Can the efficacy of bone wax sealing method in dealing with cerebrospinal fluid leakage during ACDF be alternative to that of dural patch repair method?

OBJECTIVE: Anterior cervical discectomy and fusion (ACDF) is an important treatment for cervical degenerative diseases and trauma. Cerebrospinal fluid (CSF) leakage caused by intraoperative dural injury is a rare but clinically significant complication. The current mainstay approach involves dural repair using patches and fibrin glue. The bone wax sealing method is a novel technique for managing intraoperative CSF leakage during ACDF. This study aims to analyse the effectiveness of bone wax sealing in treating intraoperative CSF leakage. METHODS: A retrospective analysis was conducted on cons

Anterior Midline Skull Base Meningiomas: A Systematic Review of Resection Rates, Functional Outcomes, and Perioperative Complications Following Contemporary Endoscopic Endonasal Versus Transcranial Approaches.

Objectives: Anterior midline skull base meningiomas, such as olfactory groove meningiomas (OGMs), planum sphenoidale meningiomas, and tuberculum sellae meningiomas pose significant surgical challenges due to their proximity to neurovascular and olfactory structures. The widespread use of the vascularized nasoseptal flap (NSF) has expanded the role of the endoscopic endonasal approach (EEA). This systematic review compares outcomes of EEA and the transcranial approach (TCA) in the NSF era. Methods: PubMed, Embase, and Scopus were searched following PRISMA guidelines for studies published Januar

CT-Guided Epidural Blood Patch for Postoperative Lumbar Cerebrospinal Fluid Leak: A Case Series and Clinical Outcomes.

Inadvertent dural injury during lumbar spine surgery is a common complication with reported incidence up to 21%. When inadequately repaired or unrecognized, this can lead to cerebrospinal fluid (CSF) leak and orthostatic headache. Current management strategies include conservative treatment, surgical dural repair, or subarachnoid drain placement. The purpose of this study was to evaluate the technical feasibility and early clinical outcomes of CT-guided epidural blood patch (EBP) as a minimally invasive option for symptomatic postoperative CSF leaks. A retrospective analysis was performed on p

Pneumocephalus Secondary to Sternutation: A Case Report.

INTRODUCTION: Sternutation is a physiological reflex that clears the upper respiratory tract through forceful air expulsion. Although it is typically considered benign, sternutation can generate substantial pressure and airflow that can result in barotrauma, including pneumocephalus. CASE REPORT: A 67-year-old female presented with shortness of breath, rhinorrhea, and a headache following sneezing. Physical exam revealed no signs of trauma or neurological deficits but did note clear rhinorrhea bilaterally. Computed tomography (CT) of the head revealed extensive extra-axial intracranial gas bi

Transforaminal Endoscopic Extradural Onlay Patch Graft Repair of Spontaneous Spinal Type 1a Cerebrospinal Fluid Leaks: A Case Series.

BACKGROUND AND OBJECTIVES: Spontaneous spinal cerebrospinal fluid (sCSF) leaks represent a significant source of morbidity, presenting with symptoms of spontaneous intracranial hypotension. Type 1a leaks are classified as ventral dural tears, with thoracic defects proving to be especially challenging to diagnose and treat. Epidural blood patches remain first-line for many cases; however, their efficacy is variable. Traditional surgical management of sCSF leaks typically involves open laminectomy with transdural direct suture repair, with attendant risks due to wide laminectomy and spinal cord

Comparative surgical outcomes of pterional, supraorbital, and endoscopic endonasal approaches for tuberculum sellae meningiomas: a systematic review and network meta-analysis.

BACKGROUND: The optimal surgical corridor for tuberculum sellae meningiomas (TSMs) remains a subject of ongoing debate. While the pterional (PT) approach is the traditional gold standard, the endoscopic endonasal approach (EEA) and supraorbital (SO) keyhole approach have emerged as viable alternatives. OBJECTIVE: To comparatively evaluate the surgical outcomes and complication profiles of the PT, SO, and EEA techniques for the management of TSMs. METHODS: A systematic literature search was conducted across PubMed, Embase, Scopus, Web of Science, and Cochrane databases through March 2026. Use of Cyanoacrylate (Superglue) for the Repair of Incidental Durotomy During Lumbar Spine Surgery: A Low-Cost Technique in a Resource-Limited Setting.

Background Incidental dural tears are a recognized complication of lumbar spine surgery and can lead to significant postoperative morbidity if not properly managed. Standard repair techniques often rely on specialized grafts and sealants that may be costly or unavailable in low-resource settings. This study reports our experience with the use of cyanoacrylate adhesive (superglue) for intraoperative repair of dural tears in a resource-limited environment. Methods We retrospectively reviewed patients who sustained incidental dural tears during lumbar spine surgery at a tertiary spine centre in a

A comparative analysis of early postoperative outcomes of autologous pericranium versus high-density polypropylene in cranial duraplasty in a tertiary care center in Central India.

BACKGROUND AND OBJECTIVES: Duraplasty is commonly required when primary dural closure is not feasible during cranial surgery. Although both autologous and synthetic dural substitutes are widely used, high-quality prospective data comparing their early postoperative safety remain limited. This study aimed to compare early postoperative complication rates following cranial duraplasty using autologous pericranium versus high-density polypropylene (G-Patch). METHODS: This prospective randomized comparative study enrolled 121 consecutive patients undergoing cranial duraplasty at a tertiary care ce

Modified surgical technique for recurrent organized chronic subdural hematoma: a preliminary retrospective case series.

BACKGROUND: The incidence of chronic subdural hematoma (CSDH) increases with age, and the recurrence rate after burr-hole drainage remains relatively high. The clinical management of recurrent organized CSDH continues to pose a challenge. This study aimed to preliminarily evaluate the safety, neurological improvement, and recurrence control of craniotomy for hematoma evacuation combined with middle meningeal artery occlusion and temporalis muscle grafting in the treatment of recurrent organized CSDH. METHODS: A retrospective analysis was performed on 17 patients with recurrent organized CSDH

CGRP-pathway exposure during the pre-EBP interval in spontaneous intracranial hypotension: an exploratory retrospective case series.

BACKGROUND: Given hypothesized involvement of calcitonin gene-related peptide (CGRP) in trigeminovascular pain and the frequent migrainous features of spontaneous intracranial hypotension (SIH), we describe temporal associations between initiation of CGRP-pathway therapies and short-term symptom changes during the interval before image-guided epidural blood patch (pre-EBP) in seven consecutively managed SIH patients. Because this is a small retrospective uncontrolled series, we report exploratory observations only. CASE PRESENTATION: Work-up included selective head-up tilt testing and heavily

A case report and literature review: Cerebral spinal fluid leak after COVID nasopharyngeal swab testing.

Coronavirus disease 2019 (COVID-19) swab testing has been commonplace since the onset of the pandemic, with few reported adverse effects beyond general discomfort. We report a rare clinical presentation of a cerebrospinal fluid leak following a nasopharyngeal COVID-19 swab. The patient, a 49-year-old female with a history of migraines, idiopathic intracranial hypertension (pseudotumor cerebri), fibromyalgia, depression, and prior traumatic brain injury, presented to the emergency department with a headache. The patient underwent nasopharyngeal swab testing 10 days before her visit. She experie

Endoscopic Craniofacial Resection for Anterior Skull Base Tumors: A Singapore Center Review.

INTRODUCTION: Anterior skull base (ASB) tumors are rare and comprise a variety of pathologies. In recent years, the surgical management of ASB tumors has undergone a paradigm shift from open craniofacial resection (CFR) to minimally invasive endoscopic approaches. Endoscopic-assisted (EaCFR) and entirely endoscopic (ECFR) CFR have shown equivalent oncological outcomes without the morbidity of open surgery. This study presents our tertiary center experience and outcomes with endoscopic approaches for ASB tumors. METHODS: A retrospective review of all patients who underwent EaCFR and ECFR for A
