Long-term follow up of trans-sphenoidal hypophysectomy for Cushing's disease.

Fourteen patients with Cushing's disease treated by trans-sphenoidal hypophysectomy between 1962 and 1975 were reviewed in 1983. Complete ablation had been attempted. There were no surgical deaths and one episode of bacterial meningitis. Two patients required a second operation for a cerebrospinal fluid leak. There have been three late deaths from unrelated causes. All patients had a biochemical remission of their Cushing's disease postoperatively and no relapse has been recorded. Most patients need some hormone replacement but residual pituitary function and sella radiography have remained st

Diagnosis and treatment of iatrogenic cerebrospinal fluid leak and brain herniation during or following mastoidectomy.

Iatrogenic brain herniation, with meninges (meningoencephalocele) or without meninges (encephalocele), following mastoidectomy is rare. However, the seriousness of the complication and the potential for its prevention necessitate periodic review and emphasis. In this study, 6 patients with small iatrogenic dural injury during mastoidectomy, 1 patient with postoperative large encephalocele, and 2 patients with three spontaneous meningoencephaloceles are reviewed. These cases, and the literature, support the following: 1. A dural injury is necessary for herniation. 2. If the arachnoid remains in

Subarachnoid space: middle ear pathways and recurrent meningitis.

Congenital bony abnormalities of the inner ear may result in a communication between the middle ear and the subarachnoid space. Patients with this anomaly often present with recurrent meningitis associated with acute otitis media or with middle ear fluid. This article presents three cases of recurrent meningitis with open middle ear--subarachnoid space connections. The first two cases involve a cerebrospinal fluid leak into the middle ear via the oval window, both patients having a Mondini-type of inner ear deformity. The pathway in the third case opened into the middle ear along the horizonta

Radionuclide demonstration of ventriculoatrial shunt-cutaneous fistula with cerebrospinal fluid leak.

Radionuclide techniques have commonly been used to assess ventriculovenous shunt function, cerebrospinal fluid dynamics, and for the diagnosis of cerebrospinal fluid leaks. All of these techniques were utilized in this unusual case of retrograde migration of a ventriculoatrial catheter into the neck, with cutaneous fistulization and CSF leak.

Meningitis caused by Mycobacterium fortuitum.

A previously healthy 16-yr-old Caucasian male developed a relapsing purulent meningitis shortly after a motor accident in Venezuela, in which he received a small wound with a retained foreign body in the sacral region. Repeated initial physical examinations, radiographs of lumbar and sacral regions, and contrast myelography failed to demonstrate a cerebrospinal fluid leak or bone involvement, and Mycobacterium fortuitum was repeatedly isolated from the cerebrospinal fluid. Finally, from the originally "cured" lumbar wound a purulent material was obtained from which M. fortuitum was also isolat

The false fontanel.

In the neonate, palpation of the anterior fontanel is recognized as a simple and reliable means for estimating intracranial pressure. With closure of the fontanel this aspect of the clinical examination is lost. The authors report a series of 15 shunted infants and children in whom a false fontanel was created by making a 2-cm craniectomy in the right parietal region and excising the underlying internal pericranium. This produces a cranial opening which, like the anterior fontanel, can be used for palpation and real-time ultrasound imaging of the brain. By removing the internal pericranium, re

Cryohypophysectomy for the relief of pain in malignant disease.

Fifty consecutive patients with metastatic carcinoma who underwent cryohypophysectomy were studied. Of these, 26 had breast cancer, 19 had prostatic cancer, one had malignant melanoma, one had cancer of the kidney, and three had metastatic adenocarcinoma from an unknown primary tumour. After cryohypophysectomy, excellent pain relief was obtained in 48% of patients, good or acceptable pain relief was obtained in 40%, and poor or no relief in 12%. Two patients died: one of aspiration pneumonia and one of an unknown cerebral cause. Sixteen patients developed diabetes insipidus, of whom three requ

[Lumbar puncture: indications and limitations].

Fast deterioration of the neurological status of patients with raised intracranial pressure after lumbar puncture is not rare. Cerebral coning or rebleeding occur in consequence of cerebrospinal fluid leak. An experimental model correlates leakage with needle gauge. Computed tomography is indicated for diagnosis at the time of the initial examination. Puncture with needles of small diameter (less than 0.5 mm) is recommended.

Facial nerve decompression complications.

Every structure contained within the temporal bone in close proximity to the facial nerve is at risk during intratemporal bone surgery on the facial nerve. We present a review of the causes of injuries to these structures and of ways to prevent such injuries. This review includes information drawn from pertinent literature, from the author's experience managing 139 patients undergoing temporal bone surgery for a variety of facial nerve disorders between 1974 and 1981, from an analysis of 43 of our patients whose hearing was evaluated before transmastoid facial nerve surgery and again 6 months

Computed tomography and cerebrospinal fluid leak.

Cerebrospinal fluid (CSF) rhinorrhea is a serious condition which if untreated may lead to meningitis. Detection of the leak site at surgery may be difficult without prior accurate radiological localization of the leak and at times repeat operation has been required. Clinical and radiological localization of the leak is not always easy and many different tests have been tried. Prior to computed tomography (CT), the most common combination of radiological tests were plain films and tomograms of the skull and CSF isotope studies. Although CT scanning may be helpful in assessing a patient with CS

Mollaret's meningitis associated with cerebrospinal fluid leak.

Mollaret's meningitis or the benign recurrent meningitis syndrome is a rare disorder not previously described in Africa. The syndrome has a characteristic clinical presentation with spinal fluid pleocytosis, often with unusual 'epithelial' cells. With contemporary techniques no causative organism has been incriminated. The aetiology remains speculative, but we report on a patient found to have a cerebrospinal fluid leak, which may represent a factor in the pathogenesis of this disorder.

Spinal cerebrospinal fluid leak demonstrated by retrograde myeloscintigraphy.

Cerebrospinal fluid (CSF) leaks may occur following spinal surgery and are commonly associated with infection. These leaks can be successfully evaluated with retrograde myeloscintigraphy using the same techniques applied in diagnosing CSF otorrhea and rhinorrhea. This report describes the utility of this procedure in differentiating a process due solely to postoperative infection from one that involves both an inflammatory site and a fistulous tract with the subarachnoid space.

Rare complications following ethmoidectomies: a report of eleven cases.

Intranasal ethmoidectomy is one of the most difficult operations to teach residents. An accurate knowledge of the regional topographic anatomy is of utmost importance. Friedman and Kerr reported complications of 1000 cases of consecutive intranasal ethmoidectomies performed at the Mayo Clinic from 1957 to 1972. The complication rate was 2.8%. Meningitis, cerebrospinal fluid rhinorrhea, loss of olfaction, and nasolacrimal duct obstruction were reported. No blindness, loss of occular motility, excision of brain tissue or intracranial vessel damage occurred in their series. We are reporting a ser

Posttraumatic lumbar cerebrospinal fluid leak: detection by retrograde in-111-DTPA myeloscintography.

A case of lumbar cerebrospinal fluid (CSF) extravasation with an unsuspected traumatic meningocele after a gunshot wound was detected by means of retrograde myeloscintography using isobaric In-111-DTPA. Our experience and a review of the literature have provided evidence retrograde myeloscintography may be useful for detecting and delineating significant traumatic thoracic and lumbar CSF leaks.

[Preoperative clinical, radiological and nuclear medical diagnostic and intra-operative findings in frontobasal cerebrospinal fluid fistula (author's transl)].

In 71 patients with rhinosurgical closed fronto-basal cerebrospinal fluid fistula the pre- and intra-operative findings are compared. Cerebrospinal fluid rhinorrhea in 80% and purulent meningitis in 44% were seen. In X-ray 52% fractures and 32% cloudiness of the paranasal sinuses were found, there were 21% false negative results. By nuclear medical examination the cerebrospinal fluid leak was identified in 59%, the result was false negative in 15%. The site of the cerebrospinal fluid fistulas could be preoperative determinated by clinical signs in 96%, by X-ray in 22%, and by scintigraphy in 4

Loss of an aminoglycoside resistance plasmid by Serratia marcescens during treatment of meningitis with amikacin.

During prophylaxis with gentamicin and amoxicillin following surgical repair of a meningomyelocele in a newborn infant, a cerebrospinal fluid leak occurred and fever ensued. Cultures of ventricular fluid yielded Serratia marcescens resistant to several antibiotics, including gentamicin and tobramycin, but sensitive to amikacin. When therapy with amikacin was substituted for that with gentamicin and amoxicillin, cultures yielded an additional colony type of S. marcescens, which was antibiotic-sensitive but of the same serotype as the original isolate, that eventually replaced the original resis

Accidental stab wound of the cervical spinal cord from in front.

An unusual case of damage to the cervical spinal cord due to a non-missile penetrating injury through an anterior route in a nine-year-old boy who fell from a bicycle is reported. There was a cerebrospinal fluid leak that lasted for two days. Clinical picture was that of incomplete Brown-Séquard syndrome and flaccid paresis of the extensor muscles of the left elbow and wrist. Treatment was conservative, with a very good functional recovery. To our best knowledge there is no myelographically documented case report of such a lesion in the literature.

External fixation of facial fractures.

Although external fixation is not indicated in every type of facial fracture, its use alone or with intermaxillary or internal fixation may be of benefit with problems resulting from avulsed segments, infection, inadequate previous reduction, delayed reduction, severe displacement and comminution, non-union, cerebrospinal fluid leak or where a combination of these factors exists. The judicious application of extraskeletal fixation can save valuable anesthetic time as well as assure accesibility of the pharynx and maintenance of an airway and routes for nutrition when these might otherwise be c
