Predictive factors of epidural blood patch success in intracranial hypotension syndrome.

OBJECTIVE: This study aimed to identify predictors of epidural blood patch (EBP) success in patients with intracranial hypotension syndrome. BACKGROUND: The epidural blood patch remains the gold standard treatment for intracranial hypotension syndrome, yet its effectiveness varies, and predictors of sustained success remain uncertain. METHODS: We performed a single-center retrospective cohort study. We analyzed 139 epidural blood patches performed for non-obstetric intracranial hypotension syndrome between April 2015 and July 2025. Demographic, clinical, biological, radiological, and procedu

Impaired Consciousness Following Cerebrospinal Fluid Leakage After Halo Vest Fixation for Odontoid Fracture: A Case Report.

CASE: An 82-year-old woman sustained an Anderson Type II odontoid fracture after a fall and underwent halo vest fixation. Eight weeks later, she developed eyelid edema and altered consciousness. Computed tomography revealed intracranial pin penetration, cerebrospinal fluid (CSF) leakage, and pneumocephalus. The halo vest was removed, and subsequent conservative treatment with bed rest and antibiotics led to full neurological recovery. CONCLUSION: This rare case of intracranial pin penetration and CSF leakage following halo vest fixation emphasizes the importance of close monitoring and early

Outcomes of endoscopic and microscopic transsphenoidal pituitary surgery: evidence from a systematic review, meta-analysis, and institutional experience.

To compare surgical outcomes between endoscopic and microscopic transsphenoidal pituitary surgery using international comparative evidence and an institutional cohort. A PRISMA-based systematic review and meta-analysis of comparative transsphenoidal pituitary surgery studies was performed. Gross total resection was the primary endpoint. Secondary outcomes included operative time, intraoperative blood loss, cerebrospinal fluid leak, epistaxis, meningitis, diabetes insipidus, SIADH, and visual outcomes. Fixed- and random-effects models were applied. Fifty-six comparative studies including 26,295

Rigid sellar floor reconstruction in transsphenoidal surgery: prevention of cerebrospinal fluid leakage attributable to the water-hammer effect.

Postoperative cerebrospinal fluid (CSF) leakage remains one of the most challenging complications of transsphenoidal surgery (TSS). Numerous reconstruction techniques have been proposed, but none has been established as the gold standard. To address this issue, we standardized rigid sellar floor reconstruction using a hard buttress, based on the concept of counteracting the water-hammer effect of pulsatile CSF pressure. We retrospectively analyzed 1,168 consecutive TSS procedures for sellar lesions performed by a single surgeon between October 2004 and March 2024. Routine rigid reconstruction

Surgical outcomes and predictors of recurrence following endoscopic endonasal marsupialization of Rathke's cleft cysts.

BACKGROUND: Endoscopic endonasal marsupialization (EEM) has been widespread adopted as drainage-based techniques for treatment of Rathke's cleft cyst. However, the effectiveness of marsupialization patency and recurrence risk remains inadequately defined. This study evaluated the correlation between sustained marsupialization opening and recurrence-free survival following EEM, and identified independent predictors of recurrence. METHODS: We performed a retrospective analysis of 54 consecutive patients with symptomatic Rathke's cleft cysts in whom endoscopic endonasal marsupialization was inte

Plastic and reconstructive surgery in spinal sarcoma resection: a review of indications, techniques, and outcomes.

PURPOSE: Soft-tissue reconstruction following oncologic resection of spinal tumors often leaves extensive dead space vulnerable to infection, cerebrospinal fluid leak, and impaired function. En bloc resection with wide margins remains standard for disease control, but closure of these complex defects is challenging. Plastic and reconstructive surgeons (PRS) are often consulted for soft-tissue closure, yet their role in spinal sarcoma care has not been comprehensively reviewed. We aimed to (1) describe indications and timing of PRS consultation, (2) summarize reconstructive options by anatomic

Spontaneous cerebrospinal fluid leaks and encephaloceles: multi-institutional collaborative retrospective analysis of 468 cases with special focus on recurrent cases.

PURPOSE: Spontaneous cerebrospinal fluid (s-CSF) leaks and encephaloceles pose significant challenges in diagnosis, treatment, and long-term management. Although their association with idiopathic intracranial hypertension (IIH), obesity, and obstructive sleep apnoea is increasingly recognized, predictors of recurrence remain unclear. This study analyses a large multi-institutional cohort of patients with s-CSF leaks, with a specific focus on recurrent cases. METHODS: A multicenter retrospective cohort study was performed. Patients with confirmed s-CSF leaks who underwent surgical repair were

Postoperative Management After Transsphenoidal Resection of Pituitary Tumors.

The postoperative period after transsphenoidal resection of a pituitary tumor is critical for the detection and management of endocrine, visual, and cerebrospinal fluid (CSF)-related complications. We describe the structured approach tailored to patients undergoing microscope-assisted transsphenoidal resection used at our institution. This step-by-step postoperative protocol, refined over years of multidisciplinary care in a high-volume pituitary surgery practice, emphasizes early recognition of diabetes insipidus, management of hormonally active tumors, careful CSF leak monitoring, and clear

Multidisciplinary management for watertight surgical closure of pneumatized mastoid breach using fascia lata graft, following neurosurgical retrosigmoid approach: a technical note.

The retrosigmoid approach is a standard surgical corridor to the posterior fossa but may lead to mastoid air cell opening and subsequent cerebrospinal fluid (CSF) leakage or infectious complications. Achieving durable closure can be challenging, particularly in revision surgery due to inflammatory changes and altered anatomical landmarks. We report the case of a 76-year-old patient who developed recurrent infectious complications following a right retrosigmoid approach for posterior fossa meningioma. Imaging revealed a subcutaneous abscess associated with persistent mastoid air cell opening. A

Surgical Management of a Large Multiloculated Thoracolumbar Spinal Extradural Arachnoid Cyst via Decompressive Laminectomy With Selective Foraminal Fenestration: A Case Report.

Spinal extradural arachnoid cysts (SEACs) are rare, benign, non-neoplastic lesions arising from focal dural defects, most frequently reported in the thoracic spine. Large multilocular with intraforaminal extension SEACs represent a challenging surgical case, as attempts for complete resection may result in dural defect and cerebrospinal fluid (CSF) leakage. The optimal surgical approach in the setting of adherent foraminal involvement remains an area of debate. We report a 32-year-old woman who presented with a three-month history of progressive low back pain, bilateral lower limb pain and wea

Risk factors of cerebrospinal fluid leakage following posterior lumbar interbody fusion for lumbar degenerative disease: A retrospective observational study of 684 patients.

This study holds the primary objective of confirming the risk factors for cerebrospinal fluid leakage (CSFL) following posterior lumbar interbody fusion for lumbar degenerative disease. The 684 patients who received posterior lumbar interbody fusion for lumbar degenerative disease at our hospital from 2017 to 2023 fell into the CSFL group (n = 115) or the control group (n = 569). We retrospectively recorded demographic, surgical, and radiographic data for the examination of the independent risk factors related to CSFL. Univariate and multivariate logistic regression analyses were conducted to

Microsurgical Untethering of Pediatric Lipomyelomeningocele: A Stepwise, Photo-Illustrated Technical Note.

Lipomyelomeningocele (LMMC) is one of the most common forms of occult spinal dysraphism, with an estimated incidence of 3-6 per 100,000 live births, and microsurgical untethering remains the cornerstone of management for symptomatic and selected at-risk children. The operation is technically demanding: reported rates of long-term symptomatic re-tethering after partial resection still reach 15-25%, and the surgeon must balance adequate untethering against preservation of the placode and lumbosacral nerve roots. In this technical note, we present a stepwise, illustrated description of our instit

Endoscopic Endonasal Approach for Resection of Clival Chordomas: A Systematic Review and Meta-Analysis.

BACKGROUND AND OBJECTIVES: Clival chordoma is a rare, locally invasive tumor originating from notochord remnants. The endoscopic endonasal approach (EEA) offers direct midline access, but the ideal balance between radicality and safety remains debated because of perioperative complications and mortality. The aim of this study was to synthesize oncologic and safety outcomes of EEA for clival chordomas, focusing on extent of resection, recurrence, and complications. METHODS: PubMed, Embase, Scopus, and Web of Science were searched through September 2025. We included observational studies of cli

Delayed diagnosis and surgical repair of a chronic post-traumatic cerebrospinal fluid fistula with encephalocele in a child.

BACKGROUND: Encephalocele is a pathological condition characterized by the herniation of intracranial contents through a cranial bone defect. Although most cases are congenital, traumatic encephaloceles can occur following blunt head injury, surgical interventions, or other acquired causes. These lesions are clinically significant due to their association with cerebrospinal fluid (CSF) leakage and the risk of life-threatening meningitis. Traumatic encephaloceles in children are exceedingly rare, particularly those involving the fronto-ethmoidal region. CASE DESCRIPTION: We report the case of

Lumbar microspur-related ventral cerebrospinal fluid leak localized by digital subtraction myelography: A case report.

Spontaneous intracranial hypotension (SIH) is commonly caused by spinal cerebrospinal fluid (CSF) leakage. Microspur-related ventral dural tears are an increasingly recognized etiology of SIH; however, lumbar lesions remain extremely rare. A 41-year-old woman presented with severe orthostatic headache and nausea. Whole-spine magnetic resonance imaging demonstrated spinal longitudinal extradural CSF collection (SLEC), suggesting spinal CSF leakage. Computed tomography myelography identified a small calcified lesion at the L1/2 level consistent with a discogenic microspur compressing the ventral
