Duration of single-agent perioperative antibiotic prophylaxis and postoperative meningitis after endoscopic endonasal skull base surgery: a systematic review and meta-analysis of 7,981 patients.

Postoperative meningitis is a rare but serious complication in endoscopic-endonasal skull-base surgery. Although prophylactic antibiotics are routinely administered, the optimal regimen and duration remain unclear. This systematic review and meta-analysis evaluated the incidence of meningitis and intraoperative cerebrospinal fluid (CSF) leakage associated with single-agent perioperative antibiotic prophylaxis. Following PRISMA 2020 guidelines, PubMed, EMBASE, and Web of Science were searched through February 2025. Eligible studies reported postoperative meningitis or intraoperative cerebrospin

A Low-Cost Device for Teaching Surgical Techniques and Evaluating Technology in Minimally Invasive Surgery.

Minimally invasive surgery (MIS) spans multiple surgical disciplines and offers multiple benefits. Widespread adoption of MIS techniques has been hindered in part by technical challenges from steep learning curves and limited options for low-cost training for surgeons. This technical report describes the design and utility of a 3-D printed MIS simulator as a platform for acquiring skills in MIS techniques, as illustrated by the demonstration and comparison of multiple endoscopic dural repair techniques. The MIS simulator was constructed from 3-D printed and commercially available components, w

Application of cervical epidural blood patch for spontaneous cerebrospinal fluid leaks: Two case reports.

Spontaneous cerebrospinal fluid (CSF) leaks are uncommon but significant causes of orthostatic headaches, often resulting from dural injuries. Although lumbar epidural blood patches (EBPs) are commonly used to treat such leaks, cervical EBPs are rarely performed due to the higher risk of complications, including spinal cord injury. This case series presents two patients with spontaneous cervical CSF leaks successfully treated with cervical EBPs under fluoroscopic guidance. The first case involved a 24-year-old female who presented with a two-month history of severe orthostatic headaches and ti

Recommendations for the Management of Spontaneous Intracranial Hypotension With Spinal Longitudinal Epidural CSF Collection: A Delphi Study.

BACKGROUND AND OBJECTIVES: Spontaneous intracranial hypotension (SIH) results from spinal CSF leakage. Epidural blood patch (EBP) is an important first-line treatment. However, pre-EBP, intra-EBP, and post-EBP practice varies widely owing to limited consensus guidance. We aimed to develop expert consensus statements to support clinical decision-making in SIH management, including peri-EBP practices, focusing on patients with spinal longitudinal epidural CSF collection (SLEC) before definitive leak type classification. METHODS: An international multidisciplinary Delphi process was conducted be

The Relationship among Obesity, Intracranial Hypertension, and Cerebrospinal Fluid Leak.

Idiopathic intracranial hypertension (IIH) is a disorder characterized by elevated intracranial pressure without identifiable lesion or secondary cause. Patients can often present with otolaryngologic manifestations of IIH, such as pulsatile tinnitus, aural fullness, and cerebrospinal fluid (CSF) leak. Obesity plays an important role as a risk factor for both IIH and CSF leak as similar pathophysiological processes underly both presentations. Understanding the nuanced presentation of this condition is important for the otolaryngologist who routinely evaluates patients with these symptoms and h

Artificial intelligence-based prediction of cerebrospinal fluid leak in skull base surgery: a scoping review.

BACKGROUND: Cerebrospinal fluid (CSF) leak remains one of the most significant complications of skull base surgery, particularly in endoscopic endonasal and transsphenoidal approaches. Accurate risk stratification may improve surgical planning and reconstruction strategies. Artificial intelligence (AI)-based models have been increasingly investigated for CSF leak prediction; however, the evidence remains methodologically heterogeneous. This scoping review aimed to map the current literature and summarize data sources, modeling approaches, validation strategies, and reported performance. METHO

The Slalom Technique as a Motion-Preserving Approach for Multilevel Lumbar Spinal Stenosis: A Systematic Review and Meta-analysis.

Multilevel degenerative spinal stenosis (DSS) is surgically challenging due to longer operative times, greater blood loss, and risk of instability. The Slalom technique, a motion-preserving, minimally invasive approach, has shown promise; however, no randomized controlled trials currently exist to validate its efficacy against standard techniques. This meta-analysis evaluated its operative outcomes in multilevel DSS. Following PRISMA guidelines and PROSPERO registration (CRD420251065160), a comprehensive literature search was performed across PubMed, Embase, and the Cochrane Library through Ja

Prospective Randomized Controlled Study Comparing Decompressive Craniectomy with Watertight Duraplasty and without Watertight Duraplasty in Traumatic Brain Injury.

BACKGROUND: Decompressive craniectomy (DC) is a life-saving procedure for refractory intracranial hypertension following traumatic brain injury (TBI). Despite its widespread use, there is no consensus regarding optimal dural management. Whether watertight duraplasty is necessary during DC remains controversial. OBJECTIVE: The aim of the study is to compare clinical outcomes, postoperative complications, and cranioplasty-related outcomes following DC performed with and without watertight duraplasty in patients with TBI. MATERIALS AND METHODS: This prospective randomized controlled study inclu

Durotomy Incidence in Degenerative Lumbar Spine Surgeries: A Systematic Review and Comparative Meta-Analysis of Proportions.

BACKGROUND AND OBJECTIVES: Spinal durotomy (SD) is a well-recognized occurrence and potential adverse event of lumbar spine surgery for degenerative conditions. Incidences have been reported between 8.11% in primary open surgery and 6.78% in minimally invasive surgery (MIS). However, given increasing surgical volume, surgical complexity, and evolving techniques, an updated synthesis of contemporary incidence is needed. The purpose here is to determine the overall incidence of SD in lumbar spine surgery for degenerative pathology and various subgroups of surgeries. METHODS: A systematic review

Relationship Between Benign Intracranial Tumor Location and the Side of Spontaneous Cerebrospinal Fluid Leak.

AIM: To present cases of spontaneous cerebrospinal fluid (CSF) rhinorrhea and otorrhea, and to conduct a systematic literature review examining the relationship between benign tumor location and the leak side. MATERIAL and METHODS: A PRISMA-based systematic review was conducted to evaluate studies on spontaneous CSF leaks caused by benign intracranial tumors, alongside three newly reported cases. RESULTS: All three cases exhibited CSF leaks ipsilateral to the tumor: Case 1 involved a pituitary ACTH-secreting macroadenoma with rhinorrhea; Case 2, a pineocytoma with hydrocephalus and nasal encep

Acute subdural hematoma in spontaneous intracranial hypotension: illustrative case and review of the literature.

INTRODUCTION: Spinal cerebrospinal fluid (CSF) leaks cause spontaneous intracranial hypotension (SIH) and are a recognized cause of chronic subdural hematoma (cSDH). In contrast, acute subdural hematoma (aSDH) without trauma as the initial manifestation of a spinal CSF leak appears to be exceedingly rare. RESEARCH QUESTION: Can a primary non-traumatic aSDH represent the first manifestation of a spinal CSF leak. MATERIAL AND METHODS: We report an illustrative case of a 52-year-old man presenting with a non-traumatic aSDH requiring emergency surgical evacuation together with a PRISMA-based lit

Prone Digital Subtraction Myelography vs Dynamic CT Myelography for Detecting Definite Type 1 CSF Leak in Spontaneous Intracranial Hypotension.

BACKGROUND AND PURPOSE: Ventral dural tears (type 1) are a major cause of spontaneous intracranial hypotension (SIH) and require precise localization for targeted treatment. Although prone digital subtraction myelography (DSM) and dynamic CT myelography (dCTM) are standard diagnostic modalities, their diagnostic performance has not been directly compared. We aimed to evaluate the diagnostic yield and radiation exposure of prone DSM compared with dCTM in a paired cohort. MATERIALS AND METHODS: In this ambispective study, we analyzed 39 patients with type 1 leaks who underwent both prone DSM an

Long-Term Outcomes of Endoscopic Endonasal Resection for Tuberculum Sella Meningiomas.

BACKGROUND AND OBJECTIVES: Tuberculum sella meningiomas (TSM) constitute 5% to 10% of intracranial meningiomas and pose surgical challenges because of their proximity to critical neurovascular structures and the optic chiasm. We aimed to evaluate the long-term efficacy of the endoscopic endonasal approach (EEA) for TSMs at a university skull base center. METHODS: A retrospective study of patients with TSM treated using EEA between 2008 and 2024 with a minimum 6 months of follow-up. Clinical outcomes, including the extent of resection, visual outcomes, complications, and recurrence rates, were

A hypertensive response to irrigation: endoscopic repair of a ventral spinal CSF leak.

Spontaneous intracranial hypotension (SIH) is most commonly caused by spinal CSF leaks, with ventral dural defects frequently requiring surgical repair when conservative measures fail. Endoscopic transforaminal approaches have emerged as minimally invasive alternatives to traditional open techniques, but the physiological consequences of continuous irrigation in the presence of a dural defect remain poorly understood. The authors report the case of a 36-year-old woman with recurrent SIH due to a persistent ventral T11-12 CSF leak despite prior laminectomy, revision repair, and multiple targete

Ultra-Rapid β2-Transferrin Lateral Flow Immunoassay Enables Accurate Point-of-Care Detection of Cerebrospinal Fluid.

BACKGROUND AND OBJECTIVES: Cerebrospinal fluid (CSF) leak diagnosis often relies on laboratory β2-transferrin testing, which is specific but not optimized for immediate bedside decision making. We air to evaluate a rapid β2-transferrin lateral flow immunoassay (LFIA) for point-of-care CSF detection. METHODS: Human CSF samples were obtained from adult patients with external ventricular drains and tested using a blinded, standardized LFA protocol. Sterile saline and non-CSF drain fluid served as negative controls. Selected CSF samples underwent 1:10 and 1:100 saline dilution. Results were class
