The Importance of Small Lateral Dural CSF Collections in Spontaneous Intracranial Hypotension: A Radiologic-Anatomic Study.

BACKGROUND AND PURPOSE: Spinal CSF leaks cause spontaneous intracranial hypotension (SIH), characterized by orthostatic headaches, but the detection of these leaks may require specialized and invasive spinal imaging. We have noted the presence of small lateral dural CSF collections of unclear significance on digital subtraction myelography (DSM) in some of these patients suspected of having SIH. The purpose of the present study was to compare radiographic and anatomic intraoperative findings in patients with such small lateral dural CSF collections who underwent surgical exploration. Clinical features of high intracranial pressure and prevalence of idiopathic intracranial hypertension among patients presenting with spontaneous CSF leaks.

OBJECTIVE: Growing evidence indicates that chronically raised intracranial pressure (ICP) might cause a spontaneous CSF leak. However, data on the prevalence of idiopathic intracranial hypertension (IIH) and associated clinical features of high ICP among patients presenting with spontaneous CSF leaks are limited. METHODS: The authors performed a retrospective review of patients presenting with spontaneous skull base CSF leaks from 2013 to 2020 at a quaternary care center. Demographic, clinical, and imaging characteristics between groups with versus without IIH were compared using the t-test,

Venous sinus stenting: where does it fit in the treatment paradigm for cerebrospinal fluid leaks?

OBJECTIVE: Spontaneous CSF leaks are traditionally managed with surgical repair. However, a growing body of evidence suggests that some of these leaks may result from idiopathic intracranial hypertension (IIH) and/or elevated intracranial pressure (ICP) due to underlying venous sinus stenosis-an association that remains underrecognized. Venous sinus stenting (VSS) may address CSF leaks by restoring normal venous outflow and reducing ICP. However, few series have evaluated VSS as a primary or adjunctive treatment for CSF leaks. Herein, the authors present a case series highlighting the role of

Conservative Management of Cerebrospinal Fluid Leaks.

Cerebrospinal fluid leaks are a complex clinical challenge with variable causes and management. Conservative therapy focuses on lowering intracranial pressure through bed rest, sinus precautions, antiemetics, bowel regimens, and lumbar drainage. Leaks persisting beyond 1 week may require surgical repair due to meningitis risk. Iatrogenic, postoperative, and spontaneous leaks rarely resolve without surgery, while small traumatic leaks tend to have the highest chance of resolution. Prophylactic antibiotics remain controversial. Management should be tailored to etiology, location, and patient-spe

Empowering front-line physicians with AI: Evaluating large language models in everyday ENT care.

PURPOSE: Artificial intelligence systems known as large language models are being evaluated for clinical decision support, yet their role in emergency and primary care remains limited. Physicians in these settings often encounter ear, nose, and throat conditions where diagnostic uncertainty, unnecessary testing, and inappropriate referrals contribute to patient risk and healthcare inefficiency. This study compared the performance of advanced large language models with physicians in diagnosis, management, and referral across common and high-acuity otolaryngologic scenarios. METHODS: Twelve cli

Comparative evaluation of two dural closure techniques for U-shaped incisions: sealing efficacy vs. site-specific infection risk.

OBJECTIVE: This study aimed to compare the outcomes of direct suturing (Method 1) and pericranium-assisted suturing (Method 2) for U-shaped dural incisions, with a specific focus on site-specific differences in cerebrospinal fluid (CSF) leak and postoperative infection. METHODS: In this retrospective cohort, 172 patients undergoing repair of U-shaped dural incisions were analyzed. Based on intraoperative feasibility, patients underwent either Method 1 (n = 94) or Method 2 (n = 78). Primary and secondary outcomes were CSF leak and postoperative infection rates, respectively. Subgroup analyses

Polyethylene glycol-coated patch versus standard dural sealing practices for preventing cerebrospinal fluid leaks in posterior fossa surgery: a randomized multicenter study.

BACKGROUND: This study evaluated the efficacy and safety of the polyethylene glycol (PEG)-coated patch as a dural sealant in elective non-traumatic posterior fossa surgeries (nTPFS) requiring dural closure. METHODS: This multicenter, randomized, controlled phase-IV study was conducted, between January and December 2022, on adult patients who underwent nTPFS requiring dural opening and closure. Patients were randomized to receive either PEG-coated patch reinforcement or standard sealing. The primary endpoint was the incidence of clinically evident cerebrospinal fluid (CSF)-leakage within four

Diagnostic Tools and Imaging for Skull Base Cerebrospinal Fluid Leak.

The diagnosis of skull base cerebrospinal fluid (CSF) leaks remains challenging and requires a multidisciplinary approach. Clinical evaluation and targeted endoscopic examination are essential, but laboratory confirmation with beta-2 transferrin or beta-trace protein provides the most reliable noninvasive evidence. Imaging modalities such as high-resolution computed tomography (CT), magnetic resonance cisternography, CT cisternography, and radionuclide studies complement detection and localization, guiding surgical planning. Intraoperative fluorescein enhances defect identification. Each diagn

The long-term outcomes of surgical approaches for managing cerebrospinal fluid rhinorrhea: a systematic review and single-arm meta-analysis.

BACKGROUND: Cerebrospinal fluid (CSF) rhinorrhea is a clinically significant condition resulting from abnormal communication between the subarachnoid space and the nasal cavity. Although multiple surgical techniques have been developed, their long-term outcomes remain inadequately defined. Our systematic review and meta-analysis aimed to assess the long-term efficacy and safety of surgical interventions used to manage CSF rhinorrhea. METHODS: A comprehensive search of PubMed, Scopus, Web of Science, and the Cochrane Library was conducted from database inception to March 2025. Studies evaluati

A Nomogram to Predict of Epidural Blood Patch Treatment Failure in Patients With Spontaneous Intracranial Hypotension and Subdural Hematoma.

BACKGROUND: Subdural hematoma (SDH) is a frequent and serious complication of spontaneous intracranial hypotension (SIH), often requiring timely intervention. An epidural blood patch (EBP) is widely recognized as the preferred interventional treatment for SIH and its complications. However, treatment failure remains a concern, and predicting outcomes in patients with SIH and a concurrent SDH post EBP remains a clinical challenge. OBJECTIVES: Our study aimed to develop and validate a predictive nomogram for treatment failure following an EBP in patients with SIH complicated by an SDH, identify

Spontaneous Intracranial Hypotension in the Postpartum Period: A Case Report.

Spontaneous intracranial hypotension (SIH) is characterized by an orthostatic variation in headache precipitation. The typical history connotes headaches that are triggered by an upright posture and partially or completely relieved in the supine decubitus. In most patients, the site of cerebrospinal fluid (CSF) leak is at the cervical or thoracic spinal level. The imaging modalities needed to establish the diagnosis of SIH include computed tomography (CT) and magnetic resonance imaging (MRI) of the brain, CT, and MRI myelography, and radionuclide cisternography. Treatment usually consists of co

Trilaminar DuraGen-Anchored Closure for Dural Repair in Posterior Fossa Surgery: Technical Note and Case Series.

BACKGROUND AND OBJECTIVES: Cerebrospinal fluid (CSF) leakage remains a common complication of posterior fossa surgery, often exacerbated by early postoperative mobilization. We describe a novel fibrin glue-free technique-trilaminar DuraGen-anchored closure-designed to enhance watertight dural sealing through circumferential suturing of DuraGen patches placed both subdurally and epidurally. METHODS: We retrospectively reviewed 20 patients who underwent posterior fossa surgery with this closure technique between 2020 and 2025. A subdural DuraGen patch approximately 1 cm larger than the dural de

The effect of modified head-of-bed elevation on postoperative headache and CSF leakage after meningioma resection.

Postoperative headache and cerebrospinal fluid (CSF) leakage are common complications after meningioma resection, which may prolong hospitalization, increase infection risk, and negatively affect patient recovery. Although maintaining a 30° head-of-bed (HOB) elevation is standard postoperative nursing care to reduce intracranial pressure, the optimal elevation angle remains uncertain. This study aimed to evaluate the efficacy of an improved HOB elevation of 30° to 35° in reducing postoperative headache severity and CSF leakage in patients undergoing meningioma resection. This retrospective coh

In-situ bone flap combined with a nasoseptal flap or a free mucosa flap for skull base reconstruction after endoscopic resection of craniopharyngiomas: A series of 115 cases.

Previous reports have indicated that in comparison with the use of a nasal septal flap (NSF) for skull base reconstruction, the use of a free mucosal flap (FMF) is associated with an increased incidence of cerebrospinal fluid (CSF) leakage in patients with intraoperative high-flow CSF leakage but less nasal trauma. The authors previously reported that the use of an in situ bone flap (ISBF) was beneficial for controlling postoperative CSF leakage. The objective of this study was twofold. The first aim was to analyze the risk factors associated with postoperative CSF leakage. Second, the CSF lea

Sternberg canal defect and intracranial hypertension: a synergistic etiopathology in spontaneous cerebrospinal fluid rhinorrhea. Illustrative case.

BACKGROUND: Spontaneous CSF rhinorrhea is an uncommon but potentially serious condition. It may arise from skull base defects and anatomical variations, often combined with triggers such as idiopathic intracranial hypertension (IIH). The proximity of these defects to critical neurovascular structures and the complexity of endoscopic repair pose surgical challenges. Distinguishing congenital skull base defects, such as a persistent Sternberg canal (SC), from pressure-induced bony erosion is challenging, and both may coexist. OBSERVATIONS: A 35-year-old woman presented with a 4-month history of

Clinical Presentation and Epidemiology of Traumatic and Iatrogenic Cerebrospinal Fluid Leaks.

Cerebrospinal fluid (CSF) leaks occur when the skull base and dura are breached, allowing CSF to escape into sinonasal or tympanomastoid cavities. Traumatic leaks, historically documented to comprise the majority of CSF leaks, happen due to blunt (e.g., motor vehicle accident) or penetrating injuries. Penetrating injuries carry higher morbidity and infection risks. Iatrogenic leaks stem from medical or surgical interventions such as endoscopic sinus surgery, pituitary, and lateral skull base surgeries, or even certain medications. Both types of leaks can lead to serious complications like meni

Cerebrospinal Fluid Leaks of the Skull Base: Physiology and Pathophysiology.

Cerebrospinal fluid (CSF) is a colorless body fluid present within the central nervous system. CSF carries out essential functions in the central nervous system through maintenance of homeostasis, protection from trauma, and control of development. Classic experiments on CSF production postulated that CSF is produced in the choroid plexus and absorbed in the arachnoid granulations, but modern studies suggest that CSF is produced and absorbed through the entirety of the neuroaxis, with its movement primarily dependent on the cardiac cycle. This review will cover current concepts of CSF physiolo
