Inpatient Deaths in Pediatric Leukodystrophies.

BACKGROUND AND OBJECTIVES: Leukodystrophies are neurogenetic diseases affecting the white matter of the central nervous system. The contributing factors for leukodystrophy mortality are incompletely understood. Our objectives were to characterize inpatient deaths of pediatric leukodystrophies, including demographics and risk factors. METHODS: This retrospective cohort analysis utilized the national Pediatric Health Information System (PHIS) database. Patients under age 19 years with an (ICD-10) diagnosis of leukodystrophy and an inpatient encounter at a PHIS hospital between 2016 and 2024 wer

Magnetic control of nano-cerebrospinal fluid dynamics in the subarachnoid region: Neurotherapeutic applications.

The dynamic behavior of nano-cerebrospinal fluid (NCSF) flow in the subarachnoid channel under external magnetic stimulation is investigated in this study, with findings aimed at advancing neurological drug delivery and treatment strategies. The Casson fluid model is employed to characterize the yield-stress behavior inherent to NCSF, enabling the simulation of transient flow within the subarachnoid space bounded by the pia and arachnoid maters. The regulating equations account for oscillatory pressure gradients, Lorentz forces due to the magnetic field, porous medium effects, and nanoparticle

A comparative analysis of early postoperative outcomes of autologous pericranium versus high-density polypropylene in cranial duraplasty in a tertiary care center in Central India.

BACKGROUND AND OBJECTIVES: Duraplasty is commonly required when primary dural closure is not feasible during cranial surgery. Although both autologous and synthetic dural substitutes are widely used, high-quality prospective data comparing their early postoperative safety remain limited. This study aimed to compare early postoperative complication rates following cranial duraplasty using autologous pericranium versus high-density polypropylene (G-Patch). METHODS: This prospective randomized comparative study enrolled 121 consecutive patients undergoing cranial duraplasty at a tertiary care ce

Comparative Evaluation of Virtual Reality versus Standard Nursing Care in Managing Pain and Fear during Lumbar Puncture Procedures: A Randomised Controlled Trial.

BACKGROUND: Meningitis is a serious infectious disease that can cause significant morbidity and long-term neurological problems. Although a lumbar puncture is a necessary diagnostic procedure, it is often accompanied by discomfort, anxiety, and fear, which can severely impact the patient's experience. Although there is few data on its application prior to lumbar puncture in adult patients with meningitis, immersive virtual reality (VR) has become a promising non-pharmacological technique for lowering procedural distress. Thus, among individuals undergoing lumbar punctures, this randomized cont

Downbeat nystagmus without ataxia as an early manifestation of spinocerebellar ataxia, autosomal recessive type 10 (SCAR10): a case report.

The differential diagnosis of dizziness is broad and can include both vestibular and autonomic pathology. Vestibular dizziness is typically described as a sensation of movement (e.g. the world is spinning) while dizziness related to autonomic dysfunction is typically described as symptoms of orthostatic intolerance (e.g. postural lightheadedness). It is important to differentiate the type of dizziness to guide proper diagnostic and therapeutic workup. We describe the case of a young patient evaluated in the autonomic clinic for dizziness, ultimately found to have a rare cerebellar neurodegener

Modified surgical technique for recurrent organized chronic subdural hematoma: a preliminary retrospective case series.

BACKGROUND: The incidence of chronic subdural hematoma (CSDH) increases with age, and the recurrence rate after burr-hole drainage remains relatively high. The clinical management of recurrent organized CSDH continues to pose a challenge. This study aimed to preliminarily evaluate the safety, neurological improvement, and recurrence control of craniotomy for hematoma evacuation combined with middle meningeal artery occlusion and temporalis muscle grafting in the treatment of recurrent organized CSDH. METHODS: A retrospective analysis was performed on 17 patients with recurrent organized CSDH

Changing Brain Death Donation Dynamics in Spain: A National Overview of Neurocritical Care and Organ Donation Trends.

BACKGROUND: Donation after brain death (DBD) has declined in Spain despite sustained transplantation activity and marked growth in donation after circulatory death (DCD). We examined whether this transition was driven by controlled or uncontrolled DCD and contextualized it alongside national trends in neurocritical care and organ utilization. METHODS: We conducted a descriptive ecological analysis of aggregated national data from the Spanish National Transplant Organization, healthcare registries, and epidemiological sources for 2015-2024. Annual DBD, controlled DCD (cDCD), uncontrolled DCD (

Plasma proteomics of cerebrovascular disease, cognitive decline, and clinical outcomes.

INTRODUCTION: The plasma proteomic signatures underlying cerebrovascular disease (CeVD) remains poorly understood. METHODS: A total of N = 2534 participants across two independent longitudinal Southeast-Asian cohorts were included. We profiled 1441 baseline plasma proteins in a memory-clinic cohort (N = 518), followed-up for 4 years. Proteins associating with baseline and longitudinal CeVD lesions (i.e., white matter hyperintensity volume, lacunes, cerebral microbleeds, and cortical infarcts) were reported. The prognostic value of CeVD-associated proteins was evaluated for incident major card

Are there missed warning signals before traffic-related traumatic spinal cord injury? An exploratory case-control study using electronic health records.

BACKGROUND: Traumatic spinal cord injury (TSCI) is a catastrophic outcome frequently resulting from road-traffic crashes. We investigated whether routinely recorded pre-injury healthcare utilization patterns in electronic health records (EHRs) can serve as pragmatic warning signals for TSCI among drivers and motorcyclists. METHODS: We conducted a retrospective age- and sex-matched case-control study using national EHR data. Adults aged 18-45 years with traffic-accident-related TSCI as drivers or motorcyclists (2024-2025) were matched 1: 1 with controls (n=35 per group). Pre-index utilization

Left Atrial Reservoir Strain as a Marker of Carotid Vascular Remodeling: Insights Into the Atrial-Vascular Aging Continuum.

BACKGROUND: Left atrial reservoir strain (LASr) has emerged as a sensitive marker of subclinical cardiac dysfunction and has been proposed as an early indicator of cardiovascular aging. However, whether impaired LASr reflects the progressive spectrum of carotid vascular aging has not been fully established. AIMS: To evaluate the association between LASr and different stages of carotid vascular aging and to determine its ability to identify advanced carotid vascular disease in adults undergoing comprehensive cardiovascular imaging. DESIGN: Retrospective, single-center, observational cross-sec

CGRP-pathway exposure during the pre-EBP interval in spontaneous intracranial hypotension: an exploratory retrospective case series.

BACKGROUND: Given hypothesized involvement of calcitonin gene-related peptide (CGRP) in trigeminovascular pain and the frequent migrainous features of spontaneous intracranial hypotension (SIH), we describe temporal associations between initiation of CGRP-pathway therapies and short-term symptom changes during the interval before image-guided epidural blood patch (pre-EBP) in seven consecutively managed SIH patients. Because this is a small retrospective uncontrolled series, we report exploratory observations only. CASE PRESENTATION: Work-up included selective head-up tilt testing and heavily

Locus Coeruleus as a Master Regulator of Diverse Functions in the Central Nervous System: Its Relevance in Ageing and Neurodegenerative Disorders.

BACKGROUND: The brainstem nucleus locus coeruleus (LC) is a rod-shaped, cylindrical, deeply pigmented neuromelanin (NM)-rich cluster of noradrenergic cells placed bilaterally in the dorsolateral tegmentum of the pons. The main neurotransmitter synthesised by these neurons is norepinephrine (NE), which polymerises to NM. Collectively, it regulates a plethora of activities, including vigilance, synaptic plasticity, memory processing, pain, stress responses, selective attention, cognition, sleep, emotion, capillary wall permeability and cerebral blood flow, thereby holding a 'master key' to sever

Adjunctive middle meningeal artery embolization after surgical evacuation of chronic subdural hematoma: An updated meta-analysis of randomized controlled trials.

BACKGROUND: Middle meningeal artery embolization (MMAE) has emerged as a promising adjunctive treatment for chronic subdural hematoma (cSDH); however, the efficacy and safety of adjunctive MMAE compared with surgery alone remains uncertain. METHODS: We systematically searched PubMed, Embase, and the Cochrane Central Register of Controlled Trials from inception through June 2026. Randomized controlled trials comparing adjunctive MMAE plus surgical evacuation with surgical evacuation alone in patients with cSDH were included. The primary outcome was hematoma recurrence. Pooled odds ratios (ORs)

Autoimmune glial fibrillary acidic protein astrocytopathy mimicking acute ischemic stroke: a case report.

Autoimmune glial fibrillary acidic protein (GFAP) astrocytopathy is a recently identified autoimmune disorder of the central nervous system. It commonly manifests as meningitis, encephalitis, myelitis, or optic neuritis. However, presentations resembling stroke are exceedingly rare. Here, we report a case of autoimmune GFAP astrocytopathy initially misdiagnosed as acute ischemic stroke. The correct diagnosis was later established through the detection of GFAP antibodies in both the patient's serum and cerebrospinal fluid (CSF) using a cell-based assay (CBA). The patient initially responded to

A case report and literature review: Cerebral spinal fluid leak after COVID nasopharyngeal swab testing.

Coronavirus disease 2019 (COVID-19) swab testing has been commonplace since the onset of the pandemic, with few reported adverse effects beyond general discomfort. We report a rare clinical presentation of a cerebrospinal fluid leak following a nasopharyngeal COVID-19 swab. The patient, a 49-year-old female with a history of migraines, idiopathic intracranial hypertension (pseudotumor cerebri), fibromyalgia, depression, and prior traumatic brain injury, presented to the emergency department with a headache. The patient underwent nasopharyngeal swab testing 10 days before her visit. She experie

Multidimensional clustering of clinical, metabolic, and ultrasound features for 30-day mortality risk stratification in ischemic stroke patients with carotid atherosclerotic plaque.

BACKGROUND: Carotid atherosclerotic plaques are a major cause of ischemic stroke, but stenosis-based assessment does not fully capture plaque heterogeneity or prognostic risk. This study aimed to explore data-driven patient phenotypes and preliminary risk stratification by integrating clinical, metabolic, and carotid ultrasound plaque features using unsupervised clustering. METHODS: Ischemic stroke patients admitted within 72 h of onset were retrospectively enrolled from four centers. Baseline demographics, National Institutes of Health Stroke Scale scores, laboratory indicators, carotid ultr

From Endometriosis to Lipedema: Toward a Neuroimmune Framework for Pain Amplification in Hormone-Sensitive Disorders.

Background: Endometriosis and lipedema are chronic female-predominant disorders characterized by persistent pain that is frequently disproportionate to anatomical lesion burden. Although traditionally interpreted within distinct lesion-centered frameworks, both conditions exhibit striking clinical and epidemiological parallels, including hormonally modulated symptom dynamics, overlap with central pain syndromes, weak correlation between structural disease severity and pain intensity, and symptom clustering during reproductive transitions such as puberty, pregnancy, and menopause. Methods: This
