time to stroke among hypertensive patients in Ethiopia. METHODS: We conducted a retrospective cohort study

Education reduces dementia incidence and mitigates genetic risk.

There is little causal evidence on factors that can protect individuals against Alzheimer's Disease and Related Dementias (ADRD) risk. We study the causal effect of education on ADRD, exploiting a regression discontinuity generated by a compulsory schooling reform. ADRD was ascertained based on medical history, hospital records, and death registries, addressing concerns about selective sample attrition. We find that education reduces incidence of ADRD. Using molecular genetic data, we show that the reform weakened the relationship between genetics and ADRD incidence, implying genetic risk of d

Antidopaminergic Medications and Clinical Outcomes in Huntington Disease: Evidence From the PROOF-HD Placebo-Controlled Trial.

BACKGROUND AND OBJECTIVES: Antidopaminergic medications (ADMs), including vesicular monoamine transporter-2 (VMAT2) inhibitors and antipsychotics, are widely used to manage motor and psychiatric symptoms in Huntington disease (HD). Observational evidence suggests that ADM use may be associated with accelerated worsening of clinical outcome measures used in HD trials, but this relationship has not been examined in a controlled trial setting. We assessed the association between ADM exposure and longitudinal clinical outcomes in the Pridopidine's Outcome On Function in Huntington Disease (PROOF-H
