BACKGROUND AND OBJECTIVES: Craniopharyngiomas are complex intracranial tumors that pose significant challenges in surgical management. We aimed to develop risk-stratified surgical outcome benchmarks for adult patients undergoing endoscopic craniopharyngioma resection by using the Registry of Adenomas of the Pituitary and Related Disorders database to facilitate comparisons across institutions and guide clinical decision-making.
METHODS: A retrospective cohort of adult patients (18 years and older) with craniopharyngioma undergoing a first-time endoscopic transnasal surgery across 14 academic

BACKGROUND: Penetrating foriegn bodies extending into brain stem are rare in children and present unique anesthetic and surgical challenges. Airway management is particularly difficult because of restricted oral access, risk of displacement leading to catastrophic hemorrhage and neurological complications.
CASE: We describe the perioperative management of achild who presented in emergency with accidentally impacted foriegn body. The metallic foriegn body occupied the oral cavity significantly limiting the airway access and posing a risk of airway obstruction. A multidisciplinary team involvin

BACKGROUND/OBJECTIVES: External ventricular drains (EVDs) are widely used in the management of intracranial hemorrhage and hydrocephalus; however, they carry a significant risk of device-related central nervous system infections, including ventriculitis and meningitis, which are associated with increased morbidity, mortality, and prolonged intensive care stays. We conducted a systematic review and meta-analysis to evaluate whether prolonged EVD duration (>10 days) is associated with an increased risk of ventriculostomy-related infection compared with shorter duration (≤10 days), and to explore

Cerebrospinal fluid (CSF) leaks, whether spontaneous or iatrogenic, can lead to debilitating post-dural puncture or intracranial hypotension-related headaches characterized by orthostatic symptoms. Epidural blood patching (EBP) has become a mainstay treatment once conservative measures (bed rest, hydration, and caffeine) fail. An EBP involves injecting autologous blood into the epidural space to seal dural defects, with success rates ranging from ∼70%-90% for post-dural puncture (iatrogenic) leaks but only ∼30% for spontaneous leaks on the first attempt. For patients with persistent CSF leakag

OBJECTIVE: Multiple closure techniques may prevent CSF leak after transsphenoidal surgery for sellar lesions. In patients with obstructive sleep apnea requiring positive pressure ventilation (PPV), the optimal closure technique remains unclear. The authors assessed the efficacy of autologous abdominal fat graft repair for preventing CSF leaks in patients reinitiating PPV early after transsphenoidal surgery.
METHODS: A retrospective cohort study was performed to review the records of consecutive patients who underwent microscopic transsphenoidal surgery with autologous fat (with or without fas

• Endoscopic endonasal repair successfully managed CSF leaks after clinoidectomy. • Multilayer reconstruction achieved durable, recurrence-free closure. • Gasket-seal and nasoseptal flap techniques provided watertight repair. • Technique selection should be individualized according to defect features.

OBJECTIVE: To investigate the incidence and treatment of cerebrospinal fluid leakage due to late presentation of dural tears (LPDT) after lumbar surgery and provide new ideas and treatment options for clinical management. This study addresses a significant knowledge gap in the specific context of delayed CSF leaks, where diagnosis and management remain particularly challenging despite existing literature on CSF leaks in general.
METHODS: Patients who underwent posterior lumbar surgery from January 2021 to December 2024 were selected. General patient information, cerebrospinal fluid leakage (C

BACKGROUND AND OBJECTIVE: Trigeminal schwannomas (TSs) are rare, mostly benign tumors that exhibit an indolent clinical course. The lateral orbitotomy approach (LOA), with or without endoscopic assistance, has emerged as a novel skull base route, particularly for accessing lesions confined to the middle cranial fossa. Herein, we elucidate the advantages and limitations of the LOA for the treatment of TSs.
METHODS: All TS cases that underwent LOA through a lateral canthus incision during the past decade in our department were retrospectively reviewed. The operative technique was detailed, and

OBJECTIVE: Endoscopic endonasal transsphenoidal surgery (EETS) has become the preferred method for treating pituitary adenomas; however, it entails a significant learning curve. This study aims to evaluate clinical features, surgical outcomes, and complication patterns during the early institutional experience with EETS and to establish a historical benchmark for tracking the progress of endoscopic pituitary surgery.
METHODS: This retrospective, single-center observational study included 46 consecutive patients who underwent EETS for pituitary adenomas between October 2008 and September 2010.

BACKGROUND: Achieving watertight dural closure in posterior fossa surgery is often challenging due to the anatomical complexity of this region, particularly when suturing is not feasible near critical venous structures. These circumstances increase the risk of cerebrospinal fluid (CSF) leakage, a well-recognized complication. Traditional single-layer techniques may be insufficient to achieve a watertight seal. To address this limitation, we developed a novel multilayer duraplasty method, termed the "Sandwich Technique," which combines an absorbable collagen matrix with autologous fascia.

OBJECTIVE: Cerebrospinal fluid (CSF) leakage remains a significant complication of endoscopic endonasal transsphenoidal surgery (EETS) for pituitary adenomas. This study evaluated incidence, predictors, and outcomes of intraoperative and postoperative CSF leaks, focusing on reconstruction strategies and intraoperative detection performance.
METHODS: A retrospective analysis was conducted on 414 consecutive patients who underwent EETS for pituitary adenomas between 2017 and 2025 at a tertiary referral center. Intraoperative CSF leaks were graded (0-III) and managed using 5 reconstruction modal

INTRODUCTION: Existing recommendations on return-to-flight following neurosurgery are broad, vary by organization, and have limited supporting evidence. Air travel after neurosurgery may increase the risk of medical events, raising the need for focused flight-clearance guidelines.
METHODS: Attending neurosurgeons were surveyed using case-based scenarios with flight-clearance timelines and preflight imaging questions.
RESULTS: Recommended flight-clearance times varied by procedure: tumor craniotomy, [mean (SD)] 2.4 (2.2) wk; subdural hematoma craniotomy, 3.8 (3.5) wk; uncomplicated transparan

BACKGROUND: Paraspinal spindle cell mesenchymal neoplasms of myofibroblastic origin are extremely rare, particularly in the high cervical region. Radiologic differentiation between dural contact and true invasion is often challenging.
CASE DESCRIPTION: A 52-year-old female presented with posterior neck pain and a palpable upper cervical mass. The magnetic resonance demonstrated a 6 × 4 cm paraspinal lesion at the C1-C2 level with suspected dural involvement. Surgical exploration revealed a well-encapsulated tumor extending to the dura without true invasion; this allowed for gross total resect

BACKGROUND: Spontaneous intracranial hypotension (SIH) results from spinal cerebrospinal fluid (CSF) leakage and typically presents with orthostatic headache. Spontaneous resolution without intervention is rare, and its underlying mechanism has not been established.
CASE DESCRIPTION: A 33-year-old female developed a SIH following chiropractic manipulation. The magnetic resonance imaging (MRI) demonstrated a ventral CSF leak at the Th2/3 level. During conservative management, she experienced acute back pain coinciding with the formation of an epidural hematoma at the same level; her headache r

BACKGROUND: Temporal bone resection (TBR) may result in large defects and facial nerve sacrifice.
METHODS: Retrospective review of adults undergoing TBR from 2008 to 2019.
RESULTS: One hundred and forty patients were reviewed. The extent of TBR was lateral (n = 135), subtotal (n = 2), and total (n = 3). Defects were reconstructed with free flaps (n = 72), regional flaps (n = 45), and cervicofacial flaps (n = 31). Complications included wound dehiscence (n = 20), infection (n = 15), hematoma (n = 6), cerebrospinal fluid leak (n = 2), and meningitis (n = 1). Smoking history was associated with

BACKGROUND AND OBJECTIVES: Spontaneous cerebrospinal fluid (CSF) leaks are often secondary to elevated intracranial pressure (ICP). Successful closure of a CSF leak does not treat the underlying elevated ICP. Thus, after repair, patients may continue to have symptoms and develop a recurrent CSF leak. Treatment options for elevated ICP include diuretics, weight loss, ventriculoperitoneal shunting (VPS), and venous sinus stenting (VSS). We present one of the largest series to include surgery, VPS, and VSS for the management of spontaneous CSF leaks in the modern era.

BACKGROUND: Cushing's disease (CD) is a rare endocrine disorder associated with significant morbidity and increased mortality if left untreated. Endoscopic transsphenoidal surgery (ETSS) is the preferred first-line treatment; however, reported remission and recurrence rates vary widely due to differences in biochemical remission criteria and follow-up duration. This study aimed to evaluate surgical outcomes, remission rates using standard and strict biochemical criteria, recurrence, and postoperative complications following ETSS for CD at a tertiary referral center.

Post-dural puncture headache (PDPH) is a common complication of neuraxial anesthesia, particularly in the obstetric population, where its incidence is higher due to the frequent use of epidural techniques and the occurrence of accidental dural puncture. The condition arises from cerebrospinal fluid leakage following disruption of the dura mater, leading to intracranial hypotension, compensatory cerebral vasodilation, and traction of pain-sensitive structures, which collectively explain its characteristic orthostatic presentation. Pregnancy-related physiological changes, including increased int

OBJECTIVE: Incidental dural tears occur in 1.6% to 17% of spinal procedures and may lead to complications including cerebrospinal fluid (CSF) leaks, pseudomeningocele, and meningitis. While primary suture repair remains the standard approach, failure rates range from 5% to 9%. This study evaluates fibrin-coated collagen patches combined with paraspinal muscle flaps for managing dural tears during spine surgery.
METHODS: A retrospective review of prospectively collected cases was conducted at a single institution. Patients with intraoperative dural tears repaired using fibrin-coated collagen p
