INTRODUCTION: Spontaneous intracranial hypotension (SIH) is a neurological disorder caused by cerebrospinal fluid (CSF) leaks, most often spinal in origin, and is characterized by orthostatic headache, nausea, and cognitive impairment that can markedly reduce quality of life. Epidural blood patching (EBP) is first-line therapy, but spinal leaks due to ventral dural tears are less likely to respond and often require surgical repair. This scoping review synthesizes current evidence on surgical management of ventral spinal CSF leaks from dural tears and presents an illustrative case of osteophyte

IMPORTANCE: The 5×FAD transgenic mouse model is widely used in Alzheimer's disease (AD) research, yet its non-cognitive phenotypes -- particularly motor coordination, olfactory sensitivity, and auditory reactivity -- remain poorly characterized across the full disease trajectory. Defining these sensory and sensorimotor deficits may enhance the translational relevance of this model and provide insight into non-cognitive features in preclinical AD studies.
OBJECTIVE: To characterize age-related motor, olfactory, and auditory changes in 5×FAD mice and assess associated AD pathologies and neuroin

PURPOSE: This multicenter, multi-surgeon retrospective study aimed to identify risk factors for postoperative cerebrospinal fluid (CSF) leak following endonasal resection of craniopharyngioma, while evaluating the "perception gap" through a surgeon survey.
METHODS: A retrospective review was conducted on 416 patients who underwent endoscopic transsphenoidal surgery (ETS) for craniopharyngioma from 20 institutions. Factors were compared between patients with and without postoperative CSF leak, and between Early (2007-2015) and Late (2016-2025) Epochs. Complementing the clinical data, a survey

STUDY DESIGN: Single-center retrospective cohort study.
PURPOSE: To evaluate tumor control, neurological recovery, perioperative morbidity, and long-term biomechanical stability of unilateral hemilaminectomy (UH) for adult intradural spinal tumors.
OVERVIEW OF LITERATURE: Total laminectomy disrupts stabilizing posterior elements and may necessitate instrumentation for deformity. UH preserves the contralateral posterior complex, offering comparable resection rates, reduced perioperative morbidity, and shorter hospitalization. However, most studies on UH involve small, methodologically heterog

BACKGROUND: Watertight dural closure in deep endoscopic endonasal surgery remains technically demanding, largely due to restricted workspace and limited instrument maneuverability.
METHOD: We describe a standardized, stepwise dural suturing technique based on three core principles: workspace optimization, adaptive grip strategy, and controlled needle handling. Detailed strategies for instrument selection, needle trajectory, and graft-based sealing are presented to enhance reproducibility.
CONCLUSION: Systematic application of these principles enables reliable, watertight dural closure in dee

BACKGROUND: Postoperative cerebrospinal fluid (CSF) leakage and sphenoid sinusitis remain major concerns after endoscopic endonasal approaches (EEAs) for midline skull base tumors. To our knowledge, this study is among the first to directly compare synthetic polyurethane foam (SPF) with balloon catheters (BCs) for sphenoid sinus packing, assessing efficacy and safety, including in high‑flow CSF leaks. METHODS: This retrospective, single-center, observational study included 116 patients who underwent EEAs between April 2016 and March 2025. Since May 2021, SPF has replaced BCs for sphenoid sinus

BACKGROUND: Endoscopic endonasal transsphenoidal (EET) approach has been widely utilized for the treatment of pituitary adenoma. However, postoperative cerebrospinal fluid (CSF) leakage is still a concern. One of the key surgical points to prevent postoperative CSF leakage is how to reduce the defect of the sellar floor and increase its support.
METHOD: We presented a step-by-step description of the incision method of the sellar dura during endoscopic transsphenoidal pituitary adenoma resection, as well as the sellar dura mater diagonal suture technique to reduce postoperative sellar dura mat

BACKGROUND: Spinal cerebrospinal fluid (CSF) leak is a disabling and often misdiagnosed condition characterised by CSF hypovolemia. Associated neurological symptoms are diverse and often leave individuals bed-bound due to their orthostatic nature. Prior literature describing the difficulties in diagnosis, treatment, and ongoing impact of CSF leak is, thus far, confined to Europe and North America. This study provides a novel account of lived experiences of spinal CSF leak in Australia and Aotearoa New Zealand (NZ).
METHODS: An online survey exploring symptoms, diagnosis, treatment, and effect

BACKGROUND: Intradiscal platelet-rich plasma (PRP) has emerging evidence for the treatment of discogenic low back pain. However, procedural access becomes technically challenging at the L5-S1 level in the presence of severe disc collapse, steep lumbosacral lordosis, or high iliac crest morphology, which may preclude standard posterolateral or transforaminal trajectories.
CASE REPORT: A 42-year-old man with chronic discogenic low back pain demonstrated severe L5-S1 degeneration (modified Pfirrmann grade 6) with approximately 3-mm residual disc height, rendering conventional intradiscal access

BACKGROUND AND OBJECTIVES: CompuFlo provides real-time pressure feedback to identify the epidural space during spinal cord stimulator (SCS) implantation.
METHODS: We compared CompuFlo with standard loss of resistance. Single-center retrospective cohort of consecutive SCS implantations by one surgeon using CompuFlo or loss of resistance. Primary outcome: composite of any cerebrospinal fluid leak, epidural blood patch, or postoperative radiculopathy. Secondary outcomes: each component, needle-pass burden (mean passes per electrode; ≥2 passes), operative time, and hospital length of stay (LOS).

BACKGROUND: Comparative evidence regarding unilateral biportal endoscopic decompression vs. tubular decompression or uniportal endoscopic decompression for lumbar spinal stenosis remains limited and inconsistent.
METHODS: We conducted a systematic review and meta-analysis of comparative clinical studies that evaluated UBE vs. tubular decompression or uniportal endoscopic decompression for lumbar spinal stenosis. Pooled analyses were performed for predefined outcomes, and comparator-specific analyses were additionally performed to improve interpretability.
RESULTS: A total of 1,395 patients f

Spontaneous cerebrospinal fluid (CSF) rhinorrhea originating from clival defects is an exceptionally rare condition, with only a limited number of cases reported, and its diagnosis is often challenging, potentially leading to delayed management and recurrent meningitis. We report 2 cases of spontaneous clival CSF leakage in male patients aged 76 and 64 years, both presenting with clear rhinorrhea associated with headaches; one case followed minor head trauma and was complicated by extensive pneumocephalus, while the second was marked by recurrent meningitis with an initially missed defect on i

OBJECTIVE: This retrospective, single-center, descriptive case series evaluates early adoption of a two-stage spine MRI protocol incorporating high-resolution, three-dimensional mesoscale constructive interference in steady state (meso-CISS) for targeted cerebrospinal fluid (CSF) leak localization.
BACKGROUND: Spontaneous intracranial hypotension (SIH) results from CSF leaks at the spinal canal, yet precise localization of dural defects remains challenging. Although fat-saturated heavily T2-weighted magnetic resonance (MR) myelography (HT2-MRM) is sensitive to spinal longitudinal extradural C

Intraoperative hemostasis and dural closure are critical components of neurosurgical procedures, directly affecting operative time, blood loss, neurological outcomes, and the risk of cerebrospinal fluid leak, infection, and reoperation. Over the past 2 decades, there has been rapid expansion in the availability of hemostatic adjuncts, dural substitutes, and liquid sealants. Despite widespread adoption, product selection is frequently guided by surgeon preference and institutional practice rather than a detailed understanding of material composition, mechanism of action, and product-specific li

BACKGROUND: Traumatic skull base fractures carry a risk of cerebrospinal fluid (CSF) leak, predisposing patients to meningitis, neurological complications, and prolonged hospitalization. Predictive factors for posttraumatic CSF leak remain incompletely defined. The primary objective of this study is to identify clinical and radiological predictors of CSF leak among patients with traumatic skull base fractures and evaluate associated outcomes at a Level-1 trauma center.
METHODS: A retrospective cohort study was conducted at King Abdulaziz Medical City, Riyadh, including all patients with compu

BACKGROUND AND OBJECTIVES: Microvascular injury to the optic nerves and hypothalamus is a common source of morbidity during craniopharyngioma (CP) surgery. An optimal strategy to minimize this risk has not been well established. We aimed to describe the "inside-out" concept in CP resection, evaluate associated outcomes, and explore global perspectives on surgical strategies.
METHODS: The surgical technique involves accessing the tumor at a superficial point, and establishing a subpial, as opposed to a subarachnoid dissection plane with the surrounding tissues. The goal is to protect the micro

INTRODUCTION: The perioperative management of spontaneous cerebrospinal fluid leaks (sCSFL) of lateral skull base remains under debate. We systematically synthesized and evaluated the current literature, to investigate the perioperative cranial-pressure assessment protocols and relief methods. The assessment corresponded to fulfillment of criteria for idiopathic intracranial hypertension (IIH) or otherwise defined in the study. Relief methods included either medical, that is, acetazolamide, or invasive, that is, lumbar puncture (LP) or drain (LD) or ventriculoperitoneal shunt (VPS).
