[Ethical Issues in Mechanical Thrombectomy for Acute Ischemic Stroke].

Mechanical thrombectomy (MT) has become the standard treatment for acute ischemic stroke (AIS) caused by large vessel occlusion, and its indications have expanded substantially over the past decade. Recent randomized controlled trials (RCTs) have extended eligibility to patients presenting in extended time windows, those with large ischemic cores, and other populations previously considered unsuitable for intervention. However, the expansion of MT indications has also raised important ethical challenges. Patients with AIS often lack decision-making capacity, requiring physicians to balance res

[Current Systems and Challenges in Acute Ischemic Stroke].

The devices and techniques used in mechanical thrombectomy have reached a high level of maturity. Over the next decade, the focus will shift from an era of developing treatments and technologies, to one of delivering treatments in a sustainable manner. Key challenges include workforce development, the establishment of healthcare systems to reduce regional disparities in access to care, reimbursement reform and healthcare economics, and physician work-style reform through task shifting and task sharing to allied healthcare professionals. As these issues are closely interconnected, comprehensive

[Artificial Intelligence in Acute Ischemic Stroke:Time-Oriented Diagnostic Support and Treatment Prediction].

Artificial intelligence (AI) and machine learning (ML) are increasingly used in acute ischemic stroke. Their clinical value depends not only on diagnostic accuracy but also on whether these systems improve time-critical decision-making. Current applications include prehospital triage, detection of large-vessel and medium-vessel occlusions, automated ASPECTS assessment, estimation of ischemic core and penumbra, collateral evaluation, and workflow notification using CT, CT angiography, CT perfusion, and MRI. AI is also being investigated to predict treatment response, futile recanalization, symp

Integrating 72-h BUN/Cr trajectories with interpretable machine learning for 28-day mortality after acute stroke: a dual-center landmark study.

BACKGROUND: The blood urea nitrogen-to-creatinine ratio (BUN/Cr) may reflect renal perfusion and systemic stress, but its early course after acute stroke is unclear. We identified 72-h BUN/Cr trajectories and assessed their role in interpretable machine-learning evaluation of 28-day mortality. METHODS: This dual-center landmark study included 2,805 ICU patients with acute ischemic stroke, nontraumatic intracerebral hemorrhage, or nontraumatic subarachnoid hemorrhage from the Medical Information Mart for Intensive Care IV (MIMIC-IV) version 3.1. Before modeling, patients were split into a trai

Specific interventions as part of a quality improvement collaborative enhance care for ischemic stroke patients.

INTRODUCTION: Adherence to guidelines for stroke care varies, despite evidence that managing fever, glycemia (sugar), and conducting swallowing screening (FeSS) reduces mortality and disability. We established a quality improvement collaborative to improve care processes for ischemic stroke, with a focus on these three recommendations. We then added two additional care process activities: screening for activities of daily living (ADL) and depression, collectively defined as FeSS+. METHODS: In this multicenter intervention study (n = 24), we retrospectively analyzed 30 consecutive patient reco

Postischemic endothelial immune reprogramming in stroke.

Large-vessel recanalization is essential in acute ischemic stroke, but it does not guarantee microvascular reperfusion. Endothelial responses may contribute to persistent tissue hypoperfusion through leukocyte trafficking and immunothrombosis. Many of these responses may begin during ischemia and may continue or intensify when blood flow returns. We therefore use postischemic or ischemia/reperfusion-associated terminology unless experiments isolate the contribution of reperfusion. We propose that endothelial immune reprogramming requires a coordinated molecular program, localization to a defin

Development of an interpretable machine-learning model to predict insufficient functional improvement after aquatic therapy in stroke.

BACKGROUND: Early identification of stroke survivors who are unlikely to achieve clinically meaningful motor improvement after aquatic therapy may help optimize rehabilitation prescriptions and support precision rehabilitation. This study aimed to develop and validate machine-learning models based on pretreatment Fugl-Meyer Assessment (FMA) data to predict insufficient motor improvement after aquatic therapy, to examine whether actual aquatic-therapy exposure improves prediction, and to identify the most important predictors. METHODS: We conducted a single-center retrospective study of patien

Effectiveness of eptinezumab 300 mg dose escalation in partial responders to 100 mg among patients with difficult-to-treat migraine: A prospective, real-world study by the GRASP study group.

AimThis prospective real-world study aimed to evaluate the effectiveness of escalating eptinezumab to 300 mg in difficult-to-treat migraine patients with a partial response after two 100 mg cycles.MethodsWe included adults with high-frequency episodic or chronic migraine who achieved a partial response (30% to < 50% reduction in monthly headache days (MHD)) after two 100 mg eptinezumab cycles and escalated to 300 mg for two consecutive quarterly cycles. The primary outcome was the proportion of patients achieving a ≥ 50% MHD reduction from baseline after each 300 mg cycle. Secondary outcomes e

Putative Transcriptomic Endotypes Reveal Neuronal, Immune, and Metabolic Heterogeneity in Temporal Lobe Epilepsy with Hippocampal Sclerosis.

Temporal lobe epilepsy with hippocampal sclerosis (TLE-HS) is a biologically heterogeneous disorder in which neuronal loss, gliosis, neuroinflammation, and metabolic stress may coexist but are unlikely to be uniformly represented across all patients. To investigate this heterogeneity at the molecular level, we analysed publicly available hippocampal transcriptomic data from individuals with TLE-HS. Our analyses identified three transcriptomically defined putative endotypes within this shared neuropathological diagnosis. These states were associated with distinct molecular programs, including a

Artificial intelligence for symptom assessment and patient-reported outcomes in neuro-oncology.

PURPOSE: Patient-reported outcomes (PROs) are critical tools for measuring how patients experience their illness and treatment. Neuropsychiatric symptoms, headaches, seizures, and neurological deficits evolve over time and impact quality of life. In this narrative review, we summarize current literature on PROs in neuro-oncology, extant artificial intelligence (AI) tools for PRO analysis, considerations for real-world application, and ethical implications. METHODS: We searched PubMed/MEDLINE, Embase, and Google Scholar for English-language articles published from January 2015 through April 20

Recanalisation quality, not first-pass effect, drives outcomes in paediatric mechanical thrombectomy: a multicentre prospective study.

INTRODUCTION: In adult endovascular thrombectomy (EVT), first-pass success is associated with better outcomes, but whether this "first-pass effect" also holds true in childhood stroke remains unknown. We investigated the association of thrombectomy pass counts with 90-day outcomes after paediatric EVT. PATIENTS AND METHODS: We analysed prospectively collected data from the international Save ChildS Pro registry (ages 0-17 years). All EVT-treated patients were included irrespective of the reperfusion grade achieved. Successful reperfusion was defined as expanded thrombolysis in cerebral infarc

The perspective of combining pulsed field ablation and left atrial appendage closure.

Pulsed field ablation (PFA) and left atrial appendage closure (LAAC) are increasingly discussed as complementary strategies in atrial fibrillation (AF) care, addressing rhythm control and long-term thromboembolic risk. The rationale is attractive, but the current evidence base is uneven. Randomized evidence for LAAC around AF ablation is strongest for thermal-ablation-era workflows, particularly the OPTION trial, which showed lower non-procedure-related major or clinically relevant non-major bleeding with LAAC than with oral anticoagulation (OAC) and non-inferiority for death, stroke, or syste

ADAR1 Inhibits ZBP1-Driven Neuronal Necroptosis After Intracerebral Hemorrhage.

Necroptosis is a critical response to secondary brain injury following intracerebral hemorrhage (ICH). Z-DNA Binding Protein 1 (ZBP1) binds to dsRNA and promotes the activation of Receptor-Interacting Protein Kinase 3 (RIPK3). This study investigated whether Adenosine Deaminase Acting on RNA 1 (ADAR1) inhibits neuronal necroptosis after ICH. ADAR1-knockdown mice and ZBP1flox/flox/Thy1-Cre+/- mice were subjected to collagenase VII-induced ICH. Brain edema and neurobehavioral deficits were assessed. The expressions of ADAR1 and ZBP1 were investigated by qPCR, western blot, immunofluorescence, an

Early Rhythm Control vs Usual Care After Acute Ischemic Stroke in Patients With Atrial Fibrillation.

BACKGROUND AND OBJECTIVES: Early rhythm control reduces cardiovascular events in patients with atrial fibrillation (AF); however, its effectiveness in the setting of acute ischemic stroke remains uncertain. We aimed to examine whether early rhythm control after acute ischemic stroke is associated with a lower risk of recurrent stroke or systemic embolism (SE) in patients with AF. METHODS: This nationwide cohort study used the DeSC-IQVIA integrated database, an administrative claims database covering approximately 16 million individuals in Japan. Adults hospitalized for acute ischemic stroke w

First-In-Human PET Imaging of Synaptic Vesicle Glycoprotein 2A with [18F]BIBD-181: Automation, Biodistribution, Dosimetry, and Preliminary Application in Epilepsy.

We report the first-in-human evaluation of [18F]BIBD-181, a novel SV2A tracer designed with a fluoropropyl side chain to enable mild, automated radiosynthesis. A fully automated cGMP-compliant production process was developed on a commercial synthesizer, yielding the tracer with >95% radiochemical purity and molar activity >130 GBq/μmol. In eight healthy volunteers, [18F]BIBD-181 demonstrated excellent safety, rapid brain penetration peaking at 15 s post-injection, and a regional uptake pattern consistent with known synaptic vesicle glycoprotein 2A (SV2A) distribution (pallidum SUV = 2.01 ± 0.
