Closed subarachnoid drainage for management of cerebrospinal fluid leakage after an operation on the spine.

A retrospective review was conducted to assess the effectiveness and safety of a temporary subarachnoid shunt to treat patients who have a leak of cerebrospinal fluid after a spinal operation. The shunt is percutaneously inserted in the lumbar spine and is removed after four days. This technique was used in nineteen patients over a ten-year period. Of the seventeen patients who had the shunt in place for the full four days, fourteen had resolution of the drainage of cerebrospinal fluid from the wound. One of two patients whose shunt was removed early also had a successful result. Eleven of the

Results of transsphenoidal microsurgery for growth hormone-secreting pituitary adenoma in a series of 214 patients.

Of 214 patients with acromegaly who underwent transsphenoidal microsurgical resection of a pituitary adenoma, 54% had growth hormone (GH) levels below 5 ng/ml and 74% had levels less than 10 ng/ml immediately after surgery. Among the 174 patients who could be contacted for long-term follow-up review (average duration 76 months), most recent GH determinations were available for 165. Of these 165 patients, 131 (79.4%) have a GH level less than 5 ng/ml and 153 (92.7%) have a level below 10 ng/ml; these represent 75.3% and 87.9%, respectively, of the total 174 patients reviewed. Fifty-two patients

Management of large pituitary adenomas by transsphenoidal surgery.

We evaluated the ability of transsphenoidal surgery to produce visual improvement, endocrine remission, and complete tumor removal in 113 patients with pituitary adenomas extending beyond the sella turcica. Eighty-one percent of patients with preoperative visual field defects had postoperative improvement, 19% had no change, and none deteriorated significantly. Seventy-nine percent of patients with diminished preoperative visual acuity had postoperative improvement; none deteriorated. Endocrine remission of functioning tumors was achieved in 43% of patients and complete resection by computed t

Use of bacitracin for neurotologic surgery.

Wound infection, cerebrospinal fluid leak, and meningitis are serious potential complications of neurotologic procedures that transgress the posterior cranial fossa dura. A study of 236 patients was made to determine the effect of perioperative intravenous antibiotics and topical bacitracin irrigation on the incidence of these complications. Of the 236 patients, 170 (72%) underwent translabyrinthine resection of acoustic tumors, while 66 (28%) underwent retrolabyrinthine vestibular nerve section. Patients were divided into four groups: those who received no antibiotics, those who received peri

Spinal cerebrospinal fluid leak demonstrated by radioisotopic cisternography.

Many conditions are known to cause a cerebrospinal fluid (CSF) fistula; one of them is lumbar puncture for contrast myelography. Reported here is the case of a man who underwent contrast myelography at the L2-L3 level and who presented with postural headaches and lumbalgia with radiation to the legs three weeks after the procedure. Tc-99m albumin isotopic cisternography at the L5-S1 level was performed and clearly depicted a functional CSF leak through the dura at the L2-L3 level and CSF suffusion along several rachidian roots. Scinticisternography may thus be used to localize accurately a CSF

Spontaneous cerebrospinal fluid otorrhea.

A case of spontaneous cerebrospinal fluid otorrhea due to a congenital tegmen tympani defect is reported. The etiology and pathogenesis of cerebrospinal fluid otorrhea and of the spontaneous congenital form in particular is discussed, together with the clinical picture and the methods of diagnosis. Reviewing the literature, a middle ear exploration is considered to be the procedure of choice to ascertain the presence and determine the exact locus of the cerebrospinal fluid leak.

Nosocomial Hemophilus influenzae type C meningitis in an adult.

Hemophilus influenzae type C meningitis developed in a 68-yr-old man with a cerebrospinal fluid leak which occurred after craniotomy for an olfactory groove meningioma. Hemophilus influenzae is an uncommon cause of meningitis in adults, and most reported cases have been due to type B or nontypeable strains.

Temporoparietal craniopagus. Case report and review of the literature.

A case of craniopagus twins joined in the temporoparietal area is presented, along with a review of the literature on craniopagus. A large area of brain was shared between the neurologically normal infants, with defects in the scalp, skull, and dura. The twins were separated in a three-step procedure. First, areas of shared brain were divided and separated with silicone sheets. The second procedure consisted of the insertion of scalp expanders to allow primary skin closure. In the third procedure complete separation was performed which was complicated by severe hypotension in one infant that w

Early repair of myelomeningocele and simultaneous insertion of ventriculoperitoneal shunt: technique and results.

During the years 1975 to 1985, 10 patients underwent simultaneous repair of myelomeningocele and insertion of a ventriculoperitoneal shunt at the Hospital for Sick Children in Toronto. The technique for carrying out these procedures in the same operative setting will be described. None of the patients developed shunt infections or myelomeningocele repair breakdown in the follow-up period of 1 to 9 years. One patient died 9 days after operation secondary to aspiration. The remaining patients underwent neuropsychological testing during their follow-up periods and over 70% were found to have norm

Craniofacial resection of extensive benign lesions of the anterior skull base.

The craniofacial resection operation was developed for the treatment of advanced nasal, paranasal and orbital malignancies. It has been refined in recent years, giving increased cure rates and better palliation when cure is not possible. When used to treat extensive benign lesions involving the anterior skull base, this procedure allows more complete and safer resection with better access for repair and avoidance of major complications of brain damage, cerebrospinal fluid leak and haemorrhage. Presented here is a technique for craniofacial resection. The study demonstrates its effectiveness an

Cryptic otalgia: a case of Munchausen syndrome in a pediatric patient.

This paper reports a case of Munchausen syndrome in a pediatric patient. The patient, a 13-year-old boy, presented with a complaint of persistent otalgia. As a result of the patient's deception of his physicians, he underwent numerous unnecessary investigations and two unnecessary operative procedures. The patient's deception included the simulation of a cerebrospinal fluid leak. The literature with respect to Munchausen syndrome in the pediatric patient is reviewed. Emphasis is placed on the fact that physicians, by their investigations and treatments, inflict most of the morbidity on this gr

Complications following enucleation and implantation of multiple glass spheres in the orbit.

The technique of subperiosteal implantation of multiple, small glass beads for correction of enophthalmos associated with anophthalmos was first described in 1967. Reported complications of this procedure include ptosis, anesthesia of the distribution of the supraorbital or infraorbital nerve, and migration of the implants into the orbit or sinuses. A case of orbital cellulitis has been reported. We now report a case in which the extremely serious complication of intracranial migration of glass bead implants, with subsequent cerebrospinal fluid leak, occurred 17 years postimplantation.

Injury to the orbital plate of the frontal bone.

Twelve cases of fracture of the orbital plate of the frontal bone are reviewed to clarify this important clinical problem and to suggest satisfactory methods of management. Common signs and symptoms include forehead laceration and deformity, and fracture of the frontal sinus. Loss of vision can occur, and in the seeing eye, diplopia can develop in several fields of gaze. Downward or upward projection of the orbital plate of the frontal bone can cause exophthalmos or enophthalmos. Cerebrospinal fluid leak, with or without meningitis, and seizures can also be part of the syndrome. Effective trea

Treatment of prolapsed conjunctiva.

Prolapse of conjunctiva from the superior cul-de-sac, an infrequent complication of levator resection surgery, has been observed by us to have occurred following craniofacial reconstruction for craniostenosis. Three cases of conjunctival prolapse are presented: two that followed maximal external levator resections, and one that occurred after a supraorbital rim advancement and a subsequent cerebrospinal fluid leak. The causes of conjunctival prolapse and its appropriate treatment are reviewed.

Spontaneous encephalocele of the temporal bone.

Postsurgical and posttraumatic encephaloceles of the tegmen tympani and tegmen mastoideum are well-recognized occurrences. Less frequently recognized, however, is the spontaneous occurrence of an encephalocele associated with cerebrospinal fluid otorrhea or rhinorrhea. This cerebral herniation into the middle ear and the mastoid cavity in adults is associated with a loss of both bony and dural support, and surgical management must deal with both the cerebrospinal fluid leak and herniation of the brain.

Management of invasive frontoethmoidal sinus mucoceles.

Invasive frontoethmoidal sinus mucoceles extending into the anterior cranial fossa or orbits are difficult to manage and can lead to lethal complications. In the past four years, nine cases of frontoethmoidal mucoceles were treated at the University of Texas Medical Branch, Galveston. Five cases were complicated by anterior cranial fossa invasion, orbital invasion, or both. The choice of surgical procedures used to manage these cases depended on mucocele extent and location, which were best determined by computed tomography. Two cases required craniotomy, one of which required an inferior-base

Pseudoepiphora from cerebrospinal fluid leak: case report.

A 4-year-old tearing child with obstruction of the nasolacrimal duct was treated with dacryocystorhinostomy three years after naso-orbital injury. However, what appeared to be tearing persisted, and meningitis developed. Coronal CT scans demonstrated traumatic encephalocele of the posterior superior orbital roof. A chronic orbital cerebrospinal fluid (CSF) leak was diagnosed. To our knowledge no case of chronic CSF leak has been reported that simulated tearing in an otherwise asymptomatic child. In the tearing patient who has a naso-orbital fracture the possibility of chronic CSF leak should b
